[The osseous location of echinococcal cysts: comments on a clinical case].

Primary bone location of hydatid disease is extremely rare. The clinical case herein reported represents a primitive bone location of echinococcus in the ileal and sacral area. The aspecific symptoms did not allow for a preoperative diagnosis. The latter was possible only after a CT scan performed for other reasons. The patient underwent a CT scan and MIR which showed a diffuse involvement of the ilium and sacrum along with the presence of two cysts (anterior and posterior). Surgery was performed through an anterolateral extraperitoneal and posterior access in the gluteal region. Asportation "en bloc" of the cysts was not possible. Therefore, they were opened, aspirated and washed with hypertonic solution (33% NaCl). Following surgery Albendazole was prescribed to the patient. At 22 days from surgery, a CT scan confirmed the disappearance of the cysts along with the presence of bone lesions.

Abnormal syndrome of iliac osteomyelitis presenting as acute appendicitis.

A case of right iliac osteomyelitis initially misdiagnosed and treated as acute appendicitis is reported. Deep-seated right lower quadrant pain persisted and a gluteal abscess appeared in the immediate post-operative period. The gluteal abscess was incised and it continued to discharge pus until appropriate diagnosis and treatment was instituted. Pain due to iliac osteomyelitis is deep-seated and may radiate to the thighs or lumbar region. Compression and distraction of the pelvis elicits pain in the affected ilium.

[Operative treatment of complicated distal femoral fractures].

OBJECTIVE: To explore an effective way for treating severe complicated distal femoral fractures. METHODS: Twenty-six patients with complicated distal femoral fracture who all belonged to 33C3.3 type according to AO/ASIF classification, were treated with a lateral condylar buttress plate or self-designed aliform anatomical plate, and operated on with allogeneic bone grafting. RESULTS: All cases were followed up for an average of 14 months (ranging 5-25 months). Twenty-four wounds were primary healing postoperatively, 2 wounds were infected and healed after dressing change. Twenty-four had bone healing after 4-11 months, 2 needed to operate again because of earlier weight-bearing resulting in fixation failure. According to shelbourne and Brueckmann score, the excellent and good rate was 88.46%. CONCLUSION: The internal fixation for complicated distal femoral fracture by self-designed aliform anatomical plate and lateral condylar buttress plate with a great deal of allograft bone is an effective surgical method. As it has long oval holes and the holes are consecutive, the aliform anatomical plate is more suitable for severe complicated fractures. At the same time, autogenous-ilium transplantation can be substituted by the allograft bone.

[Comparative studies of bullet entrance holes in trunk and extremity bones of predominantly spongious structure].

The size relation between the bullet calibre and the minimum diameter of the entrance hole in extracranial bones of predominantly spongy structure was systematically investigated. Test shots (cartridges .22 lr, 7.65 mm Brown. and 9 mm Parabellum) on bone specimens covered with soft tissue (coxal end of the femoral bone, ala ossis ilii, corpus sterni) showed the following results: The entrance holes in bone produced by solid-lead projectiles of calibre .22 showed a tendency towards larger diameters compared with the bullet (deviations at the proximal femur up to about 20%); full-jacketed bullets of calibre 7.65 mm and 9 mm mostly produced entrance holes which were smaller than the cross-section of the projectile (deviations at the sternum and os ilium up to about 12%). For perpendicular hits it was possible within one test-firing region to assign the minimum diameters of entry to one of the three calibres without any overlapping in the borderline area. The measurement results classified according to calibre and area of entry are indicated in diagrams.

Sex determination in subadults using auricular surface morphology: a forensic science perspective.

