Thoracic massage permits use of echocardiography in unanesthetized rats.

PURPOSE: The objective of the study reported here was to investigate whether massage-like stroking of the thorax and cranial portion of the abdomen might relax unanesthetized rats sufficiently to permit in vivo echocardiography. METHODS: Nine-month-old spontaneously hypertensive rats (SHR) were first conditioned to being held by hand for 10 to 15 min twice a day for seven to 10 days. During each session, the animal was placed in supine position, and the thorax and cranial abdominal area were gently stroked (approx. 5 cm/s, 12 to 14 times/min). After the conditioning period, echocardiography was initiated. We obtained serial transthoracic two-dimensional (2-D) and M-mode echocardiograms from nine-month-old SHR that were treated with isoproterenol (60 mg/kg of body weight, s.c., x 1, followed by 30 mg/kg/d x3), and from old (20 to 24 months old) SHR, studied when labored breathing, suggestive of heart failure, was evident (SHR-F). Measurements included end-diastolic volume (EDV) and end-systolic volume (ESV). RESULTS: In the isoproterenol-treated SHR, mean +/- SD echocardiographically derived EDV (2-D, 0.29 +/- 0.05; M-mode, 0.28 +/- 0.01 ml) was not significantly different from volume at necropsy (0.33 +/- 0.04 ml). Measurements of EDV and ESV by use of M-mode and 2-D echocardiography were significantly correlated (EDV R2 = 0.48, P = 0.05; ESV R2 = 0.39, P = 0.02). Echocardiography revealed pleuropericardial effusions (4/6), atrial thrombi (5/6), and left and right ventricular enlargement (6/6). The EDV and ESV were increased fivefold (P < 0.01) and threefold (P < 0.05), respectively, versus values for SHR not in heart failure (SHR-NF). Left ventricular ejection fraction of hearts from SHR-F was markedly decreased, compared with that in SHR-NF (44 +/- 7 versus 74 +/- 2%, respectively; P < 0.05). The presence or absence of left atrial thrombi and fluid in the thoracic cavity was confirmed at necropsy in SHR-F and SHR-NF. CONCLUSION: Thoracic massage permits use of echocardiography in unanesthetized rats, thereby providing a simple, non-invasive technique for assessment of cardiac structure and function in rats without the potentially adverse effects of anesthesia.

[Effect of oxygen baths and massage on hemodynamics in patients with neurocirculatory dystonia].

56 patients with neurocirculatory asthenia (NCA) of a hypertensive and cardial type aged 30 to 55 years took general oxygen baths and collar zone massage, no drugs were used. The baths and the massage improved the disease course in 92.6% patients: improved hemodynamics, relieved high vascular resistance, lowered high diastolic blood pressure, decreased heart rate in tachycardia. More effective was the treatment in hypertensive NCA.

[Treatment and mechanism study of electromagnetic stimulation and vibrational massage for patients with ejaculatory incompetence].

Studies on the effective methods and mechanism for the treatment of patients with functional ejaculatory incompetence were pursued on the basis of clinical practice. 90 patients with functional ejaculatory incompetence were treated by giving the electromagnetic stimulation to their pudendal nerve and the vibrational massage to their penis. Then the excitation conductive time and velocity of their pudendal nerve were simultaneously determined by the electro-neurophysiology technique. Of the 90 patients with functional ejaculatory incompetence, 69(76.7%) obtained cured effects, 19(21.1%) notable effects and 2(2.2%) improved effects. The total effective rate was 100%. The excitation conductive time and velocity of their pudendal nerve were both significantly different from those before treatment (P < 0.05). These suggest that it is practical to reach the ejaculatory threshold, trigger the ejaculatory reflex and complete the process of ejaculation because the nerve with improved excitational conductibility may release more acetylcholine from is endings after the electromagnetic stimulation to nerve and the vibrational massage to penis.

[A case of bilateral chylothorax following neck massage].

We report a case of bilateral chylothorax following neck massage. The patient presented with the left supraclavicular mass and bilateral pleural effusions. Both fluids obtained from thoracentesis revealed chylous and a high triglycerides content. The effusions decreased after conservative treatment. Bilateral chylothorax caused by neck massage has, to our knowledge, not previously been reported in literature. We concluded that physicians and massagists should remember this serious complication.

[Tui-Na, an oriental massage].

