Individual thyroid dose estimates for a case-control study of chernobyl-related thyroid cancer among children of Belarus--part II. Contributions from long-lived radionuclides and external radiation.

Significant quantities of long-lived radionuclides were released to the environment during the Chernobyl nuclear power plant accident in 1986. These radionuclides contributed to radiation doses due to ingestion of contaminated foods and external exposure from the ground deposition that resulted. The contributions of these exposure pathways to thyroid doses received by subjects of an epidemiologic study of children from Belarus are evaluated and presented. The analysis shows that ingestion of the long-lived radionuclides, primarily radiocesium, typically contributed a small percentage of the total thyroid dose received by the study subjects. The median and mean fractional contributions were 0.76 and 0.95%, respectively. The contribution of external exposure to the thyroid dose was generally larger and more variable, with median and mean contributions of 1.2 and 1.8% of the total thyroid doses, respectively. For regions close to the reactor site, where radionuclide deposition was highest, the contributions of radiocesium ingestion and external exposure were generally lower than those of the short-lived radioiodine isotopes (132I and 133I) and their precursors (132Te). In other areas, the contributions of these two pathways were comparable to those of the short-lived radioiodines. For all subjects, intakes of 131I were the primary source of dose to the thyroid.

Measurement of 129 I and 137 Cs in soils from Belarus and reconstruction of 131I deposition from the Chernobyl accident.

I and Cs have been measured in a large number of soil samples collected throughout the country of Belarus to support efforts for thyroid-dose reconstruction following the Chernobyl accident. Samples of soil consisting of multiple 30-cm-deep cores per site were sampled following a selection process to ensure sites were undisturbed and representative. Samples were measured by accelerator mass spectrometry (AMS) for I, gamma spectrometry for Cs, and gas chromatography (GC) for total iodine. Results show that both I and Cs are retained firmly in the top approximately 15 to 20 cm of the soil. Our results also suggest that the correlation between I and Cs deposition across the country of Belarus is poor; hence, I is a better surrogate for I than is Cs. It was also noted that total iodine concentrations in topsoil from Belarus are low compared with other regions of the world where radiogenic thyroid cancer has been studied.

Post-Chernobyl thyroid cancers in Ukraine. Report 2: risk analysis.

On April 26, 1986, the worst nuclear reactor accident to date occurred at the Chornobyl (Chernobyl) power plant in Ukraine. Millions of people in Ukraine, Belarus and Russia were exposed to radioactive nuclides, especially (131)I. Since then, research has been conducted on various subgroups of the exposed population, and it has been demonstrated that the large increase in thyroid cancer is related to the (131)I exposure. However, because of study limitations, quantified risk estimates are limited, and there remains a need for additional information. We conducted an ecological study to investigate the relationship between (131)I thyroid dose and the diagnosis of thyroid cancer in three highly contaminated oblasts in Northern Ukraine. The study population is comprised of 301,907 persons who were between the ages of 1 and 18 at the time of the Chornobyl accident and were living in 1,293 rural settlements in the three study oblasts. Twenty-four percent of the study population had individual thyroid dose estimates and the other 76% had "individualized" estimates of thyroid dose based on direct thyroid measurements taken from a person of the same age and gender living in the same or nearby settlement. Cases include 232 thyroid cancers diagnosed from January 1990 through December 2001, and all were confirmed histologically. Dose-response analyses took into account differences in the rate of ultrasound examinations conducted in the three study oblasts. The estimated excess relative risk per gray was 8.0 (95% CI = 4.6-15) and the excess absolute risk per 10,000 person-year gray was estimated to be 1.5 (95% CI = 1.2-1.9). In broad terms, these estimates are compatible with results of other studies from the contaminated areas, as well as studies of external radiation exposure.

Childhood thyroid cancer, radiation dose from Chernobyl, and dose uncertainties in Bryansk Oblast, Russia: a population-based case-control study.

