This paper aims to present the usefulness of the different diagnosis imaging methods (anatomical and functional) in the characterization of bone injury. Any data, however insignificant, is justified and should be specified. In this case, the discrepancy between the vascular and pool phases in the bone scintigraphy with 99mTc-MDP reveals revealed a lesion with an intense reaction secondary to the "foreign body effect", which is not necessarily malignant.
Twenty-two patients with unstable Frykman grade 7 or 8 intra-articular fractures of the distal radius were treated with an external fixator. The distal pins were inserted into the distal radial fracture fragments, permitting movement of the wrist and hand. Eleven patients were male and 11 female, with a mean age of 50 years. All patients had regained full function with good range of motion at mean final follow-up of 12 months. However function, pain and range of motion had returned to acceptable levels 4 weeks after removal of the external fixator. This method of external fixation provides a reliable method of maintaining fracture reduction whilst allowing early return of function.

This is a retrospective study of 10 patients (13 feet) with moderate to severe hallux valgus who underwent a chevron or modified chevron osteotomy with multiple adjunctive soft-tissue releases for surgical treatment. Radiographic and subjective results were evaluated with an average follow-up of 24 months (range, 10-41). Preoperative criteria included an intermetatarsal (IM) angle of greater than 16 degrees (average of 18.4 degrees) and painful hallux valgus deformity. Average preop hallux abductus was 35.4 degrees (range, 25 degrees-48 degrees). The average reduction in the actual IM angle was (-) 5.2 degrees with a relative IM correction of (-) 11.6 degrees. Average postoperative hallux abductus angle was 7.0 degrees (range, 0 degree-22 degrees). Subjectively, all patients were satisfied with their results and stated they would have the procedure again. No complications were noted in this patient population, including, infection, avascular necrosis, hallux varus, and recurrence of deformity. The chevron osteotomy was found to be successful in this population with high intermetatarsal angles when appropriate consideration was given to correction of soft-tissue-deforming forces and contractures.
A bone defect model was developed in the distal metaphysis of the femur for studies on bone healing in the mouse. The circular defect involving 20% of the bone circumference resulted in a 34% reduction in the bending moment compared to intact bone. The healing process was followed using histomorphometry, peripheral quantitative computed tomography (pQCT), biomechanical testing, and molecular biological analyses. Histologically, healing of the defect was characterized by filling of the medullary cavity with trabecular new bone during the first week of healing, and by closing of the cortical window by 6 weeks. Small areas of periosteal chondrogenesis were frequently observed during defect healing. In pQCT, bone mineral content (BMC) of the defect area approached that of intact control bone already by 3 weeks, reflecting the production of trabecular bone. Similarly, the bending strength and stiffness of the healing femur reached the level of intact control femur already at 3 weeks. Bone formation and remodeling was followed by Northern analyses, which demonstrated elevated mRNA levels for bone components (type I collagen and osteocalcin), and for osteoclastic enzymes (cathepsin K, matrix metalloproteinase-9, and tartrate-resistant acid phosphatase) throughout the healing period. Finally, the applicability of the defect model for gene therapy experiments was tested using adenovirus-mediated transfer of the LacZ reporter gene. Both histochemistry and mRNA analyses demonstrated that the gene was expressed in the repair tissue with the highest expression during the first week of healing. The present model thus provides a standardized environment for studies on induction and remodeling of trabecular new bone in normal and genetically engineered mice.
