Naturopathic medicine--a 10-year perspective (from a 35-year view).

The decade of Alternative Therapies in Health and Medicine's existence has seen dramatic advancement of the naturopathic profession in all areas: research, quality of practice, quality of education, licensure, inclusion in the political process, and public acceptance. Naturopathic doctors are now valued in the marketplace and in policy as a legitimate part of an overall system of healthcare. Momentum, infrastructure, acceptance, and conceptualization are all foundational to the future. Alternative Therapies in Health and Medicine was one of the first journals to invite naturopathic doctors onto its Editorial Board (i.e., Pizzorno, Standish). Our profession thanks and applauds you for inviting us.

A VR planning system for craniosynostosis surgery.

The goal of our project is to build a system which could utilize the Virtual Reality (VR) techniques for the pre-operative planning of craniosynostosis. The system includes different modules. We use the tetrahedral volume meshes for the basic structure for the models which surgery is planning on. This paper will describe the procedures of above stages, from the processing of 2D image slices, 3D modeling, smoothing, simplification, and visibility ordering, to volume meshes generation. We have demonstrated the initial results on a variety of stereo devices. The testing results show the processing time is acceptable and the rendering effect is pretty well.

CPT 2000: interventional pain management coding in the new millennium.

UNLABELLED: Current Procedural Terminology is a systematic listing and coding of procedures and services performed by physicians and other providers. The CPT is the most widely accepted nomenclature for the reporting of procedures by physicians and other providers for health-care services provided by the government, and private health-insurance programs. It is most widely accepted for claim processing, and for the development of guidelines for medical care review, and it provides the uniform language applicable to medical education, research, and utilization. The CPT 2000 includes a multitude of changes. Those of most important interest to interventional pain management specialists include neural blockade where the codes used in pain management have been totally revamped. The entire section of neural blockade codes has been substantially altered, either by deletion, modification, or addition of a new code. Various deleted codes include 62274 to 62279, 62288, 62289, 62298, and 64440 to 64445. The definitions for CPT codes 62273, 62280, 62281, 62282, 62287, 62291, 62350, 64622, 64623, and 72285 have been modified and changed. Multiple new codes not only include replacement codes for epidurals, but also creation of codes for sacroiliac-joint injection, sacroiliac-joint arthrography, percutaneous lysis of epidural adhesions, facet-joint injections at the cervical and thoracic levels, neurolytic facet-joint neural blockade for cervical and thoracic levels, transforaminal injection codes for cervical/thoracic and lumbar/sacral, epidurography and radiological examination. The several advantages and disadvantages of new codes and future directions in CPT coding are described. ABO-incompatible pediatric bone marrow transplantation: a simple method to remove red blood cells from small volume marrow grafts.

A simple and reliable technique for removal of ABO incompatible marrow red cells is described. This method requires a blood cell processor and third party red cells are used as a shelf. Using this technique, nine children age 0.5-11.5 years received allogeneic bone marrow transplantation from ABO incompatible donors. A median of 4.8 ml of incompatible red cells were transfused. There was no evidence of a hemolytic transfusion reaction in any patient. A median of 75% of nucleated marrow cells were recovered and used for transplantation. Engraftment occurred at the same time as with ABO compatible transplants. Autologous marrow red cells were reinfused into four young donors. This 'shelf' technique for red cell depletion is an acceptable method for processing small volume, ABO incompatible marrow harvests from pediatric donors.

[A pathological study on 662 autopsy cases of chronic cor pulmonale. The National Pathological Cooperative Group of Cor Pulmonale].

According to the unified diagnostic criteria and methods, 662 cases of cor pulmonale autopsy were accepted and the acceptable rate was 88.5%. Additionally, a data base with IBM PC/XT computer and dBASE-III was established in which the data of all cases accepted were stored, processed and analyzed. The results showed that chronic bronchitis and emphysema were the primary pulmonary diseases of chronic cor pulmonale in China. Chronic bronchiolitis and peribronchiolitis were considered as the late stage of bronchitis causing damage to lung tissues and constituting the central link of such complications as pulmonary hypertension and right ventricular hypertrophy. The authors emphasize also that the cellular elements and extracellular matrix were the major reason in decreasing pulmonary vascular compliance, while dilation of the lumen of pulmonary small arteries was probably the secondary change of pulmonary hypertension. According to the results obtained, it is considered that hypertrophy of the supraventricular crest can be used as one of the diagnostic criteria of right ventricular hypertrophy due to pulmonary hypertension.

