[Outcome of Doppler parameters of left ventricular systolic function during atrial stimulation as a function of coronary disease].

The authors studied the effects of transoesophageal atrial pacing on Doppler parameters derived from flow in the left ventricular out flow tract (maximal velocity (V max), velocity-time integral (VTI), mean acceleration of aortic flow (Acc), acceleration force (AF) of the left ventricle). These parameters were recorded in patients with normal left ventricular wall motion at rest, with and without coronary disease. Eight patients had angiographically normal coronary arteries (Group 1) and 21 had coronary disease (Group 2) including 10 with an isolated stenosis of the left anterior descending artery (Group 2a) and 11 with multivessel disease (Group 2b). The heart rate was increased by increments of 20 beats per minute from 90 to 130 each minute. In coronary patients, atrial pacing resulted in a fall in V max from 0.99 +/- 0.15 to 0.90 +/- 0.12 m/s, p < 0.0005 and in AF from 23.1 +/- 6.3 to 19.6 +/- 4.8 Kdynes, p < 0.0005, whereas the Acc remained stable (13.51 +/- 3.27 and 13.53 +/- 2.47 m/s/s, NS). Conversely, V max (1.04 +/- 0.11 and 1.04 +/- 0.11, NS) and AF (25.2 +/- 5.7 and 26.3 +/- 6.7, NS) were unchanged in normal controls and the Acc improved from 13.87 +/- 3.61 to 17.04 +/- 3.49, (p < 0.05). The VTI fell significantly in both groups. The percentage variations of V max, Acc and AF were significantly different in coronary patients compared with normal controls. Radiation-related coronary artery disease in Hodgkin's disease.

Coronary artery disease is a rare and only recently recognised complication of mediastinal irradiation. A 34-year-old man died suddenly eight years after mediastinal irradiation for Hodgkin's disease. Autopsy disclosed severe narrowing of all major extramural coronary arteries by atherosclerotic plaques whereas all other systemic and visceral arteries were virtually free of atheroma. Autopsy findings in the five reported cases of radiation-related coronary artery disease are reviewed.

Measuring of cardiolipin antibodies and plasma lipids in different stages of coronary disease.

The study shows that antibodies to cardiolipin which are commonly found in young postinfarction cases may present a high risk for recurrent coronary manifestations. IgG and IgM antibodies to cardiolipin (C1Ab) were determined in 50 healthy individuals and 72 patients with coronary disease: 26 with acute infarction (A) and 24 with chronic infarction (C) as well as 22 with angina pectoris (AP). Lipid status parameters as risk factors were measured in all patients: total cholesterol, LDL, HDL cholesterol, triglycerides and apolipoprotein (a). Out of 72 patients suffering from coronary disease 20 (27.7%) had elevated C1Ab IgG (12.36 +/- 3.7 GPLU/ml). Rise of C1Ab IgM was not evidenced in any of the patients. C1Ab IgG were significantly elevated (p < 0.01) in patients with chronic myocardial infarction and angina pectoris (p < 0.05) while in cases of acute myocardial infarction significant elevation of these antibodies was not evidenced.

[Value of dobutamine echocardiography in the detection of coronary disease in heart transplant patient. Groupe de Recherche VACOMED].

The value of Dobutamine stress echocardiography in the detection of coronary artery disease in heart transplant patients was studied in 64 patients at control coronary angiography 39 +/- 14 months after transplantation. Dobutamine was infused at progressively increasing doses (5 to 40 micrograms/kg/min) at 5 minute intervals, in order to reach 85% of the theoretical maximal heart rate or an ischaemic event. Echocardiography was analysed in the 4 standard views which were digitised allowing calculation of a regional wall motion score under basal conditions and at peak dosage in 16 left ventricular segments. Coronary angiography identified three groups: group I: 29 patients with normal coronary arteries; group II: 17 patients with non-significant coronary disease (diffuse or localised stenosis < 50%); group III: 9 patients with significant (> 50%) coronary disease. Dobutamine stress echocardiography showed regional wall motion abnormalities in 2/29 patients in group I, 13/17 patients in group II and all patients in group III (global sensitivity: 85%; specificity: 97%). The contractility score was significantly higher under basal conditions in group III (1.45 +/- 0.54) than in group I (1) and group II (1.17 +/- 0.23). At peak dose, the contractility score was unchanged in group I and increased significantly (p < 0.01) in the other two groups. The authors conclude that Dobutamine echocardiography is a reliable, non-invasive method of detecting coronary disease in cardiac transplant patients, and is particularly valuable for demonstrating myocardial ischaemia related to coronary lesions judged to be not significant at coronary angiography.

