[Primary hypoglycemic mesenchymatous tumor of the pleura (Doege-Potter's syndrome) : a case report].

A patient had been operated 4 years ago for a hypoglycemic tumor of the pleura. The lesion could be separated from the lung, and histological examination demonstrated an undifferentiated markedly cellular appearance of the tumor. Ultrastructural features were not in favor of a mesothelial tumor. Prognosis in such cases depends upon the histological type of tumor present, but remains uncertain, particularly in some apparently morphologically benign forms. Th most valid hypothesis to explain the genesis of this syndrome involves the role played by the non-suppressible insulin-like activity of a substance secreted by the tumor. This is a glycoprotein, molecular weight 90 000, which has recently been isolated, and which could play an important role as a non-suppressible insulin-like protein.

[Treatment of postero-superior amoebic abscess of the liver. A plea for a return to the thoracic approach with exclusion of the pleura (author's transl)].

Right anterolateral thoracotomy with exclusion of the pleura was successfully carried out in 14 cases of amoebic liver abscess. This approach is indicated for abscesses located on the posterior and superior surfaces of the liver. The operation is easy and rapidly executed. Selective drainage can be installed without any risk of pleural or peritoneal contamination, and the post-operative period is smooth.

[A case of localized fibrous mesothelioma arising from visceral pleura of the right lower lobe].

A 56-year-old male who had been followed for chronic hepatitis had cough and hemosputum. Chest X-ray and CT films showed an abnormal mass shadow in the right thorax. On suspicion of intrathoracic tumor, resection was performed. The tumor, which was on the diaphragm without invasion, was pedunculated and arose from the bottom surface of visceral pleura of the right lower lobe. From the microscopic findings and immunohistochemical staining, the tumor was diagnosed as localized fibrous mesothelioma. Some localized fibrous mesothelioma exhibit as malignant tumor. Therefore, complete resection and intensive follow-up should be done.

Solitary Fibrous Tumor Pleura.

The Solitary Fibrous Tumour of Pleura has rarely been observed. A new case has been reported. This tumour is often asymptomatic and mostly discovered by chance. A well delimited and round opacity is demonstrated on the chest X-ray. computerized tomography confirms the fleshy nature of this tumour with inhomogeneous enhancement. The treatment is a complete surgical excision. The prognosis is excellent, if diagnosed early.

[Therapeutic plasmapheresis and extracorporeal low-flow blood oxygenation in the combined treatment of patients with acute suppurative-septic lesions of the lungs and pleura].

It was shown that plasmapheresis included in the complex program of intensive therapy of patients with acute pyo-septic lesions of the lungs and pleura allowed to effectively arrest manifestations of endotoxicosis and to improve results of treatment of this category of patients. Extracorporeal hemoxygenation of small volumes added to plasmapheresis at the expense of inclusion of dialyzer DIP-02-02 in the extracorporeal contour considerably reinforced the detoxicating effect of plasmapheresis at the expense of nonspecific biotransformation of toxins.

Localized fibrous tumor of the pleura. Report of two cases with immunohistochemical study.

The light microscopy and immunohistochemical findings of two new cases of localized fibrous tumor of the pleura are reported. Both cases remained asymptomatic. One of them was incidentally found in a 69 year-old male during a thoracotomy for adenocarcinoma of the lung. The other was discovered in a 51 year-old woman in the course of a routine chest X-ray. Bundles of tightly packed benign fibroblasts within a collagen-rich stroma were the histologic hallmark. Bizarre cells and prominent sclerosis were extensive features in the first case. Only vimentin showed diffuse positive immunostaining in proliferating cells. Our results support the fibroblastic origin proposed for this lesion.

[Malignant mesothelioma of the pleura with an unusual course].

An unusual case of malignant mesothelioma of the pleura in a man aged 73 years is described. Four years after debut of symptoms a tumour emerged at the site of a previous pleural drainage and grew during the next two years to the size of a man's head. After removal of the tumour, the histological diagnosis was reviewed and confirmed. Six and a half years after the first diagnosis, the patient is still alive and in a reasonable condition in spite of bilateral pleural involvement.