Oxytocin and ovarian function.

(1) Oxytocin is synthesized in the luteal cells of all species so far studied, including the human. Vasopressin is also synthesized, but at a much lower rate. (2) The oxytocin-neurophysin gene is expressed in granulosa cells and granulosa-derived luteal cells but not in theca cells. Ovulation or spontaneous luteinization initiates the gene expression which peaks in the early luteal phase and ceases around mid-cycle. (3) Luteal oxytocin concentrations rise with considerable delay after the peak of specific mRNA and reach maximal levels around mid-cycle. Oxytocin concentrations fall to low levels in the late luteal phase and in pregnancy. (4) Thecal tissue produces substances such as catecholamines and ascorbic acid that stimulate oxytocin secretion in granulosa cells. The adrenergic innervation of thecal tissue provides a source of catecholamines and may therefore serve a modulatory function in ovarian oxytocin secretion. (5) Oxytocin has little or no direct effect on luteal progesterone production. (6) Oxytocin inhibits LH-stimulated prostacyclin production in luteal cells of cows. Oxytocin may induce the release of PGF-2 alpha or lipo-oxygenase products from the ovary but this has not yet been documented. (7) PGF-2 alpha releases oxytocin from the ovary but does not turn off its synthesis. (8) The concept that ovarian oxytocin participates in the luteolytic process is gaining acceptance. In some species (sheep, goat) ovarian oxytocin acts as a hormone causing PGF-2 alpha release from the uterus. In others it acts in a paracrine or autocrine fashion on ovarian prostanoid production (cow, possibly primates).(ABSTRACT TRUNCATED AT 250 WORDS)

Histiocytoses of lymph nodes: biology and differential diagnosis.

Two major subsets of histiocytic cells are recognized within the lymphoreticular system: the dendritic cells or antigen-presenting cells and the phagocytic histiocytes or antigen-processing cells. Reactive and proliferative lesions of histiocytes can be related to these functional subsets. Reactive proliferations with a major dendritic cell component include dermatopathic lymphadenitis and Langerhans cell histiocytosis. Rare neoplasms derived from dendritic cells have been described. Benign proliferative lesions of phagocytic histiocytes include sinus histiocytosis with massive lymphadenopathy, the hemophagocytic syndromes including familial erythrophagocytic lymphohistiocytosis, Kikuchi's disease, and the various granulomatous lesions of lymph nodes. Although the hemophagocytic syndromes were often interpreted in the past as a variant of malignant histiocytosis (histiocytic medullary reticulosis), the reactive nature of this process is now accepted. The inciting event is usually an infection, and it is hypothesized that the histiocytic proliferation may result from exaggerated lymphokine production in an immunocompromised host. Reactive lesions of histiocytes are much more common than their rare neoplastic counterparts. True histiocytic lymphomas must be distinguished from the more common sinusoidal large cell immunoblastic lymphomas which usually express the CD30 antigen.

[Ribonucleoproteins containing heterogeneous nuclear and messenger cytoplasmic RNA. Characteristics, structure and relations (author's transl)].

Following the study of Spirin, many authors have shown that cytoplasmic messenger RNA and heterogeneous nuclear RNA are complexed with specific proteins to form ribonucleoprotein particles (RNP). These RNP are heterogeneous in size and present a high protein to RNA ratio. Different observations suggest a polymeric structure for nuclear ribonucleoproteins but their protein composition is more clearly complex than that of cytoplasmic ribonucleoproteins. If we accept the following processing: heterogeneous nuclear RNP leads to free cytoplasmic RNP leads to polysomic RNP, the question arises as to what extent proteins originally present in nuclear RNP are conserved. Proteins more or less tightly bound to RNA have different roles: packing and protection of RNA, regulation during gene expression processing.

