[Primary treatment of gunshot and explosion injuries of the extremities with the Ilisarov ring fixator].

High-energy missile and explosion injuries of the upper and lower extremities are complex and require treatment over a long time. Following classic strategies of surgical therapy, bone defects can be reconstructed only after the injured soft tissue has healed. The callus distraction technique now permits the beginning of bony reconstruction at the same time as the initial surgical debridement. During the period 1 July 1987 to 30 June 1990, 25 grade III open fractures in 24 patients were primarily treated with the Ilisarov technique at Wazir Akbar Khan Hospital, Kabul, Afghanistan. Twelve months after the removal of the fixation devices the patients were examined and the following results were demonstrated: no amputations; bony, aseptic healing in 22 cases (88%); 3 non-unions, including 2 infected non-unions; 2 axial deviations > 10 degrees. The primary initiation of bony reconstruction by callus distraction offers numerous advantages in the treatment of gunshot and explosion injuries.

[The diagnosis of damages to the auditory system in the early period of explosive mine trauma and the optimization of treatment for the victims].

The wide use of the mine-explosive weapon in modern military conflicts is accompanied by high specific weight of mine-explosive traumas (MET), which achieve 30-40% of all traumas. In the departments of the hospital in 21.3-35.8% of the wounded with MET accompanying defeat of acoustical system with probable subsequent development of neurosensory hypoacusis and invalidism of the injured is established. It defines necessity of early ENT-inspections of all injured with such traumas. Steady up to 1-6 months reduction of hearing is accompanied by biochemical infringements of a cerebrospinal liquid.

[Resuscitation care in explosive mine wounds (based on the experience of delivering medical care to the wounded in the Transcaucasian region)].

From 1990 till 1994 people aged 15-32 years with mine-explosive wounds were admitted to the military hospital, 34 of them (47.2%) needed intense therapy. Multiple and combined injuries (82.4%) prevailed. The authors made an analysis of the terms of giving qualified and specialized medical aid, causes of lethal outcomes and specific features of performing intense therapy for the mine-explosive wounds.

[Characteristics of the radio-ecologic situation in the area of Totsk nuclear explosion and the immune status of schoolchildren residing in the vicinity].

Environmental pollution caused by radioactive and non-radioactive chemicals, and the immunity of schoolchildren who reside in the area exposed to the Totsk nuclear explosion (1954) are comparatively analyzed. The results indicate that the area under study has higher levels of 137Cs than that of a control area. There were immunological changes in children, which are obviously associated with the nuclear explosion.

Indirect laser-induced fluorescence detection of explosive compounds using capillary electrochromatography and micellar electrokinetic chromatography.

Mixtures of nitroaromatic and nitramine explosive compounds and their degradation products were analyzed using electrokinetically driven separations with both indirect laser-induced fluorescence (IDLIF) and UV absorption detection. Complete separations of the 14-component mixture (EPA 8330) were achieved using both capillary electrochromatography (CEC) and micellar electrokinetic chromatography (MEKC). IDLIF detection was performed using an epifluorescence system with excitation provided by a 635 nm diode laser and micromolar concentrations of the dye Cy-5 as the visualizing agent. While the sensitivity of the two detection methods was similar for the nitroaromatic compounds, the nitramines could only be detected using UV absorption due to their low fluorescence quenching efficiency of Cy-5. The detection sensitivity using IDLIF was limited by low frequency oscillations in the fluorescence background. The oscillations increased with higher electric field strength and were attributed to thermal fluctuations caused by Joule heating. Due to the more conductive running buffer and higher separation currents used in MEKC, sensitive IDLIF detection could only be achieved using low (approximately 100 V/cm) field strengths, resulting in long analysis times. CEC separations, which are typically run with low conductivity mobile phases to avoid bubble formation, are less sensitive to this effect. In CEC separations with IDLIF detection a stable fluorescence background using Cy-5 could be established using only a nonporous stationary phase. In capillaries packed with porous silica particles, anomalous migration behavior was observed with charged dye molecules and a stable fluorescence background could not be established under electrokinetic flow. This is the first demonstration of IDLIF in packed channel CEC.

Poisoning as a result of barium styphnate explosion.

