Effect of prednisolone on prostaglandin synthesis by rectal mucosa in ulcerative colitis: investigation by laminar flow bioassay and radioimmunoassay.

The effect of two concentrations of prednisolone on synthesis of prostaglandin E2 (PGE2) by 40 rectal biopsies in organ culture was investigated using both laminar flow bioassay and radioimmunoassay (RIA). Prednisolone (concentration 8.33 x 10(-7)M) reduced mean synthesis of PGE2 to 36.4% of control values (measured by bioassay) or 26.2% of control values (measured by RIA). With prednisolone (concentration 5.66 X 10(-4) M) synthesis of PGE2 was 7.7% of control values (RIA). The two concentrations are similar respectively to those achieved in plasma after oral prednisolone and delivered topically by prednisolone enemata. Inhibition of PG synthesis may thus explain prednisolone's anti-inflammatory action in the treatment of ulcerative colitis.

Colostomy care.

Psychic preparation of the patient for the necessity of colostomy is a long first step toward his adjustment to living with an artificial anus. Proper surgical placement of the outlet will ease care of the bowel. Control of fecal consistency and peristaltic rate should ideally produce constipation responsive only to habit or enemata. The object of the enema is to produce an evacuation thorough enough to prevent soiling for a day or two. The object of dietary variations is to produce a manageable volume and consistency of fecal stream. The technique of enemas and choice of diet can be individualized when the underlying principles are understood.

A survey of colonic pathology with a double contrast method and analysis of the indication for a barium enema.

A prospective study of colonic pathology with double contrast method was made of 146 patients sent to the X-ray department of the Black Lion Hospital, Addis Ababa. The barium enemata were done for complaints like constipation, diarrhoea, rectal bleeding and palpable abdominal mass often without any relevant prior physical clinical examinations. Of those examined only 26 (17.8%) patients were found to have radiologically colonic pathology and the rest, 120 (82.2%) patients were reported normal. The policy of prior clinical examination before any barium enema such as rectal digital examination, sigmoidoscopy and stool occult blood studies would reduce the normal barium enema. The survey compared bowel preparation with adequate fluid intake and laxative without cleansing enema serves as a good method of bowel preparation for ambulatory patients.

Oxamniquine treatment for schistosomal polyposis: a 1-2 year follow-up study.

A follow-up was carried out into the study of schistosomal colonic polyposis reported in the last article. Eighteen patients with schistosomal colonic polyposis were treated with oxamniquine and their progress was evaluated for 2-3 weeks after treatment, at 3 months and at 12-24 months. Sigmoidoscopy to examine mucosa on the polyps and barium enemata to determine the size and number of polyps showed variable success, but treatment with oxamniquine produced no untoward side effects.

[Chronic inflammatory bowel disease and pregnancy].

The authors have looked out the obstetric history of 13 young women who were suffering from ulcerative colitis or Crohn's disease between the years 1978 and 1982 and who had 25 pregnancies. This study made it possible to analyse the influence of these pathological conditions on the evolution of pregnancy as well as the effect of pregnancy on the evolution of the disease. The different therapeutic treatments that have to be given for these diseases, such as medical treatment using Sulphasalazine, enemata, corticosteroids and Azathioprin, or surgical treatments have been examined in their relationship with pregnancy. The results that have been obtained confirm those to be found in the literature in the sense that the two conditions are independent of one another and therapy for the pregnancy as well as for the bowel disease can be continued without too much consideration for the effect of the one condition on the other.
