BACKGROUND: AIDS in foreign migrants can have specific characteristics compared with that of Spanish patients. METHODS: Cross-sectional study, including cases from the Barcelona AIDS register diagnosed from 1988 to 1998. Global characteristics of migrants are compared with those of Spanish patients. Adjusted odds ratio (OR) with 95% confidence interval (CI) were calculated through logistic regression. A formal analysis was also performed. RESULTS: Among the 4,663 patients included, 5% were migrants. 33% of patients were diagnosed with AIDS 10 years after having migrated to Spain. They differentiate at the multivariate level by sex (OR [male]: 1.70; CI: 1.10-2.64), district of residence (OR [district I]: 2.07, IC: 1.50-2.87) and transmission pattern (OR [other than injecting drug users]: 4.08; IC: 2.77-5.99). No significant differences in survival were showed. CONCLUSIONS: The percentage of migrants among AIDS cases in Barcelona is low. Migrants are more frequently men, more often live in the inner city and are less frequently injecting drug users.
The objective of the study was to examine the effectiveness of empowering in-service training programs for foreign nurse aides working in community-based long-term care (LTC) facilities. The design was a pretest and post-test design with experiment and control groups. The sample consisted of purposeful sampling from 10 LTC facilities in the Shihlin and Peitou areas of Taipei. A total of 35 foreign nurse aides participated in this study; 16 in the experimental group and 19 in the control group. The experimental group attended the training program for a 3-month period, whereas the control group did not receive any training. The research findings reveal that the training program was effective in increasing the work stress of workload/scheduling (Z = 2.01, p </= 0.05), meaning that the training program has raised the awareness of work stress for foreign nurse aides. The results could be used as a reference when considering the development of in-service training programs in LTC facilities.
The concept of empowerment, widely accepted and utilized in many health-related disciplines, connotes a process of gaining control over one's life and influencing the organizational and social structures in which one lives. This article demonstrates an example of how empowering education can be applied on foreign nursing aides working in long-term care facilities and how differing empowering strategies, processes and effects can be adopted to address differing situations and ethnic backgrounds. How high priority issues of concern for foreign nurses are handled impacts upon their ability to perform their jobs well. Empowering strategies can help deal with such issues more effectively and, as a result, reduce work stress and improve on-the-job performance. During the preparation stage, the empowerment process focuses on building a trusting partner relationship. During the work stage, the process focuses on inspiring foreign nurses' self-awareness, encouraging their perceiving the barriers and needs at work, and, most importantly, encouraging nurses to think critically and positively and to provide feedback. The effects of empowering education include enhanced problem solving abilities, rising nurse self-confidence in his/her caretaking abilities, enhanced self-esteem, and improved adaptation to the work environment. This paper provides empirical experiences with regard to the application of empowering education in clinical settings as well as process and management strategies related to foreign nursing aides employed in long-term care facilities.

A discount rate for the consumption of future generations is typically composed of two parts. One is a "pure" time preference for immediate over postponed consumption, the other a declining marginal utility as consumption increases. The costs of greenhouse abatement, however, for at least the first 50 years, will be borne by the developed countries; the benefits will accrue to the presently undeveloped. Pure time preference always relates to one's own consumption; it has no relevance here. Consumption transfers over time will be from richer to poorer, from lower to higher marginal utility. It is a foreign aid program and it ought to have to compete with more direct foreign aid, which can benefit the very poor rather than their much-better-off descendants.

Based on the results of an inquiry among 296 Norwegian missionaries and Foreign Aid workers, we present some observations on the present rabies situation in several African countries, India and Bangladesh, Equador and Bolivia. Traditionally these countries receive a large proportion of our missionaries and Foreign Aid personnel. The incidence of rabies varies considerably in the areas covered by this investigation. When calculated for the mean length of stay (4 to 5 years) the average rating of physical exposure to proven or suspected rabies was 16% per household and 7% per individual household member. The risk of exposure was higher with children (10%) than adults (6%). the family dog equalled the stray dog as the source of suspected infection. Of the 48 persons who had been recommended post-exposure vaccination two thirds had only tended or been licked by the offending animal. However, in such animals the ratio of proven or highly probable rabies to non-proven or unknown diagnosis was 2:1. The rabies situation was considered as a minor factor of psychic stress by two thirds of the responding persons, even in highly epizootic countries. The results of this study may be of some value in the process of forming a policy for pre-exposure prophylaxis against rabies for people who are going to stay for a prolonged period of time in these areas of the world.
