The completion of the transcontinental railroad in 1869 was a milestone in American history. Seventy years later in August 1939, the westbound, new Southern Pacific Railroad streamlined passenger train "City of San Francisco," was intentionally derailed at high speed in northern Nevada by saboteurs who had moved a rail out of alignment. This resulted in a spectacular wreck, claiming the lives of 24 passengers and crew. Despite the finding of physical and trace evidence; and lengthy, exhaustive investigations by railroad police, the FBI, and forensic scientists, the perpetrators were never brought to justice. Deaths resulting from vandalism and sabotage are among the least common of railroad-related deaths; however, acts of vandalism and violence including train derailment are increasingly frequent in this country. Law enforcement officers and forensic scientists must be able to place these incidents in their proper perspective and to successfully investigate and solve them by employing all means at their disposal.
In early March 1989, China's General Office of the State Council issued an urgent Circular demanding that various local governmental bodies "do a good job in strictly controlling the blind outflow of laborers." The circular pointed out that "since the Spring Festival, large numbers of laborers from Sichuan, Henan, Hubei, Shandong, Shaanxi, Jiangsu, Zhejiang, Anhui, and other provinces have concentrated in large numbers in regions such as the northwest, the northwest, and Guangdong Province, causing a huge increase in railroad passenger traffic. There has been a large pile-up of passengers on some main railroad lines and stations, and trains have been seriously overcrowded. This has put tremendous pressure on railroad transport. After arriving in the above mentioned regions, some of these laborers hang around the streets because they cannot find work, and their life is extremely difficult. The large influx of laborers into these regions has caused confusion in local social order." In order to resolve this problem satisfactorily, the circular makes the following demands: "The people's government at all levels must rapidly get under control the blind outflow of laborers and their assembly in large numbers for moving elsewhere. It is necessary to organize forces to admonish and stop those laborers who have already assembled at the railroad stations, so that they will not blindly move elsewhere. They should also be mobilized to return to their home towns."
BACKGROUND: The extent that cigarette smoking may confound the relationship between diesel exhaust exposure and lung cancer was assessed in a retrospective cohort study of 55,395 U.S. railroad workers followed from 1959 to 1976. METHODS: The relative risk (RR) of lung cancer due to diesel exhaust was indirectly adjusted using job-specific smoking data from a case-control study of railroad workers who died between 1981-1982 and from a survey of 514 living workers from an active railroad in 1982. Adjustment factors were developed based on the distribution of job-specific smoking rates. RESULTS: The unadjusted RR for lung cancer was 1.58 (95% CI = 1.14-2. 20) for workers aged 40-44 in 1959, who experienced the longest possible duration of exposure, and the smoking adjusted RR was 1.44 (1.01-2.05). CONCLUSIONS: After considering differences in smoking rates between workers exposed and unexposed to diesel exhaust in a relatively large blue-collar cohort, there were still elevated risks of lung cancer in workers in jobs with diesel exhaust exposure.
We are reporting the case of a 29 year old male in whom we performed successful reimplantaton of both lower legs following trauma inflicted by a railroad boxcar. Five years after this accident, the patient's walk is almost normal and both deep sensitivity and two point discrimination on the soles of his feet are sufficient. The patient can walk, run and stand very well on one leg, both on even and on uneven ground.He returned to his job with the railroad 8 months after his accident. Originally the patient was employed as a railroad workman, and is now an office employee. His private life is normal and he enjoys hiking and dancing. In our opinion, sufficient function of the tibial nerve in the reconstructed extremity is important for clinically satisfactory long-term results. Both the Mangled Extremity Severity Score (MESS) and the NISSSA are helpful in making the decision on whether to primarily amputate or reconstruct Gustillo IIIC cases. Good long-term results as well as general cost reduction are achievable following reconstruction of extremities. Amputation of an extremity can be predicted with 100% certainty when MESS is 9 or more. Primary shortening and secondary lengthening of an extremity is a good method of treating Gustillo III C fractures.
