Morocco WFS.

Although richer in natural resources than many developing countries, Morocco's economic development has been plagued by repid population growth, along with declines in the world price of its main export, phosphate, and a costly war in Western Sahara. Situated on the northwest corner of Africa, the Kingdom of Morocco is dominated geographically by its long coastline and the rugged Atlas mountain range to the east ans south. Abour 1/3 of the population live in remote mountain villages and speak Berber rather than the official language, Arabic. Despite large expanses of desert, agriculture has always been an economic mainstay. Morocco has hab near self-sufficiency in foodstuffs, but is now threatened by the burgeoning population, growing at 2.9% annually. The Moroccan governemnt officially recognized the role of population in development as early as 1966 and initiated a family planning program the following year. In 1979-80, a nationwide survey was conducted in conjunction with the World Fertility Survey program in part to assess the progrss of the family planning and health delivery systems. The newly published results show that contraceptive use is higher than expected. About 23% of the married women use birth control, and almost 20% use one of the more reliable methods. The pill is the most popular method, accounting for 71% of all users, followed by withdrawal, the IUD, and rhythm. Large families are valued, women want about 5 children each, but actual fertility exceeds even these levels. The Total Fertility Rate (TFR) for the 1974-79 period was 5.9 children per woman. This indicated a significant decline in childbearing among younger women, since women aged 40 and over reported an average total of 7 children. The WFS results support evidence of decline in birth rates indicated by the 1982 Moroccan census. Declines in infant mortality also measured by the WFS have not been as dramatic, but do show constant improvement, falling from 130 in the 1950s to about 93 in 1975-80. In general, the WFS results show real movement towards the government's goal of lowering fertility rates.

[Scorpion poisonous stings in the population of Khouribga (Morocco)].

Poisonous stings caused by scorpion constitute a public health problem in Morocco because of their frequency severity and socio-economic consequences that they generate. The province of Khouribga surveyed for this study is situated in a zone of high incidence and high lethality caused by scorpion stings in Morocco. In order to analyze the epidemiological profile and the poisoning risk factors, we are presenting the results of a past study conducted from April to December 2001. It consisted in making an exhaustive follow up from admission to release of all patients stung by scorpion. These patients consulted a sanitary structure (dispensary health centre or hospital) in the province of Khouribga, Morocco. Within a total of 1212 cases of stings, 63% of them occurred at night (between 06 pm and 06 am) particularly during July and August. Youngsters under fifteen represented 36% of these cases. 3.4% of the people who got stung presented typical poisoning and envenomation. The delay expected from sting to transfer to a medical structure was 1.85 +/- 0.11 hours for the patients who recovered against 2.67 +/- 0.11 for those who did not recover. To be noticed as well that 1.3% of the subjects who got stung die. Variance analysis indicates that the type of sanitary structures receiving patients, admission classes, and age of patients influence significantly the recovery rate of people who are stung.

[Consanguinity and public health in Morocco].

This study concerns the evolution of consanguinity in Morocco in its historical, religious, legal, and sociological context with regards to Moroccan customs and undertakes to evaluate its relationship to public health. Thus it attempts to specify if consanguinity, in its present state in Morocco, poses a problem for public health. With this goal in mind, methodology has been concentrated on two approaches. The first, based upon examination of documents and oral research interviews, has made theoretical assessment possible. The second, substantiated by the study of notarial marriage acts and the analysis of 4773 medical files concerning consanguineous marriages compiled throughout the seven regions of Morocco, has enabled us to specify the importance and the evolution of consanguinity and its impact on health. This study shows a marked decrease in consanguineous unions predominantly found in rural and mountainous zones and frequent between first cousins, especially the paternal cousin. From the medical standpoint, our study has revealed the absence of pathology in 97.13% of cases studied, the transmission of various gifts in 1.08% and that of degenerative traits in 1.79% of the descendants. Consequently, in our country, a country in perpetual evolution and in contact with the outside world, consanguinity which is disappearing of its own accord does not present a preoccupying problem for public health. Its future would seem to be limited to the relation between the physician and marriage partners in search of genetic counsel.

Intention to use contraceptives and subsequent contraceptive behavior in Morocco.

In this article, the relationship between stated intention to use contraceptives and subsequent use during a three-year period in Morocco is examined. Longitudinal data are drawn from two Demographic and Health Surveys: the 1992 Morocco DHS and the 1995 Morocco Panel Survey. Reported contraceptive intentions in 1992 have a strong predictive effect on subsequent contraceptive use even after controlling for other characteristics of respondents, and the strength of the effect is second only to that of previous contraceptive use. Women who in 1992 said they intended to use contraceptives in the future but did not do so are the most likely to have had an unmet need for contraception in 1995. Weakly held fertility preferences reported by some of the women surveyed in 1992 appear to have been a contributing factor in the subsequent failure of these women to act upon their intention to practice contraception.

