Treponemal serology and blood groups on Bali island, Indonesia.

As part of a multidisciplinary study of the population of Bali, Indonesia, 2452 blood samples from people of both sexes were tested for treponemal infection and blood groups. Analysis of blood groups of the 81 patients reactive to the Treponema pallidum immobilisation (TPI) test, who were considered to have latent or inactive yaws, compared with a control group of 552 healthy Balinese, showed that the ratio of MM to MN and NN phenotypes was 2.25 times higher in the patients than in the controls (chi 2(1) = 10.2, p less than 0.005). A speculative hypothesis is that yews infection gives people with the MM phenotype a lower selective fitness. This hypothesis could explain the low prevalence of the M gene in the Australo-Melanesian populations.

Radiological and dermatological findings in two patients suffering from early yaws in Indonesia.

Two children suffering from early yaws in Indonesia are presented. Apart from skin lesions and a positive treponemal serology in both patients, involvement of tubular bones, particularly of the hands, was revealed by radiological examination. In one patient involvement of a distal phalanx was remarkable. Early diagnosis and treatment of yaws is important since a delay in treatment may result in severe and irreversible bone deformities of the late stage of the disease. This report illustrates that radiological changes, although rare, can still be detected in the early stage of yaws in areas where yaws is resurgent nowadays.

Relaxed dominance relations among female moor macaques (Macaca maurus) in their natural habitat, south Sulawesi, Indonesia.

Field observations were made of the social behaviour of moor macaques (Macaca maurus), one of several species of macaques that live in Sulawesi, Indonesia. The present study was designed to characterise the expression of their relaxed dominance style among females in their natural habitat and to examine the factors influencing the evolution of the dominance style. The dominance rank order among adult females in the study group was linear, and it was essentially stable during the study period of 4 years. Intense aggression, including biting, was rare. Few interventions in agonistic interactions by a third individual were observed. Social relationships among adult females appeared to be egalitarian but nepotistic. During moving or feeding, encounters within 1 m between adult females were frequently observed and approaches by subordinates were not infrequent. Intragroup spacing patterns during moving or feeding and those during resting were not correlated. In wild groups, differences of dominance styles seemed to be found most apparently in intragroup spacing during feeding. The ecological model of van Schaik does not provide an adequate explanation for the egalitarian relations among female moor macaques.

Genetic survey of an isolated community in Bali, Indonesia. I. Blood groups, serum proteins and hepatitis B serology.

320 adults and children of an isolated community of Bali, Indonesia, have been tested for blood groups ABO, Rh, MNS, P, Lewis, Duffy, Kell, for haptoglobin and transferrin and for hepatitis B surface antigen and antibodies. Phenotype distribution and gene frequencies are given for the total population tested and for two subgroups representative of the inbred population of the isolate and of the non-inbred part of the population. Significant differences between the two subgroups show a clear genetic drift in the inbred population. The study brings biological support to the ethnological hypothesis of population migrations in this area. Tests for hepatitis B surface antigen reveal a lower prevalence of the disease than in most other south-east Asian populations.

Genetic survey of an isolated community in Bali, Indonesia. II. Haemoglobin types and red cell isozymes.

316 adults and children from an isolated community of Bali, Indonesia, have been tested for 18 red cell enzyme systems controlled by 24 loci, and haemoglobin. 13 loci were invariant. The other 11 loci showed variations similar to those found previously in Southeast Asian populations. Of special interest is the occurrence of lactate dehydrogenase Calcutta-1 variants, indicating Indian gene admixture, and PGM92, indicating a Melanesian genetic component. A few individuals were CA1 1-3Bali and this is possibly the same as other CA1 1-3 types found in the Philippines and Guam. Nearly 10% were glucose-6-phosphate dehydrogenase (G6PD) deficient and 2% carried Hb E. A fast electrophoretic variant of G6PD was detected in 5 persons. Two sub-groups of the population were studied. Gene frequencies in the Isolate supported the view that inbreeding and genetic drift have made this sub-group genetically distinct from the non-inbred part of the population.

Territory and monogamy among Kloss' gibbons (Hylobates klossii) in Siberut Island, Indonesia.