Quantitative autoradiography using a personal computer.

A video digitizing system based on an Apple II personal computer has been developed for use in quantitative autoradiography. The selection process of an acceptable video camera, an illumination system, and the functions provided in the software are discussed. This system is in routine use and has been duplicated by several other laboratories.

Thyroid ocular myopathy.

Based upon the ultrasonographic evidence of extraocular muscle abnormalities in all patients with orbitopathy and proven thyroid disease, we conclude that the basic abnormality of thyroid orbitopathy is a panmyositis and that all of the classes described by Werner are expressions of different degrees and manifestations of the same pathologic process. This thesis is supported by presentation of cases of varying severity who have in common extraocular muscle abnormalities. We believe that the process we describe acceptably explains all of the eye signs of this common orbitopathy.

Issues in the assessment of PSA immunoassays.

The physician and laboratorian have several proven PSA assays from which to choose, and several more are likely to achieve FDA approval soon. Furthermore, an assay that is FDA approved offers considerable reassurance that the many aspects of assay design and manufacturing processes meet an acceptable standard. This will become even more critical as assays report values in the ultrasensitive range. We have attempted to review here some of the most important issues related to PSA assay performance and standardization. Standardization particularly is a complex problem for which solutions are just now appearing. Yet efforts must continue to minimize controversies, because many more assays will be forthcoming and because newer applications of PSA determinations will place more pressure on proper assay evaluations. For example, measurement of free PSA, PSA-ACT complexes, and the ratio of one to the other are intriguing possibilities for added clinical benefit. Also, ultrasensitive PSA assays add a new dimension to the detection of residual disease and may provide unique opportunities for the implementation of new therapies and their timely evaluation. In our opinion, the impact of PSA on the management of prostate cancer is still growing, and the opportunities for better and new assays are still great.

Crafting mental health policy.

The 1990s has brought a heightened interest in policy and, more specifically, health policy development. This article gives the reader an overview of the principles of crafting public policy, the components of basic policy at a national level, the techniques and pitfalls of policy formation, and the refinement needed for application of skills to mental health policy. Nurses will be able to discuss elements of and the process for policy acceptance and implementation.

[References for evaluation scales in quality assurance in rehabilitation--6. Assessment of coping processes with the Freiburg Questionnaire of Illness Coping].

Advantages of coping assessment for the understanding of individual impacts of severe illness are pointed out, the benefit for a better understanding of the rehabilitation process is demonstrated. After the basic positions and preliminary assumptions are described, the guidelines of the development of the inventory are introduced: Assessment of a broad spectrum of coping with chronic diseases with ratings by the patient himself and by others (doctors, relatives), suitability for process measuring, good acceptance, and economical application. Statistical properties of the questionnaires are reported with respect to achieved acceptance, reliability, and validity data. Using selected results as examples, possible applications in psychosomatic research are pointed out for issues such as illness-specific components of coping with chronic diseases, discrepancies between patients' and doctors' views of coping, and adaptational success. Finally, possibilities and limits of the inventory are discussed in terms of research issues and methodology.

Parental acceptance, postpartum depression, and maternal sensitivity: mediating and moderating processes.

Mothers (n = 92), fathers (n = 84), and their infants (60% male) participated in a longitudinal study of postpartum depression and maternal sensitivity. Mothers completed questionnaire measures of remembered parental acceptance, depressive symptoms, and infant distress to novelty and limits. Mothers and partners reported on marital aggression and avoidance. Maternal sensitivity was observed in the laboratory at 6 months. Characteristics of mothers, partners, and infants combined to predict postpartum depression and maternal sensitivity. Remembered parental rejection predicted postpartum depressive symptoms with prenatal depression controlled; self-esteem mediated this effect. Paternal acceptance buffered against postpartum depression when infants were highly reactive and when partners were aggressive. Paternal acceptance reduced the impact of postpartum depression on maternal sensitivity; having an aggressive marital partner exacerbated the effect.

[Measurement of the left ventricular mass using MRI with automatic determination of the endocardial and epicardial contours].

