Thoracic outlet syndromes: a plea for conservatism.

This article focuses on the controversial subtype of TOS, which presents primarily sensory sentence, normal neurologic examination, normal electrodiagnostic study results, and normal neck x-ray films. At present, this entity is so variable in its "typical" presentation that it has no characteristic profile. Although some physicians now are attempting to narrow its boundaries, the goal of fashioning a syndrome that has general acceptance remains to be achieved, consequently, neurosurgeons who do not now perform TOS surgery, but are contemplating doing so, are urged to review the literature critically on the subject first, and to err toward conservatism.

[Selective conservatism in the management of abdominal stab wounds].

Two hundred and two consecutive patients with abdominal stab wounds in a 14-year period were analysed as follows: (1) The patients were classified into 4 categories; type I (no peritoneal penetration) 12%, type IIa (peritoneal penetration, no organ injury) plus type IIb (organ injury, no repair required) 26% and type III (organ injury, repair required) 62%. Injuries caused by kitchen-knife or violence, wounds in the lower chest or epigastrium, and harakiri-injuries (Japanese traditional method of suicide) resulted in high frequencies of type III injury. (2) Hematoemesis, hematuria, unstable hemodynamics, and signs of peritonitis were reliable indications for immediate laparotomy. Both stabography and diagnostic peritoneal lavage were unuseful as adjunctive diagnostic methods. (3) A policy of selective management in 126 patients (1981-1988), comparing with a policy of mandatory exploration in 76 patients (1975-1980), reduced unnecessary laparotomy rate from 32% to 14% (p less than 0.01). (4) Injury patterns, observed in this series and the reported series from Japanese emergency centers, were strikingly different from the American series. The author emphasize the threat of morbidity caused by delayed diagnosis in the application of selective conservatism and propose the acceptable rate of unnecessary laparotomy; less than 15%.

Selective conservatism in abdominal stab wounds: the efficacy of serial physical examination.

An analysis of 186 consecutive patients with anterior abdominal stab wounds in a 2-year period was carried out to assess the efficacy of serial physical examination as the determining factor for the need for laparotomy. Of 106 patients managed by clinical evaluation, the incidence of negative laparotomy was 0.9% and three patients (2.7%) had an "unnecessary" celiotomy. There were no missed injuries or delayed detection of intraperitoneal visceral trauma. The incidence of negative laparotomy among patients subjected to wound exploration +/- peritoneal lavage (n = 41) was 2.4%, and 39 patients had mandatory laparotomy for evisceration, with a negative celiotomy rate of 20.5%. Our experience supports serial physical examination as a highly effective method of "selective conservatism" of anterior abdominal stab wounds. Mandatory laparotomy for evisceration needs further clarification.

Filial piety, authoritarian moralism, and cognitive conservatism in Chinese societies.

In the present study, I examined the role of Confucian filial piety in relation to parental attitudes, the function of personality, and social cognition, using data assembled from 11 samples of adults and students in Hong Kong and Taiwan. Results showed that (a) filial attitudes tend to be moderately associated with parental attitudes and child training that emphasize obedience and indebtedness to one's parents, impulse control, and proper conduct; (b) people endorsing traditional filial and/or child-training attitudes tend to be poorer in verbal fluency, to adopt a passive, uncritical, and uncreative orientation toward learning, to hold fatalistic, superstitious, and stereotypic beliefs, and to be authoritarian, dogmatic, and conformist; and (c) parents' attitudes rooted in filial piety tend to result in high rigidity and low cognitive complexity in their children. The results support the contention that filial piety underlies both authoritarian moralism and cognitive conservatism.

Conservatism and liberalism: a psychoanalytic examination of political belief.

A review of the historical development of conservative and liberal through reveals contradictory basic assumptions: the former views man as essentially evil by nature, and places its priorities upon self-restraint; the latter views man as essentially good, and with umlimited capabilities, and celebrates his efforts at creating a perfect society for himself. Three clinical vignettes are recounted in which the patient's intrapyschic organization found support from a hormonious political philosophy. It is suggested that conservatism and liberalism offer the individual an ideological resolution to the narcissistic injuries he experiences as a consequence of life's inevitable and developmentally determined disappointments.

Functional conservatism in mitochondrial evolution: insight from hybridization of arctic and brook charrs.

To assess the potential adaptive value of mtDNA, we evaluated functional properties and thermal sensitivity of key mitochondrial enzymes in two species that have originally evolved in different thermal environments (arctic charr, Salvelinus alpinus, and brook charr, S. fontinalis), as well as in their hybrids. We measured the activity of two enzymes of the electron transport system (cytochrome c oxidase and NADH-ubiquinone oxidoreductase), one enzyme of the mitochondrial matrix (citrate synthase), and one enzyme of the anaerobic glycolysis (lactate dehydrogenase) in the red muscle at three temperatures (6 degrees C, 12 degrees C and 18 degrees C). Surprisingly, the species presented no significant differences in enzyme activity, thermal sensitivity or thermostability of key metabolic enzymes even though they evolved in different thermal environments and present important differences in amino acid sequences. It seems that amino acid substitutions between those species have minor impact on the functional properties of mitochondrial enzymes studied. The thermal sensitivity results (Q(10)) obtained for inner-membrane mitochondrial enzymes differed somewhat from those of mitochondrial matrix or cytosolic enzymes. This result indicates the modulation of thermal sensitivity of all mitochondrial inner-membrane processes by a common parameter, which could be the structural and functional properties of membrane phospholipids.