Behavior of Kloss' gibbons was studied from July 1 to October 7, 1972 in Siberut Island, off the coast of western Sumatra, Indonesia. Reproductive groups are monogamous families with a mean family size of 3.4 individuals (n = 11 families). Such families occupy territories averaging 6.7 ha. Adults defend their territories only against members of the same sex. This intrasexual defense of territory maintains the monogamous mating system. Other social units were unmated resident females, unmated resident males, floating males, and a courting pair. Males establish territories before mating, perhaps with help from their parents. Males guard their families against predators. Females lead progression through the territory. Subadults remain peripheral to their families but other family members tend to remain within 10 m of one another.

Interaction of Hb A2 Indonesia trait with beta-thalassaemia trait and with Hb E trait.

The haematological findings resulting from the interaction of Hb A2 Indonesia trait with beta-thalassaemia trait and HbA2 Ind trait with Hb E trait are discussed. A person doubly heterozygous for Hb A2 Ind and beta-thalassaemia had mild haematological abnormalities essentially similar to those found in persons with beta-thalassaemia trait alone. A carrier for both Hb A2 Ind trait and Hb E trait had essentially normal haematological findings.

Philippines and Indonesia: on the way to a migration transition.

"This paper, in a comparative analysis of the Philippines and Indonesia, examines first under what conditions can migration favorably contribute to the process of economic development and then to what extent can economic growth impact upon reducing emigration pressures in these labor surplus economies. The paper also argues that there is still considerable scope for putting in place [an] agreed set of rules and policies to ensure better protection for the more vulnerable migrants."

Oral health condition and endemic goitre, in an iodine-deficient area in Bali, Indonesia.

Iodine deficiency is a major determinant of endemic goiter which affects bone growth. The purpose of this study was to clarify the relationship between iodine deficiency and oral health. A cross-sectional survey was carried out on 181 inhabitants aged 17 to 40 years in two villages in an iodine-deficient area in Bali, Indonesia. Thyroid swelling, serum level of thyroid-stimulating hormone, DMFT index, plaque index, gingival index, dietary habits and socioeconomic status were examined. Thirty two subjects (21%) had thyroid swelling considered to be endemic goiter, with the highest prevalence in females aged 21 to 30 years (40-41%). The DMFT, food habits, and socioeconomic status were different between both villages. Multiple regression analysis adjusted for age, sex, and location (village) did not detect any association between DMFT and serum thyroid-stimulating hormone level or thyroid swelling. Further studies will be required to confirm these findings.

Health practices and indices of a poor urban population in Indonesia. Part II: Immunization, nutrition, and incidence of diarrhea.

This second section in a two-part study of health indices and practices among a poor urban population in Indonesia focuses on problems related to immunization, nutrition, and incidence of diarrhea. Through a survey of 690 mothers and 593 children in two slum communities in Jakarta, we found that 1) 65% of the children were mildly to severely malnourished according to Gomez criteria, with malnutrition most prevalent among the poorest; 2) 75% of the mothers reported exclusive breastfeeding of their infants during the first four months after birth, but breastfeeding decreased sharply with increasing socioeconomic status; 3) of the 19% of one- to three-year-old children who were incompletely immunized, most were likely to be missing their measles and DPT3 immunizations; and 4) 28% of the mothers reported that at least one of their under-three-year-old children had diarrhea in the last month, with frequency of reporting highest among poor mothers and those using water from vendors. We concluded that selective interventions should be accompanied by development of a comprehensive health infrastructure that permits uninterrupted service delivery and follow up of those identified as high risk. Such a comprehensive primary health care system should be part of a multisectoral development strategy.

Regional study of nutritional status of urban primary schoolchildren. 2. West Jakarta and Bogor, Indonesia.

The study provides information on the nutritional status of 8- to 10-year-old primary schoolchildren in urban areas of Jakarta and Bogor, Indonesia, based on anthropometric indicators. It compares the use of the anthropometric indicators weight-for-age Z score, height-for-age Z score, weight-for-height Z score, and body mass index (BMI) to assess thinness (underweight and wasted) and overweight in children. A total of 1,367 children were examined. The nutritional status of the 8- to 10-year urban schoolchildren was better than that of urban children under 5 years old. The prevalence of underweight among urban schoolchildren ranged from 7.4% (girls) to 12.95% (boys), while underweight among urban children under 5 years old in 1998 was 29.7%. Meanwhile, the prevalence of overweight (BMI > 85th percentile) ranged from 15.3% (girls) to 17.8% (boys). There were more overweight children in the private schools than in the public schools. On average, private schoolchildren, of all ages and both sexes, were heavier and taller than public schoolchildren. The BMI indicator for "thinness" and "wasting", using the NHANES reference, indicates a false positive result. For detecting overweight in children aged 8 to 10 years, BMI is comparable to the other indices, weight-for-age, height-for-age, and weight-for-height.

