Towards a rehabilitation of methadone maintenance: integration of relapse prevention and aftercare.

Methadone maintenance was originally proposed as a long-term treatment modality for opiate addiction. However, most clients leave methadone maintenance rather than take methadone indefinitely and subsequently relapse to opiate use. In this article, the author examines relapse to opiate use by clients during and after methadone maintenance treatment in the United States. He reviews models of relapse prevention and aftercare which may be applicable to clients in methadone treatment. There now exist structured and psychotherapeutic relapse prevention methods which may be integrated into methadone maintenance treatment and could serve, in addition, to revitalize methadone maintenance treatment.

Dissociating semantic and phonological maintenance using fMRI.

Functional magnetic resonance imaging (fMRI) distinguished regions of neural activity associated with active maintenance of semantic and phonological information. Subjects saw a single word for 2 sec, and following a 10-sec delay, made a judgment about that word. In the semantic task, subjects focused on the meaning of the word and decided whether a second word was synonymous with it. In the phonological task, subjects repeated the word silently and decided whether it shared a vowel sound with a nonsense word. Analyses allowed for isolation of neural activity during the maintenance delay. Semantic maintenance elicited greater activity in bilateral inferior frontal gyrus and left middle temporal gyrus regions of interest (ROI). In contrast, there was greater activity for phonological maintenance in the left superior parietal ROI. These results show a frontal-temporal network involved in actively maintaining the meanings of words, and they indicate that semantic and phonological maintenance processes are dissociable within working memory.

Functional connectivity during working memory maintenance.

Neurophysiological experiments with monkeys have demonstrated that working memory (WM) is associated with persistent neural activity in multiple brain regions, such as the prefrontal cortex (PFC), the parietal cortex, and posterior unimodal association areas. WM maintenance is believed to require the coordination of these brain regions, which do not function in isolation but, rather, interact to maintain visual percepts that are no longer present in the environment. However, single-unit physiology studies and traditional univariate analyses of functional brain imaging data cannot evaluate interactions between distant brain regions, and so evidence of regional integration during WM maintenance is largely indirect. In this study, we utilized a recently developed multivariate analysis method that allows us to explore functional connectivity between brain regions during the distinct stages of a delayed face recognition task. To characterize the neural network mediating the on-line maintenance of faces, the fusiform face area (FFA) was defined as a seed and was then used to generate whole-brain correlation maps. A random effects analysis of the correlation data revealed a network of brain regions exhibiting significant correlations with the FFA seed during the WM delay period. This maintenance network included the dorsolateral and ventrolateral PFC, the premotor cortex, the intraparietal sulcus, the caudate nucleus, the thalamus, the hippocampus, and occipitotemporal regions. These findings support the notion that the coordinated functional interaction between nodes of a widely distributed network underlies the active maintenance of a perceptual representation.

Long-term immunotherapy with an optimal maintenance dose of a standardized Dermatophagoides pteronyssinus extract in asthmatic patients.

In a previous dose-titration study of specific immunotherapy (SIT) with a Dermatophagoides pteronyssinus extract in asthmatic patients, we proposed a dose of 3.2 micrograms of Der p and 1/1.6 micrograms of Der p 2 as an optimal maintenance dose. Changes presented by a high-dose immunotherapy group, with a maximum tolerated dose between 4 and 16 micrograms Der p 1, were compared with those of a conventional immunotherapy group, with a maximum tolerated dose of 3.2 micrograms Der p 1 or lower. After 2 years of SIT, both groups achieved the same level of clinical benefit. We now present the results of long-term monitoring of 25 of these patients, covering the 5 years of SIT in which the maintenance dose was set at 3.2 micrograms Der p 1. The aims of the study were firstly to examine if this maintenance dose could sustain the changes induced by immunotherapy in the first 2 years; and secondly, to determine if this dose is clinically effective in patients known to tolerate higher maintenance doses. The clinical severity index (CSI), medication and symptom scores, cutaneous sensitivity and specific IgE and IgG4 to D. pteronyssinus, Der p 1 and Der p 2 were measured. Positive clinical and immunological changes presented in the first 2 years of SIT were sustained (CSI, medication score, specific IgE) or even increased (symptom score, cutaneous sensitivity) after 3 additional years of SIT. In conclusion, a maintenance dose of 3.2 micrograms of Der p 1/1.6 micrograms of Der p 2 induced intense clinical and immunological changes which were sustained during a 5-year course of treatment, even in patients able to tolerate higher doses.

The cost-effectiveness of methadone maintenance.

