The article tackles the problems of interaction between the units of multidisciplinary hospitals and the feasibility of its improvement by means of introduction of a modern automated information system (AHIS). In the AHIS structure highlighted are the systems of order input and data communications ensuring more rapid interrelation of clinical departments with a pharmacy, laboratory, nutrition and other auxiliary services. The experience gained in the application of the system of order input in the central clinical hospital and abroad has been analyzed along with the role of organizational activities during the introduction of such systems. Additional functions of the order input system are pointed out. They assist not only in intensifying the curative and diagnostic process but also in raising its quality.

All healthcare facilities must have a plan for the protection of all persons on their premises and for their evacuation from the building in case of fire. Written copies of this plan must be available to all supervisory personnel. All employees must periodically trained and informed of their duties in implementing the plan. All beds must be easily movable should evacuation be necessary. Emphasis should be placed on moving patients who are in he room of fire origin and others who are directly exposed to the fire, and on maintaing in their rooms the patients who are not immediately threatened during the fire. Fire exist drills must include actual transmission of a fire alarm signal alongwith a simulation of a fire alarm signal alongwith a simulation of a fire alarm signal alongwith a simulation of a fire condition. Quarterly drills on all shifts are required. All personnel including administrative staff, maintenance personnel and internal must be trained, as well as the nurses on duty each shift. A minimum of 12 drills must behold each year. All employees must be instructed in life safety procedures and use of devices.
We describe the requirements and design of an enterprise-wide notification system. From published descriptions of notification schemes, our own experience, and use cases provided by diverse users in our institution, we developed a set of functional requirements. The resulting design supports multiple communication channels, third party mappings (algorithms) from message to recipient and/or channel of delivery, and escalation algorithms. A requirement for multiple message formats is addressed by a document specification. We implemented this system in Java as a CORBA object. This paper describes the design and current implementation of our notification system.

Conquest Hospital was a UK regional development site for a pre-Digital Imaging and Communications in Medicine (DICOM) picture archiving and communication system (PACS). The initial system was installed in mid 1992. Identification has been made of data transfer, ergonomic and single point of failure issues in the original PACS, which was called "iLAN." This has informed respecification of a DICOM/HTML PACS, the first stages of which have been hospital renetworking and installation of new DICOM 3.0 computed radiography/fluorography and computed tomography/magnetic resonance imaging segments. Final PACS elements are at contract stage. Plans are being completed for linkage of PACS to a clinical information system to create a comprehensive electronic patient record system.

A gap is in linkages between carers both within a hospital and between hospitals and community providers. A range of technologies is available to improve communication of patient clinical data and other information. This paper discusses the benefits and challenges of electronic communications and reports on a successful case of physician use of a hospital communication system (information systems, technology, physicians, health care).

The University of California at San Francisco Medical Center has made a commitment to upgrade its information and telecommunications systems infrastructure. One of the several projects being undertaken by the Medical Center, the Intelligent Console Project, demonstrates how integrating different systems, databases and technologies can improve the quality and accessibility of information, while reducing costs and stream-lining administrative activities. The Intelligent Console acts as an interface mechanism for the several constituent systems and data-bases of the Medical Center and provides a single, front-end control console by which operators can support communications using standardized procedures. Much paperwork has been eliminated and operator training and scheduling streamlined. Equipment consolidation has also freed up space at the Medical Center.
