[Health status of the workers of a polyvinyl chloride (PVC) processing plant. III. State of the respiratory tract, immunoglobulins and NBT test].

255 workers processing polyvinyl chloride were examined. All of them underwent spirometric and polycardiographic tests and determination of phagocytic activity of granulocytes, the level of serum albumins and its fractions, and the level of immunoglobulins--IgA, IgG and IgM. The results were compared with the data from healthy non-exposed subjects. It was found that hazards resulting from polyvinyl chloride processing induce slight disturbances in lungs ventilating capacity and inhibit phagocytic activity of granulocytes.

[Pathogenesis and treatment of infection].

The decrease in the microbicidal activity of peripheral blood neutrophils against autohemocultures was demonstrated with special reference to 16 patients with sepsis. This fact is discussed in the light of the data on respiratory acidosis, anemization, and blood iron reduction in sepsis.

[Granulocyte function in Duchenne's progressive muscular dystrophy].

We studied the in vitro function of polymorphonuclear leukocytes in 14 patients affected by Duchenne muscular dystrophy, aged 2-14 years, especially the chemotaxis and the bactericidal activity. The chemotaxis appeared significantly reduced, while the bactericidal activity was sometimes reduced, sometimes increased. In this disease the polymorphonuclear leukocytes seems to have a reduced capacity of movement; probably this phenomenon is connected with an alteration of the muscular cellular apparatus, which perhaps is related with an electrolytic intercellular inequality. These findings are consistent with the definition of Duchenne muscular dystrophy as a systemic disease of the membrane with alterations not only of striped muscular fibers, but of various cells and tissues.

Familial defect of polymorph neutrophil phagocytosis associated with absence of a surface glycoprotein antigen (OKMI).

Two siblings with delayed separation of the umbilical cord, recurrent skin ulceration and dental sepsis were shown to have defective neutrophil phagocytosis of opsonized yeast (S. cerevisiae) and respiratory burst to opsonized and unopsonized zymosan. Increased activity in the NBT reduction test, normal ingestion and killing of S. aureus, and normal spontaneous and directional motility were also demonstrated. These abnormalities of neutrophil phagocytosis were confined to the affected siblings; their healthy parents and brother showed normal neutrophil function. Both children had a polymorph neutrophil leucocytosis, and had normal humoral and cell-mediated immunity. SDS electrophoresis of neutrophil cell membrane preparations showed absence of a glycoprotein band of 175,000 daltons, which was present in the parents' neutrophils in reduced amounts. OKMI monoclonal antibody, which recognized the C3bi receptor (CR3) failed to bind to the affected siblings neutrophils. The findings in these children emphasize the importance of this receptor in phagocytosis, and possibly other neutrophil functions.

Depressed polymorphonuclear leukocyte functions associated with normal cytotoxic functions of T and natural killer cells during chronic hemodialysis.

The cytotoxic capacities of T lymphocytes and natural killer (NK) cells and the phagocytic functions of polymorphonuclear leukocytes were determined in a group of 20 patients with end stage disease who were being maintained on hemodialysis. The mononuclear cells from these patients were able to mediate normal T-cytotoxic functions in a xenogeneic cell-mediated lympholysis assay and normal NK cytotoxic capacity when tested against K562 target cells. Contrarily, polymorphonuclear functions in these patients were severely impaired. The reduction of Nitroblue tetrazolium in dark formazan (NBT test) and the direct phagocytosis of 2 pyogenic strains (Staphylococcus aureus and Salmonella typhimurium) were equally and deeply depressed in almost all patients. This defect was not restored either by 4 hr of hemodialysis nor by 12 months of treatment. This study confirms a major role for a defect of polymorphonuclear cell functions in the depressed defence of patients on chronic hemodialysis against infections.

[The concept of Job syndrome].

The so-called Job's syndrome belongs to the group of diseases designated "chronic granulomatous disease", characterized by recurrent suppurative infections of the skin, eczema, nail dystrophy, conjunctivitis, lymphadenitis and pulmonary infections. Leucocyte function is defective; the nature of the defect has not yet been elucidated. High immunoglobulin E levels are characteristic. The historical background to this syndrome is outlined.
