Curative treatment of peritoneal carcinomatosis arising from colorectal cancer by complete resection and intraperitoneal chemotherapy.

BACKGROUND: Peritoneal carcinomatosis (PC) is fatal, despite standard systemic chemotherapy. A new approach that combines maximal surgery with maximal regional chemotherapy has potential to cure selected patients who have colorectal PC. The authors have reported the oncologic results of this combined treatment. METHODS: The authors performed a retrospective study of 64 patients who had PC arising from colorectal adenocarcinomas, 19 (29.6%) of whom also had other metastases. These patients were treated by complete resection of all detectable tumors and by a 5-day course of early intraperitoneal chemotherapy (EPIC) with mitomycin C, then by 5-fluorouracil (n = 37), or by intraoperative intraperitoneal chemohyperthermia (IPCH) with mitomycin C, alone or combined with cisplatin (n = 27), in 2 separate trials. In the trial of IPCH, aimed at selecting the most reliable procedure in terms of spatial diffusion and thermal homogeneity, the 27 patients were treated with 7 different procedures. The extent of PC was assessed precisely by using a peritoneal index. The median follow-up period for the entire patient population was 51.7 months. RESULTS: The postoperative mortality and morbidity rates were 9.3% and 54.6%, respectively. Most severe complications occurred in patients who required extensive cytoreductive surgery. Global and disease-free survival rates were respectively 60.1% and 54.7% at 2 years and were 27.4% and 18.4% at 5 years. Results were significantly better (P = 0.04) when patients were metastasis-free (apart from PC) and when the peritoneal index was lower than 16 (P = 0.005). IPCH seemed to be more effective than EPIC for treatment of PC. CONCLUSION: This treatment plan, which combined maximal surgery with maximal regional chemotherapy, cured approximately 25% of patients. This strategy was mainly applicable to patients with limited intraperitoneal cancer volume and no extraperitoneal involvement. IPCH proved to be more effective than EPIC but more difficult to use correctly. Future results should improve through routine use of the optimal hyperthermia procedure, with improvements in the composition of instillate, better patient selection, and the reduction in the rate of complications that occurs with physician experience.

Analysis of limiting factors of DNA band separation by a DNA sequencer using fluorescence detection.

The factors affecting the electrophoretic separation of DNA bands in DNA base sequencing using fluorescence detection are analyzed. All the factors contributing to DNA band spacing and band width are evaluated; DNA diffusion and thermal effects on gels are the main considerations. The dependence of the gel's electrical resistivity on gel temperature and the variation of temperature over gel thickness are associated with a broadening of DNA band width. As a result of the analyses the maximum separable base number is represented as a function of various electrophoretic variables. The best separations are possible with an electric field strength corresponding to gel thickness. The maximum separable base number increases as the gel thickness decreases. It also increases as the migration distance increases, but it becomes saturated and has an upper limit when the migration distance is long. This upper limit increases as gel thickness decreases. DNA fragments with 600 and 601 bases can be completely separated from each other under optimum conditions for a 0.2 mm thick gel plate. Furthermore, using the band spacing information, under the same conditions, 750 bases could be assigned separately.

Synthesis and Characterization of Basic Catalysts Based on Sodium-Magnesium Mixed Phosphates and Their Use in the Conversion of 2-Hexanol.

Various catalysts consisting of sodium-magnesium mixed orthophosphates were synthesized and characterized in structural terms using X-ray diffraction, thermogravimetric analysis, differential thermal analysis, diffuse reflectance infrared spectroscopy, and solid-state (31)P NMR spectroscopy. In addition, the surface properties of the solids were determined from N(2) adsorption-desorption isotherms and their chemical properties characterized with various titrants. One of the solids obtained consists of NaMgPO(4)/Mg(2)P(2)O(7) and the rest of NaMgPO(4)/MgO. Their surface chemical properties vary depending on the particular synthetic procedure used; thus, the NaMgPO(4)/MgO systems exhibit similar populations of acid and basic sites, whereas the NaMgPO(4)/Mg(2)P(2)O(7) systems contain many more acid sites than basic sites. All the solids obtained exhibit substantial dehydrogenating activity in the conversion of gaseous 2-hexanol; in fact, some of the NaMgPO(4)/MgO systems are even more active and selective than a MgO solid tested under identical reaction conditions. Characterisation of nimesulide-betacyclodextrins systems prepared by supercritical fluid impregnation.

