Characterization of transboundary POP contamination in aquatic ecosystems of Pearl River delta.

During the past two decades, the rapid development of the Pearl River delta leads to substantial accumulation of various toxic organic compounds. This study aims to give a preliminary characterization of the existing state of contamination in this region and to provide insight into the possible fate of persistent organic pollutants (POPs) in this estuary. The available data on POPs in water, river, estuarine sediments, soil, and marine organisms within the Pearl River delta are compiled. It is shown that it may lead to transboundary POP pollution problems at both Hong Kong and Macau Special Administration Regions located at the downstream end of the region. It is noted that the levels of DDTs and HCHs in various environmental media are at alerting levels and that fresh DDT might still be applied illegally within the region. A systematic research is required to determine both the temporal and spatial variations of all POPs in various carrying media of the Pearl River delta as a whole.

Current knowledge and attitudes about organ donation and transplantation among Chinese university students.

Current attitudes toward organ donation among university students in mainland China and the differences in attitudes between Chinese students in mainland China versus overseas are unknown. To address these issues, we conducted a cross-sectional survey using questionnaires among 922 Chinese undergraduates from mainland China and overseas regions of the world. Data were analyzed by descriptive statistics, Student t tests, chi-square tests, and a logistic regression analysis. We found that blood donors showed significantly better awareness of heart, liver, lung, skin, and tendon donation among commonly transplanted organs/tissues. As to the willingness for cadaveric organ donation, 61.3% of respondents consented, 8.5% objected, and 30.3% answered "not sure." The percentage holding an organ donor card was 15.7% among students from Hong Kong; 3.0%, mainland China; 2.8%, Macau; 2.6%, Taiwan, and 4.0%, other regions of the world. In a logistic regression analysis, female students (odds ratio [OR], 2.24; 95% confidence interval [CI], 1.35 to 3.72) and blood donors (OR, 1.91; 95% CI, 1.10 to 3.32) did, but age and study specialty (medical vs nonmedical) did not show significantly more positive attitudes toward cadaveric organ donation. Compared with students from mainland China, overseas Chinese students from various regions did not show significantly different attitudes toward cadaveric organ donation. In summary, blood donors among university students have a greater knowledge of transplantation and a more positive attitude toward organ donation. Since university students are an important source of blood donors in China, they will be a potential pool of organ donors in the future.

Simulation of transboundary pollutant transport action in the Pearl River delta.

The rapid economic development in The Pearl River delta region (PRDR) has exerted serious potential pollution threats to areas in the vicinity, which have complicated the task of environmental protection in Hong Kong and Macau. In this paper, a three-dimensional numerical pollutant transport model coupled with a synchronised numerical hydrodynamic model, is developed and employed to simulate the unsteady transport of a representative water quality variable chemical oxygen demand in The Pearl River Estuary. It is demonstrated that there exists a transboundary pollutant transport action between Guangdong Province and Hong Kong for the pollutants in the wastewater discharged from PRDR.

Behavioural surveillance of sexually-related risk behaviours of the Chinese male general population in Hong Kong: a benchmark study.

The objective of the study was to establish a behavioural surveillance system (BSS) for sexually-related risk behaviours of the Hong Kong adult male general population. Benchmark data were obtained by interviewing 1,020 male respondents, age 18 to 60. The results showed that: (1) 14% of the respondents had engaged in commercial sex in the past six months, (2) 27% of the male commercial sex clients did not always use condoms when having sexual intercourse with commercial sex workers (CSWs), (3) 1.5% of the respondents had contracted sexually transmitted diseases (STDs) in the past six months, (4) 6.1% of respondents had only ever had sex with a man, (5) 0.8% of the respondents had practised unprotected anal intercourse with a man in a six-month period, (6) 4.4% of respondents practised sexually-related high risk behaviours, defined as unprotected sex with a CSW or unprotected anal intercourse with a man, and (7) 36.4% of those who engaged in commercial sex had not used condoms with their regular sex partners. Commercial sex was often practised outside Hong Kong, very commonly in Mainland China or Macau, and was often practised at multiple locations by the same client. Effective programmes have to be able to reduce the size of the at-risk population. This study together with future ones, will form the first BSS in Hong Kong for the general male population and will provide a relevant yardstick for programme evaluation.

