[Pneumoencephalography in epilepsy].

The paper deals with the methodological problems of pneumoencephalography, with clinico-morphological assessment of detected changes and also with their possible causal role in epilepsy. A purposeful examination reveals that 30% of patients have pathological changes in the cerebrospinal fluid spaces anatomically connected with the temporal lobes and adjacent structures. The morphological substrate of these changes is not quite clear, however they resemble those seen during operations for removing epileptogenic foci. In the overwhelming majority of cases, the most frequent alterations detected by the standard PEG technique (arachnoiditis, hydrocephalus) can not be cogently attributed to epilepsy per se.

Intracranial pearly tumors: the roles of computed tomography, angiography, and pneumoencephalography.

The findings is computed tomography (CT) and other radiological examinations in nine histologically proven cases of intracranial pearly tumors are presented. The CT findings include a low density lesion with no enhancement after contrast medium injection and in some cases a lesser degree of mass effect than would be expected from the size and site of such lesions. Pneumoencephalography is superior to CT in detecting suprasellar pearly tumors when they are isodense with the cerebrospinal fluid.

[Dynamics and pathogenesis of alterations in the bioelectrical activity of the brain following pneumoencephalography].

In 40 patients with remote sequalae of closed brain injury the authors studied the brain bioelectric activity before and following pneumoencephalography in dynamics. Changes in the EEG after PEG were seen in all cases in the form of decreased alpha-activity and a significant increase in the index of slow theta and delta waves. The dynamics in the EEG changes did not depend upon the clinical syndrome and were the same in patients with epileptical seizures, as well as in patients with posttraumatic syndrome. The EEG changes following PEG were transitory and were usually combined with distinct disturbances of metabolic-vegetative functions. This permits to allocate a certain place in the pathogenesis of detected EEG changes to the action of the air and x-ray on the brain membranes, as well as on the reticular structure, hypothalamic centers of metabolic-vegetative functions.

Screening of patients with Huntington's chorea by pneumoencephalography: criteria for decision and probability of error.

The problem of making a decision in case of two clinical hypotheses is treated by the application of the Neyman-Pearson criterion. As an example, a pneumoencephalographic index (frontal horn width divided by septum-caudate distance) of two different diseases, Huntington's chorea and Parkinsonism, is analyzed. As the aim of the investigator is to detect the maximum number of cases of Huntington's chorea, an appropriately high alpha, (the probability of "false alarm") should be chosen, and maximization of 1-beta (the probability of detection of Huntington's chorea) carried out.

Cerebellar atrophy: pneumoencephalography and computerized tomography correlation.

Ten consecutive patients with clinical diagnosis of cerebellar atrophy were studied by both CT and PEG. CT and PEG findings on the fourth ventricle, cerebellar cortical sulci and cisterns around the brain stem were evaluated independently and compared according to the criteria based on actual measurements. In the diagnosis of cerebellar atrophy, CT and PEG were complementary, but PEG provided better diagnostic information in detecting minimal changes than CT at this stage of development.
