Porous titanium tooth roots: clinical evaluation.

1. An inert porous vehicle can be successfully used to attach endosseous dental implants to surrounding alveolar bone. 2. Implant size and the width of keratinized gingiva surrounding the implant are critical to the success of the implant. 3. Radiography and pocket depth are useful indicators of the status of porous endosseous roots, while mobility is not.

Technique for making a customized shade guide.

A technique has been presented for making a custom shade guide that provides a direct visual means for conveying color information to the laboratory technician. The technique consists of applying pigment directly to acrylic resin denture teeth using colored drawing pencils. This technique was originated by Dr. Royal Norman of Omaha, Nebraska. One of the authors (S. W. A.) first saw Dr. Norman present it to the USAF Area Dental Laboratory Workshop, Westover AFB, Mass., in 1969.

Stabilized occlusion rims for small interarch spaces.

This article describes three methods for fabricating stabilized occlusion rims for close interarch spaces. To utilize these methods, the dentist must anticipate the lack of space and request that the rims be fabricated by one of the methods outlined prior to the maxillomandibular relations appointment.

Relationship of the incisive papilla to the maxillary central incisors.

The average distance between the most anterior point of the maxillary central incisors and the most posterior point of the incisive papilla was 12.454 mm with a standard deviation of 3.867 mm. This distance was measured when these two points were projected on a plane which was parallel to the reference plane formed by the tips of three interdental papillae; i.e., the papilla between two central incisors (A), between the first and second molars on the right side (R), and on the left side (L). The average error incurred due to inconsistency of the method employed was less than 3% or less than 0.372 mm for the position of the central incisor. It is believed that the application of this anatomic relation can provide a reliable point for arranging and checking the position of the anterior maxillary teeth for complete dentures.

The effect of relining on the accuracy and stability of maxillary complete dentures--an in vitro and in vivo study.

A technique was developed for evaluating in vitro the accuracy or fit of experimental denture bases using a low-viscosity impression material for determining the space between the master die and the processed denture base. These measurements indicated that relining heat-cured maxillary denture bases with autopolymerizing acrylic resin improved their adaptation to the ridges. Interestingly, if no teeth were present in the heat-cured denture base, markedly less distortion occurred after processing. Limited clinical measurements on the stability of maxillary dentures in function showed no statistically significant change in stability after relining, but the trend was toward increased stability with the relined denture.

A transitional anterior fixed prosthesis using composite resin.

A method has been described to temporarily restore an anterior edentulous space in a single appointment. The technique uses a composite resin to retain a resin denture tooth (pontic) supported by edgewise orthodontic wire. This approach is suggested as a transitional treatment for patients who are not amenable to either a definitive fixed or removable prosthesis.

A bilaterally balanced occlusal scheme for patients with arch width and curvature discrepancies.

A technique is described that will allow the patient maximum tooth contact in bilateral balanced occlusion without compromising esthetics or mechanics for a patient with an underdeveloped retrognathic and normal or oversized maxillae (Figs. 4 and 5). A disadvantage is maintaining the upper posterior teeth in the mold during the boil-out procedure, but this problem may be minimized with careful execution of the laboratory procedures.

Verification appointment in complete denture therapy.

Completed dentures should demonstrate the beneficial effects of an ordered, disciplined approach to the "try-in" appointment. The proposed design of the trial dentures for this appointment allows accurate verification of spatial positions, and easy access to changes in position and arrangement of the teeth. Two phases of the verification appointment should be recognized: laboratory and clinical. In the laboratory phase, the teeth are arranged according to anatomic landmarks, and the trial denture is designed to allow accurate verification of previous records. In the clinical phase, the intermaxillary dimensions are verified or corrected, and the teeth are rearranged for support of the lips and optimal esthetics. The design permits simple and effective changes in the spatial position of the teeth. When the try-in or verification appointment is carefully planned and the list of verification checks made in a disciplined and orderly manner, this appointment can progress to a meaningful and predictable conclusion.

A review of the submerged-root concept.

The purpose of this article was to review and summarize the literature on the retention of submerged roots. It appears that the submerged-root procedures may have an accepted place alongside conventional overdenture techniques in the profession's continuing effort to retard alveolar bone resorption in the field of prosthodontics.

The face-form as a guide for the selection of maxillary central incisors.

The outline forms of both maxillary central incisor teeth were compared with the "actual" and "apparent" face-forms in 70 subjects, to determine whether a close similarity of face-forms and tooth forms exists, which would support the "law of harmony" method for the selection of replacement of teeth. The results of the investigation do not support the contention of Williams but rather invalidate this method of teeth selection because more than two-thirds of the individuals showed no similarity between face-form and incisor tooth form.

Posterior peripheral seal distortion related to height of the maxillary ridge.

A statistically significant positive relationship exists between ridge height of a cast and dimensional changes in the posterior peripheral seal region of its denture base. This dimensional change might be clinically insignificant, but it should not be ignored. The relationship between distortion of the denture base and composition of the denture teeth is not significant. The relationship of distortion of the denture base at the region of the posterior peripheral seal and variables of ridge height and tooth composition were studied. A positive relationship exists between ridge height and distortion, but no significant relationship was found between dimensional change and the use of porcelain or acrylic resin teeth. Using computer analysis, an equation, table, and curve were derived for values of expected dimensional change in the posterior peripheral seal region related to various ridge heights.

Salvaging terminal dentitions with convertible periodontal prostheses.

An alternative to the complete denture, the transitional prosthesis, the overdenture, and some tooth-splinting clasped partial dentures has been presented. The convertible periodontal prosthesis has many of the advantages of the overdenture, but few of the disadvantages such as palatal coverage, the appearance of a denture, and the psychologic effect. Preliminary observations over a 7-year period indicate that the periodontal condition of the abutment teeth improves. A case for periodontal worthiness of this method of periodontal prosthesis can be made.

A new implant philosophy.

This form of tooth implant appears to exhibit great potential since it eliminates the disorganized connective tissue layer present at the implant/bone interface of most other devices. Also, as established for other implants, as long as a tight cuff of relatively immovable tissue exists around the polished cervical collar of an implant, an effective seal against the ingress of oral fluid and toxic irritants appears to be maintained, although the nature of this seal has yet to be determined. In addition, the epimobile superstructure serves to dissipate occlusal stress on the "ankylosed" implant. The prognosis of an implant is determined in part by its associated superstructure. There is a need for research into the loading that can be accommodated by an implant. Further study is required to ascertain the full potential of this implant system.
