[Corneal oxygen demand: open eye versus closed eye conditions (author's transl)].

Oxygen demands of the mammalian cornea were measured under open eye and closed eye conditions. With the lids gently sealed, oxygen availability to the cornea was found to be about 2/3's that predicted by models in the literature. While the silicone contact lens (0.25 mm thickness) tested under those closed eye conditions still allowed enough oxygen to reach the cornea to just avoid clinically detectable edema, a Hema lens of conventional cosmetic thickness (0.15 mm) would be expected to induce significant edema under those sealed lid circumstances.

The National Eye Health Education Program: increasing awareness of diabetic eye disease among American Indians and Alaska Natives.

With the highest prevalence of diabetes in the United States, American Indians and Alaska Natives are at greatest risk for diabetic eye disease (DED), a leading cause of blindness. The National Eye institute (NEI) conducted formative research to understand DED-related knowledge, identify approaches to managing this disease, and design a communication plan to increase awareness and reduce DED among these populations. The NEI conducted qualitative research at five locations in indian country with representatives from national organizations, tribal members, and healthcare providers. While diabetes ranked high on their list of primary community health issues in need of attention, study participants had only a basic level of diabetes-related knowledge, acknowledged the need for DED education, and underscored the importance of the use of interpersonal and culturally appropriate communication strategies. This is the first exploratory qualitative research study to examine the status of diabetic eye disease among American indians and Alaska Natives whose primary purpose was to inform the design of a national DED communication campaign.

[Preventive treatment of retinal detachment of the contralateral eye. Results of 5 years follow-up of 109 eyes].

In 1987, we presented a study of 141 eyes treated prophylactically following retinal detachment in the fellow eye. A hundred and nine of these patients are reviewed with a minimum follow-up of five years. Only the evolution was studied, whether a peripheral retinal lesion was present or not at the time of prophylactic treatment. The examiner noted if new lesions had occurred, or if lesions present at the time of the first examination had developed: lattice degeneration or snail track developing into a retinal tear, hole or tear causing a retinal detachment, contained or not by laser treatment. Our study contained sixty two myopes. Fifty eyes had visible, dangerous lesions, and fifty nine a normal retina. Forty five circular barrages with four anterior radial rows returning to the ora serrata were performed, and sixty four barrages with tight anterior grids and a localised barrage of visible lesions. The results were judged on one criterion only: retinal detachment requiring emergency surgery. Only one retinal detachment occurred behind the barrage, caused by proliferative vitreoretinopathy due to multiple tears, and surgical results were good. Eight tears appeared in front of the barrage, five of these in healthy retina. Two very posterior tears behind the barrage were blocked with no problem by laser treatment. Four localised detachments occurred in front of the barrage, three of these without anterior grid treatment. The advantage of this is undeniable since out of sixty three cases treated in this way, only three tears were seen to be blocked by the laser shots, and one anterior localised detachment where the anterior grid was insufficient.(ABSTRACT TRUNCATED AT 250 WORDS)

[Luminance contrast and color contrast evoked pattern electroretinogram in normal eyes and in eyes with glaucoma].

Pattern-onset ERGs obtained with color contrast (red-green, blue-yellow) and luminance contrast (green "black") square-wave-stripe patterns (spatial frequency 0.3 c/deg, 32 degree field) were studied in larger groups of normal and glaucomatous (chronic open-angle glaucoma) eyes. Colored stimuli were generated in a two-channel Maxwellian-view system using grating monochromators. In producing isoluminant lights of different colors, the peak latency of the response was used as a criterion. Luminance-contrast responses always have significantly smaller amplitude and shorter peak latency than color-contrast responses. This can be explained by the smaller number and shorter latency of the non-color-coded phasic anglion cells and by the larger number and longer latency of the color-coded tonic cells. Glaucomatous eyes respond with significantly smaller amplitudes at all contrast conditions tested; however, this response reduction is most pronounced with red-green patterns. Peak latencies are not significantly altered in glaucomatous eyes. Since red-green patterns are most appropriate in showing glaucoma damage in the pattern ERG, the largest absolute losses seem to occur among the large red-green antagonistic group of ganglion cells.