BACKGROUND: Acute barium poisoning is most often the result of accidental or suicidal ingestion of the rodenticide, barium carbonate. We describe a trauma patient whose condition was complicated by severe acute barium toxicity from an explosion of the propellant, barium styphnate. In addition to critical injuries, the patient manifested classic signs of barium toxicity including repeated profound hypokalemia, cardiac arrhythmias, respiratory failure, prolonged gastrointestinal dysfunction, paralysis, myoclonus, hypertension, and profound lactic acidosis. METHODS: The patient required lidocaine for ventricular bigeminy, massive infusions of potassium, prolonged ventilatory support, and parenteral nutrition to manage the effects of barium toxicity. RESULTS: He is the first reported case to demonstrate recurrent profound hypokalemia as an effect of blood transfusions. CONCLUSIONS: Considering the paucity of information concerning management of this life-threatening problem, the pathophysiology of barium toxicity, including the transfusion related hypokalemia, and its management is reviewed.

Investigation of the ionisation and fragmentation behaviour of different nitroaromatic compounds occurring as polar metabolites of explosives using electrospray ionisation tandem mass spectrometry.

In order to develop a liquid chromatography/electrospray ionisation tandem mass spectrometry (LC/ESI-MS/MS) method for identification and quantification of polar metabolites of explosives using a triple quadrupole system, the mass spectrometric ionisation and fragmentation behaviour of different nitrophenols, nitro- and aminonitrobenzoic acids, nitrotoluenesulfonic acids, and aminonitrotoluenes was investigated. Due to their different molecular structures, the substances concerned showed a very different ionisation efficiency in the ESI process. Interestingly, 2,4-dinitrobenzoic acid yielded no mass signals in the Q1 scan suggesting a thermal decarboxylation in the ion source, whereas the corresponding 3,5-isomer showed a high ionisation yield. Using negative ionisation polarity, carboxylic, phenolic, and sulfonic acid groups were deprotonated resulting in molecular anions, which could be fragmented in a collision cell. A pronounced dependency of the produced fragment ion series on the kind and position of substituents at the nitrobenzene ring (ortho effects) was observed and exploited for the development of substance-specific detection methods in the multiple reaction monitoring mode. In case of benzoic and sulfonic acids, decarboxylation and desulfonation, respectively, were observed as the most frequent fragmentation reactions. Furthermore, besides loss of NO(2), NO fragmentation occurred and preceded a decarbonylation of the benzene ring. The expulsion of the open-shell molecules NO and NO(2) led to a variety of distonic radical anions.

Dinitrotoluene: an assessment of occupational absorption during the manufacture of blasting explosives.

Two biological monitoring studies were carried out among workers in an explosives factory who were exposed to technical grade dinitrotoluene (DNT). In the first study urine samples from 28 workers were analysed for the metabolite 2,4-dinitrobenzoic acid (2,4-DNBA). Metabolite concentrations in urine were extremely low or non-detectable, prior to starting work at the beginning of the working week, but post-shift urine samples contained a mean 2,4-DNBA level of 17 mg/l. There were wide variations in the concentrations excreted in urine by different workers and by individual workers on consecutive days. Atmospheric levels of DNT (determined by personal monitoring) were found to be well below the recommended limit and therefore could not account for the observed excretion of 2,4-DNBA. It is suggested that skin may be the major route of absorption of DNT during this process. A second study was carried out to investigate the kinetics of absorption and excretion of DNT. Intensive urine sampling was carried out on five individuals over a 2-day period with additional samples over the subsequent 2 non-working days. Analysis for 2,4-DNBA showed that uptake of DNT is rapid and that the highest levels were normally seen in the end-of-shift specimens. Urine samples were analysed for other known metabolites of DNT which have been found in animal studies and it was shown that 2,4-DNBA is the major known metabolite which is excreted in human urine. Unchanged DNT was detected in blood samples taken during a single workshift at levels up to 250 ng/ml. It is concluded that there is a measurable uptake of DNT by the group of workers studied. The measurement of 2,4 DNBA in end-of-shift urine samples is an appropriate way of assessing the effectiveness of measures taken to reduce the absorption of DNT.

A suicidal death by explosives.