[Benign fibrous mesothelioma of the pleura].

Benign fibrous mesothelioma of the pleura is rare. The tumour may present in several ways, as shown in our two patients, one with an elevated diaphragm and the other with slowly growing intrathoracic tumour. The tumour may reach a considerable size, is localized, is normally benign, and is curable by surgical resection.

[Thymoma implants in the pleura as a variant of intrathoracic recurrence].

Extended thymectomy was carried out in 152 patients with various types of thymomas, 61 ones with noninvasive and 91 with invasive ones; in the latter case surgery was followed by chemo- and radiotherapy. Three patients with non-myasthenic invasive thymomas (NMIT) (2.0% of all patients or 10.7% of NMIT ones) developed thymoma implantations to the pleura in the course of 2 years postoperation. All these patients developed pleural or pulmonary invasion by the moment of surgery. Postoperative chemo- and radiotherapy did not prevent intrathoracic recurrence. One of the possible pathogenetic mechanisms of thymoma pleural implantation may be pleural seeding resultant from invasive tumor growth. Pleurisy associated with implantation is nonspecific and easily arrested.

[CT findings of diffuse pleural disease--alteration of pleura and extrapleural fat].

This is a comprehensive study on the usefulness of CT in the evaluation of diffuse pleural diseases in 114 patients. Alterations of the pleura and extrapleural fat were analyzed. The pleural changes were classified into five groups: no thickening (group 1), focal thickening (group 2), diffuse smooth thickening (group 3), diffuse nodular thickening less than 10 mm (group 4), diffuse nodular thickening more than 10 mm (group 5), CT findings of group 1 were mostly found in patients with transudatory effusion; group 2 in all patients with asbestosis; group 3 in those with empyema; and group 4 and 5 in those with malignant pleural diseases. In some cases with malignant pleural diseases and asbestosis, mediastinal and/or fissural thickening was present. Circumferential pleural thickening (pleural rind) was only seen in malignant pleural diseases. Pleural calcification was present in chronic empyema and asbestosis. Increased thickness of the extrapleural fat was seen in malignant pleural diseases and empyema, but homogeneous increased attenuation of the extrapleural fat might be a specific finding in acute empyema. The author concludes that CT is useful in the evaluation of pleural changes and alterations of extrapleural fat with diffuse pleural diseases, particularly in the pattern recognition based on CT classification.

[A case of recurrent breast carcinoma with metastatic foci in the lung and pleura treated successfully with biochemical modulation of 5-fluorouracil by low-dose cisplatin].

A 65-year-old female received nipple preserved total glandectomy for breast carcinoma. A 250 mg dose of epirubicin was administered as an adjuvant. After a three-year disease-free interval, breast carcinoma recurred in the lung and pleura. We then used biochemical modulation of 5-fluorouracil by low-dose cisplatin. One course of biochemical modulation resulted in eighty-three percent tumor regression and complete disappearance of the pleural effusion for over six months.

[New potentials for assessing the efficacy of extracorporeal blood correction methods in treating patients with acute suppurative destructive diseases of the lungs and pleura].

Integral mathematical analysis of changes in the peripheral blood formula was used in multiple-modality treatment of 19 patients with acute infectious destructions of the lungs and pleura. Extracorporeal methods of hemocorrection were used in the treatment. The leukocytic formula entropy, number of tension signs, degree of adaptation reaction, and leukocytic index of intoxication were assessed. Changes in the analyzed parameters were found to depend on the initial severity of patient's status, type of extracorporeal perfusion, and disease outcome. The authors came to a conclusion about the informative value of mathematical integral parameters which permit outlining the approaches to prediction of the disease outcome and plan individual treatment.

[A case of giant mesothelioma from visceral pleura which has feeding artery from pulmonary artery].