Gross and light-microscopic features of ovine pulmonary hydatidosis and verminous pneumonias in Morocco.

Verminous pneumonias appear to be common in sheep in Morocco, therefore, a systematic study was undertaken to classify the pulmonary changes induced by various parasites in Morocco. A total of 613 pairs of lungs with parasitic pneumonias were diagnosed in 1152 young and adult sheep either slaughtered at two abattoirs in Morocco (487 out of 1152 sheep) or received at necropsy services (126 out of 1152 sheep) over a 15-month period. Based on gross and microscopic findings, four types of pulmonary changes were classified. Included were those associated with: 1. The larvae of Taenia echinococcus (Echinococcus granulosus (canine tapeworm); 520 out of 613 lungs (84.8%)); 2. Muellerius capillaris (450 out of 613 lungs (79.9%)); 3. Protostrongylus rufescens (375 out of 613 lungs (61.1%)); and 4. Dictyocaulus filaria (285 out of 613 lungs (46.4%)). Mixed infestations, involving the presence of two, three, or four species of parasites in the same lung, were encountered in 325 out of 613 (53%) lungs studied. Red-to-brownish consolidated areas, confined to the cranial parts of the lung, were observed frequently in some lungs affected with M. capillaris and P. rufescens. In most of the lungs, co-existent emphysematous, atelectatic, and abscessed areas were seen.

Domesticated Argania spinosa in Eastern Morocco: HPLC-DAD/GC-MS Chemical Profiling, Antioxidant and Antidiabetic Activities, and Network Pharmacology-Guided Molecular Docking.

The argan tree (Argania spinosa) is an endemic Moroccan species known for its primary product, argan oil, which possesses exceptional nutritional and medicinal properties. The current study aimed to evaluate and compare the antidiabetic and antioxidant activities of argan oil obtained from the introduced and native argan tree in eastern Morocco, to analyze its chemical composition using HPLC-DAD and GC-MS, and to investigate the molecular mechanisms behind the obtained pharmacological activities through an in silico pharmacological networking and molecular docking study. The results revealed that argan oil from all three regions of Morocco (Oujda, Agadir, and Chouihya) is rich in oleic and linoleic acids as major constituents, along with the presence of significant tocopherols. Regarding the antioxidant assays, including DPPH radical scavenging and iron-reducing power tests, argan oil from Oujda exhibited the highest activity, with the lowest IC50 values of 15.25 ± 0.022 mg/mL and 28.5 ± 1.7 mg/mL, respectively. Concerning the antidiabetic activity, we found that oil from Chaouihya showed the strongest α-amylase inhibition, while Oujda oil had the highest antiglycation activity, indicating that even introduced argan trees retain potent bioactivity. The results of the in silico investigation suggested that tocopherols may contribute to the antioxidant and antidiabetic potential of argan oil, showing predicted antioxidant activity (Pa = 0.843-0.967) and favorable binding affinities toward iNOS (ΔG = -9.3 kcal mol-1) and α-glucosidase (ΔG = -8.2 kcal mol-1). The identified fatty acids also showed predicted insulin-promoting activity (Pa = 0.59-0.75) and moderate enzyme-binding potential. Pharmacological network analysis identified 51 shared genes associated with antioxidant, antidiabetic, and argan-related targets, with enrichment of the AGE-RAGE signaling pathway. These computational findings provide possible molecular associations that may help explain the observed biological activities, although they remain predictive and require experimental validation. Overall, the in silico analysis suggests that tocopherols could be among the contributors to the multi-target profile of Argania spinosa oil, while fatty acids may provide complementary effects related to glycemic regulation.

HCV genotypes in Morocco.

To determine the hepatitis C virus (HCV) genotypes circulating in Morocco, virus isolates from 105 chronically infected and 19 hemodialysis patients were examined using the line probe assay. Genotypes 1 and 2 only were found among Moroccan patients. Subtypes 1b (47.6%) and 2a/2c (37.1%) were the most common, whereas subtype 1a (2.8%) was less common. Among the hemodialysis patients, only genotype 1 was found with a prevalence of 68.4% for subtype 1b and 15.8% for the subtype 1a. It was also shown that the HCV genotypes distribution varies with age in both studied populations. Subtype 1b was most prevalent among older patients, whereas subtype 2a/2c was mainly found among younger ones. Although Morocco belongs to the African continent, the circulating HCV strains are similar to those observed in some American and European countries.

