[How I prevent erysipelas and its consequences and recurrences].

Erysipelas is a serious infection of the skin. In case of delay in initiating adequate antibiotic treatment, complications, sometimes dismal, can supervene. In addition, erysipelas shows a tendancy to recurrences. The prevention of an episode of erysipelas calls for correct personal hygiene and adequate use of topical antiseptics in case of skin effraction, even when minimal. When erysipelas is established, a rapidly initiated antibiotic treatment for a prolonged period prevents streptococcal gangrene complications. Elastic contention of any leg edema from venous or lymphatic origin and prophylactic antisepsis of discrete wounds help in preventing erysipelas recurrences.

Clonal spread of Acinetobacter baumannii in a general intensive care unit.

The epidemiological characterization of multiply resistant Acinetobacter baumannii isolates from a six-bed Intensive Care Unit (ICU) is described. Investigations for A. baumannii were performed in three subsequent surveillance studies. In the first study, surveillance cultures were taken from patients, health care personnel and the environment; in the second study surveillance cultures were taken at 0, 4, and 7 days from all patients admitted consecutively to the ward; and in the third study surveillance cultures were taken from patients, health care personnel and the environment. During the first study all four hospitalized patients were found to harbour A. baumannii. Hand cultures did not grow any A. baumannii when staff entered the ward from home, but 7 positive health care workers were identified out of 25 samples taken during work, and two cultures of environmental specimens grew A. baumannii. During the second study, 4 of 86 (4.6%) patients resulted colonized with A. baumannii. In the third epidemiological study, no A. baumannii was cultured from either patients, health care personnel or the environment. All isolates recovered from various patients or sources produced conserved macrorestriction Pulsed-Field Gel Electrophoresis (PFGE) patterns and showed the same antibiotic resistance; therefore, they can be considered indistinguishable. The same antibiotic resistance and macrorestriction patterns were observed in previously isolated A. baumannii strains in the ward during May 1997, suggesting the persistence of a single A. baumannii in the ICU. The present study confirms that molecular typing is an essential tool in the epidemiology and control of nosocomial infections, showing here the persistence of a single A. baumannii clone in the ICU. The origin of this strain remains unknown but, when basic infection control measures were reinforced, emphasizing the importance of hand antisepsis and judicious use of gloves, control of A. baumannii spread in the ward was achieved.

[SARS: diagnosis, therapy, and especially prevention].

The main purpose of this review is to analyze some aspects of the severe acute respiratory syndrome, SARS, in order to obtain useful data to suggest preventive actions to reduce the spreading of the disease. Many elements have been examined to reach some conclusions and to allow an updated discussion. Surgical masks protect more the patient than the caregiver. Simple or double surgical masks may be useful, as double gloving protects the hands of the surgical personnel against percutaneous transmission of HIV eventually present in contaminated blood. The frequent substitution of the external masks with a new one will improve the filtering activity against droplets produced by cough or sneezes of the patient. The use of respiratory masks may be suggested in hospitals or in restricted ventilated areas where, even if coronavirus variant is considered an environmental contaminant more than a respiratory risk, droplets nuclei may persist in the air and add consistent dangers to the heath-care givers. Considering that large and medium droplets may infect floors and surfaces, in addition to gloves, gowns, masks and eyes protection, the available list of viral and bacterial factors implicated in SARS ethiology suggests a better hand antisepsis using frequently the alcohol based gels (containing an high percentage of emollients substances), if available. A liquid soap with triclosan can also be used, if the health-care workers compliance to hand washing increases, as expected in this explosive situation. On the basis of the results of some experimental data, the environmental disinfection may be effected with ethyl alcohol 70% in water. Disinfection of floors or larger surfaces may be obtained with chlorine compounds solutions, after an accurate pre-cleaning. When corrosion, bleaching or gas production have to be avoided, chlorine compounds may be substituted by phenolic detergent disinfectants.

[Febrile neutropenia in children who have leukemia].