A connection has been suggested between glucocorticoid-induced osteopenia and an increase in the apoptosis of bone cells, and between the dimerization of the glucocorticoid receptor (GR) and the development of apoptosis. On this basis, a study has been carried out on the relationships between the occurrence of apoptotic cells and their detectable GR content, and between apoptosis frequency and changes in histomorphometric variables, in the growth plate and secondary spongiosa of rat long bones after the high-dose (10 mg/day) administration of corticosterone (CORT) and after recovery. The main results of the CORT treatment were: a significant increase in apoptotic osteoblasts, and a concomitant decrease in the histomorphometric variables of bone formation, with a reversal of both values during recovery; a nonsignificant increase in the apoptosis of osteoclasts, without changes in the histomorphometric variables of bone resorption; a significant increase in apoptotic terminal hypertrophic chondrocytes; the presence of GR in all types of skeletal cells in control rats, with different (cytoplasmic and/or nuclear) immunohistochemical detection in the same type of cell; a decrease in GR detection in proliferative chondrocytes and osteocytes in CORT and recovery groups, and in the maturative/hypertrophic chondrocytes of the recovery group; a fall in growth cartilage width, possibly due to the reduced proliferation of proliferative chondrocytes and increased apoptosis in terminal hypertrophic chondrocytes. In conclusion, pharmacological doses of CORT reduce bone formation by increasing osteoblast apoptosis; they reduce growth cartilage width, probably by inhibiting chondrocyte proliferation and increasing the apoptosis of terminal hypertrophic chondrocytes, and they reduce osteocyte GR. Although these effects appear to be mediated by the presence of GR in all skeletal cells, no precise correlation between GR immunohistochemical detection and apoptosis induction has been found.

Hydroxyapatite ceramic made of bovine spongiosa was used as structural support material in a prospective study to correct bone defects experienced after reduction in distal radius fractures. The study took place over a 3-year period (1992-1999) and comprised 14 patients. Osseous integration was analyzed via biopsies and magnetic resonance imaging. Long-term follow-up monitoring involving magnetic resonance imaging in 13 of the 14 patients showed fibrovascular growth within incorporated hydroxyapatite material. Osseous integration was demonstrated in magnetic resonance images by gadolinium uptake and by the presence of osteoid layers and endothelialized vessels. Hydroxyapatite ceramic offers a biologically acceptable alternative to autologous bone when augmenting distal radius fracture fixation.
The purpose of this study was to establish the relationship between force at the distal radius and power grip force of the hand, a common functional and rehabilitation maneuver. This information will provide limits of allowable grip forces during postfixation rehabilitation and guide design requirements for fixation systems. By designing a model of power grip using the extrinsic hand musculotendinous units, we were able to compare grip force with force at the distal radius. Our results show that to obtain 10 N of grip force, approximately 26.3 N of force is transmitted through the distal radius, 52.4 N is transmitted through the radius and ulna combined, and 30.0 N needs to be applied to the flexor tendons. Fifty-one percent of the total forearm force was transmitted through the distal radius in this model. If all forearm forces were transmitted through the radius, 52 N of force would be transmitted through the distal radius to obtain 10 N of grip force. The clinical application of this model suggests that since failure forces of tested distal radius fixation systems range from 55 to 825 N, rehabilitation grip force should not exceed 10 to 159 N, depending on the type of fixation.

Unlike the clay-shoveller's fracture in adults, stress injuries in juveniles trend to be localised at the tip of the spinous process, i.e. in the metaphysis, the apophyseal line and the apophysis. Characteristic changes are found only between 14 1/2 and 16 1/2 years, at, or immediately before, ossification of the tip of the apophysis. These fractures share with the corresponding fractures in adults similar occupational history, a similar clinical picture with a benign course of the disease. Healing is by the formation of a pseudo-arthrosis which may, however, consolidate after several months or years. Synonyms which have been used for the stress injury to the spinous process in the growing skeleton are: clay-shoveller's fractures in juveniles, apophysitis of the spinous process, aseptic necrosis of the spinous process, gross disturbance of the spinous process after chronic trauma or Schmitt's disease.