BACKGROUND: Although methadone maintenance is effective in reducing injection drug use, needle sharing, and the overall mortality associated with opiate abuse, many health plans offer little or no access to methadone, and many methadone providers do not comply with treatment guidelines regarding dose, duration of treatment, or provision of ancillary services. Moral and political judgments have helped shape the U.S. treatment system. Evaluations of methadone cost-effectiveness may play a role in changing public policy. METHOD: Cost-effectiveness analysis is used to compare a change, or changes, in treatment to that of current standard care. The cost of treatment and its effect on outcomes are used to find the incremental cost-effectiveness ratio, and determine whether the change(s) should be adopted. The literature on methadone maintenance is reviewed from an economic perspective, focusing on five policy questions: (1) whether methadone should be a health care benefit; (2) what level of ancillary services is optimal; (3) what methadone dose is appropriate; (4) what length of treatment is appropriate; and (5) whether contingency contracts should be employed. RESULTS: Expanded access to methadone maintenance has an incremental cost-effectiveness ratio of less than $11,000 per Quality-Adjusted Life Year. This is more cost-effective than many widely used medical therapies, a finding that strongly supports the inclusion of methadone in the formulary of health care plans.Ancillary services have been shown to be an effective part of methadone maintenance therapy, especially during the beginning of a treatment episode, but there is not enough information available to tell whether the optimal amount of services is being used. There is extensive evidence that many treatment programs dispense inadequate doses of methadone. The cost of additional drugs is very small compared to the benefits of an adequate dose. Many methadone programs limit treatment to 6 months or less, but such short episodes are not likely to be cost-effective. The medical model of methadone maintenance may increase the cost-effectiveness of the treatment for long-term patients. Programs that reward patients for negative urinalysis have proven effective at reducing illicit drug use, but their cost-effectiveness will need to be demonstrated before they are widely adopted. CONCLUSIONS: Cost-effectiveness researchers need to measure substance abuse outcomes in terms of Quality-Adjusted Life Years, as this will make their findings more relevant to the development of treatment policy. It will allow different substance abuse treatments to be compared to each other and to medical care interventions.

Mandibular implant-supported overdentures: a prospective evaluation of the burden of prosthodontic maintenance with 3 different attachment systems.

PURPOSE: A prospective study on mandibular implant-supported overdentures using more than 2 Brånemark implants splinted with multiple round bars evaluated a hypothesis that unnecessary prosthodontic maintenance would result from the contravention of accepted biomechanical principles. MATERIALS AND METHODS: A randomized prospective trial on 59 edentulous patients details prosthodontic maintenance and complications of 52 mandibular overdentures (11 unsplinted, 41 splinted) over a 3-year period. RESULTS: Prosthodontic maintenance was greater in the first year than in subsequent years, regardless of design. Comparison between the splinted designs with round bars revealed no statistically significant difference with either retentive clip activation or fractures. More than 70% of the retention clips in the 2-implant (single round bar) design needed activation, as compared to 44% of those with the 3-implant (double round bar) or 4-implant (triple round bar) design. Retention clip fracture occurred in 30% of patients with 3 or 4 implants and 16% of those with 2 implants. Relining the overdentures, regardless of design, revealed an excessive maintenance burden in 40% of overdentures. Remaking of overdentures was necessary in 21% of patients. CONCLUSION: Multiple round bars splinting 3 or 4 implants can still be a treatment concept in mandibular overdenture treatment. Results from prospective evaluation of this design indicate less prosthodontic maintenance of clip activation but more clip fractures than with 2 implants splinted with single round bars, although not at statistically significant levels.

[Studies on the relationship between serum albumin concentration and lipid peroxidation in the erythrocyte membrane of maintenance hemodialysis patients].

We conducted a study on the long-term prognosis of maintenance hemodialysis patients with and without diabetes mellitus(144 cases). As a result, significant differences were observed in the long-term prognosis over a 15-year period between those with a serum albumin concentration of more than 3.8 g/dl and those with a serum albumin concentration of less than 3.8 g/dl. The group with a serum albumin concentration of more than 3.8 g/dl showed a better prognosis than the group with a concentration of less than 3.8 g/dl. We investigated the levels of thiobarbituric acid-reactive substances (TBA-RS) in the erythrocyte membrane of maintenance hemodialysis patients(23 cases), pre-dialysis uremic phase patients(12 cases) and healthy controls(7 cases). TBA-RS in the erythrocyte membrane of pre-dialysis uremic phase patients was higher than that observed in the other groups. The TBA-RS in the erythrocyte membrane in the maintenance hemodialysis without diabetes mellitus group was lower than that in the pre-dialysis uremic phase group. The TBA-RS in the erythrocyte membrane in the maintenance hemodialysis without diabetes mellitus group thus indicated an inverse relationship with the serum albumin concentration, Kt/V and erythrocyte tocopherol contents. These results suggested that serum albumin protects against erythrocyte membrane lipid peroxidation, and that this function is related to hemodialysis. We thus conclude that hypoalbuminemia appears to increase the oxidative damage and acceleration of arteriosclerosis, while it also worsens the long-term prognosis of maintenance hemodialysis patients.

