[Breast asymmetry due to lympho-hemangioma of the left breast. Communication of a case report].

Breast asymmetry is relatively common condition. Mild breast asymmetry usually does not require correction. A rare case of breast asymmetry due to lymphoheamangioma of the left breast operated at 1 and 3 years of age was corrected in two stages. Expander was used during the first stage from the left side combined with the Ribeiro technique to treat the remaining constructed breast tissue. Secondary the expander was replaced with permanent mammary implant and the hypertrophic, ptotic right breast was reduced.

Eighty-four consecutive breast reconstructions using a textured silicone tissue expander.

Breast reconstruction utilizing smooth-surface silicone tissue expanders is associated with certain problems. Significant capsular contracture may develop around the expander with subsequent expander displacement necessitating repositioning or capsulectomy at the time of expander removal. Infection, pain on expansion, chest-wall compression, and complications related to the remote fill valve also have been reported. A textured-surface silicone expander with an integrated fill valve was developed to address these issues. Eighty-four consecutive breast reconstructions were performed by a single surgeon using textured expanders in 55 patients. Intraexpander pressures were measured during expansion for a group of these patients, and both initial and postinflation pressure readings were quite low (average initial pressure 2.88 mmHg; average postinflation pressure 12.87 mmHg). Eighty-one of the reconstructions have been completed without any expander losses and with minimal complications. Tissue expander volume averaged 580 cc, and the expansion duration averaged 5.6 months. Various types and configurations of permanent implants were used to complete the reconstructions. Eighty-four percent of our patients' completed reconstructions (including nippleareola reconstruction and opposite breast alteration, when necessary) were accomplished with just two procedures. Seventeen long-term adjustable textured-surface expander/implants with anatomic breast shape are still in place, all after only one operation. The inframmary fold has generally been established passively by the expansion process alone. Textured-surface silicone expanders have stayed where positioned, have expanded easily with minimal patient pain, and have created a noncontractile soft-tissue cover for the final implant. The textured expander with an integrated fill valve has simplified breast reconstruction by helping to limit the problems encountered with smooth silicone expanders. The resulting reconstructions have remained soft with a very acceptable aesthetic appearance. The patient office and hospital charts and photographs of this consecutive series were notarized and submitted to the Editor of this Journal, whose biostatistician randomly selected the cases illustrated.

[Breast reconstruction with expander prostheses].

Breast reconstruction with tissue expanders is a technique that has been known to clinical practice since 1978. The authors present their clinical experience in the Plastic and Reconstructive Surgery Service of the Virgen del Camino Hospital from 1980 to 2005. They describe breast reconstruction using tissue expanders and the evolution towards immediate breast reconstruction with durable expanders prostheses, which resemble conventional breast prostheses and try to emulate the extirpated breast. They provide an historical review of the repercussion of mastectomy and breast reconstructive surgery in the different professional and social areas, as well as the change in mentality of society and health professionals that has benefited patients. The characteristics of post-mastectomy breast reconstruction, using expanders prostheses, are set out, as well as its advantages and drawbacks.

Elliptical breast reconstruction: an improved and simplified technique.

An improved and simplified postmastectomy reconstruction by tissue expansion is presented. Utilizing a uniquely designed matched tissue expander and prosthesis, the technique is based on an elliptical concept of breast shape and corresponding volumetric relationships and permits controlled expansion of the lower chest skin. Horizontal and vertical base dimensions and the volume of the contralateral breast are used to determine the size and volume of the appropriate elliptically shaped tissue expander and, subsequently, of the mammary implant. This approach permits surgeons to achieve symmetry more readily by matching the implant to the patient, rather than the patient to the implant.

[A case of chemoradiation with tissue expander for local reccurence of rectal cancer].

The patient was a 64-year-old female who underwent an abdomino-perineal resection for advanced lower rectal carcinoma. The tumor was a mucinous adenocarcinoma, type 2 in the Japanese classification of colorectal carcinoma, and was measured 4.5 cm in size. Histologically, the tumor was considered to be stage II (a2, n1, P0, H0). She received adjuvant chemotherapy with 5-FU/LV. After 7 months from the surgery, the patient developed a hip pain. Pelvic CT scan revealed a 4.5 x 6.0 cm mass anterior to the sacrum. Consequently, a diagnosis of local recurrence with sacrum invasion was made. An operation, involving tissue expander placement in the pelvis and pelvic floor was made by dexon mesh, was performed. Radiotherapy was given to the pelvic region at a total dose of 65 Gy (given 2.5 or 3 Gy each x 25 fragments). Chemotherapy with CPT-11 (100 mg/body) and 5'-DFUR (800 mg/body) was administered for 3 cycles. CEA (ng/ml) was decreased within a normal range from maximum 13.8. No progression was detected on CT, and Hip pain had disappeared. Abscess in the pelvis by tumor necrosis was managed conservatively. In this case, tumor marker is within a normal range over 2 years and QOL was improved, suggesting that chemoradiation with a tissue expander may be effective.