Tui-Na is an oriental massage whose principles are based on traditional Chinese medicine. This medicine conceives a person as one entire entity giving as much importance to emotional and physical aspects as to everything that surrounds a person, including climate, social relationships, diet. This philosophy believes that an individual will achieve his/her ideal health state when he/she has found interior harmony and is in balance with all his/her environment. From this viewpoint, this philosophy understands pathology as an unbalance or a lack of harmony in an organism. Tui-Na massage is one of the techniques which make use of traditional Chinese medicine to maintain, or to recuperate, an organism's balance.

[Attempt of preventive treatment of striae gravidarum using preventive massage ointment administration].

Striae distensae are an appreciable cosmetic problem for many pregnant women. Preventive application of a water/oil massage cream was tested in a group of 24 gravidae (control group: 26 patients). In the untreated control group striae distensae were observed in two-thirds of the patients, whereas the prophylactically-treated gravidae showed development of striae in only one third of the group given the test preparation. Better results were obtained in women with a favourable constitutional predisposition than in patients with a tendency to overweight. The massage cream was well tolerated by all gravidae.

[Massage of transrectal heat rotating magnetic field in the treatment of obstinate hemospermia].

OBJECTIVE: To investigate the effect of transrectal heat rotating magnetic field in the treatment of obstinate hemospermia. METHODS: Sixty-eight cases of obstinate hemospermia were selected and treated by routine ways and massage by transrectal heat rotating magnetic field. The device of transrectal heat rotating magnetic field was used, 45 min/section/day, 10 sections as one sequence. The morphology of prostate and seminal vesicle by ultrasonic and red blood cells in sperm were observed and compared before and after the treatment, and then all the cases were followed. RESULTS: After one to four sequences, among 64 cases, which were caused by prostatitis and spermatocystitis, the cure rate was 81.25% (52 cases) and availability rate was 18.75% (12 cases). Sixty-two cases were followed for one to ten years, in which 6 cases intercurrently relapsed because of drinking, fatigue and frequent sexual intercourse, and then the symptom was controlled after restarting the treatment. Among 68 cases, 4 cases were verumontanum polyps and the symptom was disappeared after electrocautery. CONCLUSION: Routine treatment with massage by transrectal heat rotating magnetic field have remarkable therapeutic effects on obstinate hemospermia.

[Clinical trials of combined therapy of an oral Chinese medicine with massage for chronic nonbacterial prostatitis].

OBJECTIVE: To evaluate the clinical effect of an oral Chinese medicine combined with massage of the prostate in treating chronic nonbacterial prostatitis (CNP). METHODS: Seventy-two CNP patients were randomly divided into an experimental group (oral Chinese medicine with massage) and a control group (oral Chinese medicine only). The main parameters were compared before and two months after the treatment. RESULTS: Total efficacy rates of the experimental and control groups were 90.0% and 68.6%, respectively. Compared with the controls, patients in the experiment group had a lower score on NIH-CPSI, and the difference was significant statistically (P < 0.05). CONCLUSION: The combined therapy is more effective than oral Chinese medicine alone for the treatment of CNP.

[A case of Fournier's gangrene: was it triggered by prostatic massage?].

We have experienced a case of Fournier's gangrene which progressed rapidly after prostatic massage. The patient was a 70-year-old man who had poorly controlled diabetes mellitus, hemorrhoid, urethral stricture and benign prostatic hyperplasia. He visited an urologist complaining of pollakisuria and miction pain. Under the diagnosis of prostatitis, prostatic massage was performed. From that night, he developed a high grade fever. Simultaneously, redness, swelling and pain of the scrotum progressed rapidly, and 11 days later, he was admitted to our hospital. An X-ray examination revealed subcutaneous gas formation in the scrotum. Immediately, incision and drainage with extensive debridement of necrotic tissue were performed combined with chemotherapy using broad spectrum antibiotics and insulin therapy. About 3 months later, the gangrene and the wound were healed with granulation and scarring. Cultures of the pus and the necrotic tissue from the scrotum were positive for Bacteroides fragilis and several aerobes including Staphylococcus aureus, Pseudomonas aeruginosa, Klebsiella pneumoniae, Enterococcus and Staphylococcus epidermidis. The case proved to be non-clostridial gas gangrene.

Enhancing filtration in the early postoperative trabeculectomy refractory to digital massage.

Digital massage often can be used to lower intraocular pressure (IOP) after trabeculectomy or trabeculectomy combined with extracapsular cataract extraction and intraocular lens implantation. However, occasionally filtration, and thus IOP reduction, is refractory to massage. We present a pressure-lowering technique in which focal pressure is applied adjacent to the trabeculectomy flap firmly enough to indent the sclera and induce bleb formation.

[Traumatic complications of indirect cardiac massage].

