[Evolution after 5 years of percutaneous pulmonary valvulotomy: report of 47 cases].

47 patients aged from 2 to 59 years affected by valvular congenital or mixed pulmonary stenosis including three fallot trilogies and one patient with right congestive cardiac failure are treated by percutaneous pulmonary valvulotomy between october 1986 and december 1990. All patients have been controlled with a mean follow-up of 6.5 +/- 1.1 years. The total gradient rate between pulmonary artery and right ventricule decrease from 112 +/- 55 mm Hg to 20 +/- 8 mm Hg on the last control with disappearance of infundibular inflammation and inter auricular shunt in all the concerned cases. We observe the regression of right cardiac failure symptoms with disappearance of tricuspid insufficiency in the cases of advanced pulmonary stenosis with right ventricular dysfunction. Percutaneous pulmonary valvulotomy by its simplicity and harmlessness, its long term efficiency is a good method of treatment of pure valvular or mixed pulmonary stenosis.

[Long-term therapy with flecainide].

The effect of the new antiarrhythmic drug Flecainide was examined in a controlled long-term study (1 to 44 months [x = 17.7 +/- 12.4]) in 36 patients, aged 18 to 76 years (x = 44 +/- 16.3), suffering from ventricular arrhythmias. In 12 cases coronary heart disease, in 11 cases myocarditis, in 2 cases each of dilatative cardiomyopathy and mitral valve prolapse syndrome, and one in case each of combined aortic and mitral valve disease and postoperative condition of Fallot Trilogy was present. In 7 cases the etiology of the dysrhythmias could not be elucidated. 18 patients had been treated before by more than 3 other antiarrhythmic drugs without sufficient result. The daily dose administered was assessed by the degree of the dysrhythmias and the response to the drug. In most cases 300-400 mg Flecainide was given. The therapeutic success was assessed by 24 h Holter-ECG before and during therapy. In most cases two registrations were performed before and three registrations during therapy. In 29 patients (81%) a rate reduction of VES over 70% could be observed. Rate reduction of VT (n = 8) was total in 5 cases, in one case over 90%, in two cases over 70%. Salvos (n = 10) were abolished in 7 cases and reduced over 90% in two and over 70% in one of the cases. R-on-T-phenomena (n = 2) disappeared completely. An increase of rate or degree of the dysrhythmias was never observed. In no case had therapy to be interrupted due to severe side effects. Changes of laboratory values were not observed.(ABSTRACT TRUNCATED AT 250 WORDS)

Ultramicroscopic morphometric, biochemical and histophotometric investigations of myocardial biopsies taken from Fallot patients before and after cardioplegia with cold Kirsch's solution.

Biopsies taken from the myocardium of 5 patients with Morbus Fallot and from 1 patient with ventricle septum defect were investigated with a combination of morphometric, biochemical and histophotometric techniques in order to study the cardioprotective effect of cold Kirsch's solution. At the final phase of cardioplegia the cardiomyocytes reveal the following alterations: The volume densities of mitochondria, of their degenerated areas and that of cytoplasmic vacuoles show a significant increase whereas that of myofilaments decreases. Cristae and matrix mitochondriales, however, show only moderate alterations without statistical significance. Biochemically the total ATP-concentration and creatine phosphate (CP)-concentration were more or less diminished, in most of the cases the activity of the myosin ATPase was increased, that of the creatine phosphate kinase (CPK) diminished. Compared with the biochemical estimations of the ATPase activity, its histophotometric estimations yielded corresponding results in 2 of 4 cases. In general our findings confirm the cardioprotective effect of Kirsch's solution. The combination of methods used gives more reliable results than one technique alone.

[Clinical diagnosis and prognosis of the course of congenital heart defects with pulmonary stenosis].

An analysis of 433 observations is presented (164 cases of isolated pulmonary stenosis, 132 cases of its combination with atrial septal defect, and 137--with ventricular septal defect), the patients being 1 year 1 month to 36 years old, and the form of their heart disease being verified during surgery on a "dry" heart. The mentioned diseases were classified into 3 groups according to the tactical importance of the pulmonary stenosis. Haemodynamic studies in patients with pulmonary stenosis and an intact ventricular septum have demonstrated that with age right-ventricular hypertension progresses the faster the greater the pulmonary artery valve is stenosed. A combination of a valvular stenosis with a large ventricular communication is accompanied by a levelling of the pressure in both ventricles, its absolute figures being somewhat higher in patients over 16 years of age.

[Surgical cardiopathies and pregnancy (a 30-year report)].

The advent of heart-surgery in obstetrics has been determinant, in as much as it has improved therapeutic treatment and above all it has resulted in achievement which were impossible 30 years ago. The concepts already put forward by the school of cardio-obstetric surgery of Turin on haemodynamics in pregnancy and childbirth as biologic risk factors for the patient with a bad heart continue to be upheld. The results of Actis Dato Group's in cardiac surgery are reported. Almost all types of congenital and acquired cardiopathies in their relationships to pregnancy are described. 1750 Pregnancy cases in 997 heart diseased women are analised. The cooperation with the obstetrical team is stressed.

Open-heart surgery in 48 patients via a small right anterolateral thoracotomy.

To limit the trauma to the chest and to achieve a pleasing cosmetic result, we used 2 types of right anterolateral thoracotomy in 48 patients who required open-heart surgery: 1 was a curved incision along the lower edge of the right breast in women with developed breasts; the other was a slanted incision for men and children. These surgical procedures took place between July 1996 and November 1997. Intraoperatively, a right atriotomy was used to repair 11 atrial septal defects and 11 ventricular septal defects, 2 combined atrial and ventricular septal defects, 1 case of a single atrium, and 1 partial atrioventricular canal. A right ventricular outflow tract incision was used to repair 7 ventricular septal defects and 7 ruptured aortic sinus aneurysm. A combination of a right atriotomy and right ventricular outflow tract incision was used for 2 repairs of combined atrial and ventricular septal defects, 3 radical corrections of tetralogy of Fallot, and 2 radical corrections of trilogy of Fallot. A combined right and interatrial septal incision was used for 6 mitral valve replacements and 1 mitral valvuloplasty. Smooth bypass cannulation and satisfactory intracardiac exposure were achieved with the right anterolateral thoracotomy. There was no complication or mortality directly related to the incision. We believe that the right anterolateral thoracotomy is safer and more effective than the median sternotomy for many common congenital and acquired heart diseases. The thoracotomy causes less trauma and results in a cosmetic appearance that is more acceptable to the patient.