[Results of research on the microflora of the conjunctiva with either chemotherapeutically pretreated eyes or eyes not influenced by drugs concerning the question of the preoperative prophylaxis against infections (author's transl)].

In this study the results of 396 smears of conjunctiva are analysed concerning the microflora of chemotherapeutically pretreated eyes in comparison with eyes not treated. The results of microbiological culture indicate only a small statistically unsignificant difference in regard to the quantitity of sterile smears as well as the distribution of bacteria of both research groups. The discussion is on the question of unaimed preoperative prophylaxis of infection as conditio sine qua non before intraocular procedures on eyes free of phlogistic irritation.

Intrinsic and extrinsic signals in the developing vertebrate and fly eyes: viewing vertebrate and invertebrate eyes in the same light.

The development of complex tissues, such as the nervous system, requires coordinated interactions between cells, and defining the molecular basis of these interactions is a primary goal of developmental neurobiology. Classic experimental embryological studies of the developmental potential of embryonic neural tissues have more recently been augmented by genetic and molecular approaches to yield a picture of the mechanisms by which such complex neural structures arise during development. Perhaps coincidentally, two of the most well-studied structures in this regard have been the eyes of Drosophila and vertebrates. In the past few years, a number of laboratories have focused on the development of the eye, of both Drosophila and vertebrates in order to understand the role of cell-cell interactions in the generation of organized neural structures. Recent evidence from both the vertebrate and Drosophila models suggests that common mechanisms apply in both systems, particularly in the role of tyrosine kinases in regulating phenotypic choices during histogenesis. Thus, although the eyes of flies and vertebrates have very different appearances, we examine the possibility that similar developmental processes are used to produce tissues with analogous functions.

[Quantitative analyses of glycosaminoglycans in tear fluids in normal human eyes and eyes with corneal epithelial disorders].

The existence and pathophysiological role of glycosaminoglycans in the tear fluid in humans was investigated using quantitative analyses of hyaluronic acid and chondroitin sulfate in the tear fluid. The subjects were 42 eyes of 31 normal controls, 9 eyes of 9 patients with superficial punctate keratitis (SPK), and 13 eyes of 13 patients with epithelial defect. After an instillation of 100 microliters saline solution in the conjunctival sac, as much tear fluid as possible was collected from the lower cul-de-sac. The glycosaminoglycans in the tears were then treated with chondroitinase ABC to make fractions of unsaturated disaccharides. The quantities of disaccharides were determined by high-performance liquid chromatography. Concentrations were expressed as nanomoles of unsaturated disaccharides per protein in the tears. The concentrations of hyaluronic acid and chondroitin sulfate in the normal controls were 0.07 +/- 0.12(n mol/mg protein) and 6.91 +/- 3.63 (n mol/mg protein), respectively. The mean concentration of hyaluronic acid was significantly higher in patients with epithelial erosion than in normal controls, whereas the mean concentration of chondroitin sulfate was significantly lower in patients with epithelial erosion than in normal controls. There was no significant difference in the concentration of glycosaminoglycans between the patients with SPK and normal controls. The results of our study suggest that glycosaminoglycans are synthesized and endogenously secreted into the tear fluids and, especially in the case of hyaluronic acid, may play an important role in corneal epithelial wound healing in patients with epithelial erosion.

[Effects of long-term application of bunazosin hydrochloride eye drops on the aqueous flow rate and blood-aqueous barrier permiability in rabbit eyes].