Assessing shyness in Chinese older adults.

The Shyness Scale (SS) is a brief instrument for assessing shyness as a personality trait. The psychometric properties of the Chinese version of the SS were investigated in a random sample of 192 Macau Chinese older adults. The Chinese version of the SS possesses high internal consistency and exhibited satisfactory short-term test-retest reliability. The Chinese version of the SS exhibited acceptable convergent validity with other negative measures of psychological well-being including negative emotional states (assessed by the Negative Affect Scale), loneliness (assessed by the UCLA Loneliness Scale), and state anxiety and trait anxiety (assessed by STAI). The divergent validity of the Chinese version of the SS was demonstrated by the negative but significant association between the SS and self esteem (assessed by Rosenberg Self Esteem Inventory).

Amaricoccus gen. nov., a gram-negative coccus occurring in regular packages or tetrads, isolated from activated sludge biomass, and descriptions of Amaricoccus veronensis sp. nov., Amaricoccus tamworthensis sp. nov., Amaricoccus macauensis sp. nov., and Amaricoccus kaplicensis sp. nov.

Three isolates of gram-negative bacteria, strains Ben 102T, Ben 103T, and Ben 104T, were obtained in pure culture by micromanipulation from activated sludge biomass from wastewater treatment plants in Italy, Australia, and Macau, respectively. These isolates all had a distinctive morphology; the cells were cocci that usually were arranged in tetrads. Based on this criterion, they resembled other bacteria from activated sludge previously called "G" bacteria. On the basis of phenotypic characteristics and the results of 16S ribosomal DNA sequence analyses, the three isolates were very similar to each other, but were sufficiently different from their closest phylogenetic relatives (namely, the genera Rhodobacter, Rhodovulum, and Paracoccus in the alpha subdivision of the Proteobacteria) to be placed in a new genus, Amaricoccus gen. nov. Each of the three isolates represents a new species of the genus Amaricoccus; strains Ben 102T, Ben 103T, and Ben 104T are named Amaricoccus veronensis, Amaricoccus tamworthensis, and Amaricoccus macauensis, respectively. An isolate designated Ben 101T, which was isolated independently by Cech and Hartman in Kaplice, Czech Republic, was also characterized and belongs to the same genus. We propose that the isolate of Cech and Hartman should be placed in another new species, Amaricoccus kaplicensis.

Bacillus macauensis sp. nov., a long-chain bacterium isolated from a drinking water supply.

A novel long-chain, Gram-positive, rod-shaped, endospore-forming strain was isolated from the influent of a drinking water treatment plant in Macau, China. This facultatively anaerobic isolate (2.0-5.0 microm in length and 0.8-1.2 microm in diameter) formed long chains over 100 microm in length in both liquid and solid media. It had a growth temperature range of 20-40 degrees C, with an optimum at about 30 degrees C, and a growth pH range of 6.0-10.0, with an optimum at pH 8.5. The G+C content of the DNA extracted was 40.8 mol%. The main fatty acid components were anteiso-C(15:0) (67.2%) and iso-C(15:0) (21.7%), with small quantities of iso-C(14:0), C(14:0), iso-C(16:0) and iso-C(17:0). The main quinone component was MK-7. Phylogenetic analyses based on its 16S rRNA gene sequence revealed that this isolate is a member of the genus Bacillus, with no close relatives at the species level (sequence similarity <96.1%). These phenotypic and genetic properties suggested that this strain represents a novel species, for which the name Bacillus macauensis sp. nov. is proposed. The type strain is ZFHKF-1T (=JCM 13285T=DSM 17262T).

Survey of type 6 group variants of hepatitis C virus in Southeast Asia by using a core-based genotyping assay.