The viral origin of cervical cancer in Rabat, Morocco.

In Northern Africa, cervical cancer is the second most common cancer among women. The diagnosis is usually made in advanced stages, and mortality is high, yet few studies have investigated the role of human papillomavirus (HPV) and other risk factors in the etiology of cervical cancer. A hospital-based case-control study was completed at the Institut National d'Oncologie (INO) in Rabat, Morocco. The study included 214 cases of invasive cervical cancer and 203 controls. A structured questionnaire was used to investigate known and suspected risk factors for cervical cancer. A GP 5+/6+ polymerase chain reaction system was used to detect the presence of HPV DNA and HPV type distribution. Probes for 30 HPV types were used in one research laboratory. HPV DNA was the central risk factor and accounted for the large majority of the cases. The adjusted odds ratio (ORa) for any HPV was 61.6 (95% CI, 29.2-130) and the corresponding HPV attributable fraction (AF) was 92%. Among cases of cervical cancer, HPV 16 was the most common type (67.7%) followed by HPV 18. The HPV type-specific prevalence was similar for squamous cell carcinomas and adenocarcinomas. In multivariate adjusted or HPV-stratified analyses, in addition to the strong effect of HPV, other risk factors identified were sexual intercourse with multiple partners before the age of 20 and low socio-economic status. Use of oral contraceptives for 5 or more years and high parity were also found to be related to cervical cancer. Screening was rare in this population but offered substantial protection against cervical cancer. In Morocco, cervical cancer is a late sequel of a viral infection with certain HPV types. Developing screening programs for preneoplastic cervical lesions is a public health priority. When available, HPV vaccination would offer a relevant alternative for preventing cervical cancer.

Household health expenditures in Morocco: implications for health care reform.

The purpose of this study was to investigate the level and distribution of household health care expenditures in Morocco, and to compare the level of health care funds provided by households with the levels provided by the government and international donors. In addition, the reliance of poor and non-poor households on both public and private providers was investigated. The study was based on data collected in the 1995 Demographic and Health Survey, which included a special supplement on health care expenditures. Descriptive statistics are presented on utilization of out-of-pocket expenditures for antenatal and obstetric care, chronic care, and non-chronic care associated with illness and injury, by urban/rural status and by socio-economic status. The results indicate that government health care providers are an important source of modern health care not only for poor households, but for better-off households as well. While individuals who use private health care providers incur substantially higher costs than those who use public providers, an unexpected finding of the study is the degree to which public clients pay for health care services, despite the fact that public care is nominally priced in Morocco. We conclude by discussing the implications of our results on the design and implementation of health care reform policies.

The role of user charges and structural attributes of quality on the use of maternal health services in Morocco.

Health care decision makers in settings with low levels of utilization of primary services are faced with the challenge of balancing the sometimes competing goals of increasing coverage and utilization of maternity services, particularly among the poor, with that of ensuring the financial viability of the health system. Morocco is a case in point where this policy dilemma is currently being played out. This study examines the role of household out-of-pocket costs and structural attributes of quality on the use of maternity care in Morocco using empirical data collected from both households and health care facilities. A nested logit model is estimated, and the coefficient estimates are used to carry out policy simulations of the impact of changes in the levels of out-of-pocket fees and structural attributes of quality in order to help guide policy makers responsible for the design of pending social insurance programs. The results of the paper suggest that social insurance strategies that involve increases in out-of-pocket charges in the form of copayments could be implemented without untoward effects on appropriate use of maternity care for non-poor women, but would be contraindicated for poorer and rural households.

Detection and quantification of microcystins from cyanobacteria strains isolated from reservoirs and ponds in Morocco.