INTRODUCTION: Neutropenia febriles are the most frequent complications during the hospital treatment of children who are ill of malignant. It is urgent state end each of feverish should be treated as infection, until it is proved as opposite. It is necessary immediately to begin with antibiotical therapy according to the algoritham for therapy of neutropenia febriles. Neutropenia febriles is caused by virus, bacteria and fungus infection. RESULTS: It were 83 leukemia treated children on Hematooncology department of pediatrical Clinic in Sarajevo in period from 01.01.1997 until 31.12.2002. Neutropenia febriles were seen at 41 of children (49.4%). Between all of neutropenia febriles the causes were isolated at 27 children (65%). The most of them had sepsis, and at 40% of children had positive hemo-cultures. In the hemo-cultures the gr. positive causes were isolated at the 55% of children, gr. negative causes were isolated at 25% of children, and fungus were isolated at 20%. Between the other cultures urino-cultures were positive at 18.5% of children. RTG pulmo was isolated at 18.5%, smear pharyngs was isolated at 11%. CONCLUSION: In such state of patient it should be thought about the possibility of contamination of patient and protect him/her from superinfection, by him/her in isolation box and staff should take care about sepsis and antisepsis.

[The private society 'Utrechtsch Geneeskundig Gezelschap Matthias van Geuns' as a source of history and inspiration].

The Utrechtsch Geneeskundig Gezelschap [Utrecht Medical Society] Matthias van Geuns (1793 to the present) is the oldest still active local medical society in the Netherlands. Membership is select and limited to only one local representative from every medical specialty. Proceedings of the medical meetings have never been published, neither was a prize offered for a best essay. The minutes of the meetings, however, have been preserved. They give a fascinating insight into the way this small society reacted to the developments in medicine during two centuries. Infectious diseases, for example cholera, abdominal typhus and tuberculosis, and newly introduced surgical procedures like anaesthesia and antisepsis or asepsis were discussed. The lectures had to be attended by all members and were meant to be aimed at a general medical audience. The society members were expected to prepare themselves and take part in the discussion afterwards, everyone from his own point of view. This procedure was both instructive and stimulating. The minutes of the Utrechtsch Geneeskundig Gezelschap Matthias van Geuns are a valuable source for scientific research on medical developments in the Netherlands, particularly in Utrecht.

[Advances on biological activities of coumarins].

This paper reviewed the advances on effective constituents and biological activities of coumarins in recent ten years. Coumarins are a group of important natural compounds, and have been found to have multi-biological activities such as anti-HIV, anti-tumor, anti-hypertension, anti-arrhythmia, anti-osteoporosis, assuaging pain, preventing asthma and antisepsis. Therefore, further investigation should emphasize on improving techniques for extraction and separation, searching the effective precursory compound, and synthesizing and screening out courmarin derivatives with high activity and low toxicity.

[Modification of hybrid antimicrobial peptide CecA-mil gene and its over-secretion expression in Pichia pastoris].

According to the partiality codon of Pichia pastoris, hybrid antimicrobial peptide CecA-mil gene was reconstructed, synthesized and cloned into pPICZalpha-A to construct the recombinant expression vector pPICZa-A-CM. The pPICZalpha-A-CM was transformed into yeast host strain X-33. Under the control of the promoter AOX1 (alcohol oxidase 1), a approximately 1.9 kD cecA-mil protein was expressed with the high level of 245 microg/mL after optimized the requirements for the flask-shaking culture fermentation of the Pichia pastoris rX-33/pPICZalpha-A-CM. The hybrid antibacterial peptide had a broad spectrum antibacterial activity on both gram-positive and gram-negative bacteria, especially showed potent antibacterial activity against ampicillin-resistant and kanamycin- resistant bacteria, such as Staphylococcus aureus (cowan I) and pathogenic E. coli (O1) from chicken. In addition, the hybrid antibacterial peptide showed an extreme heat-stable and acid-stable characteristic. Based on these characteristics, the recombinant antibacterial peptide CecA-mil display application foreground in the field of antisepsis of food, prevention of disease, additives of animal feedstuff and so on.

[Surgical site infections: risk factors and methods of prevention.].