A 23-year-old man committed suicide by detonating an explosive substance clutched to his abdomen. The face was singed, the hands were lacerated and the chest and the upper part of abdomen were badly mutilated. The death was due to the destruction of several vital organs. The injuries had a directional nature. Only the consideration of all the facts of the case in relation to the situation, nature, distribution and extent of wounds and the preceding medical and social history allowed a reconstruction of the circumstances.

Traumatic autonephrectomy due to explosion: a case report and literature review.

A 47-year-old man presented 33 years after major trauma resulting from an explosion with asymptomatic microhaematuria. An IVU, showed a completely calcified non-functioning right kidney with hypertrophic left kidney. Investigations for other causes of haematuria were negative. The results of long-term conservative management of traumatic renal infarction are reviewed.

Post-traumatic stress disorder in pre-school aged children after a gas explosion.

UNLABELLED: To evaluate the sensitivity of diagnostic criteria for post-traumatic stress disorder (PTSD) in pre-school aged children involved in a gas explosion, post-traumatic symptoms of the children were investigated four times after the accident, immediately, 10 days, 6 months, and 1 year later. Using symptoms at 6 months after the accident, sensitivity of diagnostic criteria was assessed by comparing the fourth edition of the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders (DSM-IV), and the alternative criteria for infants and young children [10]. In addition, the published Child Post-Traumatic Stress Disorder Reaction Index (CPTSD-RI) and its modified version proposed by us were also evaluated their sensitivity to rate the symptoms. Girls had a tendency to show more post-traumatic symptoms than boys. Although no children met DMS-IV criteria for PTSD, 8 children out of 32 were diagnosed as having PTSD with alternative criteria. With our modified CPTSD-RI, all eight children were decidedly more statistically distinguishable from those without PTSD than with original index. CONCLUSION: our data indicate that the sensitivity of the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders (4th edition) and the Child Post-Traumatic Stress Disorder Reaction Index in rating symptoms of pre-school aged children is not sufficient. The alternative criteria of the former and modified version of the latter would be the better choice in this age group.

Modern cell technologies in the treatment of patients with tympanic membrane injury inflicted by mine explosion.

We evaluated the efficiency of transplantation of cultured human allofibroblasts onto tympanic membrane damaged by mine explosion in combination with Tampograss dressing (Paul Hartman). Transplantation of cultured human allofibroblasts was effective in 100% cases. Application of the film with fibroblasts onto perforation occupying from 1/4 to 1/2 of the tympanic membrane was more effective by 15% (by 59% in subtotal perforation) than tympanoplasty with amnion membrane. The mean duration of tympanic membrane restoration after spontaneous healing and amnionoplasty is virtually the same, while transplantation of allofibroblasts accelerated the process in comparison with other groups in perforation of any size; in subtotal defect the duration of tympanic membrane restoration was shorter by 14 +/- 1 days.

Biodegradation kinetics of the nitramine explosive CL-20 in soil and microbial cultures.

The cyclic nitramine explosive CL-20 (C(6)H(6)N(12)O(12), 2,4,6,8,10,12-hexanitro-2,4,6,8,10,12 -hexaazaisowurtzitane) is a relatively new energetic compound which could be a persistent organic pollutant. To follow its biodegradation dynamics, CL-20 was added to soil alone or together with organic co-substrates and N-source and incubated under oxic and anoxic conditions. Without co-substrates, the CL-20 degradation was detectable only under anoxic conditions. The highest degradation rate was found under aerobic conditions and with the addition of co-substrates, succinate and pyruvate being more efficient than acetate, glucose, starch or yeast extract. When added to intact soil, CL-20 degradation was not affected by the N content, but in soil serially diluted with N-free succinate-mineral medium, the process became N-limited. About 40% of randomly selected bacterial colonies grown on succinate agar medium were able to decompose CL-20. Based on 16S rDNA gene sequence and cell morphology, they were affiliated to Pseudomonas, Rhodococcus, Ochrobactrum, Mycobacterium and Ralstonia. In the pure culture of Pseudomonas sp. MS-P grown on the succinate-mineral N(+) medium, the degradation kinetics were first order with the same apparent kinetic constant throughout growth and decline phases of the batch culture. The observed kinetics agreed with the model that supposes co-metabolic transformation of CL-20 uncoupled from cell growth, which can be carried out by several constitutive cellular enzymes with wide substrate specificity.

