Contraception survey: Denmark 1988.

In 1988 a survey** was conducted in 1,033 women living in Denmark, whose ages ranged from 15 to 44 years. The main aims of the study were to identify the influence, if any, of demographic and socioeconomic factors on contraceptive use and to assess the women's knowledge of selected facts concerning their own fertility. The study's additional objectives were to gain insight into factors influencing communication on the subject of contraception, such as advice obtained from professional counsellors, general information dissemination and discussion between partners, and to investigate women's attitudes and opinions regarding contraceptive methods and their use. The results are discussed in relation to the findings of other studies on the use of contraceptives both worldwide and in a number of European countries. The Danish findings are also examined in the context of possible action to reduce the relatively high rate of legally induced abortions in Denmark.

Contraceptive use and attitudes in Great Britain.

In order to update current knowledge on contraceptive use and attitudes in Great Britain (i.e. England, Scotland and Wales), a survey was conducted among 1753 randomly selected British women aged 15-45. Replies were received from 967 women (55.2%). Seventy-three percent (73%) of fertile, sexually active women who wished to avoid pregnancy were using reliable methods of contraception, viz. oral contraceptives (OCs), intrauterine devices (IUDs) or sterilization. However, it was found that adolescents and women over 40 who wished to avoid pregnancy were, nevertheless, especially likely not to be using any contraceptive method at all. The women surveyed were concerned about weight gain, cardiovascular and cancer risks associated with OC use, and infection and infertility risks associated with IUD use. Sixty percent (60%) perceived sterilization as a major and risky surgical operation. It was concluded that contraceptive practice in Britain had not improved greatly in recent years. The latest scientific findings regarding the true advantages and disadvantages of OCs, IUDs and sterilization, therefore, need to be brought to the attention of the lay public more effectively. Special efforts need to be directed towards providing adolescents and women over 40 with proper information. Physicians and the mass media could play a considerable role in this respect.

Television and adolescent sexuality.

Existing studies of the sexual content of television programming and advertising and the effects of this content on adolescent viewers are reviewed. Content studies show that the frequency of sexual references have increased in the past decade and are increasingly explicit. Studies of the effects of this content, while scarce, suggest that adolescents who rely heavily on television for information about sexuality will have high standards of female beauty and will believe that premarital and extramarital intercourse with multiple partners is acceptable. They are unlikely to learn about the need for contraceptives as a form of protection against pregnancy or disease. Suggestions for future research and trends in television programming policies are explored.

Effects of different socioeconomic conditions on menarche in Turkish female students.

PURPOSE: The aim of this study was to determine the age at menarche, the menarcheal features, and the association between menarcheal age and socioeconomic status in an urban area in Turkey. In addition, we tried to assess whether there is a relationship between age at menarche and body composition. METHODS: We asked some questions about menarche of 1017 female adolescent students in the high schools of Manisa region. Height and weight were measured. The body mass index (BMI; kg/m2) was used as an index of relative weight. Adolescent girls were grouped into three socioeconomic status according to the educational and occupational levels of their parents. The age at menarche and the menarcheal pattern were evaluated according to the socioeconomic status. RESULTS: The ages of girls involved in the study ranged between 14 and 18 years, with a mean of 15.7+/-1.1 years. Although the menarcheal age was found to be lower in girls with higher socioeconomic status, there was no significant difference between the three different socioeconomic status. In all of the three groups, menarche was more common in summer and fall than in spring and winter. Although the mother was an important source of knowledge in all groups, it was significantly more important in the group with high socioeconomic status. Adolescent girls with low socioeconomic status had fewer premenstrual complaints. However, there was no significant difference between the groups. We found an inverse correlation between menarcheal age and postmenarcheal weight and the BMI (r=-0.14, p=0.000). However, there was no correlation between menarcheal age and postmenarcheal height. CONCLUSION: These results indicate that as the social status differences decrease, the difference observed in menarcheal age and pattern disappears in urban areas of developing countries. Menarcheal age may be an indicator of socioeconomic development. It does not influence postmenarcheal height; however, as menarcheal age decreases, BMI increases.

Family delivery from the standpoint of fathers--can stereotypes of participant or non-participant father be fully justified?