Combating iodine deficiency: lessons from China, Indonesia, and Madagascar.

This study investigated the factors contributing to a successful and sustainable elimination of iodine-deficiency disorders, drawing from salt fortification experiences in China, Indonesia, and Madagascar. Government officials, salt farmers, salt producers, and wholesalers were interviewed to collect data during field visits. Analyses used in the study include simple correlation, and wherever data permit, regression. The study found that measures crucial for combating iodine deficiency include raising public awareness of the disorders, ensuring easy access to iodated salt, promoting compliance in the salt industry, and monitoring and enforcement. Factors that ensure a reliable supply of iodated salt are equally important as those that create the demand for it. Governments must ensure that surveillance and enforcement mechanisms are functioning right from the time that salt iodation is made compulsory. For sustainability during later years, the adequacy of iodine in iodated salt must be monitored, and incentives must be modified as needed to increase compliance rates in the salt industry. Once national coverage of iodated salt reaches over 90%, the government can concentrate on fine-tuning and targeting resources at areas with a low consumption of iodated salt. Elimination of micronutrient deficiencies has a long-term impact on public health; moreover, poorer segments of the population, who are more vulnerable to such deficiencies, have more to gain from fortification programs. Thus, lessons from the successful elimination of iodine-deficiency disorders are valuable for future similar micronutrient activities.

Food patterns during an economic crisis among pregnant women in Purworejo District, Central Java, Indonesia.

A cross-sectional study was conducted between 1996 and 1998. Six 24-hour recalls were performed during the second trimester of pregnancy among 450 women in Purworejo District, Central Java, Indonesia. The objectives of the study were to assess the food intake and food pattern among pregnant women before and during the economic crisis. Before the crisis, rich women had the highest intakes of animal foods, fats and oils, and sugar. Food intake among the urban poor and the rural landless poor subgroups was influenced by the emerging economic crisis. Although the price of rice increased, the intake of rice also increased among all subgroups. Rural poor women with access to rice fields increased their intake of rice and decreased their intake of nonrice staple foods (p < .05). There were significant decreases in the consumption of chicken by rich women and rural poor women with access to rice fields (p < .05). Rice was a strongly inferior good and remained an important supplier of energy, protein, and carbohydrate. Nuts and pulses were important suppliers of calcium and iron, and vegetables were an important supplier of vitamin A. Rich women increased their intake of nuts and pulses, vegetables, fats and oils, and sugar when their intake of rice increased (p < .05). The food patterns were based on rice, nuts and pulses, and vegetables, i.e., plant food. All but the rich women decreased their intake of nutritious foods such as meat, chicken, and fruits. The intake of nuts and pulses and of vegetables increased, whereas the intake of cooking oil and sugar remained constant.

PCR-based detection and identification of the filarial parasite Brugia timori from Alor Island, Indonesia.

Brugia timori is widely distributed on Alor Island, Indonesia, where it causes a high degree of morbidity. The HhaI tandem repeat of B. timori was found to be identical to that of B. malayi, for which sensitive PCR-based assays have already been developed. Using one of these assays, a single microfilaria (mf) of B. timori, present in a spot of dry blood on filter paper, could be detected. The assay was equally sensitive in the detection of B. timori and B. malayi. When the collected mosquitoes were pooled according to species and tested with the assay, 39 (64%) of the 61 Anopheles barbirostris pools (containing a total of 642 mosquitoes) were positive. As none of the 33 Culex pools tested (which contained 624 mosquitoes) gave a positive result, and An. barbirostris is the only Anopheles species commonly caught on human bait in Alor, An. barbirostris is assumed to be the main and perhaps only local vector. Brugia timori could be differentiated from B. malayi by restriction-endonuclease digestion of the PCR-amplified mitochondrial cytochrome oxidase subunit 2. A few distinct nucleotide exchanges were also found in the second internal transcribed ribosomal spacer of the filariae, and in the 16S rDNA and FTSZ gene of their Wolbachia endobacteria. The results show that B. timori can be effectively detected using the PCR-based assay developed for B. malayi and can then be differentiated from B. malayi by other molecular markers. PCR-based techniques targeting the HhaI repeat can therefore be employed for monitoring B. timori in the framework of the Global Programme to Eliminate Lymphatic Filariasis.