The determination of sex in subadult skeletons remains a problem for several areas of biological anthropology. To date, univariate and multivariate assessments of sex in the young using adult indicators have failed to produce reliable results. However, research in this area continues. In 1980, Weaver proposed a modification of adult differences in auricular surface morphology as an effective means for sex determination in subadult remains. His method was indirectly evaluated by Hunt through a comparison of the sex ratios produced by this technique and the expected 1:1 ratio. The present investigation expands upon both studies by using a sample of subadults of known sex, and by evaluating Weaver's method from two perspectives: 1) what percentage of individuals can be correctly sexed using Weaver's criteria? and 2) what is the probability that an individual case will be correctly sexed based on the presence or absence of auricular surface elevation? The first is of interest to those reconstructing population patterns, while the second is critical to the forensic investigator faced with the diagnosis of an individual case. The sample used in this study consisted of 58 ilia from subadults of known sex ranging in age from birth through 18. In each case, sufficient soft tissues were present to allow absolute sex diagnosis. Each ilium was subjected to a blind examination using Weaver's criteria for auricular surface elevation. Weaver's technique proved most effective on the males in our sample, with an overall accuracy of 85.3%; however, accuracy in sexing females was only slightly better than chance at 58.3%. Our results corresponded closely to Weaver's own values of 85.4 and 57.7% respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Transverse plane pelvic rotation measurement.

AIM: To determine how pelvic rotation in the transverse plane relates to coronal plane anatomical landmark location. The problem this addresses is the observation that iliac crest medial lateral width as seen in the coronal plane Cobb is often asymmetrical in patients with idiopathic scoliosis before treatment, and sometimes after treatment. MATERIALS AND METHODS: A pelvis was marked with radiographically opaque markers at symmetrical sites in the ilium, sacrum and acetabulum; mounted in a jig; and radiographed at varying degrees of transverse and sagittal plane angulation. The medial lateral distance from two similar landmarks on opposite sides of the pelvis was then determined and a left/right ratio correlated with the degrees of transverse plane angulation. In addition, a theoretically derived equation was developed to describe the effect of pelvic shape on the degree of transverse plane rotation. RESULTS: The left/right ratio for the distance from the sacro iliac joint to the anterior superior iliac spine proved to be the most reliable for determining the degree of pelvic rotation in the transverse plane. The relationship is nearly linear up to 25 rotation and is little influenced by the degree of sagittal plane angulation. Based on theoretical considerations, the importance of the anterior to posterior angular relationship of the two coronal plane landmarks measured influences the degree of rotation but not the linear relationship. CONCLUSION: Within limits the transverse plane rotation of the pelvis can be determined by a left/right ratio of the distances between two similar landmarks on each side of the pelvis.

[Treatment of developmental dislocation of the hip with improved Pemberton method].

OBJECTIVE: To find a more perfect method of treating developmental dislocation of the hip(DDH). METHODS: From March 1994 to December 2002, on the basis of the ordinary operative method, Pemberton method was improved for 48 cases of DDH 49 articulatio coxae which had superficial and small acetabula but big heads of femur. Osteotomy points were moved higher to enlarge area of bone flap turned over. A piece of full thickness ilium including periosteum was removed as a free bony graft. Capsula articularis growing thicker was made into 2 layers, of which the inner layer was used to tighten articularis and the outer layer was used to stabilize bone flap. RESULTS: All patients were followed up for 18 months to 5 years, the function of articulatio coxae and degree of bony union, inclusion area to head of femur and ischemic necrosis of femoral head were observed through X-ray. According to Zhou Yongde's criteria for the curative effect of congenital dislocation of the hip, there were 18 cases of 26-30 scores(3 hips), 27 cases of 21-25 scores, 2 cases of 16-20 scores(3 hips), and 1 case under 15 scores, and the total superior rate was 93.8%. CONCLUSION: Improved Pemberton method can enlarge inclusion area to head of femur, avoid decreasing acetabular volume, and stabilize articulatio well, which made operative indication bigger than original Pemberton method.

Acetabular augmentation at six- to 30-year follow-up. A biochemical and histological analysis.

