[Forceps delivery under local-regional anesthesia in current obstetrical practice. Apropos of 400 cases].

We herewith report 400 cases of forceps delivery under local regional block anesthesia of the internal pudental nerves. They were carried out together with routine episiotomy. The indications were the purely classical ones and the results were good. There were very few fetal complications and these were in all instances mild. There were very few maternal complications. So long as some precautious are taken we have in our hands an excellent method for extracting a baby with instruments and it should not have come into disrepute as it has (in comparison with the vacuum extractor).

Setleis (bitemporal 'forceps marks') syndrome in a German family: evidence for autosomal dominant inheritance.

The Setleis syndrome is a rare disorder characterized by predominantly facial findings, including bitemporal skin changes resembling forceps marks. Autosomal recessive inheritance of this distinct condition has been proposed. We report on a typically affected German boy whose father shows a much milder expression, thus suggesting autosomal dominant inheritance.

Two new laparoscopic instruments: bipolar sterilizing forceps and uterine manipulator.

Two new instruments have been found useful in laparoscopic surgery. One is a bipolar forceps used primarily for sterilization operations performed on the Fallopian tube with which discrete lesions can be made. The bipolar principle avoids electrical trauma at distant sites. The other, an intrauterine manipulator, permits better uterine positioning during the procedure without the need for an assistant. Tubal lavage can also be performed with this instrument.

Laparoscopic removal of an artery forceps.

Numerous foreign bodies, such as surgical gauze, pads and instruments, and other items, have been left behind in the abdominal cavity during open surgeries. These have been traditionally removed at redo open surgeries. Here we describe a case of an artery forceps left behind at a previous surgery (open cholecystectomy and appendicectomy) performed 5 years earlier that was removed by laparoscopy.

Clinical interpretation of the histological typing of gastric cancer using endoscopic forceps biopsy.

BACKGROUND/AIMS: Histological typing of gastric cancer is important for determining the treatment strategy and predicting the prognosis. We compared the histological types obtained by endoscopic forceps biopsy with the finally determined histological types of surgically resected specimens to investigate the reliability of histologically typing gastric cancer by biopsy. METHODOLOGY: Agreement between the biopsy typing and the final histological typing based on the predominant histology of the resected tumor was studied in 115 consecutive gastric cancers. RESULTS: The overall agreement rate of histological typing of gastric cancer was 75.7%. In patients with early gastric cancer, the agreement rate was a high 82.5% and the final histological type was usually predicted. In patients with advanced gastric cancer, the agreement rate was 72.0%, which was significantly lower than for early gastric cancer (p<0.05). The agreement rate for advanced differentiated cancer was also significantly lower than that for early cancer (63.6% vs. 90.0%) (p<0.05). CONCLUSIONS: The reliability of using histological typing by biopsy to set the indications for endoscopic treatment and for preoperative prediction of the prognosis is expected to be good for differentiated early gastric cancer, but it might not be adequate for advanced cancer or undifferentiated cancer.

A particular case of a "captive forceps" in the esophagus.

We present a particular case of a "captive" forceps in the esophagus in an attempt to remove a foreign body in a patient with previous esophageal post-caustic stenosis. The mechanism of incarceration and the surgical therapeutic option consisting of open thoracic surgery are detailed and argumented. Postoperative course was favorable, though the patient developed a small esophageal fistula visible at radiology without any clinical expression. This case emphasizes the difficulties that may occur but in the management of esophageal foreign bodies in patients with esophageal post-caustic stenosis, which may lead finally to open surgery.

[A method of tying a surgical knot with one hand which allows the surgeon to maintain tension on the ligature while simultaneously releasing the forceps].

The author describes the preparation of a surgical Knot with one hand, whereby one end of the ligature during ligation of the artery is held by the thumb and index finger and the second one is twisted round the little finger of the same hand in the distal interphalangeal groove. When tightening the knot, simultaneously the forceps is released, using the other hand.

[A biopsy forceps with a needle for a flexible endoscope].

Biopsy forceps with a suture needle were developed. They will enable to increase considerably the efficacy and accuracy of diagnostic procedures in early cancer and precancerous conditions, and the differentiation of malignant and benign affections of internal organs as well.

[Closure of the vagina after colpohysterectomy using an automatic suture forceps (author's transl)].

Closure of the lower third of the vagina using a TA 55 automatic suture forceps is an easy procedure which offers the best guarantee from a septic and haemostatic standpoint during the peroperative phase. We have used the technique on 50 occasions. In all cases, it gave a linear vaginal scar without granuloma. Used in association with sub-peritoneal suction drainage it brings about a reduction in the number of lymphoceles and their complications.

[The use of a new type of haemostatic forceps in trauma of the liver (author's transl)].

Clamping of the hepatic pedicle is an emergency act, limited in time and ineffective in cases of vena cava-hepatic vein reflux. The single forceps suggested are particularly well suited to the morphology of the right lobe, affected in 70% of cases of liver trauma, providing equipotential parenchymatous stereocompression. They have the advantage of being selective, complete, and reversible, and may be applied for an indefinite period. They provide rapid biliostasis and haemostasis and provide time for thorough exploration of the abdominal cavity without any risk of total hepatic ischaemia.

[Biopsy of chorionic villi by forceps under ultrasonic control: technic and results].

We describe an original technique for villi sampling using a rigid forceps under ultrasound control. 120 chorion biopsies were performed, the success rate of sampling is 100%, the samples are pure without any maternal contamination in 90% of cases. The fetal loss rate needs further evaluation because the number of continuing pregnancies is still too small to allow enough accurate assessment. No miscarriage was observed among our last 70 patients. At the beginning of our experience we preferred to start our diagnoses only for high genetic risk diseases: haemoglobinopathies, hereditary metabolic diseases, lethal X-linked diseases. We insist on the efficiency and the quickness of this method and on the quality of the samples which makes the diagnosis safer.

Comparison of transbronchial fine needle aspiration biopsy, aspiration of bronchial secretion, bronchial washing, brush biopsy and forceps biopsy in the diagnosis of lung cancer.

The accuracy of morphologic diagnosis from transbronchial fine needle aspirates (TBN), bronchial secretions (BS), bronchial washings (BW), brush biopsy (BB) and forceps biopsy (FB) sampled via a flexible fiberoptic bronchoscope, was studied in 60 malignant lung tumours. In visible tumours the best results were obtained with FB (85%) while TBN was positive in 65%. In cases were bronchoscopy only showed secondary signs of tumour, the sensitivity of FB, BB and TBN was higher than that of BW and aspiration of BS. In tumours not visible through the bronchoscope, the diagnostic accuracy of BB, BW and aspiration of BS was higher than that of FB and TBN. In the diagnosis of carcinoma of the lung the diagnostic accuracy of combining FB and one cytologic method (BW) was higher than that of any of the single methods (p less than 0.05) while no appreciable increase was achieved by using additional methods.
