The Social Security Administration's Death Master File: the completeness of death reporting at older ages.

We examine the completeness of death reporting in the Social Security Administration's Death Master File (DMF) through comparison with deaths by year and age group reported in official U.S. vital statistics. For most years since 1973, results suggest that the DMF includes 93 percent to 96 percent of deaths of individuals aged 65 or older. Although studies have shown that the National Center for Health Statistics' National Death Index provides superior coverage of deaths, for many researchers the DMF may be a desirable choice. Some advantages of the Death Master File are discussed.

[Follow-up, with Internet medical file, of patients who have undergone radical prostatectomy].

The development of Internet and the need for regular follow-up of patients often living a long way from hospital have led us to develop a follow-up dossier for patients with localized prostate cancer treated by laparoscopic radical prostatectomy. This feasibility study is based on 100 patients who agreed to test this system. After approval by the Commission Nationale d'Informatique et de Liberté (CNIL) (French Computers and Privacy Commission) ensuring medical secrecy and confidentiality of data, the website was opened on a server specifically devoted to this project and presenting all of the required computer securities. The website is composed of pages comprising the hospital discharge summary, and the operative and histology reports. A quality of life questionnaire based on assessment of urinary continence and sex life and a PSA assay form are also included. The patient is therefore able to enter his PSA level and complete the questionnaire at home and the results are then sent to the doctor who treated him. A contact page allows the patient and the doctor to exchange information by text. 92 of these 100 patients connected regularly to the site with a mean connection rate of 8 per patient (range: 1 to 22). 98% of patients were satisfied with the various sections of the site and 95% were satisfied with their medical file. 11% of patients encountered connection problems and 14% reported technical problems essentially attributed to incorrect PSA data entry or incorrect functioning of videos due to the absence of appropriate software. This type of Internet medical service for patients who have undergone a surgical operation requiring regular follow-up appears to be a useful approach for the future by allowing: maintenance of close contact between the patient and his doctors while avoiding the problems related to hospital visits, regardless of the patient's place of residence. It also provides general practitioners with access to the patient's file, with the patient's permission.

Automated discovery of patient-specific clinician information needs using clinical information system log files.

Knowledge about users and their information needs can contribute to better user interface design and organization of information in clinical information systems. This can lead to quicker access to desired information, which may facilitate the decision-making process. Qualitative methods such as interviews, observations and surveys have been commonly used to gain an understanding of clinician information needs. We introduce clinical information system (CIS) log analysis as a method for identifying patient-specific information needs and CIS log mining as an automated technique for discovering such needs in CIS log files. We have applied this method to WebCIS (Web-based Clinical Information System) log files to discover patterns of usage. The results can be used to guide design and development of relevant clinical information systems. This paper discusses the motivation behind the development of this method, describes CIS log analysis and mining, presents preliminary results and summarizes how the results can be applied.

Implementing a MIRC interface for a database driven teaching file.

This poster describes the feasibility of integrating an existing database driven teaching file with the Radiological Society of North America's (RSNA) Medical Imaging Resource Center (MIRC). MIRC is the product of an RSNA sponsored initiative to enable medical institutions to share their electronic medical content (images, text, and multimedia) by creating a distributed repository accessible from the Internet. A description of MIRC's query/storage service architecture is provided along with an overview of the author's experience with implementing a storage service front end for an existing, database driven teaching file system.

[Expansive development of the French regulation of the genetic print files after the recent reforms (Part I)].

French regulations related to "genetic prints" and its later incorporation to an automatized file in the frame of the penal process, initially deserved (1998) a positive judgement due to the guarantees surrounding such techniques, considering that with its use an interference was made with the freedom and rights of the individual. This primary regulation is watching a legislative evolution that brings serious doubts about the current guarantee system. A couple of legal reforms with security as their main axis (2001 and 2003) give more importance to the "genetic print" file by extending the causes in which it starts functioning going against the proportionality that must be observed when freedoms and rights of the individual can be affected.

Clinical attachment loss produced by curettes and periodontal files.

