Imaging of the stomach, and localization of the stomach meridian & its acupuncture points in a human cadaver by the use of the indirect "Bi-Digital O-Ring Test Imaging Technique".

Since 1984, using the "Bi-Digital O-Ring Test (Molecular Identification, Localization and) Imaging Technique" originally developed by Y. Omura, the imaging of the outline of normal and abnormal internal organs has been successfully applied for clinical diagnosis and evaluation of therapeutic effects, without the use of expensive imaging instrumentation. Not only has it become possible to non-invasively image each internal organ and localize specific malignant tumors of specific internal organs, as well as neurotransmitters, but it has also recently become possible to demonstrate the presence and exact locations of the meridians corresponding to specific internal organs and their acupuncture points along the meridians through the use of either the Direct or Indirect "Bi-Digital O-Ring Test (Molecular Identification, Localization and) Imaging Techniques" while holding a microscopic slide of the specific human internal organ tissue. However, very little information was available to demonstrate the presence of the meridians and their acupuncture points in a human cadaver. In this study, we were able to demonstrate by use of the Indirect "Bi-Digital O-Ring Test (Molecular Identification, Localization and) Imaging Technique" the presence of meridians corresponding to specific internal organs and acupuncture points on the meridians, all of which were nearly identical to those found in a living human. For example, the authors found that the stomach meridian at both ends of one of the most well-known acupuncture points, Stomach 36, is located within 1 mm of the surface of the skin, and the average diameter of the meridian is less than 1 mm. Stomach 36 in this particular cadaver had a round shape with a diameter of approximately 1.8 cm at the skin surface. The acupuncture point, St. 36, in this cadaver at the skin surface was round in shape and extended to the deepest underlying muscle layer of the tibialis anterior muscle, in three dimensions. In spite of the fact that the Indirect Bi-Digital O-Ring Test Imaging Technique showed a distinct pathway for the stomach meridian and the location of St. 36, there was no apparent difference to the naked eye between the meridian and the surrounding non-meridian tissue, nor between St. 36 and surrounding non-acupuncture point area. However, there seems to be a more dense connective tissue network between the skin layer and the fascia on the muscle tissue at the acupuncture point, as compared with the surrounding non-acupuncture point area.(ABSTRACT TRUNCATED AT 400 WORDS)

Cancer precursor lesions in intact stomach Helicobacter pylori gastritis and in resected stomach gastritis.

Hemigastrectomy for benign disease and Helicobacter pylori infection are risk conditions for the development of gastric cancer. Aim of the study was to compare gastric histology and precursor lesions of malignancy in these two conditions. The hemigastrectomy group included 351 consecutively endoscoped subjects operated for gastroduodenal benign disease. Six to ten biopsy specimens were routinely taken from the residual gastric mucosa. The intact stomach group included 2097 consecutively endoscoped symptomatic subjects, who did not receive eradication therapy against H. pylori. The histological findings were classified as normal mucosa (NM), chronic non atrophic gastritis (CNAG), chronic atrophic gastritis (CAG), intestinal metaplasia (IM) and dysplasia (DYS). One thousand and three intact stomachs were H. pylori negative, and 1094 showed H. pylori colonization. The age over fifty was a significant risk factor for the occurrence of IM (OR 2.52, P < or = 0.001) and DYS (OR 3.46, P < or = 0.001), while Hp-positivity was a risk factor for CNAG (OR 1.81, P < or = 0.001) and CAG (OR 3.88, P < or = 0.001). Gastroresection was associated to higher risk for CNAG (OR 1.53, P < or = 0.001) and DYS (OR 4.31, P < or = 0.001) and to a lower risk of CAG (OR 0.49, P < or = 0.001). Both in males and females the risk for CNAG was significantly higher in Hp-positive (males OR 1.92, P=0.000; females OR 1.70, P=0.000) and gastrectomized subjects (males OR 2.06, P=0.000; females OR 2.43, P=0.000). Gastrectomized males, furthermore, showed an increased risk for DYS (OR 5.82, P=0.000). The aged Hp-negative and Hp-positive subjects evidenced a significant risk for IM (respectively OR's 3.42, P=0.000 and 4.85, P=0.000); the risk for DYS was significant in aged Hp-negative subjects (OR 4.09 P < or = 0.020). The Hp-positive individuals evidenced a significant risk for metaplastic mucosal changes (OR 38.17, P=0.000). Subjects aged over forty at the time of surgery and those with a longer postoperative follow up endoscopy presented an increased risk for CNAG of the residual mucosa (respectively OR's 2.75, P=0.000 and 5.25, P=0.000). CNAG and IM were the most frequently observed mucosal lesions both in subjects operated for duodenal and gastric ulcer (respectively OR's 4.02, P=0.000 and 3.00, P=0.000). Our data support that hemigastrectomy for benign disease and H. pylori infection may induce an increased incidence for histological precursor lesions for gastric malignancy and suggest that carcinogenesis in a resected stomach may be different from that in the intact stomach.