This study describes a method of automatic border detection of the left ventricular endocardium and epicardium associating three methods of segmentation (increase of region, border detection and adaptive threshold), applicable to the evaluation of ventricular mass and volume by magnetic resonance imaging. Despite slight underestimation, the spin-echo sequence used in 9 small pigs provided a value of left ventricular mass close to that observed ex vivo (r = 0.97, SEE = 6.05 g). Clinical validation using a rapid gradient-echo sequence was undertaken and compared with manual border detection carried out by three independent, trained operators. The study population included healthy subjects and patients with global or segmental left ventricular dysfunction with or without ventricular deformation. The correlations between automatic and manual detection were satisfactory both for calculation of left ventricular mass (r = 0.93, SEE = 13 g) and measurement of surfaces (r = 0.91, SEE = 1.4 cm2). The concordance of the two methods was confirmed by the Bland and Altman test. Cardiac magnetic resonance imaging may provide accurate and reproducible measurements of left ventricular mass within acceptable acquisition and image processing times for routine use. Although the clinical value of such a method is accepted both for establishing the prognosis and guiding management, studies of the cost/efficacy ratio should be undertaken to situate magnetic resonance imaging with respect to other non-invasive techniques of investigation of left ventricular function.

Computational fluid dynamics analysis for process impact assessment during thermal pasteurization of intact eggs.

Transient temperature and albumen velocity profiles during thermal pasteurization of intact eggs were studied using a commercial computational fluid dynamics (CFD) package. Simulated temperature profiles were in close agreement with experimental data for eggs of different sizes. Convective heat transfer only occurred in the egg white fraction, and conductive heat transfer only occurred in the yolk. For process assessment, a generally accepted kinetic inactivation model for Salmonella Enteritidis was incorporated into the CFD analysis. Minimum process times and temperatures needed to provide equivalent pasteurization at 5-log reductions of the target microorganism were obtained on a theoretical basis. The combination of CFD analysis and inactivation kinetics can be very useful for assessing pasteurization of intact eggs and can enable processors to gain a better understanding of these processes and to establish process conditions for consumer-safe eggs.

Collaborating with physicians in the drug-usage evaluation process.

The Joint Commission requires that DUE be performed by the medical staff with pharmacy department participation. To gain physician acceptance of the DUE process, it must be promoted as a means of ensuring quality care and improved patient outcomes, employing an educational rather than punitive approach. Peer interaction is the ideal method to enforce and reenforce desired practices. The pharmacy department can play a major role in ensuring appropriate prescribing practices by reporting information, serving as an educational resource, facilitating physician participation, and overseeing the DUE process.

A simple procedure for morphometric analysis of processes and growth cones of neurons in culture using parameters derived from the contour and convex hull of the object.

Morphometric estimation of neuronal processes is currently laborious and time-consuming, since the individual processes (axons and dendrites) have to be traced manually. In order to facilitate the measurement of cellular processes, we have tested a series of parameters derived from the contour and the convex hull of an object and estimated to which extent they reflect process length and number. The parameters included the area, perimeter and form factor of the object and convex hull, their ratios as well as object length, breadth, width, length/width and spreading index. Some new parameters derived from the contour and convex hull of the object, were also computed: process index (the number of areas contained within the convex hull outside the object contour), process domain (the total area contained within the convex hull outside the object contour), their ratio and the square root of the process domain (SR process domain). In total, 18 parameters were estimated. Populations of motoneurons, growth cones of cerebellar granule cells and N2a neuroblastoma cells were utilized due to their diversity in morphological features. The processes of each object were drawn by hand to establish the actual length and number. Total process length per object correlated strongly with object perimeter, process domain and SR process domain. The number of processes per object correlated well with perimeter ratio, process index and form factor, whereas object length, convex hull perimeter and spreading index correlated acceptably with the average process length. Using these parameters for the evaluation of neurite outgrowth in developing of hippocampal neurons in vitro, variables such as object perimeter, process domain and SR process domain were found to be very well suited for estimation of the total length of neurites. We conclude that based on the contour and convex hull of an object it is possible to calculate a series of parameters which may substitute direct measurements of process length.