In Morocco, the occurrence of toxic cyanobacteria blooms is confirmed in some water bodies used for recreational and/or as drinking water reservoirs. According to WHO recommendations, the establishment of a monitoring program for microcystins is a necessity. This paper presents toxicological studies of 19 toxic cyanobacteria strains of Microcystis, Synechocystis, Pseudanabaena, and Oscillatoria. These strains were isolated from various water bodies including natural lakes, reservoirs, and ponds located in central regions of Morocco. The isolation, culture, and biomass production of these strains was made on Z8 or BG13 media under laboratory controlled conditions. The hepatotoxicity of cyanobacterial lyophilized material was confirmed by mouse bioassays. The amount of microcystins produced by each strain was determined by the enzyme-linked immunosorbent assay (ELISA). The detection and identification of microcystin variants was performed by high performance liquid chromatography (HPLC) with photodiode array detection. Almost all strains showed medium to high toxicity, the estimated LD50 i.p. mice bioassay ranged between 28 to 350 mg/kg body weight. The concentrations of microcystins varied between 2.16 to 944 micrograms/g and 26.8 to 1884 micrograms/g dry weight determined by ELISA and HPLC, respectively. The screening of bloom-forming and microcystin producer cyanobacteria strains in these fresh water bodies leads us to propose the need for the establishment of a survey of cyanobacteria and a cyanotoxin-monitoring program.

Achlya abortispora, a new oomycete isolated from water samples taken from a water reservoir in Morocco.

Achlya abortispora sp. nov. was found in water and floating organic matter taken form a dam near Rabat, Morocco. The new species is described and compared with other species of the genus. Distinguishing characteristics of A. abortispora are the production of long fusiform sporangia with achlyoid and aplanoid discharge of zoospores; smooth-walled spherical to club-shaped oogonia, which are usually lateral, but at times intercalary, containing 1 to 20 oospheres. The oogonia can also bear 1 to 5 appendages, which may indicate oogonial proliferation. Most of the oospheres do not mature and are thus abortive. The antheridial branches supplying the oogonia are predominantly diclinous, but at times these may be monoclinous and androgynous. Antheridial branches coil and wrap around the oogonia. Morphologic features of the oomycete and the sequence of the ITS region of its rDNA, as well as their comparison with related species, are discussed. This is the first report of the occurrence of a saprolegniaceous oomycete from Morocco.

Seasonal fluctuations of phlebotomine sand fly populations (Diptera: Psychodidae) in the urban area of Marrakech, Morocco.

Phlebotomine sand flies (Diptera: Psychodidae) were collected continuously, using sticky traps, during 1 year from October 2002 to September 2003, in an urban area of Marrakech city (Morocco). A total of 3277 specimens were collected belonging to five species. Phlebotomus (Phlebotomus) papatasi (54.6%) is the predominant species followed by Sergentomyia (Sergentomyia) minuta (20%), S. (S.) fallax (11.3%), P. (Paraphlebotomus) sergenti (10.3%) and P. (Larroussius) longicuspis (3.8%). Data analyses showed a mono-modal annual pattern for P. sergenti and a bi-modal one for the other species. P. papatasi, the proven vector of Leishmania major in Morocco, was active throughout the year. This species did not diapause in this region. P. papatasi population peaked in June and November, which relating to the periods of risk in this area. Its preferred temperature ranged between 32 and 36 degrees C but no significant correlation was found between its density and the temperature. Considering the high density and long activity period of P. papatasi, the area of Marrakech should be regarded as a potential focus for L. major. This suggests the need for a continuously surveillance to prevent risk of zoonotic cutaneous leishmaniasis.

Tolerance and biomarkers as useful tools for assessing environmental quality in the Oued Souss estuary (Bay of Agadir, Morocco).

The aim of this study was to assess aquatic environmental quality of Oued Souss (Agadir, Morocco). This estuary has been subjected for a long time to large amounts of sewage discharges and industrial effluents. Since November 2002, no waste outlets have been discharged in this site due to their connection to a wastewater purification plant. Firstly, we have compared metal tolerance of the annelid polychaete (Nereis diversicolor) originating from Oued Souss and a relatively clean site (Oualidia, Morocco). Secondly, we have evaluated with a multi-marker approach (acetylcholinesterase [AChE], glutathione-S-transferases [GSTs], catalase, thiobarbituric acid reactive substances [TBARs]) responses of worms to the pollution gradient. Results have shown that worms from Oued Souss have acquired tolerance to copper and zinc due to a long-term sub-lethal metal exposure and this metal tolerance was maintained in spite of the end of wastewater discharges in this site. Higher catalase, GSTs and TBARs values have been observed in worms from Oued Souss sampled before implantation of wastewater treatment. The multi-marker approach confirms that these worms have been submitted to various contaminants. In contrast, high inhibition in AChE activities measured in worms from Oued Souss could be explained by the continuous agricultural influence of nearest areas. The level of contamination was probably maintained since biomarker values were generally higher in worms from Oued Souss when compared to Oualidia.

Mutational heterogeneity in low-density lipoprotein receptor gene related to familial hypercholesterolemia in Morocco.