The purpose of this study was to apply the supercritical CO(2) impregnation process for preparing solvent-free nimesulide (NMS)-betacyclodextrins (BCD) association systems with enhanced drug dissolution rate. Several drug-to-carrier molar ratios were tested (1:1; 1:2.5; 1:3.5) at different conditions of temperatures (40, 100, and 130 degrees C) and pressures (140, 190 or 220 bar). The physical and morphological characterisation of the systems using powder X-ray diffraction, thermal analysis, diffuse reflectance Fourier transform-infrared spectroscopy and scanning electron microscopy was carried out to understand the influence of this technological process on the physical status of single components and binary systems and to detect possible interactions between drug and carrier. These analyses provided no evidence of a complete inclusion of NMS in the carrier but the existence of interactions between drug and carrier together with a partial dehydration of the BCD and the formation of drug crystallites with lower melting point and heat of fusion than the native NMS. These phenomena were more intense when severe conditions of pressure and temperature (220 bar and 130 degrees C) were used during impregnation trials and when the amount of BCD augmented in the systems. These activated solid state of the impregnated systems promoted an enhancement of drug dissolution rate that, in keeping with the results of the physical characterisation, was function of the process conditions and BCD content.

Development of a passive sampler to measure personal exposure to gaseous PAHs in community settings.

A sensitive, simple, and cost-effective passive sampling methodology was developed to quantify personal exposure to gaseous polycyclic aromatic hydrocarbons (PAHs). A Fan-Lioy passive PAH sampler (FL-PPS) is constructed from 320 sections of 1-cm long SPB-5 GC columns (0.75-mm i.d. and 7-microm film thickness), similar to a mini-honeycomb denuder. Given the unique feature of the GC column stationary phase, gaseous PAHs are collected on the inner surfaces of the columns by molecular diffusion and thermally desorbed to GC/MS for analysis. The sampling rates of FL-PPS were determined in the laboratory using a controlled test atmosphere containing eight PAHs for a range of face velocity, temperature, relative humidity, PAH concentration, and sampling duration. The sampling rate (mean, %RSD, cm3/min) was 26.7 (21%) for acenaphthylene, 37.6 (25%) for acenaphthene, 56.2 (13%) for fluorene, 49.1 (25%) for phenanthrene, 62.7 (22%) for anthracene, 65.4 (24%) for fluoranthene, and 64.4 (18%) for pyrene over a sampling duration of 8-48 h. The sampling rate for naphthalene was approximately 14.1 (12%) cm3/min over a sampling period of 8 h but decreased along with an increase of sampling time. The effects of temperature, humidity, face velocity, and PAH concentration on the sampling rate were not significant for all the compounds tested. A reasonable agreement (<30%) was obtained for most compounds measured by FL-PPS and a conventional active PAH sampling method colocated side-by-side in the field, but a sampling time of 24 h or longer was required for detection of less abundant PAHs in community settings.

Raman microspectroscopic study on polymerization and degradation processes of a diacetylene derivative at surface enhanced Raman scattering active substrates. 2. Confocal Raman microscopic observation of polydiacetylene adsorbed on active sites.

Confocal Raman microscopic measurements were performed at room temperature on the Langmuir-Blodgett (LB) monolayer of 10,12-pentacosadiynoic acid (DA) prepared on surface enhanced Raman scattering (SERS) active Ag island films, two-dimensional (2D) Raman images of which exhibit bright and dim spots on a dark background. The measurements performed by focusing the excitation laser light (488 nm) on the dark background indicate the prompt appearance of the Raman bands (1515 and 2115 cm(-1)) due to polydiacetylene (PDA) in the red phase and subsequent diminution of the Raman bands. On the other hand, the spectra observed by focusing the excitation laser spot on the dim and bright spots exhibit almost random fluctuations, giving rather narrow Raman bands in the 1620-1000 cm(-1) region, which appear and disappear temporarily with varying intensities under the continuous irradiation at 488 nm. Broad Raman bands appear around 1580 and 1360 cm(-1), which are ascribable to amorphous carbon, at a later stage of the observation, the intensities from the bright spot being more than 100 times stronger than those from the dim spot. The narrow bands are ascribed to a series of carbonaceous intermediates such as polyenes, graphite sheets with various sizes, and folded or reorganized forms of the sheets including carbon nanotubes and fullerenes, which are formed during the conversion of PDA to amorphous carbon. The random spectral fluctuation was interpreted by considering that the intermediates undergo thermally activated diffusion and get temporarily in contact with the SERS-active site, resulting in the enhancement of their Raman bands and the fluctuation.

