[Experimental stapesplasty with the CO2 laser].

Perforation of the footplate in otosclerosis can be performed accurately, when the large and high energetic CO2 laser focus is reduced to 0.5 mm in diameter by an aluminium diaphragm. The operation was performed on three dogs. BAEPs were evoked by bone-conducted acoustic stimuli. Hearing threshold was unchanged immediately after operation as well as after 2 and 6 weeks postoperatively. The new method has still to be performed in man.

[Experience with fibrin glue in type-III tympanoplasty with stapes elevation].

In a retrospective study, type III tympanoplasties with stapes elevation on two groups of patients were compared. In group I (101 patients) the stapes elevations, the temporalis fascia grafts for tympanic membrane reconstruction, the grafts of periosteum and cartilage used for repair defects of the posterior bony meatal wall and the Stacke-II plasty replaced in its original position after tympanoplasty were also fixed with fibrin glue (Tissucol). Group II (control group of 102 patients) was operated with fibrin glue. In group I the fascial grafts of the tympanic membrane healed faster, more securely and without fewer complications. The external auditory canal however healed equally well in both groups. Early hearing results of type III tympanoplasty could be improved by fixing the stapes elevations with fibrin glue.

Sodium fluoride: effectiveness of treatment for cochlear otosclerosis.

We examined the effectiveness of sodium fluoride in treatment of cochlear otospongiosis in ninety-four patients with cochlear otosclerosis and ninety-eight patients with stapedial otosclerosis and sensorineural hearing loss. The drug halted or slowed the progression of sensorineural hearing impairment in 63% of the patients with cochlear otosclerosis and 46% of the patients with stapedial otosclerosis. The single factor that best predicted which patients would respond most favorably to treatment was rate of progression before treatment. Sodium fluoride therapy was successful for 79% of the patients losing their hearing at a rate of 5 dB or more per year at one or more of the speech frequencies. The finding that patients with more rapid rates of progression responded most favorably to sodium fluoride therapy suggests that patients with the most active otospongiotic processes will be the most responsive to treatment.

Middle ear surgery in fibrous dysplasia of the temporal bone.

Fibrous dysplasia of the temporal bone is uncommon. When it does occur, surgery may be necessary because of cholesteatoma or chronic infection behind bony obstruction of the external auditory canal. Cosmetic surgery may also be performed because of bony deformity. When a surgical procedure is undertaken for hearing improvement, special problems may be encountered. The case presented illustrates middle ear pathologic changes found with fibrous dysplasia and specific surgical difficulties.
