Crowding effect on adult growth, pre-patent period and egg shedding of Fasciola hepatica.

Fascioliasis pathogenesis depends on fluke burden. In human hyperendemic zones, individual infection intensities reach very high levels and the majority of infected subjects should be in the advanced chronic phase. The rat model offers a useful approach for pathological research in the advanced chronic period. The influence of infection intensity per rat on fluke development, pre-patent period and egg shedding (eggs/g faeces/worm) was analysed in 3 groups (I: 1-3 worms/rat; II: 4-6; III: 7-9). Ontogenetic trajectories of fluke body measures followed a logistic model. Results showed that when the burden increases, the maximum values of fluke measures decrease. The crowding effect is manifest when fluke measures approximate their maximums in the advanced chronic stage. The pre-patent period and egg production decrease when the burden increases. This means that measurements of eggs per gramme of faeces tend to underestimate the fluke burden. The present study demonstrates how to quantify the fascioliasis experimental rat model crowding effect on adult growth, pre-patent period and egg production. This quantification may be of great interest in epidemiological studies and in experimental research on the in vivo actions of different anthelminthic drugs and vaccines, pathology, immunology and resistance studies.

Patent ductus venosus.

BACKGROUND: Patent ductus venosus is extremely rare with only 14 cases reported in the world literature. We present a case of patent ductus venosus. METHODS AND RESULTS: A 29-year-old male was admitted with melaena stool caused by gastric haemorrhagic ulcers. Laboratory data disclosed severe anaemia; however, liver function tests were normal. Serum ammonia was also within the normal range. Serological viral markers for hepatitis B or C were all negative. The abdominal ultrasonography and computed tomography indicated a 12 mm diameter shunt located in the left lobe of the liver, which connected the portal vein with the left hepatic vein. After treatment for gastric ulcers, percutaneous transhepatic portography was performed and an enormous shunt connecting the umbilical portion of the portal vein with the left hepatic vein was revealed. CONCLUSIONS: Histological findings of the liver biopsy showed that portal venules could not be observed in the portal areas and that no fibrosis or inflammatory cell infiltration were shown. Because of the anatomical position of the shunt, the case was diagnosed as patent ductus venosus.

Dacryocystorhinostomy for epiphora in the presence of a patent lacrimal system.

PURPOSE: To evaluate the success rate of dacryocystorhinostomy (DCR) for epiphora in patients with a clinically patent lacrmal drainage system. METHODS: A series of 51 patients, 12 men and 39 women, who underwent DCR surgery for epiphora with a clinically patent lacrimal drainage system in the background of normal eyelid examination, were evaluated. All patients underwent fluorescein dye disappearance testing and Jones 1 and 2 testing with dacryocystograms in borderline cases. A standard DCR was performed with bicanalicular silicone tubing inserted in all cases. The average time for the removal of the silicone tubes was 9.6 weeks. RESULTS: In 48 cases (94%) there was improvement in symptoms with minimal or no significant epiphora postoperatively. CONCLUSIONS: Epiphora with a patent lacrimal drainage system obstruction can be successfully treated by DCR based on fluorescein dye disappearance tests and Jones 1 and 2 tests, with dacryocystography in borderline cases.

[Multiple paradoxical emboli in patent foramen ovale].

HISTORY AND CLINICAL FINDINGS: A 38-year-old man was admitted because of angina pectoris with concomitant dyspnoea. Three months previously he had suffered an ischaemic stroke of the right middle cerebral artery and was treated in a neurological department. At that time, no aetiologic diagnosis was possible. There was no history of other diseases. Pulse rate was 100 beats per minute with a blood pressure of 140/60 mm Hg. The left calf had a 4 cm greater circumference without any symptoms. The rest of the physical examination in the markedly overweight patient was normal. INVESTIGATIONS: The ECG showed sinus rhythm and negative T-waves in leads V1-V4 and a slightly elevated ST-segments in II, III and aVF. An acute coronary thrombosis was ruled out by left heart catheter-angio. DIAGNOSIS, TREATMENT AND COURSE: Within the following hours, embolic occlusion of the left popliteal artery developed and was treated with a Fogarty catheter. On the first postoperative day, the patient complained about mild dysaesthesia of his right arm. Duplex sonography showed a floating thrombus in the left carotid bifurcation. The thrombus was removed surgically. Later a pulmonary embolism due to deep vein thrombosis in the left thigh and calf was found. Transoesophageal echocardiography performed in another hospital previously was repeated and a patent foramen ovale (PFO) with a middle-sized shunt was found. The patent foramen ovale was closed percutaneously by implanting a Cardioseal-Starflex occluder. There was neither a complication nor a residual shunt. Neurological symptoms disappeared completely within the next few months. The patient has now been free from new neurological events for 11 months. CONCLUSION: In patients with PFO, paradoxical embolism remains a challenging diagnosis that can be made highly probable by documentation of venous thromboses, pulmonary embolism, missing evidence of atherosclerosis in the vessels of the embolized organ and exclusion of other cardiovascular sources of emboli and prothrombotic coagulation disorders. Interventional closure of a patent foramen ovale appears to be the treatment of choice in proven paradoxical embolism.