[Tissue expansion under the cicatrix].

OBJECTIVE: To investigate a more simple and effective method to repair cicatrix by tissue expansion. METHODS: The dilator with the capacity of 80 - 500 ml was implanted into the subcutaneous pocket under the cicatrix. After dilating for one to two months, the dilator was taken out and the wound surface of the cicatrix was removed. The expanded skin flap was advanced or rotated to cover the defects. The procedure was used on 203 cases. RESULTS: The dilatation was achieved successfully in all the cases, followed by cicatrix removing and repair. The incision scar was not noticeable. CONCLUSIONS: Tissue expansion under the cicatrix has the advantages of safety, less trauma and less extra incisions. It is a reasonable choice to obtain more flexible surgical designs and more economical skin flap applications. It is suitable for most of the treatment for cicatrix.

[Strategic approach to immediate breast reconstruction with submuscular-fascial tissue expanders].

Breast reconstruction can be accomplished with implants, autologous tissue or both. Thanks to the widespread of microsurgical techniques autologous tissue reconstruction has become a frequent option in many departments although it is mostly considered a second choice. The aim of this study was to review our experience covering the past five years in immediate breast reconstruction with tissue expanders. From January 2000 to January 2005 279 patients underwent immediate breast reconstruction with submuscular tissue expanders. Tissue expander was chosen according to dimension, shape and volume of the healthy breast. Operating time, early and late complications, interference with neo-adjuvant therapies as well as timing and surgical techniques of second-step reconstruction were also considered. No major complications occurred. Tissue expander, beyond proving itself a valid and safe option in immediate breast reconstruction, also decreased the psychological distress following a mastectomy.

[The relationship between the changes in the blood flow of the exceedingly expanded skin and the viability of the flap].

OBJECTIVE: To investigate the relationship between the blood flow of the exceedingly expanded skin and the viability of the flap. METHODS: The piglets were set up as the animal model. They were divided into 7 groups according to different injection volume of the tissue expander, i. e. injection to 1/2 content, 3/4 content, full content, 50% over content, 100% over content, sham-operation control and normal control. At different stages of expansion, the blood flow and the amplitude of the expanded skin were examined by laser Doppler. The survival length of the expanded flap was measured. Furthermore, statistical analysis was performed. RESULTS: As the expansion volume increasing, the blood flow and the amplitude of the expanded skin increased, the survival length of the expanded flap increased also. In the meantime, the amplitude instability and vessel permeability increased. When the injection volume exceeded to 100%, the blood flow and the survival length of the expanded flap decreased. CONCLUSIONS: In soft tissue and skin expansion, increasing injection volume could increase the survival length of the expanded flap, while over-expansion to 100% content may cause blood flow disturbance.

[Repair of skin soft tissue defects with new overlapping tissue expansion techniques].

OBJECTIVE: To improve the efficiency of skin soft tissue expansion with the overlapping tissue expansion techniques. METHODS: From June 2003 to March 2005, 5 cases of skin soft tissue defects were treated with the overlapping tissue expansion technique-two overlapped expanders in one soft tissue pocket, which was different from the traditional technique--one expander in one soft tissue pocket. Five patients included 3 males and 2 females, aging from 11 to 28 years. The defect was caused by scar of forearm in 2 cases, by melanotic nevus in 1 case and by cicatricial baldness in 2 cases. The disease course was 1.5 to 24 years. The defect size ranged from 12 cm X 5 cm to 13 cm X 12 cm. RESULTS: Skin expansion process was satisfactory and skin defect was completely repaired with the expanded skin tissue in one operation in 5 cases. After operation, the wound of donor-recipient site healed by first intention. All patients were followed up from 3 to 15 months, no contracture, pigmentation and scar occurred at the expanded skin area. The long-term appearances were satisfactory. CONCLUSION: Compare with the traditional tissue expansion techniques, the new overlapping tissue expansion techniques can apparently improve the efficiency of skin soft tissue expansion. It is suitable for the patients whose expandable skin is limited or no more skin tissue can be dissected near the skin defect and who need more expandable skin to repair skin defect.

[Kyphoplasty using an enhanced balloon expander: an experimental study].