Chromosome banding pattern conservatism in birds and nonhomology of chromosome banding patterns between birds, turtles, snakes and amphibians.

The G-banded karyotypes of 4 species of birds representing the orders Galliformes, Columbiformes and Musophagiformes were compared. Banding pattern homology between orders was limited t 5o 5 major chromosome arms and the Z chromosome. Even in these major chromosome arms pericentric and paracentric inversions produced alteration of the banding pattern sequences. Addition of constitutive heterochromatin was responsible for changes in banding patterns in the Z chromosome. The chromosome banding patterns of an emydid turtle, Terrepene carolina, 5 species of boid snakes of the genera Liasis, Acrantophis, and Sanzinia and the African clawed-frog. Xenopus muelleri, were also compared to the bird chromosome banding patterns. No homology was observed between any of these major groups: bird, snake, turtle, amphibian. However, intergroup homology was apparent. - The data obtained do not support reports of broad interordinal direct homology of the macrochromosomes of birds and refutes the idea of a primitive bird karyotype with 3 pairs of "Agroup' chromosomes and 3 pairs of "B group' chromosomes. - The major mechanisms responsible for chromosome evolution in birds appear to be centric and tandem fusions, paracentric and pericentric inversions, and addition or deletion of heterochromatin.

Conservatism of sites of tRNA loci among the linkage groups of several Drosophila species.

The sites of seven tRNA genes (Arg-2, Lys-2, Ser-2b, Ser-7, Thr-3, Thr-4, Val-3b) were studied by in situ hybridization. 125I-labeled tRNA probes from Drosophila melanogaster were hybridized to spreads of polytene chromosomes prepared from four Drosophila species representing different evolutionary lineages (D. melanogaster, Drosophila hydei, Drosophila pseudoobscura, and Drosophila virilis). Most tRNA loci occurred on homologous chromosomal elements of all four species. In some cases the number of hybridization sites within an element varied and sites on nonhomologous elements were found. It was observed that both tRNA(2Arg) and tRNA(2Lys) hybridized to the same site on homologous elements in several species. These data suggest a limited amount of exchange among different linkage groups during the evolution of Drosophila species.

Neutropenic typhlitis. A plea for conservatism.

Neutropenic typhlitis is an acute inflammation of the cecum occurring in neutropenic patients. Two cases are described, both presenting typical features of the disease. It is our experience that the treatment approach should consist of bowel rest, fluid therapy, and massive administration of broad spectrum antibiotics. In some cases, full recovery will follow. If abdominal signs persist, surgery should be deferred until hematologic convalescence occurs. At this time, cecostomy and drainage should be adequate. Only in very severe cases, with wide-spread necrosis, should a right hemicolectomy be performed.

Abdominoscrotal hydrocoeles: little place for conservatism.

Abdominoscrotal hydrocoeles (ASH) are infrequently reported in children. The presence of bilateral ASH is even rarer, with fewer than five cases reported to date. ASH are present at birth and tend to be progressive in nature. Spontaneous resolution has not been documented. The pathogenesis is uncertain and their presence in the neonatal period suggests a preformed abnormality of the processus vaginalis. Diagnosis is suspected on clinical examination and confirmed by ultrasonography, which is a reliable diagnostic tool. Undescended testes frequently coexist. Early surgery is the preferred management, and complications are frequent if left unattended. Surgery is simply an extension of a hernia operation, without the need for a laparotomy.

The role of the cognitive activity context in the conservatism of unconscious visual sets.

Studies in healthy adults were performed to compare measures of the rigidity of a verbal set in three series of experiments: in series 1, pseudowords were presented at the set-forming stage of the experiment, while common words were presented in the test stage; series 2 used the pseudoword/word conditions of series 1 with the additional task of identifying the position of a target stimulus in a matrix, requiring discrimination from other symbols in terms of two characteristics; in series 3, the pseudoword/word test was followed by an initial task consisting of identifying the matrix position of a target stimulus in conditions in which the need to discriminate was minimized. The results supported the hypothesis that the rigidity of a visual set depends on the cognitive activity context. This property is significantly dependent on the loading applied to working memory and the cognitive tasks solved by the subject, particularly the ratio of involvement of the ventral and dorsal visual systems in the cortical processing of sequentially acting verbal and non-verbal visual stimuli. The cognitive set paradigm serves as a model for experimental studies of the roles of the ventral and dorsal visual systems in organizing recognition functions.