OBJECTIVES: Family delivery is an important aspect of modern obstetrics. The aim of this study was to estimate the phenomenon of family delivery from the standpoint of participating and non-participating fathers. STUDY DESIGN: Two types of questionnaires were used in the study. Five hundred and five men were divided into two groups: (A) delivery participants and (B) delivery non-participants. RESULTS: The participants were significantly younger than non-participants. Better educated men dominated in group (A), whereas men with trade education were the majority in group (B). Low birth classes attendance among women in group (B) was noticed. All men in group (B) knew about the possibility of taking part in the delivery. Almost 100% of participants declared their satisfaction with family delivery. For non-participants, wife and media were the main source of knowledge, while for participants it was the medical staff and labor schools. CONCLUSIONS: Family labor is commonly accepted by both partners and is generally connected with positive emotions. This phenomenon is generally approved by couples with higher economical status. Family labor resignation, common among pairs with lower economical status, is caused generally by fear and lack of proper knowledge. Health care providers, through labor schools and outpatients visits, should concentrate on getting to couples, whose attitude towards family labor is negative. Educational programs and popular press articles should be prepared more thoroughly, focusing on the whole population of potential fathers.

Molecular epidemiology and disinfectant susceptibility of Listeria monocytogenes from meat processing plants and human infections.

We have investigated the molecular epidemiology of Listeria monocytogenes from the meat processing industry producing cold cuts and from cases of human listeriosis by discriminative pulsed-field gel electrophoresis (PFGE). A subset of the isolates was also investigated for susceptibility to a disinfectant based on quaternary ammonium compounds (QAC) frequently used in the meat processing industry. The purpose of this investigation was to obtain knowledge of sources, routes of contamination and genetic types of L. monocytogenes present along the production line in the meat processing industry, and to compare meat industry isolates and human isolates. Of the 222 isolates from four meat-processing plants, 200 were from two plants responsible for nearly 50% of the production of cold cuts in the Norwegian market. The strain collection included historical routinely sampled isolates (1989-2002) and isolates systematically sampled through a one year period (November 2001 to November 2002) from fresh meat and production environments in three plants. No isolates were obtained in samples from employees (throat, faeces). Human strains included all available reported isolates from Norwegian patients in selected time periods. The L. monocytogenes PFGE data showed a large genetic heterogeneity, with isolates separated into two genetic lineages and further subdivided into 56 different PFGE profiles. Certain profiles were observed on both sides of production (before and after heat treatment) indicating contamination of end products by fresh meat or fresh meat environments. While fresh meat isolates almost exclusively grouped within lineage I, isolates from end products showed a more balanced distribution between lineages I and II. Ten profiles were common among isolates from human and meat industry. Typing of human isolates identified a previously unrecognised outbreak. Generally, a higher QAC resistance incidence was observed among isolates from the meat processing industry than among human isolates although large plant to plant differences were indicated. No correlation between resistance and PFGE profile or resistance and persistence was observed.

The comparative study on concept representation between the UMLS and the clinical terms in Korean medical records.

OBJECTIVE: The National Library of Medicine's Unified Medical Language System (UMLS) is a rich source of knowledge in the biomedical domain. The UMLS is used for research and development across wide range of different applications. In this paper, we evaluated the coverage of UMLS as compared with medical terms extracted from Korean medical records and identified differences in concept representation between two terminology sets. DESIGN AND MEASUREMENT: We measured the concept coverage of the UMLS. For this study, we mapped the clinical terms extracted from the discharge records of Seoul National University Hospital (SNUH) to the UMLS. RESULTS: Thirty-five percent of the entry terms used in chief complaint of SNUH were conceptually matched with the UMLS 'Sign or Symptom' concepts. Fifty-eight percent of the terms were found to be matched with the UMLS 'Disease or Syndrome' concept rather than the 'Sign or Symptom' concept. The remaining 7% were not found in the UMLS concepts. We then analyzed some of different expression patterns used by the two term sets and addressed issues to be taken into consideration. CONCLUSION: We found out that the UMLS was comparable with Korean medical records, since most of concepts of Korean medical records were covered with the UMLS concepts.

Oxidative stress in malaria parasite-infected erythrocytes: host-parasite interactions.

Experimenta naturae, like the glucose-6-phosphate dehydrogenase deficiency, indicate that malaria parasites are highly susceptible to alterations in the redox equilibrium. This offers a great potential for the development of urgently required novel chemotherapeutic strategies. However, the relationship between the redox status of malarial parasites and that of their host is complex. In this review article we summarise the presently available knowledge on sources and detoxification pathways of reactive oxygen species in malaria parasite-infected red cells, on clinical aspects of redox metabolism and redox-related mechanisms of drug action as well as future prospects for drug development. As delineated below, alterations in redox status contribute to disease manifestation including sequestration, cerebral pathology, anaemia, respiratory distress, and placental malaria. Studying haemoglobinopathies, like thalassemias and sickle cell disease, and other red cell defects that provide protection against malaria allows insights into this fine balance of redox interactions. The host immune response to malaria involves phagocytosis as well as the production of nitric oxide and oxygen radicals that form part of the host defence system and also contribute to the pathology of the disease. Haemoglobin degradation by the malarial parasite produces the redox active by-products, free haem and H(2)O(2), conferring oxidative insult on the host cell. However, the parasite also supplies antioxidant moieties to the host and possesses an efficient enzymatic antioxidant defence system including glutathione- and thioredoxin-dependent proteins. Mechanistic and structural work on these enzymes might provide a basis for targeting the parasite. Indeed, a number of currently used drugs, especially the endoperoxide antimalarials, appear to act by increasing oxidant stress, and novel drugs such as peroxidic compounds and anthroquinones are being developed.