Previous surveys of the prevalences of genotypes of hepatitis C virus (HCV) in different populations have often used genotyping assays based upon analysis of amplified sequences from the 5' noncoding region (5'NCR), such as restriction fragment length polymorphism (RFLP) or hybridization with type-specific probes (e.g., InnoLipa). Although highly conserved, this region contains several type-specific nucleotide polymorphisms that allow major genotypes 1 to 6 to be reliably identified. Recently, however, novel HCV variants found in Vietnam and Thailand that are distantly related to the type 6a genotype (type 6 group) by phylogenetic analysis of coding regions of the genome often have sequences in the 5'NCR that are similar or identical to those of type 1 and could therefore not be identified by an assay of sequences in this region. We developed a new genotyping assay based upon RFLP of sequences amplified from the more variable core region to investigate their distribution elsewhere in southeast (SE) Asia. Among 108 samples from blood donors in seven areas that were identified as type 1 by RFLP in the 5'NCR, type 6 group variants were found in Thailand (7 from 28 samples originally identified as type 1) and Burma (Myanmar) (1 of 3) but were not found in Hong Kong (n = 43), Macau (n = 8), Taiwan (n = 6), Singapore (n = 2), or Malaysia (n = 18). Although this small survey suggests a relatively limited distribution for type 6 group variants in SE Asia, larger studies will be required to explore their distribution in other geographical regions and the extent to which their presence would limit the practical usefulness of 5'NCR-based genotyping assays for clinical or epidemiological purposes.

Toward implementation of a regional quality assurance program in cytopathology: the Hong Kong experience.

OBJECTIVE: To develop a local quality assurance program in cytopathology based on circulation of patient specimens on glass slides, with limited resources. STUDY DESIGN: A working group was set up for design and running of the program. Participation is on a laboratory basis. The scope and frequency of testing are defined. Well-documented cases (including gynecologic, nongynecologic and fine needle aspiration cytology) with commonly encountered diagnoses are collected. Consensus concerning the diagnosis, interpretive menu and scoring system is sought before the actual slide circulations using express mail. After returning their answers to the program organizer, the participating laboratories receive immediate feedback on their scores, with reference answers, explanatory notes, "whole-mount" images of glass slides and cumulative responses of peer laboratories for on-site checking. At the end of each year, an electronic file containing representative photomicrographs of all cases examined is provided to individual laboratories for their permanent records and training purposes. RESULTS: The program was launched in mid-2003. There were 24 and 27 participating laboratories from Hong Kong (and Macau) in 2003 and 2004, respectively. To date, >150 well-documented cytology cases are available in the slide pool and ready for circulation. As the revenue is mainly to cover the expenses of express mail, the program can be carried out at a relatively low cost. CONCLUSION: In order to have any cytology quality assurance program accepted by local laboratories, it has to be fair and practical. Strict confidentiality needs to be observed throughout the process. This program emphasizes both performance assessment and educational value. Adequate representation from experienced local cytology workers, detailed documentation support from authorities and assistance from dedicated staff are essential to the success of any external proficiency testing scheme. Regular review and evaluation are also necessary for continuous improvement. The Hong Kong experience can serve as an example of running a glass slide-based cytology quality assurance program in a small region with limited resources.

Depersonalization in kindergarten teachers in relation to rejection of help by fellow teachers and emotional support from family.

The present study investigated the external validity of a model describing the reactions of spurned helpers by examining the relation of spurning with burnout among kindergarten teachers. A second objective was to explore whether emotional support from one's family could reduce the negative effects of feeling spurned on burnout. 48 kindergarten teachers in Macau responded to a questionnaire measuring feeling spurned, emotional support from family, and depersonalization. Analyses indicated the effects of feeling spurned on depersonalization and also the benefit of emotional support; however, no evidence was obtained for he proposed buffering effect of emotional support.

Universal antenatal human immunodeficiency virus testing in Hong Kong: consensus statement.

Following the recommendations of the Advisory Council on AIDS, Hong Kong, the Hospital Authority announced plans to introduce universal antenatal screening for human immunodeficiency virus infection and hence, a consensus conference was held to discuss strategies for implementing such screening in Hong Kong. This paper reports the discussions of the consensus conference. The consensus meeting group consisted of 15 clinicians and scientists from Hong Kong, Macau, and Thailand. Seven commonly asked questions concerning mother-to-child transmission of human immunodeficiency virus were selected for discussion by the participating panellists. Information on the laboratory diagnosis of human immunodeficiency virus infection and the efficacy of preventive measures in reducing mother-to-child transmission of human immunodeficiency virus were reviewed. Data from local studies was also presented and discussed. The timing, potential problems, and cost issues involved in testing all pregnant women in Hong Kong for human immunodeficiency virus were then considered.

