Communicable disease control: an introductory course for MPH students.

Persons preparing for careers in public health practice need a solid academic grounding in the principles of communicable disease control before arriving on the job. We have developed an introductory course in infectious disease control for the Master of Public Health program in Jerusalem, which includes instruction in the following broad areas: How do micro-organisms spread and cause disease? How do we investigate and control an outbreak? What are the basics of primary prevention by immunization and what can mass immunization accomplish? What is the importance of routine ongoing communicable disease surveillance? What are the essentials of Travel Medicine? How can public health officials provide useful information to a concerned citizenry by intelligent cooperation with the media? How can immunization programs and other programs for infectious disease control be kept current with the help of expert advisory committees? What are the best resources available to the public health practitioner in the area of infectious disease control? Armed with the essentials, the practitioner will have the tools to approach communicable disease problems in an orderly and rational way even in an atmosphere of public and professional ignorance and apprehension.

Improving the quality of communicable disease control: the example of meningococcal disease.

Improving the quality of health services is central to current attempts to reform the National Health Service. The main approaches to quality improvement in communicable disease control to date have been the development of practice guidelines and audit projects. Descriptions of the impact of quality initiatives in public health generally and communicable disease in particular have been limited, objective measures of quality are rare and few reports outline improvements that have been achieved in practice. We describe a project to develop a set of quality indicators for meningococcal disease control. A set of candidate indicators was developed and screened using standard approaches based on local consensus. We outline how they could be further tested and used to promote quality improvement.

Communicable disease control in a migrant seasonal workers population: a case study in Norway.

Reliable data on the health status of migrant seasonal workers in Europe is scarce. Access to public health care for this population depends on national regulations, and their legal status in host countries. In this manuscript we describe a case study of a salmonellosis outbreak that occurred in Norway, and highlight the difficulties encountered in applying control measures in a population of seasonal migrant farm workers. Surveillance and control of infectious diseases need to be supported by legislation which makes implementation of control measures possible. Efforts have been made to improve the rights for migrants in Europe with regard to healthcare, but seasonal migrant workers still remain largely outsiders where these measures are concerned. Special attention should be given to this disadvantaged group in terms of social rights and healthcare. Preparedness plans should be improved to deal with contagious pathogens involving the seasonal migrant population.