Role of O2 in inducing intensive fluctuations of surface-enhanced Raman scattering spectra.

Confocal Raman microscopic measurements were performed on silver electrodes covered with hydrogenated amorphous carbon (a-C:H). When short accumulation time was used, the subsequently measured surface-enhanced Raman scattering (SERS) spectra exhibited fluctuations. As previously reported for other systems, the intensity of fluctuations of SERS spectra significantly decreases if O2 was removed from the ambient medium. In this contribution we show that intensive SERS fluctuations can be also observed for a-C:H/Ag samples immersed in the deoxygenated electrolyte after applying a negative potential pulse to the silver electrode. It means that the O2-mediated Burstein mechanism of SERS fluctuations, which has been previously proposed to explain the SERS O2 effect, is not adequate for these results. We suggest that oxygen chemisorbed on the silver surface decreases the average strength of the interaction between a-C:H clusters and the metal surface (and hence the speed of movement of a-C:H clusters across the metal surface) and that the SERS O2 effect should be rather explained using the "classical" model of SERS fluctuations, in which fluctuations are interpreted as a result of the thermally activated diffusion of carbon segments in and out of the SERS "hot spots". A numerical algorithm for modeling of the fluctuations of SERS intensity has been proposed, and some example simulations of SERS fluctuations have been carried out. For the first time, strongly fluctuating bands due to the stretching vibrations of significantly weakened C-H bonds have been identified.

Relationship between structural order and the anomalies of liquid water.

In contrast to crystalline solids--for which a precise framework exists for describing structure--quantifying structural order in liquids and glasses has proved more difficult because even though such systems possess short-range order, they lack long-range crystalline order. Some progress has been made using model systems of hard spheres, but it remains difficult to describe accurately liquids such as water, where directional attractions (hydrogen bonds) combine with short-range repulsions to determine the relative orientation of neighbouring molecules as well as their instantaneous separation. This difficulty is particularly relevant when discussing the anomalous kinetic and thermodynamic properties of water, which have long been interpreted qualitatively in terms of underlying structural causes. Here we attempt to gain a quantitative understanding of these structure-property relationships through the study of translational and orientational order in a models of water. Using molecular dynamics simulations, we identify a structurally anomalous region--bounded by loci of maximum orientational order (at low densities) and minimum translational order (at high densities)--in which order decreases on compression, and where orientational and translational order are strongly coupled. This region encloses the entire range of temperatures and densities for which the anomalous diffusivity and thermal expansion coefficient of water are observed, and enables us to quantify the degree of structural order needed for these anomalies to occur. We also find that these structural, kinetic and thermodynamic anomalies constitute a cascade: they occur consecutively as the degree of order is increased.

The gated gait of the processive molecular motor, myosin V.

Class V myosins are actin-based molecular motors involved in vesicular and organellar transport. Single myosin V molecules move processively along F-actin, taking several 36-nm steps for each diffusional encounter. Here we have measured the mechanical interactions between mouse brain myosin V and rabbit skeletal F-actin. The working stroke produced by a myosin V head is approximately 25 nm, consisting of two separate mechanical phases (20 + 5 nm). We show that there are preferred myosin binding positions (target zones) every 36 nm along the actin filament, and propose that the 36-nm steps of the double-headed motor are a combination of the working stroke (25 nm) of the bound head and a biased, thermally driven diffusive movement (11 nm) of the free head onto the next target zone. The second phase of the working stroke (5 nm) acts as a gate - like an escapement in a clock, coordinating the ATPase cycles of the two myosin V heads. This mechanism increases processivity and enables a single myosin V molecule to travel distances of several hundred nanometres along the actin filament.

Re-examining the origins of spectral blinking in single-molecule and single-nanoparticle SERS.

Single metal nanoparticles and nanoaggregates are known to emit intense bursts of surface-enhanced Raman scattering (SERS) in an intermittent on and off fashion. The characteristic "blinking" timescales range from milliseconds to seconds. Here we report detailed temperature dependence (both heating and cooling) and light-intensity studies to further examine the origins of this intriguing phenomenon. The results indicate that blinking SERS contains both a thermo-activated component and a light-induced component. Several lines of evidence suggest that the observed fluctuations are caused by thermally activated diffusion of individual molecules on the particle surface, coupled with photo-induced electron transfer and structural relaxation of surface active sites or atomic-scale roughness features.