Homocysteine, endothelial dysfunction, and coronary artery disease: emerging strategy for secondary prevention.

Atherosclerosis is an important medical problem of the 21st century, but traditional risk factors could only account for 50% of the problem. Hyperhomocysteinemia is emerging as an independent atherosclerosis risk factor, associated with folate deficiency, renal failure, and relative deficiency of MTHFR (C677T polymorphism) or other enzymes depending on gender, age, and smoking status. Hyperhomocysteinemia has been reported to occur in 11-22% of western people, in 3-5% of normal asymptomatic Chinese subjects aged 18-70 years in Hong Kong, Macau, Sydney, and San Francisco, 23-36% of Chinese in Hong Kong with premature coronary artery disease, and 29% of a nonselective series of coronary subjects in Hong Kong. Evidence is accumulating that documents its associations with atherosclerosis disease in both case-control observations and prospective cohort studies, in vitro experiments, and in vivo experimental models in both animals and human subjects, as well as the successful improvement by homocysteine-lowering of endothelial function as surrogate atherosclerosis endpoints in asymptomatic human and coronary patients (secondary prevention). A number of large scale homocysteine-lowering trials are currently underway for stroke and heart attacks prevention. Collectively these trials will include more than 65,000 patients at high-risk for cardiovascular and stroke events, and should provide a reliable evidence-base for prevention.

Some observations on the cholera (E1 Tor) epidemic in 1961-62.

The author discusses some of the features of the cholera epidemic caused by El Tor vibrios in 1961-62 in the Western Pacific. The disease originated in the Celebes and spread from there to other parts of Indonesia, to Sarawak and, possibly, to Kwangtung. Hong Kong and Macau were most probably infected from Kwangtung. Subsequently the disease reached the Philippines, progressing from Manila southwards to the other islands, whence it invaded British Borneo. The El Tor epidemic did not differ clinically or epidemiologically from other cholera outbreaks observed during the past decade. The disease attacked poor, under-nourished people living under insanitary conditions. It spread along the coastline and, to a limited extent, along inland waterways. The authorities in the affected territories recommended that the quarantine regulations, sanitary measures and treatment methods used against cholera caused by the so-called "true" cholera vibrios be applied also to cholera caused by El Tor vibrios.

[A national survey on current status of the important parasitic diseases in human population].

In order to understand the current status and trends of the important parasitic diseases in human population, to evaluate the effect of control activities in the past decade and provide scientific base for further developing control strategies, a national survey was carried out in the country (Taiwan, Hongkong and Macau not included) from June, 2001 to 2004 under the sponsorship of the Ministry of Health. The sample sizes of the nationwide survey and of the survey in each province (autonomous region and municipality, P/A/M) were determined following a calculating formula based on an estimation of the sample size of random sampling to the rate of population. A procedure of stratified cluster random sampling was conducted in each province based on geographical location and economical condition with three strata: county/city, township/town, and spot, each spot covered a sample of 500 people. Parasitological examinations were conducted for the infections of soil-transmitted nematodes, Taenia spp, and Clonorchis sinensis, including Kato-Katz thick smear method, scotch cellulose adhesive tape technique and test tube-filter paper culture (for larvae). At the same time, another sampled investigation for Clonorchis sinensis infection was carried out in the known endemic areas in 27 provinces. Serological tests combined with questionnaire and/or clinical diagnosis were applied for hydatid disease, cysticercosis, paragonimiasis, trichinosis, and toxoplasmosis. A total sampled population of 356 629 from the 31 P/A/M was examined by parasitological methods and 26 species of helminth were recorded. Among these helminth, human infections of Metorchis orientalis and Echinostoma aegypti were detected in Fujian Province which seemed to be the first report in the world, and Haplorchis taichui infection in Guangxi Region was the first human infection record in the country. The overall prevalence of helminth infections was 21.74%. The prevalence of soil-transmitted nematodes was 19.56% (including hookworm infection 6.12%, Ascaris infection 12.72% and Trichuris infection 4.63%), and the estimated number of population infected with soil-transmitted nematodes was 129 million (with 39.3, 85.93 and 29.09 million for hookworm, Ascaris and Trichuris infections respectively). The prevalence of Taenia infection was 0.28% with an infected population of 550 000. The prevalence of Clonorchis sinensis in the national survey was 0.58%. From the survey in the known Clonorchis endemic areas with a sample of 217 829, terobius vermicularis infection in children under 12 years old was 10.28%. The positive rate of serological tests for hydatid disease, cysticercosis, paragonimiasis, trichinosis, and toxoplasmosis was 12.04% (4 796/39 826), 0.58% (553/96 008), 1.71% (1 163/68 209), 3.38% (3 149/93 239) and 7.88% (3 737/47 444) respectively. In comparison to the last national survey in 1990, the prevalence of hookworm, Ascaris lumbricoides and Trichuris trichiura infections has been reduced by 60.72%, 71.29% and 73.60% respectively, and the number of infected people by soil-transmitted nematodes has declined remarkably. However, the prevalence of Clonorchis infection significantly increased in the provinces of Guangdong, Guangxi and Jilin by 182%, 164% and 630% respectively. A remarkable increase of the prevalence of Taenia infection was found in Sichuan and Tibet, by 98% and 97% respectively. Echinococcosis is important in the Western part of China. Many parasitic diseases are still highly prevalent in the rural and pastoral areas with higher prevalence, morbidity and certain case fatality in farmers and herdsmen, especially in women and children.