AIM: Aim of this study was to determine whether coating of titanium implants of various surfaces with BMP-3 would improve the osseous integration of the implants into the orthotopic bony implant bed. METHOD: In this experimental study 190 micro g per implant of highly purified bone morphogenetic protein 3 (BMP-3) precipitate isolated from porcine bone were available for the coating of each of 24 cylindrical test implants (12 with hydroxyapatite and 12 with plasmapore surface). The remaining 24 test implants with the same surface makeup served as negative controls. Implantation sites were randomly assigned for the 4 versions of implants available and all implants were embedded into the medial or lateral femoral condyle of both legs of 12 German shepherds. The drilling holes were performed in such a matter that after embedding the cylindrical devices a gap of 1 mm surrounding the implants remained. A biomechanical testing and histological evaluation was performed on the explants 42 days after surgery. RESULTS: In biomechanical testing forces necessary to extract the implants from the explanted bones in BMP-3 coated devices were up to 70% higher compared to the ones in the non-coated reference groups. Quantitative histomorphometric examination showed in BMP-3-coated implants an increasing formation of new bone close to their own surface (gap-healing) which was higher than in the corresponding non-coated controls (hydroxyapatite + BMP-3 32.1%, hydroxyapatite controls 20.3%, plasmapore + BMP-3 30.2%, plasmapore controls 13.1%). The extent of direct bone implant contact as percentiles of the corresponding implants perimeter (ongrowth) was also significantly higher in the BMP-3-coated implants compared to the non-coated controls (hydroxyapatite + BMP-3 37.7%, hydroxyapatite controls 22.4%, plasmapore + BMP-3 15.3%, plasmapore controls 6.4%). CONCLUSION: In this study it was proven the first time that implants of various surface textures as used in endoprosthetics are able to be coated by the osteoinductive growth factor BMP-3. In that way metallic implants can achieve osteogenic properties which have positive effects in osseointegration.
AIM: Under early mobilisation and full weightbearing the primary stability of two different cementless hip stems should be examined by using Roentgen Stereometric Analysis (RSA). METHOD: 26 patients (slashed circle 60.8+/-7.5 J., 16 m, 10 w, BMI 27.7+/-3.9 kg/m2) received a total hip arthroplasty with an anatomical designed image-stem (IS) (Smith & Nephew, Schenefeld, Germany), 20 patients (slashed circle 60.0+/-10.9 J., 12 m, 8 w, BMI 27.5+/-3 .7 kg/m2) the Bicontact-stem (BS) (Aesculap, Tuttllingen, Germany). RSA measurements were done before mobilizing, 3 and 6 weeks, 3, 6, 12 and 24 months postoperatively. Furthermore the patients had to estimate their pain on a visually scale (VAS). We registrated the Harris-Hip-Score (HHS) and the Western Ontario and McMasters Universities Osteoarthritis Index (WOMAC). RESULTS: Both stems showed the maximum of subsidence in the first 3 months (IS 0.647 mm, BS 0.54 mm). 6 months postoperative the measured migrations were in the range of the accuracy. After 2 years the subsidence was 1.07 mm+/-0.07 mm for the IS-group, 0.97 mm+/-0.18 mm for the BS-group. There were no significant differences in the examined parameters between the both groups. An excellent prostheses function and high activity grade with a high WOMAC score, a high HHS and a low VAS was founded for both groups. CONCLUSION: Both cementless implanted titanium hip stems showed a sufficient primary and midterm stability with excellent clinical results. The measured migrations do not differ from those given in literature for cemented stems.

Because most scholars take it for granted that medicine is concerned with healing and problems of ill health, the way in which various medical systems define good health has not been adequately studied. Moreover, good health as such is usually regarded as a natural, normative state of being even by most medical anthropologists, who otherwise take a critical, relativist perspective on the subject of illness, pain, and disease. Using the case of Ayurvedic medicine, this article shows that there is a very different way of looking at the question of how health is embodied. This perspective is proactive and concerned with overall fitness rather than reactive and primarily concerned with either illness or disease. The argument presented here therefore seeks to go beyond the limiting--although extremely useful--orientation of remedial health care and suggest a radical challenge to some of the most basic ontological assumptions in the cross-cultural comparative study of medical systems.
