Combined surgical procedure for the prevention of blindness caused by progressive high myopia.

A combined prophylactic procedure consisting of the strengthening of the sclera by fascia lata implants and the laser photocoagulation of peripheral retinal degenerations has been developed for the prevention of blindness caused by progressive high myopia. More than 7 years of experience gained with over 400 patients showed that none of the treated eyes developed retinal detachment or other severe complications of high myopia. Moreover, the degree of myopia decreased by a mean value of 3.0 D in 90%, and the visual acuity improved by a mean value of 0.15 in 70% of the patients treated. The authors suggest that their combined prophylactic procedure should be tried out and introduced to general ophthalmological practice.

Development of retinal arteriolar tortuosity in previously unaffected family members.

A family affected by retinal arteriolar tortuosity over a nine-year period has been observed, and the appearance of the abnormality in three previously unaffected family members in late teen-age has been documented. The proband has suffered visual loss due to optic disc infarction, this complication has not previously been reported. We believe that this is the first report of retinal arteriolar tortuosity in a British family.

Development of retinal synaptic arrays in the inner plexiform layer of dark-reared mice.

In the central area of the retina of mouse the rate of synaptogenesis in the inner plexiform layer (IPL) drops precipitously at about the time the eyes open. To determine if the visual input at eye opening provides a signal for the neurons to stop adding synapses, mice were raised in darkness during the period of maximal synaptogenesis and through eye opening. Retinal synaptic arrays of dark-reared and normally reared animals were compared quantitatively. The rate of synaptogenesis after eye opening in dark-reared mice indicated that the onset of visual stimulation was not the cue to stop synaptogenesis. However, the synaptic arrays of the IPL of dark-reared mice consistently had more conventional synapses than those of normally reared mice. It is concluded that the number of conventional synapses in the central retina was increased by dark-rearing.

Prevention of aphakic retinal detachment by retinocryocoagulation.

A retrospective study of 36 second eyes with cataract, who have undergone prophylactic retinocryocoagulation prior to cataract extraction, is presented. All 36 patients suffered an aphakic retinal detachment in the first eye in the past. Postoperative complications included iritis, transient cycloplegia and retinal hemorrhages; two patients developed retinal tears following cataract surgery. During the average followup period of 8.4 years, no retinal detachment occurred among this group of patients. This study strongly supports the value of preventive treatment (retinocryocoagulation) of the second eyes prior to cataract extraction, which have a high risk of aphakic retinal detachment.