Trapped thrombus in a patent foramen ovale.

We report two cases of impending paradoxical embolism through a patent foramen ovale. A 73-year-old male had recurrent pulmonary embolism and a large thrombus trapped in a patent foramen ovale. The other patient, a 70-year-old male had septic mediastinitis after prior bypass surgery and a large thrombus lodged in a patent foramen ovale. Cardiac embolectomy and closure of the foramen ovale was performed in both cases because of impending paradoxical embolism. In addition pulmonary thromboendarterectomy was performed to relieve pulmonary hypertension. Therapeutical options are discussed.

Patent foramen ovale presenting as platypnea-orthodeoxia: diagnosis by transesophageal echocardiography.

Platypnea-orthodeoxia is a rare syndrome that is often associated with interatrial shunting through a patent foramen ovale or atrial septal defect. We describe the case of a 65-year-old woman with progressive dyspnea and hypoxia when standing and walking, which was relieved by assuming the recumbent position. The diagnosis was confirmed by tilt-table transesophageal echocardiography demonstrating a large right-to-left shunt through a patent foramen ovale while the patient was in a semiupright position and no significant shunt while in a recumbent position. This case demonstrates that platypnea-orthodeoxia caused by a patent foramen ovale can be clearly demonstrated by the technique of contrast transesophageal echocardiography performed on a tilt table.

Lacrimal drainage capacity and symptomatic improvement after dacryocystorhinostomy in adults presenting with patent lacrimal drainage systems.

AIM. To determine the rate of symptomatic improvement after dacryocystorhinostomy (DCR) in patients with epiphora and insignificant obstruction to preoperative lacrimal syringing, and to measure the conductance of the postoperative lacrimal drainage anastomosis. METHODS. Retrospective cohort study of patients with symptomatic epiphora and patent tear ducts, with at least two-year follow-up after DCR. Patients underwent structured telephone interview together with clinical examination for lacrimal drainage capacity using the saline drop test.(1) RESULTS. Thirty-seven of 383 adult patients having had DCR were noted to have patent tear ducts at preoperative examination. Ten patients with previous surgery were excluded and two other patients were excluded because of other factors contributory to epiphora. Of the remaining 25 patients, 22 (aged 35-75 years at surgery; median 56 years) underwent a structured telephone interview: Of the 13 (60%) who felt that symptoms had improved, 9 (41%) were relieved of indoor epiphora and 11 (50%) thought the procedure was an overall success. Seventeen patients underwent clinical examination. The lacrimal drainage anastomosis was patent in all patients and the saline drop test result was well within the normal range in 3/4 patients who had persistent indoor epiphora. CONCLUSION. Of patients with epiphora due to minor nasolacrimal duct stenosis, open DCR produces a marked improvement or cure of symptoms in 50%. The lacrimal drainage capacity was well within the normal range in 3/4 patients with persistent postoperative lacrimal symptoms, suggesting that other factors (such as hypersecretion) may be significant in this group of patients with 'functional block'.

RT-PCR and immunohistochemical evaluation of sentinel lymph nodes after in vivo mapping with Patent Blue V in colon cancer patients.

OBJECTIVE: Lymph node status is the most important predictive factor in the treatment of colorectal cancer. As sentinel lymph node (SLN) biopsy might upstage stage II colon cancer, it could have therapeutic consequences in the future. We investigated the feasibility of in vivo SLN detection with Patent Blue V dye and evaluated nodal microstaging and ultrastaging using cytokeratin immunohistochemistry and reverse transcriptase-polymerase chain reaction (RT-PCR). MATERIAL AND METHODS: In 30 consecutive patients operated on for colon cancer, subserosal injection with Patent Blue dye was used for SLN detection in four different hospitals under the supervision of one regional coordinator. In searching for occult micrometastases, each SLN was examined at three levels. In tumor-negative SLNs at routine hematoxylin-eosin (H&E) examination (pN0) we performed CK8/CK18 immunohistochemistry (IHC) and RT-PCR for carcinoembryonic antigen (CEA). RESULTS: The procedure was successful in 29 out of 30 patients (97%). The SLN was negative in 18 patients detected by H&E and IHC. In 16 patients the non-SLN was also negative, leading to a negative predictive value of 89% and an accuracy of 93%. Upstaging occurred in 10 patients (33%) - 7 by IHC and 3 by RT-PCR. Aberrant lymphatic drainage was seen in 3 patients (10%). CONCLUSIONS: The SLN concept in colon carcinoma using Patent Blue V is feasible and accurate. It leads to upstaging of nodal status in 33% of patients when IHC and PCR techniques are combined. Therefore, the clinical value of SLN should be the subject of further studies.

