The survival and well-being of patients treated for Cushing's syndrome.

OBJECTIVE: The main aim of the study was to evaluate the survival, well-being and working capacity of patients treated for Cushing's syndrome. DESIGN: The study was carried out by retrospectively analysing patient records from years 1981-94. Follow-up time was extended from the time of diagnosis to the end of 1996. A questionnaire dealing with symptoms prior to and after therapy, and the quality of life estimated on a Visual Analogue Scale (VAS) was sent to all surviving patients. SETTING: The study was performed in a university hospital. MAIN OUTCOME MEASURES: Survival, subjective well-being, working capacity before and after treatment and disappearance of symptoms after treatment of Cushing's syndrome. RESULTS: During the follow-up time 10 patients died out of a total of 74. The overall standardized mortality ratio (SMR) was 168% (95% CI 81-309%). The SMR of patients with pituitary disease was 267% (89-525%), and in patients with adrenal adenoma it was 135% (16-489%). Forty-six per cent of the surviving patients stated that they felt fully recovered from the disease, but the proportion of patients having persisting symptoms after treatment was also noteworthy. The mean VAS score (range 0-100) was 19 (SD 14) before treatment and 82 (SD 18) after treatment (P<0.001). After treatment, 81% of the patients were able to return to work, 11% retired because of disability, 5% retired because of age and 3% were on sick-leave at the time of answering the questionnaire. During the follow-up time, 42% of the patients with pituitary disease suffered a relapse. However, the effect of the relapse on well-being was not significant. CONCLUSIONS: The mortality risk of patients treated for Cushing's disease was not significantly increased compared with that of the general population. Many symptoms persisted even years after therapy. After clinical recovery, working ability was not always regained.

[Clinical criteria for evaluation of hemophilia severity in expert estimation of patient's working capacity].

Data are presented on the evaluation of hemophilia severity during expert estimation of the patient's working capacity according to the information of the Republican Centre. The authors have developed the most optimal classification for the evaluation of the disease severity; the influence of hemorrhagic symptoms and concomitant diseases on hemophilia course and prognosis has been considered.

Functional capacity evaluations as a post-offer employment: a case study.

Functional capacity evaluations in the post-offer phase of employment has the capacity to minimize work related injuries and promote wellness on the worksite. This paper includes a discussion of the value of post-offer evaluations and possible assessment options. To investigate the feasibility of administering post-offer evaluations, a case study was initiated. The Physical Work Performance Evaluation (PWPE) was administered to a new employee, VA, who was employed for the task of kneading dough, at a major food company. When compared to detailed work demands, there was no match between the job demands, and the client's capability in two sub-tests: lift from waist to eye level and trunk rotation. Based upon findings, recommendations for an initial graded work schedule, basic strengthening exercises, and modifications of the job site and task were made to the employer. The project was initiated with the hope of raising employee's and employer's awareness of workers health issues in the post-offer phase as it express itself in post-offer evaluation that aims to prevent musculoskeletal problems. This article aims to expand occupational therapists' awareness to the potential benefits of such a process.

[Evaluation of occupational and work capacity after myocardial infarct].

Innovations in medical technics increase in volume. In consequence the medical expert has more difficulties in judging the rank of the different tests and medical examinations. With this publication we intend to help to value the results of specific medical tests and the checkup of a patient after a myocardial infarction with regard to the "Berufs- und Erwerbsunfähigket". Furthermore there is a table, showing criterions and various results to answer the question, if a patient after a myocardial infarction is able to have furthermore an occupation and if you may expect heavy labour of someone. In this article, written by a doctor working in a hospital treating patients with diseases of the heart-circulation system, special aspects were considered, concerning the "Anschlu ss heilbehandlungsma ss nahme", which is a common practice for a patient after a myocardial infarction.

Expert review of an approach to functional capacity evaluation.

Functional capacity evaluation (FCE) is a widely used tool in work rehabilitation, despite the limited examination of the soundness of its measurement properties. This paper outlines the development of a new approach to FCE, the GAPP FCE, and reports on the findings of an expert review of aspects of its content validity and technical adequacy and how it meets established test criteria. Five expert occupational therapists reviewed the materials of the GAPP FCE then completed a questionnaire related to the content validity, technical adequacy and safety, reliability, validity, practicality and utility of the GAPP FCE. The experts gave support to most aspects of these criteria. The main issue identified by the review was related to interpretation and extrapolation of the FCE results for return to work. This and other issues are discussed in relation to recent developments in FCE and plans for future development of the GAPP FCE.

Disability methodology redesign: considerations for a new approach to disability determination.

Disability determination meets important societal needs, involving billions of dollars and millions of people every year. However, disability determination decisions often are incorrect, and the high proportion of decision appeals and reversals creates additional administrative expense and difficulty for the people that the disability determination system is intended to support. Projects funded by the United States Social Security Administration explored these issues and developed new conceptual models and tools to improve the accuracy and fairness of disability determination. This paper provides an introduction to the projects and the papers in this special issue of the Journal of Occupational Rehabilitation.

Postural compensation after intratympanic gentamicin treatment of Menière's disease.

We have treated 93 patients with severe Menière's disease with gentamicin (GM) applied with 1 to 4 or intratympanic injections in a prospective study. The patients were tested at frequent intervals and followed up for two years. Postural stability was evaluated on posturography. Unsteadiness and gait between attacks, working capacity and vertigo were scored. Before commencing the treatment 47 of the patients complained from moderate to severe postural instability. After treatment moderate to severe postural instability was met in 19 patients. Initially the sway velocity was 27 mm/s on average being significantly worse than in referents. Two weeks after treatment the sway velocity increased to 33 mm/s. Thereafter the sway velocity slowly started to decline to 31 mm/s after two years. Patients with severe attacks of rotatory vertigo had a good postural outcome with GM treatment. In logistic regression analysis the most significant risk factor for poor recovery after GM treatment was severely reduced gait before commencing the treatment with odds ratio of 2.0. Subjectively, after two years, the patients rated their postural stability significantly improved when compared to pretreatment values. The good subjective rating of postural stability is related to postural training program where in the absence of attacks, the patients have learned to cope with daily tasks.