Chloroquine or sulfadoxine-pyrimethamine for the treatment of uncomplicated, Plasmodium falciparum malaria during an epidemic in Central Java, Indonesia.

A recent malaria epidemic in the Menoreh Hills of Central Java has increased concern about the re-emergence of endemic malaria on Java, which threatens the island's 120 million residents. A 28-day, in-vivo test of the efficacy of treatment of malaria with antimalarial drugs was conducted among 167 villagers in the Menoreh Hills. The treatments investigated, chloroquine (CQ) and sulfadoxine-pyrimethamine (SP), constitute, respectively, the first- and second-line treatments for uncomplicated malaria in Indonesia. The prevalence of malaria among 1389 residents screened prior to enrollment was 33%. Treatment outcomes were assessed by microscopical diagnoses, PCR-based confirmation of the diagnoses, measurement of the whole-blood concentrations of CQ and desethylchloroquine (DCQ), and identification of the Plasmodium falciparum genotypes. The 28-day cumulative incidences of therapeutic failure for CQ and SP were, respectively, 47% (N = 36) and 22% (N = 50) in the treatment of P. falciparum, and 18% (N = 77) and 67% (N = 6) in the treatment of P. vivax. Chloroquine was thus an ineffective therapy for P. falciparum malaria, and the presence of CQ-resistant P. vivax and SP-resistant P. falciparum will further compromise efforts to control resurgent malaria on Java.

Hypothyroidism in an area of endemic goiter and cretinism in Central Java, Indonesia.

In an area of severe endemic goiter in Central Java, Indonesia, clinical overt or mild hypothyroidism appeared to be present in 7 out of 20 cretins and also in 12 out of 94 non-cretinous subjects, all 5-20 years of age, living in the village of Sengi. Hypothyroidism was not found in a control group of 70 subjects of the same age living in Londjong just outside the edemia. In hypothyroid subjects the plasma PBI-concentration was 0.98+/-0.32 mug/100 ml (mean+/-SD) vs. 2.72+/-1.24 mug/100 ml in euthyroid subjects from Sengi and 4.86+/-0.80 mug/100 ml in controls from Londjong. Values for T3 were 56.3+/-3.17 ng/100 ml in hypothyroids, 140.5+/-38.5 ng/100 ml in euthyroids from Sengi and 121.6+/-27.4 ng/100 ml in controls. The TSH levels (geometric mean and range) in these 3 groups were, respectively, 210.1 (108.0-342), 15.6 (3.0-372) and 4.1 (0.8-7.0) muU/ml. The differences between the mean concentration of PBI, T3 and TSH in the hypothyroid and euthyroid groups were highly significant (P less than 0.001). These data strengthen the clinical diagnosis of hypothyroidism in cretins as well as in non-cretinous subjects. All hypothyroid subjects had a PBI less than 1.8 mug/100 ml and T3 less than 120 ng/100 ml and TSH greater than 100 muU/ml. In 8 hypothyroid subjects, restudied 18 months after iodized oil injection, hypothyroidism was either corrected or markedly improved. It therefore appears that iodine deficiency per se in postnatal life may lead to (juvenile) hypothyroidism, which can be corrected by iodine therapy. Our findings have implications for the definition and diagnosis of endemic cretinism. Not all hypothyroid subjects in an area of endemic iodine deficiency should be classified as cretins.

Indonesia medicinal plants. III. On the constituents of the bark of Fagara rhetza (Rutaceae). (1): Alkaloids, phenylpropanoids, and acid amide.

Two new phenylpropanoids, named O-geranylsinapyl alcohol (1) and O-geranylconiferyl alcohol (2), and a new acid amide, named hazeleamide (3), were isolated from the bark of Fagara rhetza (Rutaceae), an Indonesian medicinal plant from Flores Island, Indonesia. The chemical structures of 1, 2, and 3 have been elucidated on the basis of their chemical and physicochemical properties. Among the three new compounds, hazeleamide (3) was found to show a pungent taste and to exert a moderate antimalarial activity in an in vitro test system.