Inter- and intra-observer variability in detection of patent foramen ovale with contrast echocardiography.

Patent foramen ovale is detected in-vivo using the technique of contrast echocardiography. A positive test depends on the ability of an observer to recognise micro-bubbles passing from the right to the left heart, and to differentiate them from background noise and/or chordae tendinae. To assess the inter- and intra-observer variability of this technique four observers reviewed independently, and on two separate occasions, 100 consecutive contrast studies stored on video tape. The proportions of agreement (95% CI) for each separate observer on the two occasions for the presence of patent foramen ovale were 0.91 (0.76-0.98), 0.88 (0.73-0.92), 0.80 (0.63-0.92) and 0.78 (0.60-0.91), and for its absence were 0.96 (0.88-0.99), 0.95 (0.87-0.99), 0.90 (0.81-0.96) and 0.91 (0.82-0.96), respectively. The proportions of agreement (95% CI) between all observers on the first viewing were 0.78 (0.72-0.84) for the presence and 0.91 (0.87-0.93) for the absence of patent foramen ovale. For the second viewing these were 0.81 (0.75-0.86) and 0.91 (0.88-0.94), respectively (P = NS, Chi-squared test). These results indicate that contrast echocardiography is subject to clinically acceptable inter- and intra-observer variability.

Hypoxemia from right to left shunting through patent foramen ovale.

We report a patient with severe hypoxemia from a large (41%) right to left shunt through a patent foramen ovale after right ventricular myocardial infarction, and review 18 previous descriptions of patients with right to left shunting through patent foramen ovale. These shunts occur when right atrial pressure is elevated above left atrial pressure, or when the anatomic relationship of the interatrial septum to the inferior vena cava is altered. Since 15-35% of the population have a potentially patent foramen ovale, interatrial right to left shunting may occur more frequently than had previously been recognized, and should be considered in a differential diagnosis of hypoxemia.

Transhepatic balloon dilation of anastomotic biliary strictures in liver transplant recipients: the significance of a patent hepatic artery.

PURPOSE: To determine the significance of hepatic artery steno-occlusive disease on the patency of anastomotic biliary strictures in liver transplant recipients after transhepatic balloon dilation. MATERIALS AND METHODS: A retrospective review of records of all patients undergoing transhepatic balloon dilation for anastomotic biliary strictures after orthotopic liver transplantation was performed over an 8-year period. Patency of the anastomosis was based on subsequent cholangiography. The presence of hepatic artery steno-occlusive disease was determined by Doppler ultrasound and/or angiography. The anastomotic biliary stricture patency rates were calculated by the Kaplan-Meier method. RESULTS: Thirty-eight patients who had undergone liver transplants underwent 53 balloon dilations for anastomotic biliary strictures (nine patients for arterial disease, 26 patients had patent arteries and three patients had arteries of indeterminate patency). Eight of the 53 strictures treated (15%) were refractory to balloon dilation: 10.5% of first comers and 27% of restenotic lesions. Two of the 53 strictures treated (4%) had significant complications: hemobilia requiring blood transfusion and ductal rupture. One-year cumulative primary patency rates for anastomotic biliary strictures for patients with arterial disease, patent hepatic arteries, and all-comers were: 0%, 45% (P = .01), and 36%, respectively. One-year cumulative primary patency rates for choledocho-choledocal and choledocho-jejunal anstomoses in patients with patent arteries were 43% and 48%, respectively (P = .10). CONCLUSIONS: In the presence of hepatic artery disease there is a lower patency of anastomotic biliary strictures after balloon dilation. Imaging of the hepatic artery should be considered to stratify patients who will have a successful outcome.

Congenital patent urachus in an adult: a case report.