[Anterior seromyotomy of the body and the functional part of the stomach combined with posterior truncal vagotomy and ulcer excision in the surgical treatment of complicated stomach ulcer].

Anterior seromyotomy of the body and fundus of the stomach was combined with posterior truncal vagotomy and excision of the ulcer in 23 patients with gastric ulcer complicated by bleeding or perforation. Seventeen patients had chronic ulcers of the body of the stomach (type I), 3 patients had concurrent ulcers (type II), and 3 more patients had acute ulcers of the body of the stomach. Operation was undertaken for active bleeding from the ulcer in 20 patients and for perforating ulcer in 3 patients. One patient died. Mild disorders of evacuation of an aqueous barium sulfate suspension from the stomach were noted in 4 patients.

Catechol strongly enhances rat stomach carcinogenesis: a possible new environmental stomach carcinogen.

Catechol (CAS: 120-80-9) is present in the environment, being a major industrial chemical as well as a major phenolic component of cigarette smoke. Continuous oral treatment of rats with 0.8% catechol for 51 weeks after a single intragastric dose of 150 mg/kg of N-methyl-N'-nitro-N-nitrosoguanidine strongly enhanced both forestomach and glandular stomach carcinogenesis. In addition, and more importantly, catechol alone induced adenocarcinoma and adenomatous hyperplasia in the pyloric region of the glandular stomach. These results clearly indicate that this environmental contaminant merits classification as an enhancer of forestomach and glandular stomach carcinogenesis with complete carcinogenic potential for the glandular stomach. Its significance for gastric tumor development in man requires elucidation.

[Current state and possibilities of radiotherapy in the inter- disciplinary treatment of malignancies of the stomach, pancreas and bile ducts. I. Stomach cancer].

Surgery is undoubtedly the therapy of choice in case of the advanced carcinoma of the stomach. In spite of more and more radical and extended operation techniques, the extremely unfavorable prognosis could not be improved. Based on experiences gained with palliative irradiations, the efficacy of radiotherapy can be considered to be proved. Its difficulties and problems are due to the topographic position of the stomach and to the radiosensitivity of the stomach and the adjacent organs. An additional application of radiotherapy seems sensible regarding the high rate of local recurrences and regional lymph node metastases following surgery (about 90%). The greatest effect of radiotherapy is to be expected in case of intraoperative application - alone or combined with postoperative percutaneous irradiation. However, the therapeutic effect of fast neutrons, hyperthermia, radiosensitizers has not been explored yet. The occurrence of remote metastases besides local recurrences in about 25% of all cases and the available results of some smaller studies suggest a therapeutic advantage to be obtained by an additional systemic cytologic therapy. The authors present the surgical, radiotherapeutic, and chemotherapeutic results achieved hitherto in the treatment of the advanced carcinoma of the stomach. Further possibilities for the future use of radiotherapy are proposed in order to encourage the establishment and application of interdisciplinary therapy conceptions.

DNA single-strand scission in the pyloric mucosa of rat stomach induced by four glandular stomach carcinogens and three other chemicals.

Induction of DNA single-strand scission by four glandular stomach carcinogens and three other chemicals was studied in the pyloric mucosa of rat stomach after gastric intubation. DNA single-strand scission, as was measured by the alkaline elution method, was induced by four glandular stomach carcinogens; N-nitroso N-methylurethane at doses of 1 and 9 mg/kg body wt, 4-nitroquinoline 1-oxide at 20 and 30 mg/kg body wt. N-ethyl-N'-nitro-N-nitrosoguanidine at 30 and 100 mg/kg body wt and N-propyl-N'-nitro-N-nitrosoguanidine at 30 and 100 mg/kg body wt. DNA single-strand scission was also induced dose-dependently by a direct acting mutagen, 1-nitrosoindole-3-acetonitrile at doses of 100, 500 and 800 mg/kg body wt. Omeprazole, a proton pump inhibitor, was equivocal in its effect in this assay at 30-500 mg/kg body wt: induction was statistically significant by Cochran-Armitage binomial trend test. Loxtidine, an H2-receptor antagonist, did not induce DNA single-strand breaks in the pyloric mucosa at a dose of 400 mg/kg body wt. The present results together with previous information suggest that DNA single-strand scission is a good marker for tumor-initiating activity in rat stomach mucosa.