We investigated the effects of long-term instillation of bunazosin hydrochloride--alpha 1 specific adrenergic antagonist--on the aqueous humor dynamics and blood-aqueous barrier in rabbit eyes. We examined intraocular pressure, aqueous flow rate, and blood-aqueous barrier permiability for albumin after four weeks application of 0.05% bunazosin. The effect of bunazosin on blood-aqueous barrier destruction by laser iridophotocoagulation was also examined. During four weeks, bunazosin reduced the intraocular pressure by 1.7 +/- 0.6 mmHg (mean +/- SEM). Continuous application of bunazosin had no significant influence on aqueous flow rate and fluorescein isothiocyanate-labeled rabbit albumin (FITC-Alb) concentration in the anterior chamber. Bunazosin had no effect on the rise of the FITC-Alb concentration after iris photocoagulation, but the intraocular pressure in bunazosin treated eyes was significantly lower than in control eyes one hour after photocoagulation.

Familial aggregation of lens opacities: the Framingham Eye Study and the Framingham Offspring Eye Study.

Eye examination data from 1,086 parents examined in the Framingham Eye Study (1973-1975) and 896 offspring examined in the Framingham Offspring Eye Study (1989-1991) were used to study familial associations for nuclear, cortical, and posterior subcapsular lens opacities. Associations between spouses, between parents and offspring, and between siblings were explored. The generalized estimating equations (GEE2) approach to logistic regression was used to simultaneously model risk of lens opacity and familial associations. Specifically, this provided odds ratios, adjusted for age (the age range was 50-74 years) and sex, for the familial associations. For any pair of siblings, the odds of nuclear opacity for one sibling was estimated to more than triple if the other sibling had a nuclear opacity. Similar findings were noted for posterior subcapsular opacity. No significant associations were noted for any of the opacity types between spouses or between parents and offspring. The strong associations between siblings for nuclear and posterior subcapsular opacities suggest there is clustering of lens opacities within families. The clustering may be due to genetic or environmental factors.

[Functional magnetic resonance imaging of the frontal eye fields during saccadic eye movements].

We evaluated activity-induced signal intensity changes in the human cerebral cortex during horizontal saccadic eye movements using functional magnetic resonance imaging (fMRI) based on the blood-oxygenation-level-dependent (BOLD) contrast method. Compared with central fixation, significant signal increases were observed bilaterally in the middle frontal gyrus (Brodmann area 8) during saccadic conditions. The location of the activated area was consistent with that of previously reported frontal eye fields (FEF). These results suggest that fMRI has potential merit for the study of cortical control of eye movements in humans.

Penetrating eye injuries related to assault. The National Eye Trauma System Registry.

From 1985 to 1991, 648 cases of assault-related penetrating ocular injury were reported to the National Eye Trauma System Registry. We examined the circumstances surrounding and initial clinical findings related to this case series of injuries. Eighty-three percent of the patients were male. The median age was 28 years; 77% were younger than age 40 years, and 17% were younger than age 20 years. The ocular injury was part of multiple trauma in 34% of cases. There was evidence of alcohol and illicit drug use by at least 48% and 6% of the injured persons, respectively. Seventeen percent of the injuries involved powder or nonpowder firearms. Fists, glass fragments, metal rods or pipes, knives, and scissors were among the other objects causing injury. Posterior segment trauma, which occurred in 70% of cases, included vitreous hemorrhage (40% of cases), retinal detachment (11%), presence of intraocular foreign bodies (6%), and optic nerve damage (4%). The initial visual acuity after injury was hand motion or worse in 74% of the cases. Strategies to prevent such injuries focus on the origins of violence in general and are more difficult to devise than strategies to prevent eye injuries in other settings. Public health efforts to reduce the use of alcohol, drugs, and firearms may reduce the incidence of these injuries.

The pink-eyed dilution locus controls the biogenesis of melanosomes and levels of melanosomal proteins in the eye.

