[Metastasizing chondrosarcoma of the pisiform bone: a case report].

We report about a chondrosarcoma of the pisiform in a 39-year-old man. 1.5 years after exstirpation of the tumor, an extended recurrence developed. Two years after the first operation, an amputation was performed at midshaft of the upper arm. One year later, multiple metastases in both lungs were found.

[Vascular pedicled transposition of the pisiform bone for treatment of lunate malacia].

In a prospective study we investigated the results of 18 patients with Kienböck's disease stage II as defined by Decoulx, treated with transposition of the pedicled pisiform. In eight cases of minus variance of the ulna, a radius shortening osteotomy was performed. There was an average follow-up of 30 months, X-ray investigations were done every six months after operation. 17 patients had less pain, 14 patients showed an improved range of motion of 30 degrees. Magnetic resonance imaging proved vitalizing of the pisiforme in 16 cases.

[Long-term results of pisiform bone transposition in lunate necrosis].

Kienböcks disease is one of the most frequent manifestations of avascular necrosis. Multiple different surgical and conservative methods for treatment have been described. The transposition of pisiform on its vascular pedicle to replace the reamed out necrotic portion of the lunate has permitted revascularization and healing of the lunate. In the following article, the operative technique is described and the results of a five year follow-up study on 32 patients who underwent this operation are presented. On the basis of this study, we found, even in advanced cases of Kienböcks disease, that 50% of the 14 followed-up patients became free of symptoms or had pain only under stressful conditions.

Erosion of the triquetrum and pisiform bones caused by a foreign body granuloma.

A 16-year-old boy had pain and swelling in the left wrist 6 months after he had fallen on a twig. Erosion of the triquetrum and pisiform was seen on x-ray films. A granulomatous process was identified at operation. Despite numerous cultures and special stains, no organisms were identified. Wood debris was seen under high-power magnification. A wooden foreign body may cause bone and joint destruction through a granulomatous process. In this case, surgical debridement and excision stopped the destructive process.

[A rare case of metastasis of urinary bladder carcinoma in the pisiform bone. A case report].

Metastases to the hand are rare. The incidence of secondary tumors in the hand is about 0.15%. The lung, breast, and kidneys are the most common sites of primary lesions that metastasize to the hand. The authors report about a 66-year-old female who showed an osteolytic metastatic lesion in the os pisiforme which was ulcerated on the ulnopalmar aspect of the right wrist, 18 months after radical cystectomy because of a bladder carcinoma. In agreement with the patient, as well as with colleagues from the pathological, oncological, and radiotherapeutical field, the hand was not amputated as this was a solitary metastatic lesion.

[Fracture of the pisiform bone in inline skating].

During the last years inline-skating became one of the most familiar kind of sports with more than 12 million inline-skaters. The danger of injuring is often underestimated. Protectors and technique are important for an effective prophylaxis of injury. The upper extremity is mainly endangered. But despite effective protectors there is still the danger of injury. In particular protectors of the hand can shift the force when falling and result in a fracture of the Os pisiforme. This fracture can be missed in standard x-rays and only be diagnosed with special x-rays.

[Isolated fractures of the pisiform bone].

The literature has been discussed and two patients followed up for this injury have been presented. The patients were treated conservatively. Good results were obtained.