The author analyzed injuries in 238 subjects where before death indirect cardiac massage was applied by skilled health workers. The ribs were fractures in 27.7% and the sternum in 16% of the deceased, injuries of other organs were detected 13 times (5.5%). In women injuries were more frequent and fractures of the sternum markedly more frequent than in men. While in men rib fractures on the left side were more frequent than on the rights side, in women this difference was not observed. Rib fractures were mostly in the medioclavicular line at the level of the 3rd-5th rib. The fractures of the sternum were also most frequently at the level between the 3rd and 5th rib. No fractures of the 10th-12th rib were found. The author observed an age-depunende cy of rib fractures in men and women, while fractures of the sternum were age-dependent only in men. There was no relationship with age as regards injuries of other organs. Only in two deceased subjects the injury which developed during indirect cardiac massage was the immediate cause of death. Once it was rupture of the interval mammary vein with haemorrhage into the mediastinum and haemorrhage into the chest, in the other case rupture of the right ventricle with cardiac tamponade.

[Maintenance of the circulation and the restoration of heart function in the donor body by means of direct mechanical heart massage for subsequent transplantation (an experimental study)].

The possibility of maintenance of circulation and subsequent restoration of the function of the heart in the organism of the donor, after 10-minute arrest of circulation, for transplantation of the heart was investigated. Study of the hemodynamic, biochemical, metabolic, and structural changes in the myocardium after 2-hour mechanical massage without and with pharmacological protection of the myocardium (glucose--insulin--potassium, hydrocortisone, dipyridamole), without and with restoration of cardiac activity in the organism of the donor and the recipient after transplantation of the second heart by the method suggested by Moory et al. in artificially created left ventricular insufficiency showed that direct mechanical massage of the heart may be used for its short-term preservation in the donor organism.

[Rupture of the liver secondary to external cardiac massage in a patient with coronary disease : recovery after partial hepatectomy and aortocoronary by-pass (author's transl)].

A 55-year-old man developed ventricular fibrillation during an attack of acute Prinzmetal-type angina, and was treated by external cardiac massage and a defibrillator. Hemorrhagic shock due to laceration of the left side of the liver developed 48 hours later. A liver lobectomy was performed. The postoperative course was uneventful enabling a coronarography examination to be followed by an aortocoronary by-pass, good results being still present after one year. Complications of cardiac massage, especially those involving the liver, are discussed, as well as the treatment of traumatic liver lesions.

[Cardiac massage in the early days of the technique. An instrument of dubious usefulness].

Authors expose a short historical profile of heart massage techniques purposed and realized in the second part of the 19th and beginning of the 20th century. Some methods were well known and diffused, and object of discussion, as demonstrated by Macaura's Blood Circulator, an instrument used for external heart massage, found in U.K., obtaining no practical results; but constructed and sold only for gain.

Iris incarceration associated with digital ocular massage.

Iris incarceration into the trabeculectomy site occurred in three patients following the use of digital ocular massage to maintain satisfactory aqueous humor filtration. The iris incarceration occurred within the first six weeks after surgery and the institution of ocular massage. There had been no complications at the time of surgery, and immediately postoperatively the iris was in normal position. The success of the trabeculectomies was compromised by the iris incarceration.

[Experimental studies on the efficacy of pressure wave massage (Lymphapress) in lymphedema].

The following effects of a massage by the Lymphapress apparatus could be demonstrated by girth measurements and nuclear medical investigations on the treated extremity: 1. Volume reduction (in lymphedema in average 4,6% of the basic extremity volume after 3 h). 2. Improvement of the lymph kinetics in cases with intact or decreased lymph transport. If isotopic lymphography failed to show any lymphatic transport as in cases with severe, indurated lymphedemas no effect on the lymph kinetics could be demonstrated. 3. Decrease of albumin content in the tissues. However there is an increase of local albumin concentration in lymphedema by overproportional water reduction. Therefore compression bandages should be applied between the Lymphapress massages for maintenance of the effect.

[Prevention of postoperative phlebothrombosis in the femoral vein by means of external pneumatic massage of the extremity].

The postoperative thrombo-embolism belongs to the most dangerous complications in the postoperative phase. The frequency of the postoperative phlebothrombosis increased in the last years because more risk patients were operated. External pneumatic massage of the extremity produces acceleration of the blood flow in the veins and a increase of the fibrinolytic activity of the venous vascular wall. A significant retrogression of the thrombus size was proved in animal experiments in dogs by application of external pneumatic massage after artefactitiously induced thrombosis in the femoral vein.