[Studies on the protein and amino acid metabolism of laying hens using 15N labeled casein. 2. 15N incorporation into the N fractions and basic amino acids of the hen stomach and into the contents of the crop and stomach].

4 colostomated laying hens received 15N labelled casein as sole protein source for a period of 6 days. Two birds each were slaughtered on the 8th and 12th day of experiment. 23.5% of the total N in the glandular stomach and the gizzard were TCE soluble. The amino acid composition of the gastric proteins was broader than that of the proteins of flesh, with the exception of lysine and histidine. The atom % 15N' of the N of the stomach was roughly the same as that of the protein N; the level of labelling in the TCE soluble N fraction was only slightly higher. 2 and 6 days after the final administration of tracer N an appreciable proportion of 15N was found in the N fractions and in the basic amino acids contained in the crop and the stomach. From this is may be concluded that a considerable portion of the N-containing substances previously absorbed were secreted into the lumen of the crop and the stomach.

Induction of changes in the pepsinogen content and the pepsinogen isoenzyme pattern of the pyloric mucosa of the rat stomach by short-term administration of stomach carcinogens.

Changes in pepsinogen isoenzyme patterns were examined in the pyloric mucosae of the stomachs of noninbred male Wistar rats after short-term administration of gastric carcinogens. N-Methyl-N-nitro-N-nitrosoquanidine, N-ethyl-N1-nitro-N-nitrosoguanidine, and N-propyl-N-nitro-N-nitrosoguanidine, which induce stomach cancer in rats, decreased the content of pepsinogen isoenzyme 1 )Pg 1), which was separated by poly-acrylamide gel electrophoresis. They also decreased the pepsingoen content of the pyloric mucosa. 4-Nitroguinoline 1-oxide, which induces a low incidence of stomach cancer in rats, rarely decreased the Pg 1 content or the pepsinogen content of the pyloric mucosa, and the incidence of such decreases was not statistically significant. However, diethylnitrosamine and dimethylnitrosamine, which do not induce stomach cancer in rats, did not cause any decrease in pepsinogen content. Ethyl methanesulfonate, a direct-acting carcinogen used as a control, also did not decrease the pepsinogen content.

[Occurrence of Helicobacter pylori in mucous membrane of stomach in patients with duodenal ulcer and inflammation of stomach mucous membrane].

In 1990-1991 microbiological studies were performed aimed at determination of frequency of occurrence of Helicobacter pylori in patients treated because of peptic ulcer in Department of Gastroenterology in Katowice. Studies were performed in 112 patients. Out of these, in 73 duodenal ulcer was detected endoscopically (group I), in 26--inflammation of stomach mucous membrane with erosion (group II), and in 13--both duodenal ulcer and erosion of stomach membrane (group III). Samples from mucous membrane of pylorus and of stomach taken endoscopically were tested microscopically and in culture. Helicobacter pylori was found in 74 patients (66.1%). Frequency of isolation was different in individual groups. Helicobacter pylori was found in 51 patients (70%) of group I, 15 (58%) of group II and in 8 (61%) of group III. Helicobacter pylori was most frequently isolated from patients with duodenal and stomach ulcer (81.3%).

[Esophagoplasty in patients with the "waning" stomach or without the stomach].

An experience with 48 intrathoracic esophagoplasties in patients with "waning" stomach or its absence is generalized. The results obtained show that scarry-ulcerous damages of the pyloroduodenal and cardioesophageal portions, local surgical procedures, gastrostomy included, are not considered as deterrent factors for using the stomach as the plasty material for esophagoplasty. For the resected stomach or its absence the small intestine should be preferred in the formation of the anastomosis within the limits of the thoracic cavity and large intestine--when putting anastomosis on the neck. A complete clinical effect was obtained in 45 patients. Three patients died (6.25%).