Control group and maintenance treatment with bacillus Calmette-Guerin for carcinoma in situ and/or high grade bladder tumors.

PURPOSE: Intravesical instillations of bacillus Calmette-Guerin have demonstrated satisfactory results in the treatment of vesical carcinoma in situ and high grade superficial bladder tumors. We designed a protocol to evaluate the decrease in tumor recurrence with maintenance therapy. MATERIALS AND METHODS: Between June 1989 and May 1995 an initial course of 6 intravesical instillations of Connaught strain bacillus Calmette-Guerin was administered in patients with carcinoma in situ and/or high grade superficial bladder tumors. Six months later 131 disease-free patients were randomly assigned to a control group or a maintenance therapy group that received 6 instillations every 6 months (6 x 6) for a 2-year period. RESULTS: Of the 126 evaluable patients at a mean followup of 79 months there were no significant differences in recurrence nor progression. A total of 16 patients (26.2%) in the control and 10 (15.1%) in the maintenance group had superficial relapse at a mean of 24 and 20 months, respectively (p = 0.07). Eight patients underwent radical cystectomy due to bladder contraction in 1, high grade superficial recurrence in 4 and disease progression in 3. Of the 65 patients on maintenance therapy 22 (33.85%) completed the planned 2-year treatment. CONCLUSIONS: Six-month maintenance therapy in patients treated initially for carcinoma in situ and/or high grade superficial bladder tumors who are disease-free at 6 months did not significantly decrease recurrence or progression.

Evolving methodologies in bipolar disorder maintenance research.

Background During the development of a new treatment for bipolar disorder, maintenance studies are used to evaluate the ability of the putative mood stabiliser to prevent relapse and recurrence of further episodes. Comparisons with the early bipolar disorder maintenance studies indicate that the methodologies of recent trials have evolved substantially. Aims To review the methods used in the first- and second-generation maintenance studies, highlighting the differences of the various designs. Method Literature review. Results Methods that have evolved the most include patient enrolment, randomisation schemes and the use of outcome measures and statistical analyses. In addition, regulatory and commercial issues have also influenced study design. Conclusion There is little consensus on the methodology of bipolar disorder maintenance studies. As the integration of newer therapies into routine clinical practice is dependent on the evidence from controlled studies, it is essential that future maintenance trials in bipolar disorder achieve adequate methodological rigour without sacrificing overall feasibility.

An evaluation of breathing nervous regulation of opiate dependent patients during in 6 months' methadone maintenance treatment programme.

During the methadone maintenance treatment an individually specified doses of methadone should prevent a withdrawal syndrome and may stabilise the activity of respiratory centres of opiate dependent patients. The aim of the study was to evaluate the direction of changes in respiratory pattern parameters of 34 opiate dependent patients (14 women and 20 men) during 6 months' methadone maintenance treatment at the Kraków Department of Clinical Toxicology. Respiratory pattern parameters and occlusion pressure were measured by means of a computer spirometer using "on line" during unrestrained breathing with atmospheric air. Examination of respiratory pattern were performed three times: I--initial examination--before administration of methadone maintenance treatment; II--control examination after 3 months' methadone maintenance treatment; III--control examination after 6 months' methadone maintenance treatment. An increase in values of occlusion pressure and minute ventilation were observed. The times of expiration and total times of inspiration-expiration cycle were shorter in III examination than in the examination before methadone administration. The values for the breathing cycle neuromuscular drive (VT/Tin) were relatively stable, but they were higher than those obtained in the control group. It may be related to the anxiety states and mood alteration. The largest individual variation in consecutive examination were observed for Tin/Ttot index being the timing component of respiratory patterns characteristic for parasympathetic control of breathing.

Sustained Downregulation of Cortical Adrenergic Receptor Density with Maintenance Electroconvulsive Stimulation.