A population-based case-control study was conducted to estimate the radiation-related risk of thyroid cancer in persons who were exposed in childhood to (131)I from the Chernobyl accident of April 26, 1986 and to investigate the impact of uncertainties in individual dose estimates. Included were all 66 confirmed cases of primary thyroid cancer diagnosed from April 26, 1986 through September 1998 in residents of Bryansk Oblast, Russia, who were 0-19 years old at the time of the accident, along with two individually matched controls for each case. Thyroid radiation doses, estimated using a semi-empirical model based on environmental contamination data and individual characteristics, ranged from 0.00014 Gy to 2.73 Gy and had large uncertainties (median geometric standard deviation 2.2). The estimated excess relative risk (ERR) associated with radiation exposure, 48.7/Gy, was significantly greater than 0 (P = 0.00013) but had an extremely wide 95% confidence interval (4.8 to 1151/Gy). Adjusting for dose uncertainty nearly tripled the ERR to 138/Gy, although this was likely an overestimate due to limitations in the modeling of dose uncertainties. The radiation-related excess risk observed in this study is quite large, especially if the uncertainty of dose estimation is taken into account, but is not inconsistent with estimates previously reported for risk after (131)I exposure or acute irradiation from external sources.

Significance of beta-radiation skin burns in Chernobyl patients for the theory and practice of radiopathology.

In the year 1986 the Clinical Department of the Institute of Biophysics of the Ministry of Health of the USSR was the main specialized center in the country for the treatment of radiation disease. More than 300 people involved into the Chernobyl nuclear power plant accident were submitted to a specialized clinical observation, and the acute radiation syndrome (ARS) was diagnosed in 115 of the patients. The author of the article worked as a chief of surgical team responsible for the radiation burns treatment.

Relationship between caesium (137Cs) load, cardiovascular symptoms, and source of food in 'Chernobyl' children -- preliminary observations after intake of oral apple pectin.

Seventeen years after the nuclear power accident at Chernobyl, most of the radio-contamination among the population of Southern Belarus is caused by incorporation of long-lived radioisotopes. The varying levels of 137Cs observed among children in this area are explained by the source of their food, especially by the consumption of contaminated milk produced privately. We stratified children from rural areas of Belarus (caesium [137Cs] contamination >5 Ci/km(2)) by their 137Cs loads into three distinct groups (group 1, <5 Bq/kg body weight [BW]; group 2, 38.4 +/- 2.4 Bq/kg BW; group 3, 122 +/- 18.5 Bq/kg BW). We determined the relationship between the 137Cs load and the children's main source of food and recorded their cardiovascular symptoms. Cardiovascular symptoms, ECG alterations, and arterial hypertension were significantly more frequent in children with high 137Cs burden than in children with very low 137Cs burden. Children with moderate and high 137Cs loads (groups 2 and 3) received apple pectin, a food additive, for 16 days. Apple pectin significantly decreased 137Cs loads in these groups (39% and 28%, respectively). ECG alterations improved, while cardiovascular symptoms and hypertension did not change in any group.

[Age-related peculiarities of effect of low dose ionizing radiation on blood antioxidant enzyme system status in Chernobyl's accident liquidation participant].

Age-dependency of activity of key blood antioxidant enzymes--superoxide dismutase (SOD), glutathione peroxidase and glutathione reductase has been estimated in 104 men and women aged 25-60 years participated in the liquidation of the Chernobyl's accident since 6 years after irradiation. Control group includes 35 age-matched men and women. The results of study on 18 children aged 7-15 years and 5 children aged 2-6 years born by irradiated parents are given as well. Nineteen children were in the control group. Low-dose irradiation was found modify the pattern of age-related dependency of all enzymes studied. Most susceptible chain was enzymes of glutathione cycle both in liquidators and children. Study of late effects has shown that young people (<30 years) as well as children are most susceptible to low-level irradiation whereas most resistant were middle-aged people. This observation should be taken into consideration at selection of high-risk groups in an industry linked with chronic low-dose irradiation.

[Peculiarities of human lung prenatal morphogenesis at the first trimester of pregnancy in different periods after Chernobyl accident].

The comparative histological and morphometric analysis of the bronchial structures of human fetal lungs was performed. Fetal lungs were taken from pregnant women living in radionuclide polluted zones (Novozibcov, Bryansk region) or in control regions (Moscow) in different periods after Chernobyl accident. Relative areas of the bronchial epithelial tubes and mesenchyme, quantity of bronchial branches and buds (end sections) of epithelial tubes on cut area unit were determined. The dates received in 1992-1993 showed the delay of fetal bronchial growth and branching in comparison with control. It can be estimated as tissue dysplasia of lungs from fetuses of mothers living in Novozibcov. The material received in 1995 showed the more intensive growth of bronchial branches in human fetal lungs than the material from Novozibcov and of the Moscow control, taken in 1992-1993. These date suggest the heterogenous character of lung prenatal morphogenesis reactions of offspring from pregnant women lived in different periods on controllable territories of Bryansk region.