Surgical site infections are the most important problems of surgery. They increase morbidity, mortality, hospital stay and hospital costs. Infection occurring within 30 days after the operation if no implant is left in place or within 1 year if implant is in place is described as surgical site infection. These infections are classified into two categories; incisional or organ/space specific infections. Incisional infections are further divided as superficial or deep incisional infections. Development of surgical site infections is mainly dependent on type of micro-organism and patients' factors related to host resistance. The endogenous flora of the patients' skin, mucous membranes, or hollow viscera is the main source of microorganisms responsible for surgical site infections. Development of such infections can be influenced via a lot of risk factors such as nutritional status, diabetes, smoking, altered immune response, and length of preoperative stay, skin antisepsis, preoperative shaving, duration of operation, antimicrobial prophylaxis, operating room ventilation, inadequate sterilization, the presence of foreign material in the surgical site, and surgical techniques. With the improvement of some of these factors, risk of developing surgical site infections may be decreased. The aim of this study was to discuss the risk factors and prevention measures of surgical site infections in the lightening of current literature.

Safety of neonatal skin cleansing in rural Nepal.

OBJECTIVE: A high proportion of deaths during the neonatal period are attributed to infections. Neonatal skin plays an important role in protecting the newborn from invasive pathogens. In preparation for a study of newborn skin cleansing with chlorhexidine in Nepal, we evaluated the feasibility, acceptability, and safety of the newborn cleansing procedure. STUDY DESIGN/SETTING: Observational pilot study of full-body cleansing of newborns in rural Nepal. METHODS: Thirty two newborn infants were wiped with commercially available non-antiseptic baby wipes. Pre and post-procedure axillary temperatures were recorded to estimate the impact of cleansing on body temperature. Skin aggravation, residual moisture, removal of vernix, and maternal satisfaction were assessed qualitatively. RESULTS: Body temperature of newborns decreased an average of 0.40 C (95% CI: 0.31 to 0.49 C, p < 0.0001) during the procedure. There was no evidence of skin aggravation, injury or removal of vernix, and mothers expressed satisfaction with the procedure. The procedure was simple and project workers were easily trained. CONCLUSION: Care must be taken to promptly wrap infants after skin cleansing procedures as slight temperature decrease was noted after the procedure. These pilot data indicate, however, that gentle cleansing of newborn skin poses minimal risk to infants. This procedure is safe and appropriate precautions can be taken to deliver safe skin antisepsis with chlorhexidine to infants in the community.

Outlined history of the development of the world and Polish cardiac surgery.

It was the dream of humanity to perform surgery on an open non-beating heart. Scientific and medical discoveries five thousand years ago in China, partially adopted by the Western civilization, laid, through ancient Egypt, Mesopotamia, Greece, Rome and, later on in the Renaissance, the foundations for the development of empirical medicine. The 19th and the 20th centuries shoved dynamic scientific and technical development in various fields including medicine and surgery whose importance grew with the necessity to help the patients wounded in the wars. A break-through event in the development of surgery was overcoming of pain and discovery of reasons of infections and the control thereof, and, in the case of cardiology and cardiac surgery, the discoveries in physiology of circulation and the diagnostics of cardiovascular system diseases. This review contains a brief description of medical science in the past centuries, emphasizing the most important discoveries. A focus has been placed on the contribution of general surgery and thoracic surgery to the development of Polish and World cardiac surgery. The I Congress of the Polish Surgeons was held in 1889 in the Austria occupied territory of Cracow, which celebrated its one hundredth anniversary. The main obstacles in the development of clinical cardiac surgery included intratracheal general anesthesia, antisepsis and aseptics, hypothermia, oxygenators, extracorporeal circulation, transfusions, blood clotting and thromboses and cardioplegia. The spectacular heart and aorta surgical operations performed for the first time in the world and in Poland as well as the names of cardiac surgeons employed by the important cardiac surgery centers in Poland have been mentioned. The Department of Heart, Vascular and Transplantology Surgery of Cracow, the role and the share of Fundacja Rozwoju Kardiochirurgii COR AEGRUM in Cracow (COR AEGRUM Foundation for the Development of Cardiac Surgery in Cracow) in the construction of the new facilities for the Department of Cardiac Surgery of Cracow consecrated on June 9, 1997 by pope John Paul II, have been discussed. The contribution of the Club of Polish Cardiac Surgeons to the integration of surgical community and to development of the Polish cardiac surgery has been emphasized. In summary, it has been outlined that the contemporary standards of the Polish cardiac surgery do not differ from cardiac and vascular surgery and transplantology in developed countries.

Investigation of the influencing factors on severe acute respiratory syndrome among health care workers.