Central beta-adrenergic receptor downregulation is a robust and reversible neurochemical finding in animals after a course of electroconvulsive stimulation (ECS) or antidepressant drug administration. In addition, beta-receptors are essential components of monoamine-based models of affective disorder pathogenesis and treatment. We tested whether the downregulation in rat cortical beta-receptor density observed after an initial series of ECS is maintained over 2 months of maintenance ECS. Our findings confirm prior reports, in the absence of maintenance ECS, that receptor density is decreased after a course of 10 daily ECS, with recovery to baseline 16 days after the last ECS. In contrast, when maintenance ECS was given for 2 months after an initial series of 10 ECS, a schedule of maintenance ECS at two per week was sufficient to sustain the receptor downregulation. The results are discussed in terms of the neurochemical study of ECS in animals and implications for maintenance ECS.

Benzodiazepine abuse in a methadone maintenance treatment clinic in Israel: characteristics and a pharmacotherapeutic approach.

OBJECTIVE: 1) To study the prevalence, course and correlates of benzodiazepine (BZD) abuse in an Israeli methadone maintenance treatment (MMT) clinic; and 2) to present preliminary results of an open clinical study on the treatment of BZD addiction in MMT patients using BZD maintenance. METHODS: Study 1 was carried out on 196 patients who were in MMT for at least one year. Objective information on drug use (urine testing) was recorded, and the one-year retention rate was measured. Demographic data and history of abuse were obtained, and a self-rated psychopathology and psychological distress questionnaire was administered. The second study was an open one. Twenty BZD-addicted patients were given clonazepam maintenance treatment (CMT) and were started on a daily regimen of 6 mg. followed by a gradual tapering until an individual dose was reached. This maintenance dose was the lowest level at which the patient still expressed satisfaction with the BZD dose without reporting craving or overdose. Failure was defined by three daily consecutive BZD uses above the permitted dose. RESULTS: BZD abuse was significantly reduced during the first year of MMT (from 55 to 43%, P < .000), although a number of patients started to abuse BZDs during this period. BZD abusers (BAs) were found more often to be polydrug abusers, their methadone dosage was higher than non-abusers (NBAs), they started to abuse drugs in general and heroin in particular earlier than NBA, and they had higher self-rated psychopathology and psychological distress scores. The second study showed that CMT was successful in 75% of the patients over a period of six months and that it is, therefore, a promising mode of treatment for BZD-dependent MMT patients. CONCLUSIONS: MMT patients who abuse BZDs are at an increased risk for continuing polydrug abuse. Although our results are to be taken cautiously due to the inherent limitation of a non-controlled non-randomized pilot study, ongoing maintenance with clonazepam seems to be well tolerated and a stabilizing and satisfying pharmacotherapy for BZD dependent MMT patients.

Maintenance biotherapy for metastatic melanoma with interleukin-2 and granulocyte macrophage-colony stimulating factor improves survival for patients responding to induction concurrent biochemotherapy.

PURPOSE: A prospective Phase II study of a novel maintenance biotherapy regimen after induction biochemotherapy was conducted in patients with metastatic melanoma in efforts to maintain responses and improve survival. EXPERIMENTAL DESIGN: Thirty-three patients with poor prognosis metastatic melanoma who achieved a partial response (PR) or stable disease (SD) to induction concurrent biochemotherapy were treated with chronic low-dose interleukin (IL)-2 and granulocyte macrophage-colony stimulating factor, and intermittent pulses of intermediate/high-dose decrescendo IL-2 over a 12-month period. The outcome of these patients was compared with a control group of patients at our institution who were treated recently with induction biochemotherapy and achieved a PR or SD. RESULTS: Five patients (15%) achieved a complete response, and 4 patients (12%) maintained SD for at least 6 months on maintenance biotherapy. The median progression-free survival (PFS) and overall survival (OS) were 8.1 months and 18.5 months, respectively, compared with historical controls, which were PFS 5.9 months (P = 0.0015) and OS 9.3 months (P = 0.0004). Administration of maintenance biotherapy was a significant predictor of PFS (P = 0.0008) and OS (P = 0.0001) in multivariate and matched-pair analyses (P = 0.002). The maintenance biotherapy regimen was well tolerated with no dose-limiting acute or cumulative toxicities. CONCLUSION: In this single institution study, maintenance biotherapy with IL-2 and granulocyte macrophage colony-stimulating factor in patients achieving PR or SD to induction biochemotherapy improved PFS and OS compared with historical controls. A larger multicenter Phase II trial has been initiated in an effort to confirm these results.

Maintenance Enforcement Act, 11 May 1988.