In developmental dysplasia of the hip, a deficient acetabulum may be augmented by placing local autogenous iliac osseous graft, or the ilium itself, over the head of the femur with the expectation that the added bone will function as a bearing surface. We analysed this bone obtained en bloc during subsequent surgery which was performed for degenerative osteoarthritis in three patients at 6, 25 and 30 years after the initial augmentation procedure. In each patient, the augmentation comprised of red cancellous bone covered on its articulating surface by a distinct layer of white tissue. Microscopy of this tissue showed parallel rows of spindle-shaped cells lying between linearly arranged collagen bundles typical of joint capsule. Biochemical analysis showed type I collagen, the principal collagen of joint capsule and bone, with no significant quantity of type II collagen, the principal collagen of cartilage. While the added bone produced by acetabular augmentation was durable, histological and biochemical analyses suggested that it had not undergone cartilage metaplasia. The augmented acetabulum articulates with the head of the femur by means of an interposed hip joint capsule.

Extracorporeal bypass preserved composite anterior thigh free flap (periosteo-musculo-fascio-cutaneous) for hemipelvectomy reconstruction: utilizing the periosteal component for abdominal wall fascial reconstruction.

We present a case of a right external hemipelvectomy for chondroblastic osteosarcoma originating from the right ilium and reconstruction with salvage parts; cardiopulmonary bypass preserved anterior thigh free flap. The resection required sacrifice of the right common iliac artery and vein and the entire right gluteus maximus muscle. The first stage of the procedure was a high above-knee amputation on the ipsilateral leg. The amputated extremity was placed on extracorporeal bypass utilizing the common femoral artery and vein. This technique, previously unreported, supported the successful utilization of the anterior thigh free flap for both soft tissue and abdominal wall facial reconstruction.

Unicameral bone cysts of the pelvis: a study of 16 cases.

Unicameral bone cysts of the pelvis are extremely rare. This study summarizes the clinical, radiologic and pathologic features of 16 cases. Patients ranged in age from nine to 69. Most lesions were in the anterior portion of the iliac wing; many appeared to be related to an open iliac crest apophysis. This suggests that the pathogenesis of unicameral bone cysts in this portion of the ilium is similar to that seen in the proximal humerus and the proximal femur. The correct diagnosis was made preoperatively in only five cases. This indicates that, although they are well documented, unicameral bone cysts of the pelvis remain a diagnostic problem. Patients received a spectrum of treatments from curettage to observation. There appeared to be no difference in the outcome after any form of treatment. Therefore, unicameral bone cysts of the pelvis can be managed conservatively. The choice to manage patients conservatively depends on making the correct diagnosis based on clinical history and imaging. The most effective imaging is a combination of plain radiographs, computed tomography (CT) and magnetic resonance imaging (MRI).

[Reconstruction of calcaneal thalamus by bone autograft and subtalar arthrodesis for antiquated intra-articular calcaneal fractures].

OBJECTIVE: To introduce a clinical treatment for the reconstruction of calcaneal thalamus by bone autograft and subtalar arthrodesis for antiquated intra-articular calcaneal fractures. METHODS: From July 2000 to October 2003, 11 cases with antiquated intra-articular calcaneal fractures were treated. Of the 11 cases, 9 were males and 2 females, whose ages ranged from 21 to 48 years. All unilateral calcanei were involved in the 11 cases. The modified extended L-shaped approach lateral to calcaneus was adopted. The primary treatment was manipulation and immobilization with the reconstruction of calcaneal thalamus by bone autograft and subtalar arthrodesis. The average volume of the ilium for the autograft was 3.0 cm x 2.5 cm x 1.8 cm. Protruded osteophyte from the lateral wall of calcaneus were ablated in all 11 cases. RESULTS: All the 11 cases were followed up for 3 to 18 months, 11.5 months on average. The reconstructed calcaneal thalamus was healed 10 to 12 weeks after the operation. Some of the patients could walk with load 8 weeks after the operation, and the average time for all the patients to walk with load was 13.2 weeks. There were 4 cases of excellent result, 5 cases of good result, and 2 cases of fair result, according to ZHANG Tieliang's foot score system. Plain radiograph showed that Böhler angle, width of calcaneus and height of calcaneal thalamus were nearly restored to normal. CONCLUSION: With the improvement of calcaneal abnormality and restoration of the shape and function of hind foot, reconstruction of calcaneal thalamus by bone autograft combined with subtalar arthrodesis is effective in treating antiquated intra-articular calcaneal fractures.

