Ultrapotent topical corticosteroid treatment of hemangiomas of infancy.

BACKGROUND: Superficial cutaneous hemangiomas of infancy represent a therapeutic challenge. Two small case series using ultrapotent topical corticosteroids for periocular hemangiomas were reported in the ophthalmologic literature. The use of this therapy for hemangiomas of infancy at other sites on the body has not been reported. OBJECTIVE: We sought to assess the clinical effects of short-term application of ultrapotent topical corticosteroids for the treatment of hemangiomas of infancy. METHODS: The records of 34 infants with proliferating hemangiomas of infancy that were treated with ultrapotent topical steroids were reviewed retrospectively. Treatment response was based on: (1) cessation of growth; (2) shrinkage or flattening of the lesion; and (3) lightening of the surface color. Lesions demonstrating responses of two of the three criteria were judged to have good response; one criterion, partial response; and no improvement, no response. RESULTS: Of the patients, 35% demonstrated good response, 38% partial response, and 27% no response. CONCLUSIONS: Hemangiomas in 74% of the infants demonstrated either good or partial response to treatment with ultrapotent topical corticosteroids. Of the responders, the majority reported cessation of growth before what would have been expected for their age. Improvement varied, with thinner superficial hemangiomas demonstrating better cosmetic improvement than thicker lesions.

Reconstruction of a lower eyelid defect using the temporalis muscle.

Massive defects of the cheek and simultaneous involvement of both eyelids preclude the use of local flaps and standard techniques for total lower eyelid replacement. Regional flaps may not provide enough coverage nor bulk, or they may require multiple delay procedures. Sagittal splitting of the temporalis muscle and anterior rotation of its lateral head deserves consideration for reconstruction of uncommon, large periorbital defects. A case is presented in which globe salvage was possible using this technique as a single stage procedure.

Increased suicide risk among Danish women with non-melanoma skin cancer, 1971-1999.

More suicides than expected (standardized mortality ratio=1.3; 95% confidence interval=1.1-1.6) compared to the population risk were found among Danish women with a diagnosis of non-melanoma skin cancer in the period 1971-1999 but not among men. The increased risk might be associated with a different personality type in this cancer group.

Unusual skin lesions in chronic myelomonocytic leukemia.

Chronic myelomonocytic leukemia (CMML) is a relatively rare, heterogeneous syndrome classified as a myelodysplastic syndrome according to the French-American-British classification system. The patient's presenting symptom was a pigmented skin nodule that, although common for cases of acute monoblastic leukemia, is peculiar for CMML. This case should increase awareness of the inclusion of CMML in the differential diagnosis of a discolored nodule and highlight the clinicopathologic considerations and therapeutic challenges consistent with the diagnosis of CMML.

Cosmetic principles in surgery on the face.

Excisions from the face are performed to cure diseases and to remove blemishes. Satisfactory cosmetic appearances can be achieved in most cases, even when large defects result from the excisions, if the surgeon employs cosmetic surgical principles and techniques in the repairs. Some esthetic concepts and artful methods are presented to illustrate this feasibility.