This Act establishes a Director of Maintenance Enforcement in Prince Edward Island, Canada, to enforce maintenance "in such manner, if any, as appears practicable." Under the Act, extraprovincial garnishments are recognized, and maintenance orders may be registered against land with authority to force the sale of that land. In addition, a debtor may be required to file a financial statement and appear before a court to explain the default. At a default hearing, a debtor shall be assumed to have the ability to pay arrears, unless the contrary is shown. A debtor may also be required to discharge the arrears by periodic payments; provide security in such form as the court directs, which may later be sold; report periodically to the court; and be imprisoned continuously or intermittently for not more than 90 days; among other things. Support and maintenance orders are given priority over other judgment debts regardless of when an enforcement process is issued or served. Similar laws were enacted in Alberta and Newfoundland. See the Maintenance Enforcement Act 1985 (Alberta, 5 June 1985) and The Support Orders Enforcement Act (Newfoundland, 8 July 1988). For a description of the major provisions of the Alberta Act, see Commonwealth Law Bulletin, July 1986, pp. 621-22.

[The influence of expansion maintenance period upon in vivo skin tension and immediate stretch-back rate of the expanded skin].

OBJECTIVE: The study was to investigate the influence of maintenance period of expansion upon skin tension and immediate stretch-back rate of the expanded skin. METHODS: 30 expanders (240 ml) were implanted in six adult dogs, which were divided into four groups: group A with 2-week saline injection of the expander; group B with 6-week injection; group C with sham-operation and group D as the blank control group. The first two groups were subdivided into three groups respectively according to different maintaining times of one week, two weeks and four weeks. When full inflation was finished, a 4 cm x 7 cm expanded flap was created and in vivo skin tension and stretch-back ratio of the flap were measured. The Comparison was made in terms of in vivo skin tension and immediate stretch-back ratio between different injection and different maintenance periods. RESULTS: With expansion maintenance time increasing, the in vivo skin tension and immediate stretch-back ratio decreased. After the capsule of the expand skin flap was removed, the in vivo skin tension and immediate stretch-back rate significantly reduced. Based on the experimental results, 16 patients were treated with implantation of 23 expanders. Their average expander injection time was 14 days and the average maintenance time was 25 days. All expanders obtained good results except one extrusion. CONCLUSION: In skin and soft tissue expansion, in vivo skin tension and immediate stretch-back ratio can be reduced through prolonged maintenance of expansion and capsule removal from the expanded skin flap.

Maintenance energy requirement of llamas.

Five castrated male llamas (mean body weight, 94 kg) were studied in an energy balance trial to determine maintenance energy requirement of llamas. Llamas were fed a 50% oat hay-50% pelleted concentrate diet (2.43 Mcal of metabolizable energy/kg of diet dry matter) at approximately 1.6% of body weight (BW). An 8-day total collection digestion trial was used to determine fecal and urine energy losses. Heat production and methane emissions were determined via indirect respiration calorimetry measurements on each llama fed at the same level of intake as during the digestion trial and subsequently on days 3 and 4 of a period of nonfeeding. Fecal, urine, and methane energy losses of the llamas fed near-maintenance intake were 32.5, 3.5, and 7.1% of gross energy intake, respectively. The postabsorptive metabolic rate, commonly called nonfed (fasting) heat production, was 59.3 kcal/BW0.75. Using a linear relation between postabsorptive and maintenance energy requirement and efficiency of energy use below maintenance of 0.702, metabolizable energy requirement at maintenance was determined to be 84.5 kcal/BW0.75.

Effect of drug therapy on the relapse of peptic ulcers. 2nd communication: the effect of maintenance therapy.

One-year maintenance therapy with pirenzepine (Gastrozepin, CAS 28797-61-7) (75 mg/d) or placebo was performed in a double-blind fashion in 251 patients to investigate the relapse by means of endoscopic examinations repeated every 3 months. Enrolled were the patients who had been cured within 3 months by acute therapy with either pirenzepine or cimetidine randomly given by the envelope method. When the results were analyzed by Cutler-Ederer's life table method to monitor manifestation of relapse, the maintenance therapy with pirenzepine did not show any significant difference from placebo in all the patients enrolled. However, stratified analysis according to various combinations of background factors revealed that there was significant difference in 1-year cumulative non-relapse rates between the pirenzepine maintenance therapy group and the placebo group, suggesting prophylactic effect of the maintenance therapy with pirenzepine on relapse. An increase of daily dose more than 75 mg in maintenance therapy may be expected to display the prophylactic effect of pirenzepine on ulcer relapse in a definite manner.