OBJECTIVE: To investigate the protective factors and risk factors of nosocomial infection of severe acute respiratory syndrome (SARS) among health care workers (HCWs), and thus provide the scientific basis for prevention and control of nosocomial infection. METHODS: With the case-control study, a standardized questionnaire was used for data collection in three general hospitals where nosocomial infection had occurred. Univariate analysis was done at first. All concerned factors about SARS infection were scanned by using Chi-square test and Fisher's exact test one by one, and determined as to whether they were risk factors or protective factors according to odd ratio (OR) score. Then, multivariate unconditional logistic regression analysis was used to re-analyze the picked-out factors for finding out which factors played independent roles. RESULTS: Twenty-two factors (nineteen protective factors and three risk factors), among the total fifty-six factors, were significantly associated with SARS infection. Multivariate unconditional logistic regression revealed that factors such as double exposure suits (OR=0.053), education (OR=0.072), gloves (OR=0.102), hands sterilized by iodine (OR=0.231), room air ventilation (OR=0.32), were significantly protective; conversely, tracheal intubation (OR=30.793) was a significant risk factor. CONCLUSION: Strict defense and antisepsis measures were pivotal in preventing SARS infection among high-risk medical personnel. Education about associated knowledge and effective air ventilation were also important factors.

[PDT photodynamic therapy in orthopedic inflammatory conditions?].

The infections in orthopedic surgery and traumatology represents important medical problem. The results of treatment of motor-organs infections in high degree were improved by the introduction of antisepsis and asepsis in XIX century and the antibiotics' discovery in the beginning of XX century. However, widespread usage of antibiotics leaded to the rise of refractory tribes of bacteria on their activity, which caused higher percentage of fails in the therapy. Also the rapid civilization development, which flown on arising of a new invasive methods of operative trauma protection and stabilizations of fractures in motor-organs area increased the number of infectious complications during treatment. These facts gave a reason to look for a more effective therapeutic methods. It seems that photodynamic therapy gives us the new possibilities of infectious treatment, which avails oneself activity of therapeutic light laser with proper wave length on the human tissue with photosensitizer included. Indeed there are known until now splendid results of PDT in neoplasma treatment, however it seems, based on experimental investigations and publications in medical literature, that this method can be a chance of effectual and non-invasive treatment of the soft tissues and bones inflammations.

[Fetal urinary dilatation due to vesico-ureteral reflux].

We have retrospectively studied 163 newborns referred to us between 1986 and 1990 for sonographic diagnosis of intra-uterine upper urinary tract enlargement. All were studied by paediatric radiologists on days 5th or 6th after birth by means of sonography and voiding cystourethrogram (VCU). Fifty percent had also IVP. No urinary tract disease could be found in 44 children, obstructive uropathy or cystic disease were diagnosed in 58 children and 61 patients had vesico-ureteric-reflux (VUR). They form the basis of the present report. There was a male-to-female ratio of 3.3 to 1. Neonatal ultrasonography was normal in 11 cases, 42 had pyelectasia (bilateral in eight cases) and eight had ureterohydronephrosis (bilateral in seven). VUR was bilateral in 28 cases, right in 16 and left in 17. Out of the 89 refluxing units, VUR grade was I in 11, II in 16, III in 24, IV in 14 and V in 23. There was intra-renal reflux in 17 units. Some other form of urinary tract disease was detected in 29 babies (47.5 per 100). Twenty-eight patients were operated upon and the remaining are under long-term urinary antisepsis with only five episodes of infection. The considerable proportion of false negatives for VUR in neonatal ultrasonography (30 per 100) and the high grade of some of these cases lead us to strongly advise routine VCU for patients with intra-uterine urinary tract dilatation.

[The prevention of suppuration of postoperative wounds].

A method to prevent operative wound suppuration is offered. An experimental animal model of operative wound suppuration was elaborated to specify rational antibiotic-antiseptic prophylaxis and adequate wound drainage. This yielded a complex of measures to prevent purulent complications on the part of the operative wound. In addition, an original turbidimetric technique to assess the resistance of the biological fluid microflora to antisepsis was elaborated. The complex prophylaxis against suppuration of infected wounds was clinically tested on 124 patients operated on for different acute surgical conditions of abdominal organs. The wound process was controlled by means of an original endoscopic-bacteriologic technique. The postoperative period was complicated by the wound suppuration in two of the 124 patients (1.6 per cent). The postoperative hospital stay in most cases lasted 5-7 days.