55-year-old woman was admitted to our hospital for cough. Chest X-ray films showed a giant tumor shadow in right anterior pleural cavity. Pulmonary arteriography showed a feeding artery from the branch of right superior trunk. We diagnosed the tumor might be mesothelioma from visceral pleura with stalk. We operated the patient and the tumor was turned out to be mesothelioma pathologically. The patient had a postoperative re-expansion pulmonary edema and recovered in a week. She lives well without recurrence for 3 years 8 months.

Diseases of the pleura.

In this review, three aspects of pleural disease are discussed. Although it was thought for many years that the origin of pleural fluid was the capillaries in the parietal or visceral pleura, recent evidence suggests that in many cases the origin of pleural fluid is the interstitial space of the lung. The interstitial space of the lung appears to be the source of the pleural fluid in patients who have congestive heart failure, parapneumonic effusions, pulmonary embolism, and lung transplants. The Hantavirus pulmonary syndrome is characterized by rapidly progressive, noncardiogenic pulmonary edema in relatively young, previously healthy individuals. The mortality rate with this syndrome is approximately 60%, and at autopsy most patients have large pleural effusions. Patients after lung transplantation frequently have profuse drainage from their chest tubes because most of the fluid that enters the lung must exit through the pleural space. The incidence of pleural effusion is very high in patients who have a complication of their lung transplantation, but the pleural fluid findings in patients after lung transplantation have not been well studied. Similarly, virtually all patients who undergo liver transplantation have a right-sided pleural effusion. The effusion usually reaches its maximum size around the third postoperative day. If the effusion increases in size after this time, serious complications should be suspected. The approach to pleural diseases has been altered with the advent of videothoracoscopy. Videothoracoscopy should be considered in patients who have undiagnosed pleural effusions and are not improving; in patients who have had recurrent pneumothorax, or a spontaneous pneumothorax with a persistent airleak or unexpanded lung; or in patients who have a traumatic hemothorax with clotted blood.

[Successful operation on giant solitary fibrous tumor of the pleura].

A 68-year-old man began complaining of dizziness and dyspnea on exertion 2 months ago. Chest X-ray and CT scan taken on admission revealed a giant tumor occupying almost all of the right thoracic cavity. The mediastinum, especially RA and IVC, was markedly oppressed and shifted by the tumor. MRI showed no invasive lesions of the tumor into adjacent organs. An operation was performed under the tentative diagnosis of solitary fibrous tumor (SFT) of the pleura by transcutaneous needle biopsy. The tumor was encapsulated and resected from the lower lobe of the right lung using a surgical stapler. Then the tumor was carefully dissected from the chest wall, pericardium and diaphragma, and removed completely. It weighed 3920 g and measured 22 x 20 x 20 cm. Various immunohistochemical properties of the specimen were morphologically compatible with SFT. The postoperative course was uneventful and the patient is now free of complaints. Because malignant cases of SFT have been reported a careful follow-up will be performed.

[Clinico-diagnostic role of ferritin and lactoferrin assays in benign and malignant affections of lungs and pleura].

Immunochemical tests of blood sera from 160 healthy residents of the town of Astrakhan were performed to determine standard quantities of ferritin and lactoferrin in the serum. Examinations of the saliva from 280 healthy subjects provided the frequency of occurrence and levels of excretory lactoferrin. These iron-containing proteins were also assessed in different biosubstrates (blood serum, sputum, saliva, pleural fluid, lung tissues) from 550 patients with benign and malignant lesions of the lungs and pleura. Tests for ferritin and lactoferrin proved useful in evaluation of bronchopulmonary inflammation activity, early detection of pulmonary and intrapleural supputation, differential diagnosis of chronic nonspecific pulmonary diseases and lung cancer. The excretory salivary lactoferrin provided sufficient information on the condition of adaptive mechanisms of local defense of the lungs in premorbid state in 420 workers of the gas-processing plant exposed to aggressive occupational pollutants.