[The anatomic and radiologic picture of the acetabulum (author's transl)].

The basic shape of the facies lunata shows at the inner margin of each of the three participating bones a dell into which the floor of the acetabulum progresses. These dells originate in nutritive vessels which enter the bone and claim this place. There always are groups of vessels and wide channels which may be visible on radiographs. The so far unexplained picture of a vascular canal in the roof of the acetabulum corresponds to the canal leading into the body of the ilium. Due to its position there is often restructuring leading to filling or covering of the canal. The entrance to this canal is shown as a gap in the arc of the acetabular cavity which would have to be added to that of the "tear figure" known so far. The closed arc, as always drawn, reults only when this entry has been filled up.

Osteomyelitis of the pelvis due to Fusobacterium nucleatum.

Anaerobic pelvic osteomyelitis occurs rarely in children. A case of Fusobacterium nucleatum osteomyelitis of the right ilium [correction of ileum] in a child without apparent risk factors is described. This case emphasizes the value of diagnostic aspirates. Therapy with a combination of intravenous and oral clindamycin led to successful resolution of the infection.

Roentgenographic assessment of skeletal ages of Asian junior youth football players.

Skeletal age of 50 football players participating in the Asian Junior Youth Tournament was assessed in an attempt to verify their stated chronological age to have a fair competition. Roentgenograms of hand and wrist (Greulich - Pyle standards), ilium and ischium were used for evaluation. More than half of the players had a bone age over the 16 year age limit of the Tournament and 30 percent of them had reached the maximum skeletal maturity of 19 years of age or over. Skeletal age is a reliable and dependable guide for assessing the physical maturity in this age group provided that proper local or international standards are set up.

A test of the auricular surface aging technique.

A recently developed technique used in the estimation of age at death is based upon the metamorphosis of the auricular surface of the ilium. This technique was designed to be used in seriation to estimate the age distribution of a skeletal sample. However, the technique has also been used in forensic science cases, which must be analyzed on a case-by-case basis. The present study examines the use of the auricular surface technique as the single aging factor. Two hypotheses are tested using a sample of 189 individuals drawn from the Terry Collection, housed at the Smithsonian Institution. The hypotheses are (1) is the method equally applicable across race and sex, and (2) how well does the method perform as the single aging factor? The results indicate that the amount of degenerative change in the auricular surface is not dependent upon race or sex in any given age category. However, the rate of degenerative change is too variable to be used as a single criterion for the estimation of age; the range of estimation error is simply too large for forensic science purposes.

[A hist-morphometrical study of undecalcified sections of bone obtained in patients with cervical disorders].

Histomorphometrical parameters were used to investigate bone metabolism in patients with symptomatic cervical disorders, especially those patients with ossification of the posterior longitudinal ligament (OPLL) of the cervical spine. There were 19 patients in this study. The patients were divided into two groups depending on the presence of OPLL. Six patients had OPLL (OPLL group), 13 patients did not (non-OPLL group). All patients received pre-operative labelling with tetracycline. There was a positive correlation between vertebral body and iliac bone in the parameters of trabecular osteoid specific volume, relative osteoid volume and relative formation surface. The vertebra/ilium ratio of the mineral apposition rate, which suggests bone formation in vertebral body relatively, was higher in OPLL group significantly. The present study concludes that OPLL can be considered a condition of hyperostosis though, bone formation ability in the vertebral body of OPLL is higher than in the ilium.