[Advantages of the combined use of nuclear medicine procedures in the diagnosis of coronary disease].

Investigations on 106 patients with coronary artery disease were performed to improve the diagnosis of myocardial ischemia by combining myocardial scintigraphy and gated blood studies. The results show that it is possible to enhance the detection of coronary artery disease and to classify haemodynamic efficiency. In addition we found that in investigations performed on patients with arrhythmia and under stress conditions the "hybrid-technique" is superior to the framemode acquisition method.

Association of Bartonella species and Coxiella burnetii infection with coronary artery disease.

Coronary artery disease is an inflammatory condition associated with several infections. We prospectively evaluated 155 consecutive patients undergoing coronary angiography for evidence of Bartonella species and Coxiella burnetii infection. All Bartonella cultures were found to be negative. Multivariable logistic regression analysis that controlled for potential confounding factors revealed no association between coronary artery disease and seropositivity to Bartonella henselae (odds ratio [OR], 0.852; 95% confidence interval [CI], 0.293-2.476), Bartonella quintana (OR, 0.425; 95% CI, 0.127-1.479), C. burnetii phase 1 (OR, undefined), and C. burnetii phase 2 (OR, 0.731; 95% CI, 0.199-2.680). The geometric mean titer (GMT) for C. burnetii phase 1 assay was slightly higher in persons with coronary artery disease than in those without such disease (P<.02). B. henselae, B. quintana, and C. burnetii seropositivity was not strongly associated with coronary artery disease. On the basis of GMTs, C. burnetii infection may have a modest association with coronary artery disease.

Dietary habits, plasma polyunsaturated fatty acids and selected coronary disease risk factors in Tanzania.

OBJECTIVES: To compare dietary habits and percent compositions of omega (omega)-3 fatty acid (FA) in plasma phospholipids and to examine if there is any association between fish intake and selected coronary disease risk (CHD) factors in this population. DESIGN: Cross sectional population study. SETTING: Three areas in Tanzania: urban Dar es Salaam (D), rural Handeni (H) and pastoralist population of Maasai in Moduli (Mo). SUBJECTS: One hundred and five participants (Dar 36, Handeni 37 and Monduli 32) aged 47-57 years. MAIN OUTCOME MEASURES: Fatty acids composition in plasma phospholipids. RESULTS: The frequency of intake of fish, meat, coconut milk and fruits was higher in Dar than in Handeni or Monduli (all p< 0.0001). The Maasai from Monduli had the highest percentage consumption of milk in a week (p<0.0001) and lowest intake of fish (p<0.0001). Participants from Handeni had the highest prevalence of consumption of green vegetables (p<0.0001). Percent compositions of arachidonic acid (AA; C20: 4omega-6) and docosahexaenoic acid (DHA; C22: 6omega-3) in plasma phospholipids were the lowest in Monduli (p<0.0001). Selected coronary disease risk factors were higher in the urban area Dar. The frequency of intake of fish per week correlated negatively with total cholesterol (TC) and LDL-C and HBA1c percent but not with blood pressure. The percent composition of omega-3 FA in plasma phospholipids was positively correlated with the frequency of intake of fish a week. CONCLUSION: Our results indicate that, there are significant differences in dietary patterns among the three study areas, and that the intake of fish is inversely associated with selected risk factors for coronary heart disease.

Oxidative status of lipoproteins in coronary disease patients.

Oxidized low-density lipoprotein (LDL) may play an important role in atherogenesis. The oxidative status of isolated LDL and very low-density lipoprotein (VLDL) were investigated in 23 patients with proven coronary disease and in 23 healthy asymptomatic control subjects. Oxidized cholesterol (4-cholesten-3-one and 20 alpha-OH cholesterol) was identified in LDL and VLDL from both groups. The content of cholesterol and 4-cholesten-3-one in LDL from patients was significantly increased in comparison with values from the control subjects. Lipid peroxidation, as assessed by malondialdehyde (MDA) formation, was barely detectable in native LDL and VLDL from the two groups. However, after incubation with a free radical-producing system, MDA levels in LDL from patients were significantly higher than those in control subjects. Lysine reactivity in LDL after incubation with an oxidizing agent, CuSO4, was similar between groups. However, lysine reactivity to CuSO4 in VLDL from patients was less than that in control subjects. Our results suggest that LDL levels from patients with coronary disease have an elevated oxidized cholesterol content and are more susceptible to peroxidative modification. Conversely, the LDL apoprotein does not appear to have been oxidatively modified in these patients. The data are consistent with a role for oxidized LDL in coronary artery disease and indicate that the LDL lipid may be an important oxidation site.
