[New evidences for adapting Qian jin yao fang (Thousand golden essential prescriptions) by Office of Revising Medical Books of the Song dynasty].

Qian jin yao fang (Thousand Golden Essential Prescriptions) written by Sun Simiao in the Tang dynasty has two kinds of extant editions in circulation: one is the revised edition, Bei ji qian jin yao fang (Essential Prescriptions Worth a Thousand Gold for Emergencies) revised by the Office of Revising Medical Books of the Song dynasty, the other is the Song edition, Sun zhenren qian jin fang (Master Sun's Thousand Golden Prescription) (20 volumes extant) not revised by Song people. There are great discrepancies between these two editions not only in the order of chapter contents, including the order of items in the same chapter, but also in its texts. Concerning the causes of this difference, we already made preliminary analysis in our former paper. However, there are new evidences to further explain it with our recent research.

[A study of the Chinese books quoted in "Chotei Kaitai-schinsho", "Yi Xue Yuan Shi" and "Wu Li Xiao Shi"].

Gentaku Otsuki quoted from "Yi Xue Yuan Shi" and "Wu Li Xiao Shi" when revising "Kaitaishinsho". He quoted from them in three ways; quoting the original text, quoting them to annotate the difficult terms, and quoting them to correct errors in the original text. When Western medicine was first introduced to Japan, the interpretation and the explanation of Chinese classics was utilized to translate Dutch medical books. The study has clarified for the first time that "Yi Xue Yuan Shi" and "Wu Li Xia Shi" contributed greatly to the accomplishment of "Chotei Kaitai-shinsho" and to the importation of Western medicine into Japan.

Midwifery basics. Antenatal care--the booking history.

Joannna is in the first trimester of her first pregnancy. She has seen her General Practitioner who gave her some general advice about what she should and should not be eating. Aware of her previous medical history, the GP told Joanna to go to the reception desk and to make an appointment for the community midwife to visit her at home to undertake the 'booking'. A week later she received a package of information and a letter from the midwife confirming the appointment time and asking Joanna to produce an early morning specimen of urine (EMSU) on the day of the visit.

Books for children about death.

Books about death for preschool and schoolaged children are reviewed in terms of child development, children's understanding of death, and literary quality. The article is written to help pediatricians and nurses become aware of this literature and to aid them in selectively advising concerned parents about its constructive use to help children understand death. Preventive mental health is the focus rather than use with fatally ill children.

Predicting shared parent--child book reading in infancy.

This study examined the degree to which parental contextual factors and infant characteristics predicted whether parents read aloud to their 8-month-old infants. Discriminant function analysis revealed that mothers with higher family incomes and those who reported less parenting stress and fewer general hassles were more likely to read to their infants. Gender and temperament of the infant did not significantly predict whether mothers would engage in shared reading. Furthermore, there was no evidence that mothers who reported reading aloud to their infants display more enriching parenting practices in the laboratory. Paternal contextual factors did not discriminate readers from nonreaders, but infant temperament did. Fathers who read aloud had infants who were less soothable and who displayed longer durations of orienting. The possibility that book reading could serve as 1 mediator of the temperament-cognition relationship is discussed.

Clinical application of two computerized diabetes management systems: comparison with the log-book method.

In two consecutive studies the clinical application and suitability of two computer-assisted data management systems (Camit and Cadmo) were evaluated in a prospective manner. In each study nineteen long-standing, stable insulin-dependent patients were randomly assigned to one of two groups. In study I assessment of metabolic control and insulin dose adjustments were based either on the Camit S1 data analysis or on the conventional log-book method, whereas in study II the Camit S2 and the Cadmo simulation programs were evaluated. HbA1c values decreased significantly in both studies (p < 0.05). A clear decline in hypoglycemic events as well as a significant reduction of the percentage of glucose values below 4.0 mmol/l (p < 0.005) and a marked increase (p < 0.05) in the percentage of glucose levels in the target range (4.0-10.0 mmol/l) were observed. We found both computerized assessment systems to be reliable and suitable for the assessment of blood glucose control and for insulin dose finding. The graphical and statistical presentation of the numerous glucose and insulin data allowed a better summary of blood glucose control and metabolic trends. More time could be spent for problem solving, which proved to be much less exhausting with the computer for the attending physician. Further studies should address the educational potential of computerized systems for the patient as well as for the physician.