The aim of this study was to clinically evaluate the effect of immediate trauma from instrumentation after scaling and root planing with different instruments. Ten subjects with moderate chronic periodontitis, presenting probing depths of 3.5-6.5mm on anterior teeth, maxillary and/or mandibular, were selected. Teeth were randomly assigned to one of the following groups: PF group - scaled and planed with Hirschfeld periodontal files; CC group - scaled and planed with Gracey conventional curettes. The selected teeth were probed with a computerised electronic probe, guided by an occlusal stent and subjected to scaling and root planing. Immediately following instrumentation, teeth were probed again. The difference between relative attachment level (RAL) immediately before and after instrumentation was considered as trauma from instrumentation. Intra-group analysis revealed statistically significant differences between RAL immediately before and after instrumentation in both groups (1.28 +/- 1.13 for PF group; and 0.83 +/- 0.41 for CC group - p<0.05). However, inter-group analysis did not show statistically significant difference in trauma from instrumentation caused by the different instruments (p=0.063). Within the limits of this study, it was concluded that root instrumentation causes a mean immediate attachment loss of 1.06mm, with no differences between instrumentation with periodontal curettes or periodontal files.

Pediatric hospitalizations for asthma: use of a linked file to separate person-level risk and readmission.

INTRODUCTION: Disparities in asthma hospitalization by gender, age, and race/ethnicity are thought to be driven by a combination of 2 factors: disease severity and inadequate health care. Hospitalization data that fail to differentiate between numbers of admissions and numbers of individuals limit the ability to derive accurate conclusions about disparities and risks. METHODS: Hospitalization records for pediatric asthma patients (aged one to 14 years) were extracted from New Jersey Hospital Discharge Files (for the years 1994 through 2000) and then linked by patient identifiers using a probabilistic matching algorithm. The analysis file contained 30,400 hospital admissions for 21,016 children. Hospitalization statistics were decomposed into persons hospitalized and number of hospitalizations. Analysis of readmission within 180 days of discharge used additional records from 2001 to avoid bias due to truncated observation. RESULTS: Overall, 22.9% of children in our analysis had repeat asthma admissions within the same age interval, accounting for 30.9% of all hospitalizations. Also among all children, 11.7% had at least one readmission within 180 days of a prior discharge. The risk of hospitalization was higher for boys, decreased by age for both genders, was lowest for white children and highest for black children. Readmission rates were higher for black and Hispanic girls than boys in older age groups, but were otherwise relatively uniform by gender and age. CONCLUSION: Decomposition of ratios of total hospitalizations to population illuminates components of risk and suggests specific causes of disparity.

Shaping of curved root canals instrumented with stainless steel and nickel titanium files.

The cleaning efficiency and the quality of the surgical preparation is mainly conditioned by root canal anatomy, the instruments and the technique employed. The purpose of the present study was to evaluate the extent of straightening of curved root canals and the changes in endodontic anatomy caused by the use of Flex-R, Onyx-R and Nitiflex files with the balanced forces technique. Sixty mandibular molars were selected for evaluation of angulation and transportation. Pre and post instrumentation radiographs were taken of the mesiodistal and buccolingual aspects of each tooth. The radiographs were projected on a transparent sheet, the contours of each tooth and file were outlined and the canal curvature was measured. Pre and post instrumentation tracings were overlaid and the extent of transportation was measured with a micrometric caliper. Statistical analysis of the data was performed employing Student's "t" test, Analysis of Variance and Bonferroni's test. The results showed that all the instruments reduced canal curvature significantly. Transportation in the mesiodistal direction did not reach statistical significance whereas transportation in the buccolingual direction was statistically significant.

[Scanning electron microscope study of chemically disinfected endodontic files].

Forty stainless steel endodontic files were observed at scanning electron microscopy after being subjected to ten disinfection cycles of 10 minutes each one, immersed in different chemical disinfectants. Corrosion was not observed on the surface of the files in circumstances that this study was made.

Matching waveform audio files with toxicall data: Record linkage in a poison control center.

Analysis of coded communications and electronic patient record data present a unique opportunity for nursing knowledge discovery, the integration of communication content with clinical data. However, joint analysis of coded communications and clinical data depends on the ability to link waveform audio files with patient records. A procedure has been developed to link waveform audio files with Toxicall patient records, using date/time stamps and station identification numbers.

