The oculocardiac reflex occurred in a patient with an anophthalmic socket undergoing surgery for inferior fornix shortening and laxity of the lower lid. This reflex occurs only rarely in anophthalmia; it has not been previously reported during socket surgery.

K1 preprotoxin is the 316 residue precursor of the K1 killer toxin secreted by the yeast Saccharomyces cerevisiae. The SP beta la reporter consists of the mature, secreted form of beta-lactamase (beta la) fused to S and P, two fragments of preprotoxin. S is the N-terminal 34 residues, including the secretion signal. P, a 67 residue 'processing' segment with three sites for N-glycosylation, terminates in a Lys Arg site for cleavage by the Kex2 protease. Expression of SP beta 1a in yeast results in efficient secretion, processing by signal peptidase and glycosylation in the endoplasmic reticulum, producing pro beta la. Kex2 cleavage of pro beta la in the lumen of a late Golgi compartment releases beta la, which accumulates stably in culture media buffered at pH 5.8-7. The half-life of secretion is 11 min at 30 degrees C; 10-12% of the total activity in exponential-phase cells is intracellular, mostly in the form of pro beta la, indicating that transit from the endoplasmic reticulum to the Golgi is rate limiting. We have used SP beta la expression in single- and multi-copy vectors to compare the PGK, GAL1, GAL10, PHO5 and CUP1 promoters under varying nutritional conditions. In exponential-phase cells, secretion of beta la over a 40-fold range and up to several micrograms/ml was proportional to transcript level, demonstrating that SP beta la can be employed as a convenient secreted reporter of promoter function in yeast.
Malignant papillary mesothelioma of the tunica vaginalis testis is an extremely rare tumor arising from the mesothelium of the tunica vaginalis. A total of 14 cases have been reported, with only two exhibiting cutaneous metastases. We present a third case of cutaneous metastases arising from this unusual tumor. In addition, this is the first reported case of the occurrence of pagetoid epidermal invasion in this condition.

In 1956 a disease of nursing pigs previously unreported in Denmark, was observed in several swine herds. The disease syndrome appeared identical with that later described by Roe and Alexander in 1958 and by S. F. Cartwright and M. Lucas in 1968. Since 1956 there have been no reports of such outbreaks in Danish herds. To investigate whether the virus is still present, a survey was carried out to detect neutralizing antibodies in sera from a number of boars using a micro technique. In 144 sera from Jutland 12% had titers ranging from 10 to 32 ND-50, 22% from 32 to 90, 37% from 90 to 256, 14% from 256 to 724 and 3% above 724 ND-50 with a total of 86% positive. Of 174 sera from the rest of the country 16% had titers ranging from 10 to 32 ND-50, 18% from 32 to 90, 28% from 90 to 256, 13% from 256 to 724 and 3% exceeding 724 ND-50 with a total of 79% positive.

Teratoma ranks second in frequency to yolk sac carcinoma among testicular germ cell tumors in infants, accounting for 25% of cases. Testicular teratomas are uniformly benign in children younger than 24 months old. Epidermoid cysts are tumor-like lesions of unknown etiology, probably monodermal teratomas, comprising 3% of all pediatric testis tumors. Bilaterality is rare for both tumors. We report a case of a synchronous left testis teratoma and a right testis epidermoid cyst. Appropriate therapy is discussed.
BACKGROUND: The Vanderbilt University Medical Center (VUMC) has designed and deployed the Chemotherapy Incident Reporting and Improvement System (CIRIS), which is embedded into daily care processes. The system uses commercial information technologies, including handheld computers, to create a mobile Web-based chemotherapy incident reporting system for nurses and pharmacists. Two phases--(1) development and implementation of the CIRIS incident reporting safety registry and (2) development of the handheld-computer interface--were implemented. The final phase entails integration of the computerized order entry system into the front end of the CIRIS architecture. The voluntary incident reporting system data are stored over time for use by the multidisciplinary safety improvement team. RESULTS: Staff buy-in has been demonstrated by increased reporting rates, the high number of provider-initiated improvements made to the reporting tool during the first year of implementation, and specific chemotherapy safety interventions conceived from analysis of the reported data. CONCLUSION: The CIRIS model for pediatric chemotherapy safety improvement has been implemented in the inpatient setting but could easily be configured for a variety of other clinical applications in inpatient or outpatient settings. CIRIS has been effective, especially in the chemotherapy pharmacy, where incident reporting has increased dramatically.
A renal biopsy of a 57-year-old man with nephrotic syndrome revealed marked glomerular deposition of Congo red-positive, bire-fringent material characteristic for amyloid. Ultrastructurally, however, this material is composed of nonbranching, haphazardly arranged fibrils of approximately three times the thickness of typical amyloid fibrils. To our knowledge, there has been no report of such a finding, and this unique case enlarges the morphologic spectrum of renal fibrillosis.