Pancreatic trauma from a book.

CONTEXT: An early diagnosis of pancreatic trauma can be challenging and difficult because of the lack of correlation between the initial presenting features, radiological and laboratory findings, and the severity of the trauma. A high degree of suspicion is essential to diagnose pancreatic injury particularly in patients with blunt trauma to the abdomen. A computerised tomography scan is useful in making an early diagnosis of pancreatic trauma, localizing the site of the injury and in the identification of main pancreatic duct injury which has major implications in the management of the patient. CASE REPORT: Here in, we report an interesting case of a 40-year-old woman who sustained a tear in the tail of the pancreas following a blunt injury to the pancreas while she was carrying a book in front of her abdomen and collided against an edge of a door-frame. She was managed conservatively without any complications. CONCLUSIONS: A history of abdominal trauma, however trivial it may sound, needs to be appropriately investigated.

A comparison of booked and unbooked mothers delivering at the Port Moresby General Hospital: a case-control study.

A case-control study of unbooked mothers delivering at the maternity unit of the Port Moresby General Hospital was undertaken over a period of 7 months. 48 mothers who had no antenatal attendances during pregnancy were recruited with 96 booked controls. Reasons for non-attendance, understanding of the importance of antenatal clinics and socioeconomic and demographic factors were recorded to assess likely risk factors for non-attendance. Almost half the mothers cited financial difficulties as the reason for non-attendance at antenatal clinics despite most of them knowing that it was important to have antenatal care during pregnancy. The two most important risk factors for being unbooked were mothers having no education (p <0.001) and the type of employment of their spouse (p <0.01). Unbooked mothers were more likely to have preterm babies (OR 16.1; 95%CI 3.4-75.7) and all 6 perinatal deaths occurred in babies born to unbooked mothers. Remedial approaches would need to take into account maternal education, education of partners and the fact that despite free antenatal services in urban clinics financial difficulties in terms of other costs involved still remain an obstacle to overcome.

[The cookery book of the monastery library of Michaelbeuern (Man.of cart. 81). Edition and commentary].

We present the edition of 12 cookery recipes which are found in the manuscript Man. cart. 81 of the monastery of Michaelbeuern near Salzburg (Austria). The manuscript dates from the middle of the 15th century and contains mostly religious texts; some medical texts as well as texts belonging to the liberal arts indicate that it may have been used as a school book for the monks. The dialect of the recipes shows upper German and Bavarian peculiarities, according to the place the manuscript is preserved. The edition of the recipes is followed by a commentary and a glossary. Eight of the recipes concern lenten meals, which means that they were probably collected especially for the religious community of the monks.

The Biblical Book of Job: advice to clinicians.

Study of the Book of Job clarifies the particular adaptation to trauma that underlies the suffering of certain difficult patients. In addition, the misdirected efforts of Job's comforters in the Bible story help us understand why, with such patients, an analyst's attempts to address unconscious guilt and defenses against it will prove counterproductive. A case example is presented.

[Hospital morbidity statistics: the accuracy of the diagnosis reported in the discharge registry book].

BACKGROUND: The registry book of admission and discharges (RBD) is operative in most public and private hospitals of this country. Currently its reformation is contemplated, consisting in a modification of the minimal set of basic data and its computerization. The aim of the present study was to evaluate the accuracy of the discharge diagnosis recorded in the RBD and to analyze the possible factors implicated in the current quality level. METHODS: In a sample of 9 hospitals from the Seville province, 2,634 discharges were selected with a stratified sampling process which was proportional to the number of discharges in each stratum during 1985. The accuracy of the diagnosis recorded in the RBD was estimated by comparing it with that in the clinical record (CR). RESULTS: The accuracy indexes showed that, for the large diagnostic groups, only in 45% of discharges the diagnosis coincided in both documents. The accuracy increased to 89% when the estimation was made after excluding those discharges where the diagnosis was not recorded in the RBD and/or in the CR (n = 1303). CONCLUSIONS: The fulfillment of the RBD appears as the main error source, followed by the selection of the major diagnosis and its transcription in the different documents of the circuit. It is concluded that a precondition for the success of a reformed RBD is to guarantee the fulfillment of data.