[Surgery with the laser scalpel. Physical principles and clinical effectiveness].

BACKGROUND: The conventional surgical method for cutting vascularized tissue with scalpel and scissors may now be improved by use of the laser scalpel. In particular, the frequent interruption necessitated and the poor visibility caused by bleeding may be mitigated owing to the haemostatic properties of laser light. This instrument may be of particular value for tasks involving extrabulbar structures, such as extraocular muscles and lacrimal tissue, as well as for facial surgery. MATERIALS AND METHODS: The mode of action and physical properties of a new laser scalpel are described and its characteristics compared with those of other instruments on the market. The probe consists of a clad, sculptured silica fibre with a core diameter of 0.6 mm and a conical 0.15 mm-diameter cutting tip. Radiation generated by a low cw-Nd:YAG laser module is fed into the probe, at the exit point of which a maximal power density of 57 kW/cm2 is attained. Radiated laser energy penetrates the tissue as an incision is made, thereby inducing an efficient blood flow stasis which is amplified by thermal energy diffusing from the immediate surroundings of the scalpel tip. RESULTS: In this report, the laser scalpel is implemented for the excision of a vascularized, amelanotic, facial naevus. Both the cutting and haemostatic effects were found to be excellent, occurrence of the latter phenomenon being supported by ultrastructural findings. The healing response was comparable to that observed after conventional surgery. CONCLUSION: The new laser scalpel represents an inexpensive and effective cutting and haemostatic tool powered by a standard cw Nd:YAG laser module, with a wide spectrum of potential applications.

Effect of cardiac output on extravascular lung water estimates made with the Edwards lung water computer.

The Edwards lung water computer system uses the thermal-dye indicator technique to estimate the lung extravascular fluid volume (EVLW). The authors tested the effect of changes in cardiac output (CO) on EVLW estimates made with the lung water computer in six dogs anesthetized with halothane. Baseline CO was 2.5 +/- 1.3 l/min (mean +/- SD); CO subsequently was increased either by 220% or decreased by 70% by either giving 0.5 mg/kg of isoproterenol or increasing the inspired halothane (1-4%), respectively. There was a significant correlation between the estimated EVLW and CO in each animal (P less than 0.05) such that a 50% decrease in CO from baseline caused an approximately 40% increase in estimated EVLW. Postmortem examination showed that the lungs were not edematous, even though the lung water computer data indicated that severe pulmonary edema had developed at reduced COs. At increased COs, estimated EVLW decreased. The authors conclude that the Edwards lung water computer overestimates lung water, possibly because the thermal indicator diffuses into nonpulmonary as well as pulmonary tissue. The overestimate is greatest at low cardiac outputs.

Relationship between global and cortical cerebral blood flow in patients with head injuries

OBJECTIVE: The goal of this study was to analyze the use of thermal diffusion-cerebral blood flow (TD-CBF) monitoring as a continuous monitoring method for patients with head injuries in the intensive care unit. METHODS: The TD-CBF probe was placed on normal-appearing frontal or parietal cortex at the time of surgery to treat traumatic intracranial hematomas in 35 patients, and cortical cerebral blood flow (CBF) was monitored for up to 7 days after surgery. We compared TD-CBF values with global CBF values measured by the standard Kety-Schmidt technique, and we compared changes in TD-CBF with changes in jugular venous oxygen saturation observed during intracranial pressure elevations. RESULTS: The average value for the global CBF measurements was 50.5+/-0.9 ml/100 g/min and that for the TD-CBF measurements was 60.5+/-1.4 ml/100 g/min; the average difference was 9.3+/-1.2 ml/100 g/min. The overall slope of the regression between the global CBF and TD-CBF measurements (n = 206) was 0.636 (comparison of observed slope with a slope of 0, P < 0.001). The relationship between the TD-CBF and global CBF values during 546 episodes of increased intracranial pressure was examined by comparing the changes in TD-CBF with the changes in jugular venous oxygen saturation. When the change in TD-CBF was at least 10 ml/100 g/min during an intracranial pressure elevation, the TD-CBF change reflected the change in jugular venous oxygen saturation on 85% of the occasions. CONCLUSION: The TD-CBF method is very convenient because of the continuous and automatic nature of the measurements. Most of the time, a change in TD-CBF indicated a similar change in global CBF. However, the limitations of local measurements of CBF must be kept in mind during therapeutic decision-making.