OBJECTIVE: To test the efficacy of kyphoplasty using an enhanced balloon expander in restoring the height of vertebral compression fractures (OVCF). METHODS: Fifteen lumbar vertebral bodies (L1-L5) were harvested from 3 young male fresh cadavers and separated into individual vertebral bodies with the bilateral pedicles of the vertebral arch removed. Before operation, plain X-ray films of all the vertebral bodies were obtained. All the vertebral bodies were compressed lengthwise to approximately 80% of their original heights using a universal material-testing machine to result in compression fractures. Post-compression vertebral bodies were then repaired using an enhanced balloon expander, and the delivery of the bone cement into the vertebral bodies was observed. The heights of the anterior and posterior borders of the vertebral bodied were measured before and after compression as well as after kyphoplasty. RESULTS: The inflation of the balloon expander averaged 2.95-/+0.18 ml and the pressure was 122.67-/+27.89 psi (1 psi=6895 Pa). Kyphoplasty resulted in significant restoration of the vertebral body height lost due to the compression (P<0.01). CONCLUSION: Kyphoplasty using an enhanced balloon expander may restore vertebral body height damaged by compression and correct the kyphotic deformity. The balloon expander can be a effective and economic choice for kyphoplasty for its relatively low cost.

Reconstruction of facial burn sequelae utilizing tissue expanders with embodiment injection site: case report.

Although highly specialized burn centers have significantly reduced mortality rates following extensive total body surface area burns, survivors are often left with grotesque facial disfigurement. Hypertrophic scars and tissue defects are the most common cause of functional and aesthetic problems in the head and neck region. Plastic surgeons use full-thickness or split-thickness skin grafts, pedicled flaps, free flaps, transplantation of bone or cartilage and tissue expansion. The authors present a case of a patient who suffered from third-degree flame burns to the face. Prior skin grafting procedures left him with severe scar deformity of the face. The patient was treated utilizing multiple tissue expansion. Facial animation has retained and facial integrity has been aesthetically restored and, with the use of make-up, it is near normal in social settings at conversational distances. The tissue expansion technique is advantageous in facial reconstruction because it makes it possible to resurface even wider defects with neighboring skin, similar in colour and texture, and superior to skin obtained elsewhere.

[Contribution of external valves in skin expansion].

The filling period is a delicate phase of skin expansion procedures. Many of these difficulties are due to problems encountered during puncture of the valves when they are buried in the skin (pain, leaks, loss of the valve, inverted valve). The authors report their experience of the use of external filling valves. Based on a series of 68 valves, they describe the implantation technique, indications and their results.

[Rapid tissue expansion: value of the measurement of the transcutaneous oxygen pressure and intraprosthetic pressure].

Tissue expansion is often unpleasant for the patient because of its long duration. The authors present a series of 16 rapid expansion prostheses implanted in 10 patients. The expansion was monitored by measuring the percutaneous oxygen partial pressure and intraprosthetic pressure. The expansions were performed without any complications. Monitoring of the expansion by measuring the pcPO2 appears to ensure good results.

[Prevention and treatment of postoperative complications after skin soft tissue expansion for scar alopecia].

OBJECTIVE: To summarize the prevention and treatment of postoperative complications after the skin soft tissue expansion for scar alopecia. METHODS: From January 1995 to June 2005, 57 patients with scar alopecia were admitted to our department for treatment. Of the patients, 25 were males and 32 were females with their ages ranging from 5 to 55 years. The causes were burn in 33 patients, trauma in 14, alopecia after head surgery in 8, and other causes in 2. Their disease courses ranged from 6 months to 15 years. For treatment, 89 therapeutic expanders were utilized in 57 patients. The retrospective analysis on the complications and their prevention and treatment were performed. RESULTS: The follow-up for 3-12 months averaged 6 months revealed that 81 areas undergoing the expander insertion healed well and the hair grew well, too. Eight areas undergoing the expander insertions had complications, including expander exposure in 2 patients, infection in 2, hematoma in 1, expander rupture in 1, necrosis of the flap tip in I, and scar necrosis at the injection port in 1. The results also revealed that there was a significantly increased rate of complications in the patients aged 5-10 years and the patients older than 50 years (P<0. 05). The complication rate in the patients who received 2 expanders at one time was significantly higher than that in the patients who received only 1 expander (P<0. 05). However, there was no significant difference in the complication rate in the other kinds of patients. All the complications were effectively treated with a satisfactory therapeutic result. CONCLUSION: The skin soft tissue expansion for scar alopecia can effectively prevent and treat postoperative complications. If the complications are identified early and treated properly, the therapeutic results will be satisfactory.