[A comparative analysis of the impact on biota and man in the 30 km zone of the Chernobyl NPP].

A methodological approach is described for a comparative assessment of ionizing radiation effects on man and biota, based on the use of indices of radiation impact--ratios of actual exposure doses to environmental objects (including humans) and critical ones. As such doses, standards limiting radiation exposure and doses at which phenotypical effects were absent after the Chernobyl accident were employed, respectively for man and biota. For the test site chosen within the 30 km ChNPP zone (region of the Borshchovka settlement), dose burdens to reference biota species and the population (with and without evacuation) and the corresponding radiation impact indices were calculated. For the long term period after the accident radiation safety standards for man are shown to ensure radiation safety for biota as well. At the same time in the early period after the accident the emergency regulations do not guarantee adequate protection of nature, some species of which can be subject to irradiation more than man, even if countermeasures like evacuation are not applied. A conclusion has been made on the necessity of a more detailed and comprehensive analysis of situations when the anthropocentric principle "if radiation standards protect man then biota are also adequately protected" is violated.

[Demographic situation in Chernigiv area and its connection with Chernobyl catastrophe].

The article presents statistics concerning demographic "crisis" in the regions of Chernigiv area and in Ukraine, as a whole with connection to consequences resulted from Chernobyl catastrophe. The crisis is characterized by increase in population mortality and birth rate reduction, that caused negative natural growth tendency of population. The factors causing reduction in population number of Chernigiv Oblast and in Ukraine as a whole are the following: the effect of ionizing radiation, social-economic indices (the reduction of population profits, unemployment, worsening of medical care, psychoemotional stress caused by the crisis situation in Ukraine, migration of the considerable number of population abroad).

[Results of dynamic cytogenetic study of children and teenagers living in areas, radioactive by contaminated after the Chernobyl accident].

Results of multiyear cytogenetic study of children and teenagers living in areas, radioactive by contaminated after Chernobyl accident, were adduced. Mean density of 137Cs contamination in two compared living areas were 111 and 200 kBq/m2 and mean external doses accumulated for 1986-2001 were 6.7 and 11.4 mGy correspondingly. Averaged thyroid doses receives by subjects of all age groups in the second area were approximately 1.5 times higher than in the first area; in the youngest group (0-1 year) the doses were 114.3 and 174.3 mGy. During 17 years cytogenetic investigation approximately from 30% to 60% of examined persons were observed the increased level of chromosome aberrations in lymphocytes of peripheral blood. Average frequency of unstable aberrations (acentrics, dicentrics and centric rings) constituted about 0.4 per 100 cells (0.22 per 100 cells in controls) during all period of observation. Level of marker aberrations (dicentrics and centric rings) was increased almost all times of study and varied within 0.04-0.19 per 100 cell (0.03 in control group). The parallel investigation of frequency of stable aberrations by FISH method showed up their level about 3 times exceeding observed dicentrics level. Comparably higher indexes of cytogenetic disturbances were revealed in group exposed in utero during period of accident.

[The use of computer system in the introduction of countermeasures in agriculture after the Chernobyl accident].

In the development of strategies for countermeasures aimed at the rehabilitation of contaminated areas, essential is to identify criteria for assessing their effectiveness and factors influencing the priority of the criteria chosen. A method is suggested for assessing effectiveness of countermeasures in agriculture based on a multicriterium analysis of radiologic, economic, regulatory and socio-psychological indicators characterizing the use of countermeasures. Rating of countermeasures strategies is presented depending on financing of works on the rehabilitation of the Chernobyl affected areas.

[Long-term morphofunctional changes in the skin of Chernobyl clean-up workers].

Musculocutaneous samples taken from 19 Chernobyl clean-up workers were morphofunctionally examined to trace the changes, which had happened in them under the lasting exposure to low doses of ionizing radiation. The analysis showed that the greatest changes had taken place in the epidermis in the form of thickening of corneal and cellular layers and inflammatory infiltration of lymphocytes provided with the productive panvasculitis in the majority of arterioles. It is supposed that after the long low-doce irradiation two forms of reactions developed in the skin: a defense reaction in the form of proliferative hyperkeratosis and an immunopathological reaction. The latter is a consequence of activation in the cellular layer of epidermis combined with the appearance of effector component of immune response, which stimulates the interaction of epidermal T-lymphocytes with the endothelial cells of derma vessels and systemic involving of blood vessels of microcircular blood stream.