The efficacy of povidone-iodine pessaries in a short, low-dose treatment regime on candidal, trichomonal and non-specific vaginitis.

Povidone-iodine pessaries (Betadine vaginal pessaries) containing 200 mg of povidone-iodine (PVP-I) in a water soluble base, are a widely used gynaecological preparation for treatment of vaginitis. We conducted a study on PVP-I pessaries at the reduced dose of one pessary daily for 7 days to suit conditions in Macau where patients are eager for a simple, short treatment course with confirmed clinical efficacy benefiting their professional and household essential requirements. Thirty-eight cases were selected for this particular clinical trial. These consisted of housewives, factory workers and professional girls (a sexually high risk group) who were suffering from vaginitis and complaining of vaginal discharge and irritation due to trichomonas, candida or non-specific vaginitis. After routine examination, including the collection of samples for microbiology, patients were treated with PVP-I pessaries 200 mg once a day for 7 days. The second microbiology samples were collected after the 7 day treatment period. Among the 38 cases, we had 30 cases with a complete record to allow us to make a summary and analysis of the trial. There were 14 cases of vaginitis due to yeasts and fungi infection, 3 cases of protozoa (trichomonas), and 13 cases due to non-specific pathogenic infections. 73.3% of cases had a complete symptomatic and microbiological cure and a further 16.7% had a microbiological cure with a good improvement in symptoms. No complications or side effects were found in the 7 days consecutive treatment course and inflammation quickly subsided during the course of treatment. PVP-I pessaries, used once daily for 7 days, seem to be an ideal treatment for cases who are likely to be unable to follow a longer treatment course.

Authentication of the Chinese drug "ku-di-dan" (herba elephantopi) and its substitutes using random-primed polymerase chain reaction (PCR).

DNA fingerprinting and polymorphism among the Chinese drug "Ku-Di-Dan" and its substitutes were demonstrated with arbitrarily primed polymerase chain reaction (AP-PCR) and random amplified polymorphic DNA (RAPD). Distinctive, reproducible genomic fingerprints from DNA of the Chinese drug Ku-Di-Dan and 9 species of Compositae were generated with six long (18-24 mer) and one short (10 mer) random-chosen primers with PCR. Ku-Di-Dan samples can be distinguished according to the banding patterns of their amplified DNA on agarose gels. Results showed that AP-PCR and RAPD fingerprints of the commercial samples of Ku-Di-Dan retailed in Fujian, Taiwan, Hong Kong and Macau markets are identical with that of Elephantopus scaber L. The relatedness of Ku-Di-Dan among 9 plants may also be estimated by the Similarity Indexes values of the genomic DNA fingerprints.