A rare case of the congenital patent urachus in an adult is reported. The patient, a 42-year-old man, was admitted with the chief complaint of macroscopic hematuria. During the cystoscopy perfusion water discharged from the umbilicus. Diagnostic imaging revealed a tubular fistula between the bladder and umbilicus and the patient underwent urachal resection and partial cystectomy. Microscopically the lumen was lined by transitional, columnar and squamous epithelium and there was no evidence of malignancy. To date 11 cases of congenital patent urachus in an adult have been reported in Japan. From a review of the Japanese literature, the common chief complaint is urine discharge from the umbilicus accompanying cystitis and/or urachal inflammation. Urachal resection is performed in all cases. We also clarified the epithelial histology of the patent urachus in this patient.

Patent omphalomesenteric duct: a case report and review of Japanese literature.

Patent omphalomesenteric duct (umbilical enteric fistula) was diagnosed in a 7-day-old infant. The duct closed spontaneously, but at the age of 4 months the infant was readmitted for a resection of the duct. A review of the literature disclosed that 65 cases of patent omphalomesenteric duct have been reported in Japan. The male/female ratio was 2.8:1. Ten out of 36 infants (27.8%) were premature. Surgery was performed in as many patients as possible--55 out of 59 cases (93.2%). The ducts averaged 3.8 cm in length and 1.1 cm in diameter. Errant gastric mucosa was found in 3 out of 30 cases (10.0%). Prolapse of the ileum was present in 28 out of 53 patients (52.8%)--a relatively high incidence. Ten out of 55 patients died (18.2%); 8 of these had a prolapse of the ileum. In view of the high mortality rate of patients with a prolapse of the ileum and the strong possibility of intestinal obstruction, patent omphalomesenteric ducts should be resected surgically.

Reoperation in patients with patent coronary bypass grafts.

As the population of patients having undergone myocardial revascularization increases, the number of reoperations will escalate and reoperations of patients with patent grafts will become increasingly frequent. Successful management of this new surgical entity requires a clear understanding of the probability of atherosclerotic involvement of patent vein grafts and careful attention to technical detail. Patent vein grafts more than five years old should be replaced to avoid the potential for embolization of atheromatous material and late vein graft closure.

Transcatheter closure of patent foramen ovale using the Amplatzer septal occluder to prevent recurrence of neurological decompression illness in divers.

OBJECTIVE: Large flap valve patent foramens may cause paradoxical thromboembolism and neurological decompression illness in divers. The ability of a self expanding Nitinol wire mesh device (Amplatzer septal occluder) to produce complete closure of the patent foramen ovale was assessed. PATIENTS: Seven adults, aged 18-60 years, who had experienced neurological decompression illness related to diving. Six appeared to have a normal atrial septum on transthoracic echocardiography, while one was found to have an aneurysm of the interatrial septum. METHODS: Right atrial angiography was performed to delineate the morphology of the right to left shunt. The defects were sized bidirectionally with a precalibrated balloon filled with dilute contrast. The largest balloon diameter that could be repeatedly passed across the septum was used to select the occlusion device diameter. Devices were introduced through 7 F long sheaths. All patients underwent transthoracic contrast echocardiography one month after the implant. RESULTS: Device placement was successful in all patients. Device sizes ranged from 9-14 mm. The patient with an aneurysm of the interatrial septum had three defects, which were closed with two devices. Right atrial angiography showed complete immediate closure in all patients. Median (range) fluoroscopy time was 13.7 (6-35) minutes. Follow up contrast echocardiography showed no right to left shunting in six of seven patients and the passage of a few bubbles in one patient. All patients have been allowed to return to diving. CONCLUSION: The Amplatzer septal occluder can close the large flap valve patent foramen ovale in divers who have experienced neurological decompression illness. Interatrial septal aneurysms with multiple defects may require more than one device.

Incomplete myocardial reperfusion despite a patent coronary bypass: a generally unrecognized shortcoming of the surgical approach to coronary artery disease.

When assessing a coronary artery bypass graft, the patency rate is virtually the sole criterion of success. However, this fails to identify grafts which are patent by incompletely reperfuse the diseased vessel. Angiograms of 82 patients with 172 bypassed coronary arteries confirmed patency in 126 (73%); however, in 30 reperfusion was incomplete though the graft was patent, making the rate of complete reperfusion only 56% (96/172). Causes included localized narrowings in the region of the anastomosis [9], significant narrowing along the graft [6], pre-existing additional stenoses in the native vessel, remote from the anastomosis and the lesion [7], stenoses not present preoperatively but seen postoperatively in the native vessel, remote from the anastomosis and original lesion [6], and separate obstructions of unbypassed branches [2]. While incomplete reperfusion is often unavoidable and is certainly preferable to total occlusion, evaluation based purely on patency rates is misleading, since it overlooks the fact that a significant proportion of patent grafts will not totally reperfuse the diseased vessel.