Coping patterns as a valid presentation of the diversity of coping responses in schizophrenia patients.

This study aimed to identify coping patterns used by schizophrenia inpatients in comparison with those used by healthy individuals, and to explore their association with selected clinical and psychosocial variables. The Coping Inventory for Stressful Situations (CISS) was used to assess coping strategies among 237 inpatients who met DSM-IV criteria for schizophrenia and 175 healthy individuals. Severity of psychopathology and distress, insight into illness, feelings of self-efficacy and self-esteem (self-construct variables), social support, and quality of life were also examined. Factor analysis, analysis of covariance and correlations were used to examine the relationships between the parameters of interest. Using dimensional measures, we found that emotion-oriented coping style and emotional distress were significantly higher in the schizophrenia group, whereas the task-oriented coping style, self-efficacy, perceived social support and satisfaction with quality of life were lower compared with controls. When eight CISS coping patterns were defined, the results revealed that patients used emotion coping patterns 5.5 times more frequently, and task and task-avoidance coping patterns significantly less often than healthy subjects. Coping patterns have different associations with current levels of dysphoric mood and emotional distress, self-construct variables, and satisfaction with quality of life. Thus, the identified coping patterns may be an additional useful presentation of the diversity of coping strategies used by schizophrenia patients. Coping patterns may be considered an important source of knowledge for patients who struggle with the illness and for mental health professionals who work with schizophrenia patients.

Relative contributions of surgeons and decision support systems.

Evidence-based medicine came into focus in 1992 when scholars recognized the need for valid information required for optimal patient care. Because of the increasing volume and uncertain quality of new knowledge, traditional sources of information such as books and journals failed to meet the needs of busy practitioners.Evidence-based medicine promoted strategies for identifying and appraising relevant information and making it readily available. Surgeons face unique challenges in the recognition and application of best evidence. Evidence-based surgery requires careful appraisal of the existing evidence, expanding the pool of level 1 evidence,and improving the availability of best evidence. Ultimately, the organization of systems will incorporate best evidence into the processes of care and will document the outcomes of care.

Seasonal and predator influences on adrenal function in adult Steller sea lions: gender matters.

Chronically heightened adrenal activity indexed by fecal corticosteroids has been shown to be a valid descriptor of stress in many species. As part of an ongoing investigation of adrenal activity in Steller sea lions (SSL), adrenocorticotropic hormone (ACTH) challenges were performed during the summer at the Alaska SeaLife Center (ASLC). These results were compared to earlier data from winter months. Additionally, adrenal response of free-ranging females SSL to a presumed in situ stressor, pup predation by killer whales (Orcinus orca), was evaluated as a field trial of developed methodologies. Summer ACTH results indicated that gender-dependent differences in baseline fecal corticosterone concentrations exist, with summer baseline fecal corticosterone concentrations higher in males than in females, based on season when compared with previously reported values in winter ACTH trials for this species. ACTH trials in the male in the summer resulted in 2468 ng/g basal to 10,937 ng/g maximal fecal corticosterone concentrations, a fourfold change. Female 1 exhibited a 30.5-fold increase 24 h post-ACTH stimulation (27.9-852.0 ng/g dry weight), with a return to just above baseline concentrations by hour 25. Additionally, female 2 exhibited a 64.4-fold increase at 25 h post-stimulation (31.7-2042.0 ng/g dry weight), with a return to just above baseline concentrations by hour 45. In situ female fecal corticosterone tripled 24h subsequent to orca predation on pups (54.6+/-18.5 ng/g mean pre-attack, 542.5+/-252.7 ng/g mean post-attack) during the field trial. Data from both the summer ACTH and subsequent field trial underscore the necessity of gender determination of collected scat source and knowledge of conditions at collection sites for proper interpretation of fecal corticosterone data, particularly in studies that focus on population-wide stress. With elimination of gender bias and delineation of acute response to natural stimuli, corticosterone data can now be evaluated within context and provide meaningful information about stress and potentially reproductive physiology in free-ranging Steller sea lions.

