The case of a five year old girl with a bifid thumb is presented. The radial thumb showed opposition function at the metacarpal but the phalanges were crooked and stiff. The ulnar thumb could flex and extend actively. The treatment consisted of amputation of the crooked phalanges of the radial thumb and transfer of the ulnar thumb on its neurovascular bundles to the first ray to provide extension, flexion and opposition. The final functional result is nearly normal.

In nine years 26 duplicated thumbs were seen in the hand clinic of a hospital for crippled children. Using a classification based on the level and degree of duplication and a knowledge of the abnormal anatomy, reconstruction was done. This surgery was done with proper skin incisions, reconstructions of the collateral ligaments, centralization of the flexor and extensor tendons, and alignment of the bones and joints by corrective osteotomies. Simple ablation or incomplete correction produced complications such as skin contractures, increased angular deformities, unstable joints, and intrinsic weakness. In all of the patients, the appearance was improved, opposition was maintained, and postoperative function was not impaired.

A case of perinatal lethal dwarfism is described: owing to its clinical, radiological and histologic features the case can be classified as SRP syndrome type III (Verma-Naumoff). On the basis of the radiological features and--particularly--of those of the growing cartilage, stress is laid on the importance of these studies for a proper classification of such rate and not completely known chondrodysplastic dwarfisms.