[In vitro activity of an antiseptic, chlorquinaldol, against Neisseria gonorrhoeae and Chlamydia trachomatis].

The activity of chlorquinaldol, a derivative of hydroxy-8-quinolein used for local antisepsy, was studied against Neisseria gonorrhoeae and Chlamydia trachomatis. The weak solubility of the product and the special growth conditions of the organisms made an adaptation of the AFNOR norm necessary. For 0.1 to 0.2% (W/V) chlorquinaldol concentrations, a reduction of about 10(4) organisms was obtained after 60 minutes for N. gonorrhoeae and C. trachomatis. However, for technical problems, the concentrations tested were 10 to 100 times lower than the doses usually recommended for this antiseptic.

[Preliminary study of the antimicrobial activity of traditional plants against E. coli].

The microbiological quality of drinking water is a major public health priority in developing countries. As an alternative proposal to chemical treatment of water, we have studied the antibacterial effectiveness against E. coli of plants traditionally used for antisepsis. Traditional plants were freshly sampled from Burkina Faso and Vietnam. Two of them, Tamarinier and Derris elliptica respectively, were selected for the study. Various part of the plants, i.e. seeds, cloves and fruits were prepared in both decoction and maceration. Their antibacterial activity was assessed using standardized preparations of E. coli (Pasteur 792) in water (10(5), 10(6) and 10(7) germs/ml). Some preparations of Tamarinier showed encouraging results, whereas Derris elliptica was uneffective. Solutions of Tamarinier seeds produced a 4.5 log decrease of the germ concentration in standardized 10(7) germs/ml preparation. Decoction and maceration are still effective 24 h after a 10(4) bacteria/ml overloading. On the other hand, acid pH of clove and fruit solutions explains the slight effectiveness (1 log UFC/g of dry extract/48 h) of these solutions. Though the results are not meeting with the AFNOR requirements for disinfection, this preliminary study demonstrates a bacteriostatic effect of traditional plants against E. coli in water. Further study is needed to valid this property in the field.

[Cytologic diagnosis of abdominal lesions with fine needle aspiration guided by ultrasound].

The purpose of this work was to analyse the advantages and disadvantages of puncture-aspiration with fine needle, guided by ultrasonography, trying to determine its usefulness in our hospitals and its reliability in the diagnosis of intraabdominal lesions of different locations. 29 punctures were practiced on 19 patients, 9 women and 10 men of ages comprised between 34 and 94 years, with lesions in different organs of the abdominal cavity diagnosed by ultrasonography with real time equipment and lineal 3.5 and 5 MHz transducers. After cleaning and antisepsis a Chiba needle is introduced under ultrasonographic vision, up to the location of the lesion, the guide is removed and under a negative pressure, the sample is taken, which is later dried into the air and coloured using the May-Grünwald-Giemsa Technique. Only in one case it was not possible to obtain adequate material for the cytological study. There were 11 positive cases for malignity and 7 negative, one of which was a false negative. The sensibility of the method was of 91.6% with a specificity of 100% and a reliability of 89.4%. We definitely believe that the method is practical, very easy to carry out in our hospitals with a minimum amount of risk, and most of all, dependable to clarify certain diagnoses.

Bacterial transference during electrodesiccation and electrocoagulation.

Electrodesiccation and electrocoagulation are commonly used to control bleeding and destroy tissue. In certain outpatient settings, the clinician routinely uses one of these electrosurgical modalities on successive patients without sterilization or antisepsis of the treatment electrode tip. In controlled laboratory experiments using electrodesiccation and electrocoagulation, we investigated bacterial transference of Staphylococcus aureus from inoculated tissue to sterile electrode tips and from inoculated electrode tips to sterile tissue. With use on inoculated tissue, sterile electrode tips remained sterile after electrocoagulation but not after electrodesiccation. Bacterial transference from inoculated electrode tips to sterile tissue occurred with electrodesiccation but not with electrocoagulation. These results are consistent with bacterial destruction by electric current and suggest that bacterial transference via the treatment electrosurgery electrode from one patient to another is possible but much more probable during electrodesiccation than during electrocoagulation.