Binding theory and grammatical specific language impairment in children.

This study investigates the intrasentential assignment of reference to pronouns (him, her) and anaphors (himself, herself) as characterized by Binding Theory in a subgroup of "Grammatical specifically language-impaired" (SLI) children. The study aims to (1) provide further insight into the underlying nature of Grammatical SLI in children and (2) elucidate the relationship between different sources of knowledge, that is, syntactic knowledge versus knowledge of lexical properties and pragmatic inference in the assignment of intrasentential coreference. In two experiments, using a picture-sentence pair judgement task, the children's knowledge of the lexical properties versus syntactic knowledge (Binding Principles A and B) in the assignment of reflexives and pronouns was investigated. The responses of 12 Grammatical SLI children (aged 9:3 to 12:10) and three language ability (LA) control groups of 12 children (aged 5:9 to 9:1) were compared. The results indicated that the SLI children and the LA controls may use a combination of conceptual-lexical and pragmatic knowledge (e.g., semantic gender, reflexive marking of the predicate, and assignment of theta roles) to help assign reference to anaphors and pronouns. The LA controls also showed appropriate use of the syntactic knowledge. In contrast, the SLI children performed at chance when syntactic information was crucially required to rule out inappropriate coreference. The data are consistent with an impairment with the (innate) syntactic knowledge characterized by Binding Theory which underlies reference assignment to anaphors and pronouns. We conclude that the SLI children's syntactic representation is underspecified with respect to coindexation between constituents and the syntactic properties of pronouns. Support is provided for the proposal that Grammatical SLI children have a modular language deficit with syntactic dependent structural relationships between constituents, that is, a Representational Deficit with Dependent Relationships (RDDR). Further consideration of the linguistic characteristics of this deficit is made in relation to the hypothesized syntactic representations of young normally developing children.

Learning to read with underdeveloped phonemic awareness but lexicalized phonological recoding: a case study of a 3-year-old.

Case studies on very precocious readers are useful for examining what sources of knowledge and processes are necessary in the acquisition of reading. This is a case study of a 40-month-old child with a word reading age of 8 years 6 months. Tests indicated that she had no phoneme awareness beyond initial phonemes, and that her productive spelling was undeveloped. In reading she was highly proficient at rapid phonological recoding, both by correspondences that were contextually sensitive and those that were not. The former determined her high level of irregular pronunciations for irregular consistent non-words. Experiments indicated that she had well-specified orthographic lexical representations. It was concluded that her phonological recoding was an implicit process based on sublexical relations induced from her lexical representations rather than explicitly taught letter-sound correspondences. The implications of the results for major developmental models of reading acquisition are examined.

Psychosocial and behavioral factors associated with risk of sexually transmitted diseases, including human immunodeficiency virus infection, among urban high school students.

The main purpose of this study was to evaluate the role of multiple psychosocial and knowledge-related antecedent factors that may predict sexual and alcohol and drug use behaviors that are associated with the transmission of sexually transmitted diseases (STDs), including human immunodeficiency virus infection. Five hundred forty-four ninth-grade urban high school students were surveyed regarding knowledge, attitudes, and behaviors related to STDs and acquired immunodeficiency syndrome (AIDS). Results of multiple regression analyses indicate that higher levels of STD and AIDS knowledge were associated with lower levels of STD and AIDS anxiety (R2 delta = 0.09; p less than 0.001), fewer negative attitudes toward people with AIDS (R2 delta = 0.09; p less than 0.001), stronger perceptions of self-efficacy (R2 delta = 0.03; p less than 0.01), and stronger peer affiliation (R2 delta = 0.02; p less than 0.05). Negative attitudes toward people with AIDS were inversely related to knowledge (R2 delta = 0.08; p less than 0.001), social support (R2 delta = 0.02; p less than 0.01), and perceived self-efficacy (R2 delta = 0.01; p less than 0.05). Predictors of alcohol and drug use included perceived peer norms (R2 delta = 0.08; p less than 0.001) and strong peer affiliation (R2 delta = 0.05; p less than 0.001). The best predictor of sexual risk behavior was alcohol and drug use (R2 delta = 0.07; p less than 0.001). Lower levels of knowledge (R2 delta = 0.14; p less than 0.01) and perceived peer norms (R2 delta = 0.05; p less than 0.05) predicted nonuse of condoms. Our results indicate that several factors relate to adolescent risk for STDs: the connection between peer influence and adolescent risk behaviors, the link between alcohol and drug use and sexual risk behavior, and the role of knowledge in determining nonuse of condoms.