[Studies on trace elements in cancerous stomach tissue of the patients with stomach cancer].

This study was performed to find out whether copper, zinc, manganese, selenium and iron concentrations in the cancerous and normal stomach tissues of the patients with stomach cancer vary within the malignant stages and Borrmann classification or not, and to investigate the interaction of copper, zinc, manganese, selenium and iron concentrations in blood of these patients. Copper concentration in cancerous tissues was not statistically significant as compared with normal tissues. Plasma and whole blood copper concentration of Stage IV showed a significant higher level than that of stage I. Zinc concentration in cancerous tissues was not statistically significant as compared with normal tissues. Selenium concentration in cancerous tissues showed a statistically significant high level as compared with that in normal tissues. Plasma selenium concentration of Stage III showed a significant lower level than that of stage I. Iron concentration in cancerous tissues showed a significantly lower level than that in normal tissues at stage IV. Whole blood iron concentration was low levels in proportion to the progress of stomach cancer. The correlation of selenium concentration between in cancerous tissues and in whole blood of these patients was significant with the correlation coefficient of 0.340. The correlation of iron concentration between in cancerous tissues and in whole blood of these patients was significant with the correlation coefficient of 0.423. The correlation between iron concentration in cancerous tissues and hemoglobin concentration in whole blood of these patients was significant with the correlation coefficient of 0.361.

[Epithelioid leiomyosarcoma of the stomach. Clinical experiences with a rare stomach tumor].

Gastric epithelioid leiomyosarcoma (epLMS), which generally occurs in mid- or late adult life, is a rare smooth muscle tumor of the stomach. Out of 25 soft tissue tumors of the stomach operated at the Department of Surgery, University of Graz, two epLMS were diagnosed. This paper presents the case of a 67-year-old male with an epLMS in the corpus and of a 80-year-old female with an epLMS in the fundus of the stomach. The tumors were not diagnosed by gastroscopy; they were localized by sonography and CT-scan. In both cases the tumor was completely removed surgically, using a TA 90 4.8 mm respectively a TA 55 4.8 mm stapler. Diagnosis was reached by histological and immunohistochemical examination of the tumor tissue. Surgical excision with wide tumor-free resection margins is the therapy of choice in this tumor group.

[Immunogenetic features of patients with ulcer disease of the stomach and duodenum and the significance of genetic markers in prognosis of complications of stomach and duodenal ulcers in Azerbaijan].

The work has shown that patients with ulcer disease of the stomach and duodenum have immunogenetic features. In particular, genetic markers were detected for ulcer disease of the stomach and duodenum whose presence allows to predict with certain probability the development of ulcer disease of the stomach and duodenum. The presence of antigen HA-At and haploids--AxB18 and HLA--A10B27 is prognostically unfavorable factors for the development of complications of this disease.

[Dynamics of endogenous bacterial nitrite formation in the stomach. 2. Follow-up observations with standardized secretion from the porcine stomach].

The bacterial nitrite synthesis in gastric juice from porcine stomach mucin and the decrease of nitrite after adding acid, which has often been described in literature-results of an examination with a standardizable biomodel. A decrease of nitrite concentration was only seen after adding a small volume acid of high concentration. Simulating the acid secretion by a large volume of low concentrated acid mixed with pepsin-solution (artificial gastric secrete, DAB 8) a considerable increase of nitrite concentration can be observed. An explanation can be the structure of stomach mucous gel can be solubilized in water and thus the ions are liberized inside of the stomach mucin. In the discussion of the endogenous synthesis of cancerogenic N-Nitroso-compounds it must be considered if this phenomena also can be observed in human secretes.

Long-term results of operation for carcinoma of the stomach in T1/T2 stages: critical evaluation of the concept of early carcinoma of the stomach.