The pink-eyed dilution (p) locus is known to control the quantity of melanin pigment made within melanocytes and retinal pigment epithelium (RPE) in the eye. We have examined the effects of several mutant allele combinations at the murine p locus on the number and morphology of melanosomes in choroidal melanocytes and RPE cells as well as on the levels of four proteins known to be present within melanosomes: tyrosinase, tyrosinase-related proteins 1 and 2 (TRP-1 and TRP-2) and lysosome-associated membrane protein-1 (LAMP-1). By electron microscopy, we observed a modest diminution in the size and number of choroidal melanosomes in pbs/pJ mice but a more dramatic decrease in the RPE in comparison with wild-type P/P mice. By contrast, a drastic reduction in melanosome size and number was present in the choroid and RPE of pun/pun and p6H/pcp mice, and in the RPE of p6H/pcp mice, melanosomes were essentially undetectable. In wild-type mice, levels of tyrosinase, TRP-1 and TRP-2 were high at birth and showed a second peak of expression at 10-14 days of age, declining to undetectable levels by 42 days. All three mutant allele combinations reduced the levels of these melanosomal proteins with the relative severity of effects being p6H/pcp>pun/pun>pbs/pJ. In the null p6H/pcp mice, levels of these proteins were extremely low at birth, no postnatal peak was observed, and levels declined to undetectable by 14 days. Levels of LAMP-1 in wild-type mice rose initially and then declined whereas in the mutant mice, levels decreased gradually from birth. Higher levels of LAMP-1 were observed in each of the mutants than in the wild-type mice at 21 days of age. Our results demonstrate that mutations at the p locus affect the size, number, shape and contents of melanosomes, implicating the p gene product in the normal biogenesis of this organelle.

Comparing eye movements recorded by search coil and infrared eye tracking.

OBJECTIVE: The performance of a new video-based infrared eye tracker (IR) was compared to the magnetic search coil technique (SC). Since the IR offers interesting possibilities as a diagnostic tool in neuro-ophthalmology, it was investigated whether the new device has overcome shortcomings that were reported from former IR systems. METHODS: Horizontal saccades were recorded using the IR and the SC. The IR allowed eye movement recordings at different sampling rates ranging from 250 Hz to 1000 Hz while the SC recorded at 1000 Hz. RESULTS/CONCLUSIONS: The results show that the IR and the SC were in good agreement and produced similar results. In contrast to other studies, the influence of the sampling rate of the IR was small. The saccade main-sequences did not show significant differences. The latency times observed for both systems were mainly in the short-latency range.

Objective measurement of postocclusion surge during phacoemulsification in human eye-bank eyes.

PURPOSE: To objectively compare the postocclusion vacuum surge among different phacoemulsification machines and devices. DESIGN: Experimental study. METHODS: Infiniti, Legacy, Millennium, and Sovereign were tested in an eye-bank eye. All the machines were tested with 20-gauge non-ABS tips, 430 mm Hg vacuum pressure, 24 ml/minute aspiration rate, peristaltic pump, and 75 cm bottle height. In addition, Infiniti and Legacy were also tested with 20-gauge bypass tips (ABS), 125 cm bottle height, and 40 ml/minute flow rate. We also tested 19-gauge tips with Infiniti and Sovereign and the venturi pump for Millennium. RESULTS: Significant differences were found between all the machines tested with Millennium peristaltic generating the least and Millennium Venturi the most surge. ABS tips significantly decreased the surge for Legacy but not for Infiniti. Cruise Control (CC) had a significant effect on Sovereign but not on Millennium. Increasing the bottle height decreased surge while increasing the flow increased surge for both Infiniti and Legacy. The 19-gauge tips increased surge for both Infiniti and Sovereign. CONCLUSIONS: Surge varied over a range of 40 microm to more than 2 mm. ABS and CC decrease surge, especially when the machine is not functioning near the limits of surge prevention. Certain parameters, such as a 19-gauge tip and high flow, dramatically increased surge, whereas elevating the bottle ameliorates it. Understanding the impact of all these features will help in minimizing the problem.

Pax6 heterozygous eyes show defects in chamber angle differentiation that are associated with a wide spectrum of other anterior eye segment abnormalities.