Specificity and treatment of thermal and inhalation injury following an explosion in a cement manufacturing kiln.

Six patients sustained a unique type of burn injury while working during the manufacture of cement in China. These patients suffered a combination of chemical and thermal injuries with extensive deep burns and inhalation injury, when they were involved in an accidental explosive leakage of cement from a manufacturing kiln. This report discusses the aetiology of the burn injury and the principles of treatment. All of these burns were preventable.

The breakdown of an emergency system following a gas explosion in Osaka and the subsequent resolution of problems.

Osaka, a modern urban metropolis in Japan, experienced a tragic gas explosion in 1970 when the dispatch room of the City Fire Department was in the process of being moved to a new building. Many unforseen problems arose during this disaster: eg, there was an overall lack of leadership, confusion of communication, a need for triage, and lack of control of mass media. The Osaka Medical Association organized a committee to resolve these problems. Their conclusions and recommendations were that a control headquarters be established at the scene of disaster, the number of ambulances and EMTs be increased, disaster tags be utilized, a special radio frequency be created, and a computer-aided command and control system for fire fighting and ambulance services be introduced. These recommendations have all been followed.

Application of neutron generators for high explosives, toxic agents and fissile material detection.

The portable pulsed neutron generators developed and designed by VNIIA are presented. The neutron methods are good tools for non-destructive assay of dangerous materials. The samples of various systems for the detection of high explosives in luggage, identification of toxic agents in chemical munitions, and also for the prevention of illicit trafficking of fissile elements in passengers' luggage are discussed.

Terror-related injuries: a comparison of gunshot wounds versus secondary-fragments-induced injuries from explosives.

BACKGROUND: Terror-related injuries caused by secondary fragments (SF) from explosive devices show a distinctive pattern in severity, distribution, outcomes of assault, and health-system resource use as compared with terror-related penetrating injuries caused by gunshot wounds. STUDY DESIGN: A case-comparison study conducted in a tertiary university hospital and the only Level I trauma center in the Jerusalem vicinity. During a period of 4 years, over 1,500 casualties of terror-related injuries were treated in one Level I trauma center. The study included 533 patients who were admitted for hospitalization. Excluded from the study were victims who were dead on arrival or who succumbed to their injuries within 30 minutes of arrival at the emergency department. Data were collected from trauma registry records. RESULTS: Gunshot-wound victims were mostly men, aged 19 to 30, and SF victims were more evenly distributed between the genders and across the age spectrum. Injury Severity Score (ISS) was considerably higher in SF victims, although critical mortality rates were higher in gunshot-wound victims. More than 40% of SF victims were injured in three or more body regions, as opposed to < 10% in gunshot-wound victims. Use of imaging modalities and ICUs was considerably higher for SF victims. CONCLUSIONS: Terror victims suffering from SF wounds have more complex, widespread, and severe injuries than victims suffering from gunshot wounds. They tend to involve multiple body regions and use more in-hospital resources. Attenuation of bus seats and protective vests can lead to a reduction in severity of these injuries.

Dissolution kinetics of high explosives particles in a saturated sandy soil.

Solid phase high explosive (HE) residues from munitions detonation may be a persistent source of soil and groundwater contamination at military training ranges. Saturated soil column tests were conducted to observe the dissolution behavior of individual components (RDX, HMX, and TNT) from two HE formulations (Comp B and C4). HE particles dissolved readily, with higher velocities yielding higher dissolution rates, higher mass transfer coefficients, and lower effluent concentrations. Effluent concentrations were below solubility limits for all components at superficial velocities of 10-50 cm day(-1). Under continuous flow at 50 cm day(-1), RDX dissolution rates from Comp B and C4 were 34.6 and 97.6 microg h(-1) cm(-2) (based on initial RDX surface area), respectively, significantly lower than previously reported dissolution rates. Cycling between flow and no-flow conditions had a small effect on the dissolution rates and effluent concentrations; however, TNT dissolution from Comp B was enhanced under intermittent-flow conditions. A model that includes advection, dispersion, and film transfer resistance was developed to estimate the steady-state effluent concentrations.