[S.E.M. study of surface condition of ultrasonic K files].

The present study was not about the interest of the association of ultrasonic instrumentation with irrigation in the root canal therapy, but about the wear of the K files themselves. The results showed with the assistance of the sono-synergistic system, files are almost as clean as the sterile are. This is important for the prevention of possible contamination.

[Magneto-optical disk filing system for oral diagnostic reports and their synchronized images].

The rewritable and compact media of magneto-optical disk (MOD) is firstly applied to store medical images with spoken reporting. The MOD has multi-media function of communication and filing. Our system is optimized in terms of recording length of speech based upon our clinical research. The results will have influences over the future PACS. One of their examples is possibility of multimedia PACS with images, speeches and demographic data. Another example is prevention of dispersion of images which have no interpretations by radiologists. Our next step is to recognize filed speech in background processing for printing reports.

Endodontic file design and dynamics in automated root canal preparation. Concepts, materials and methods.

Many treatment failures in endodontics are caused by problems that occurred during canal preparation. This article reviews principles of endodontic instrument design and analyzes the design as it relates to an automated handpiece. After a review of the problems of automation, there will be an introduction of a new automated handpiece and a new modified file known as the Anticurvature File.

[Effects of paraformaldehyde steam sterilization on K-files].

Success of endodontic therapy depends upon both operator's skill and quality of the employed instruments. K-files have been examined for they probably are the most commonly used endodontic instruments. It is particularly important to take into consideration, together with the original quality of the instruments, its possible changes due to the wear from sterilization. In this paper have been therefore evaluated the characteristics of brand-new K-files from three different manufacturers together with the wear of the same instruments after paraformaldehyde steam sterilization.

[A new computerized system for electroencephalography at the Kochi Medical School Hospital: the present status and problems of electroencephalogram data filing systems].

The usual electroencephalography (EEG) recording consumes great amounts of paper, considerable storage space for records and much time and energy for their search and retrieval. In addition, we can not perform digitized analyses of the records with the present method. To solve these problems, our laboratory developed a new computerized system for EEG, in which data are retained in optic disks, and which has been in service for routine examination since December, 1988. The functions of the system and EEG filing system, include the collection, retention, retrieval, transmission and analyses of data with the reproduction of the original EEG and editing function of summary reports to be filed in the medical records. The summary report consists of summary, characteristic wave patterns picked up and edited from EEG, and spectral array and topographical mapping by digitized analyses of EEG. The condition for the collection of EEG data was 200 Hz/8 bit, and the reproduced wave patterns were accepted by all clinicians. The merits of the system include; (i) saving of paper, space and time needed for EEG, (ii) enabling the comparison of the wave patterns in the form of summary reports and (iii) the capability of digitized analyses of EEG by retaining the EEG data in the data base. The problems remaining to be improved for the system are the longer time required for examination (5-10 min) and the higher running cost (yen 460/order). Regarding the latter problem, a revised method which dispenses with recording paper is under consideration. That is, in the case of screening examinations, summary reports for medical records alone would be delivered to clinicians. This idea has been accepted by some clinicians. To realize the revised system, we presently are planning to establish a method to display EEG on CRT.

Computers help keep patient files confidential.

File folders stuffed with papers documenting a patient's hospital stay can be an administrative nightmare. Such medical records are easily misplaced and might already be checked out when they need to be reviewed. In addition, because so many eyes glance at such files, confidentiality is a problem. That's why some hospitals are purchasing information systems for their medical records departments.

Analysis of models of single-file diffusion.

Theoretical models of single-file transport in a homogeneous channel are considered. Three levels of channel populations were specified for which different approximations could be used. The results of these approximations are in good agreement with the results of a computer experiment (Aityan and Portnov 1986). At low populations, the pair correlation functions were negligibly small and allowed the use of linear approximation for unidirectional fluxes and populations. The value of the pair correlation function and the respective approximation for fluxes was obtained by the two-particles random-walk technique. At extremely high populations, the "divider" technique was proposed to describe the single-file transport. The divider technique allowed to explain the exponential shape of the pair correlation function FABn, n + 1 profile at extremely high populations. At medium populations the finite difference superposition approximation was valid.