Nondissipative diffusion of lattice solitons out of thermal equilibrium.

We perform Langevin dynamics simulations for pulse solitons on atomic chains with anharmonic nearest-neighbor interactions. After switching off noise and damping after a sufficiently long time, the solitons are only influenced by the thermal phonon bath which had been created by the noise. The soliton diffusion constant D is considerably smaller than before the switch-off, and it is proportional to the square of the temperature T , in contrast to the diffusion due to the noise which is proportional to T . We derive a diffusion equation for a soliton which is scattered elastically in an ensemble of phonons and derive general expressions for D and for the drift velocity v(d) . These expressions can be evaluated for the case of the Toda lattice for which the soliton shift due to the phonon scattering is known explicitly. D is indeed proportional to T2 and agrees well with the simulation results, while v(d) is much smaller than the soliton velocity and cannot be measured in the simulations due to the large fluctuations of the soliton position. We express D in terms of soliton characteristics which are known also for solitons on other anharmonic chains in the continuum limit: namely, velocity, amplitude, and width. The results agree well with the simulations if the soliton shape is the same as in the Toda case. If the shape is different, only an estimate of the order of magnitude can be given.

Onset of convection in Soret-driven instability.

It is common knowledge that light fluids rise while heavy fluids sink in the gravity field. The most obvious case is the isothermal Rayleigh-Taylor instability when a heavy fluid is placed on top of a light one. In the nonisothermal case, while heating from above, the density stratification is stable in a pure liquid. However, unstable density stratification might be established in a binary mixture with a negative Soret effect in the case of heating from above: the heavier liquid is accumulated on the top of the lighter one. Due to the large differences between viscous, thermal, and diffusion times the system has a tendency to fingering buoyant instabilities. At some moment the flow may be initiated. Near the onset of convection the flow pattern has a columnar convective structure: for a relatively low applied temperature difference Delta T the lighter and colder liquid is drawn up in the central part of the cell and the heavier liquid flows down along the walls. For finite size systems the situation is reversed at higher Delta T. Here we present results of three-dimensional direct numerical simulations of heat and mass transfer in a system with a negative Soret effect. While the development of Soret-induced convection is similar for a wide class of liquids: water based mixtures, colloidal, and polymer solutions, the parameters of the chosen system correspond to a realistic binary mixture of water (90%) and isopropanol (10%) enabling comparison of theoretical predictions with planned experimental studies.

Compositional patterning in systems driven by competing dynamics Of different length scale

We study an alloy system where short-ranged, thermally driven diffusion competes with externally imposed, finite-ranged, athermal atomic exchanges, as is the case in alloys under irradiation. Using a Cahn-Hilliard-type approach, we show that when the range of these exchanges exceeds a critical value labyrinthine concentration patterns at a mesoscopic scale can be stabilized. Furthermore, these steady-state patterns appear only for a window of the frequency of forced exchanges. Our results suggest that ion beams may provide a novel route to stabilize and tune the size of nanoscale structural features in materials.

Driven alloys in the athermal limit.

Static molecular simulations of binary alloys under extrinsic forcing show that complex ordered or segregated structures may evolve even in the absence of thermally activated diffusion. This result is in opposition to the standard theoretical framework for so-called "driven alloys," which assumes that extrinsic driving is an ideally randomizing process, and therefore predicts only random atomic configurations in the athermal limit. We propose a qualitative modification to the theory that introduces a new control parameter and use additional Monte Carlo simulations to demonstrate the physical plausibility of this modification. New research directions in nonequilibrium dynamic systems are also suggested by this analysis.

Nonlinear responses of electronic-excitation-induced phase transformations in GaSb nanoparticles.

We studied electronic-excitation-induced phase transformations in nanoparticles using transmission electron microscopy. GaSb particles excited by 75 keV electrons transform to two phases consisting of an antimony core and a gallium shell or an amorphous phase, or remain in the original crystalline phase, depending on particle size and/or temperature. It is suggested that such nonlinear responses of the phase transformations may arise from synergistic effects of bond instability, localized excitations, enhanced diffusivity, or thermal equilibrium in reactions.