BACKGROUND: The significance of nodal metastasis in patients with early gastric cancer (ECG) (T1) is unknown. It has been suggested that patients with T2, N0 carcinoma of the stomach have a comparable survival rate to patients with T1 carcinoma of the stomach. STUDY DESIGN: A retrospective review and survival analysis of 321 patients with T1/T2 adenocarcinoma of the stomach treated between 1979 to 1991 were performed. RESULTS: Patients were divided into four groups: group 1, 214 patients with node-negative EGC (T1, N0); group 2, 13 patients with node-positive EGC (T1, N+); group 3, 49 patients with node-negative T2 disease (T2, N0); and group 4, 45 patients with node-positive T2 disease (T2, N+). Excluding deaths from causes other than recurrence, the survival rate for patients in groups 1 and 3 was 100 percent, in contrast to the ten-year survival rate of 72.7 percent for group 2 and 62.5 percent for group 4 patients (p < 0.001, groups 1 versus 2, groups 3 versus 4). The ten-year survival rate for patients with node-negative T2 disease (group 3, 100 percent) was significantly better than that of patients with node-positive EGC (group 2, 72.7 percent) (p < 0.001). Although differences in the survival rates were noted according to lymphatic or venous invasion and whether or not patients had EGC or T2 carcinoma, the most significant factor was lymph node invasion. CONCLUSIONS: The postoperative survival rate for patients with node-positive EGC was poorer than that for those with node-negative T2 carcinomas. Reevaluation of the concept of EGC may be necessary. Post-operative chemotherapy does not appear necessary in patients with T2, N0 disease.

The physiology of gastrocystoplasty: once a stomach, always a stomach.

To determine the physiology of acid secretion after gastrocystoplasty with the body of the stomach we performed a prospective standardized 3-day study in 13 children (median age 12.5 years) who had undergone bladder augmentation/replacement (median postoperative period 2 years). Urinary pH and titratable acid, and serum gastrin levels were measured after gastric distention with a meal and bladder distention with urethral filling at baseline and after medication with a histamine-2 receptor antagonist or an anticholinergic agent. Five children underwent cystoscopy and biopsy of the gastric and native segments of the gastrocystoplasty. In the fasting state pH was neutral, there was no titratable acid in the urine and serum gastrin level was normal in all cases. After a meal urinary acid secretion and serum gastrin level increased markedly. After each medication half of the patients demonstrated marked inhibition of urinary acid secretion after a meal while response was partial in the remainder. In none of the patients was there significant alteration in the pattern of gastrin secretion. Bladder distention did not result in urinary acid secretion or gastrin secretion. The cystoscopic and histological appearance of the native bladder and stomach segment of the gastrocystoplasty in the 5 patients was normal. We conclude that the gastric body segment used in gastrocystoplasty continues to secrete acid as though it were part of the stomach. The secretion of acid in the urine can be decreased with histamine-2 receptor antagonist or anticholinergic medication.

[Clinical studies of intra-thoracic stomach function in patients after esophagectomy and reconstruction by whole stomach].

We usually use the stomach to hung up into the chest and to the neck for esophago-gastrostomy in the patients of esophageal cancer. We had studied the intrathoracic stomach function in patients after esophagectomy with isotope 99m Tc labelled 717-resin semisolid meal for scintigram. We measured the gastric emptying time (GET) and fund the GET1/2 was no difference between the preoperative group and contrast group (P > 0.05). The study indicated that GET1/2 was faster obviously in postoperative patients with pyloroplasty than without pyloroplasty (P < 0.01). It was proved that to perform pyloroplasty with esophagostomy should be used routinely for preventing the pylorospasm, dilatation of the intrathoracic stomach and gastroesophageal reflux. At the same time, we found fasting serum gastrin (FSG) was increased (P < 0.01) in patients after esophagectomy than before, but basal acid output (BAO) decreased. It indicated that vagotomy caused the BAO decreasing and PH increasing. There were some relations between high level of FSG and postoperative diarrhea.

[Immunohistologic study of the localization of organ-specific antigens of the stomach and intestine and carcinoembryonic antigen in normal and pathologically altered stomach tissues].

Localization of organospecific gastric and intestinal antigens, as well as of carcinoembryonic antigen (CEA) was studied by an indirect immune peroxidase method on the sections of fetal stomach, normal definitive stomach, gastric mucosa with features of superficial and deep gastritis, enterolysis and dysplasia, as well as in gastric tumours. Normally, pepsinogen was found to localize in zymogen cells and to disappear in enterolysis and dysplasia of gastric mucosa. Intestinal antigen is absent from the normal mucosa, but is found in all the cases with enterolysis and dysplasia. CEA is most specific for dysplasia of gastric epithelium. In cancers of intestinal type pepsinogen was found in 54%, intestinal (colonic) antigen in 37.5%, CEA in 62.5%. In diffuse type cancers pepsinogen was absent, intestinal antigen was found in 76.9%, CEA in 92% of tumours.