BACKGROUND: Familial hypercholesterolemia (FH) is an autosomal dominant disorder caused by mutations in the low-density lipoprotein receptor (LDLR), apolipoprotein B (APOB) and proprotein convertase subtilisin/kexin type 9 (PCSK9) genes. Until now, molecular data concerning FH in Morocco is still limited. To gain more information in this field and to assess the contribution of these three genes in the cause of FH determinism, we analyzed six unrelated Moroccan probands and twenty-five of their family's members. METHODS: After LDLR and APOB genotype analysis, we screened the LDLR gene for mutations using southern blot and PCR-sequencing analysis. We also screened the APOB gene for the two common mutations R3500Q and R3531C by PCR-mediated site-directed mutagenesis. The PCSK9 gene was analyzed by direct sequencing. RESULTS: We identified three novel mutations (C25X, IVS3+5G>T, D558A) and two mutations previously described (D151N, A480E) in the LDLR gene. The R3500Q and R3531C mutations are absent in our probands and for 1 proband, the implication of LDLR, APOB and PCSK9 genes was excluded, supporting the implication of a fourth gene in the determination of FH. CONCLUSION: These data are in agreement with our previous study that suggests a heterogeneous mutational spectrum of FH in Morocco.

Heavy metal contamination from mining sites in South Morocco: 2. Assessment of metal accumulation and toxicity in plants.

Metalliferous soils cover a relatively large surface area in Morocco, and up to now no hyperaccumulating plants have been identified on these mining or these industrial sites. The aim of this work was to assess the extent of metal accumulation by plants found in three mining areas in southern Morocco with the ultimate goal of finding metal hyperaccumulating species by using the MetPAD biotest. The biotest helps to obtain information on the selective metal toxicity of aqueous extracts from the plants. A strong metal toxicity, as revealed by the biotest is an indication of a hyperaccumulating plant. Toxicity tests were run concurrently with chemicals analyses of metals in plants and their water extracts. The chemical analyses allow the determination of the hyperaccumulated metal(s). Specimens of the plant species mainly growing on and in the vicinity of the three mines were sampled with their corresponding soils. The results show that all plants analyzed had lower heavy metal content and toxicity despite the relatively very high soil concentrations. A comparison of our results with the criterion used to classify the hyperaccumulator plants indicates that plants we collected from mining sites were hypertolerant but not hyperaccumulators. This was confirmed by transfer factors generally lower than 1. Nevertheless, these tolerant plants species can be used as tools for revegetation for erosion control in metals-contaminated sites (phytostabilization).

Limited survey for the occurrence of aflatoxins in cereals and poultry feeds from Rabat, Morocco.

Fifty eight (58) samples of cereals for human consumption (20 corn flour, 17 wheat flour) and poultry feeds (n=21) purchased from popular markets of Rabat in Morocco were analyzed for aflatoxins (AF) by HPLC with immunoaffinity column (IAC) clean-up and fluorimetric detection. The incidence of AF in corn, wheat flour and poultry feeds was about 80, 17.6 and 66.6% respectively. High contamination levels were found in corn and poultry feeds samples. Levels of contamination of analyzed samples with AFB1 ranged from 0.23 to 11.2, 0.03 to 0.15 and 0.05 to 5.38 ng/g for corn, wheat and poultry feeds respectively. Results showed also that the contamination of 10% of samples of corn was higher than the limit set by EU regulations for AFB1 and total AF. This is the first report on the determination of AF in corn, wheat and poultry feeds from Morocco by HPLC and IAC.

Presence of aflatoxin M1 in pasteurized milk from Morocco.

Fifty four samples of pasteurized milk produced by five different dairies from Morocco were surveyed for the presence of aflatoxin M1 (AFM1) using immunoaffinity columns and liquid chromatography coupled to fluorescence detection. Confirmation of AFM1 identity in positive samples was based on the formation of AFM1 hemi-acetal derivative (AFM2a) after derivatization with trifluoracetic acid. Analytical results showed that 88.8% of the samples were contaminated with AFM1; 7.4% being above the maximum level of 0.05 microg/L set by the Moroccan and European regulations for AFM1 in liquid milk. The incidence of AFM1 in milk from these dairies was 100, 92.3, 90, 83.3 and 77.7% respectively, with AFM1 levels ranging from 0.001 to 0.117 microg/L and a mean value of 0.0186 microg/L. Based on the results presented in this study, the estimated daily intake of AFM1 was 3.26 ng/person/day. In this work, data on the natural occurrence of AFM1 in pasteurized milk produced in Morocco is presented for the first time.