The development of the chamber angle was studied in the eyes of heterozygous Pax6(lacZ/+) mutant mice (Nature 387 (1997) 406). Mutations in PAX6 cause aniridia, a condition that is frequently associated with glaucoma, a blinding disease that may be associated with chamber angle defects. Mesenchymal cells were seen in the chamber angle at P1-P5. In wild-type mice, these cells differentiated into typical trabecular meshwork (TM) cells next to Schlemm's canal. In Pax6(lacZ/+) mice, TM cells remained undifferentiated and Schlemm's canal was absent. From P1 to P4, staining for beta-galactosidase and immunoreactivity for Pax6 were observed in chamber angle mesenchyme, but were absent later. Cultured murine TM cells expressed Pax6. The defects in chamber angle and TM differentiation were associated with a wide spectrum of other anterior eye defects, which included various degrees of iris hypoplasia and corneal haze, isolated iridocorneal adhesions and atypical coloboma, and a vascularized cornea in all adult animals. A third of the animals showed Peters' anomaly including corneal opacity and iridocorneal adhesions. The separation of the lens from the cornea was incomplete, and epithelial layers of lens and cornea were continuous. Pax6 activity is directly required for differentiation of the chamber angle. Variations in phenotype of Pax6(lacZ/+) mice appear not to involve direct dominant-negative or dose-dependent effects.

Developmental genetics: vertebrates and insects see eye to eye.

The results of a number of recent studies indicate that eye development in insects and vertebrates may have more features in common than hitherto suspected. The results support the possibility that insect and vertebrate eyes evolved from a complex ancestral organ.

Increase in retinyl palmitate concentration in eyes and livers and the concentration of interphotoreceptor retinoid-binding protein in eyes of vitiligo mutant mice.

Retinyl esters play an important role in the visual cycle because they are involved in regeneration of 11-cis-retinal for use in rhodopsin formation. In the present study, retinyl ester concentrations were significantly elevated in eyes and livers of mice homozygous for the vitiligo mutation (mivit/mivit). Vitiligo mice demonstrate a slowly progressing retinal degeneration characterized by gradual loss of photoreceptor cells and rhodopsin as well as uneven pigmentation of the retinal pigment epithelium (RPE). Analysis of retinoids by h.p.l.c. indicated that the retinyl palmitate level was increased fivefold in eyes of affected mice at 10 weeks postnatally and was threefold higher at 22 weeks of age. Accumulation of retinyl palmitate occurred in the RPE rather than the neural retina. Furthermore, the concentration of all-trans-retinol was elevated in the RPE of vitiligo mice. Levels of interphotoreceptor retinoid binding protein (IRBP) were increased in vitiligo mice between ages 4 and 14 weeks, but returned to normal by 16 weeks. Increased IRBP levels were not due to increased protein synthesis because IRBP mRNA levels did not differ significantly between control and affected animals. To examine possible systemic involvement in vitiligo mice, retinoids were evaluated in liver and plasma. Mean hepatic total vitamin A levels in affected mice were approximately 1.7 times higher than controls. Analysis of esterified and non-esterified retinoids in liver showed that the concentration of retinyl palmitate was elevated. Plasma retinol levels were normal. This study provides the first evidence of altered systemic retinoid metabolism in vitiligo mice, which occurs, significantly, under normal dietary conditions.

Allergic and immunologic disorders of the eye. Part I: immunology of the eye.

Immuno-ophthalmology evolved during the 20th century as a subspecialty linking ophthalmologists and immunologists. This emerging subspecialty has focused on the use of immunology to better understand and treat ocular disorders. To help the allergist/clinical immunologist better appreciate the growing field of immuno-ophthalmology, this 2-part review series (Part II: Ocular Allergy will appear in the December issue of the Journal) will provide an overview of the impact that immunology has had on our understanding and treatment of allergic and immunologic eye diseases. The current review will focus on mechanisms by which mast cells, T cells, eosinophils, cytokines, and other inflammatory constituents contribute to the unique features of eye disease and their link to allergic responses that occur in other organs of the body.
