Mouth opening limitations following on soft tissue lesions are subdivided according to aetiology into collagen disease connectival lesions, cutaneous and mucosa lesions consequent on the action of physical agents and, finally, cicatricial lesions of the cheeks and lips as a result of surgery for cancer or trauma. The commonest forms of plastic surgery used to correct such lesions are reviewed.
The general principles and historical background of TNM classification was briefly discussed, with advantages as well as weakness and limitations of the system. The current TNM staging of lip and oral cavity, pharynx. The currant TNM staging of carcinoma in lip and oral cavity, pharynx, larynx, nasal cavity and paranasal sinuses and major salivary glands was presented, pointing out the last changes in the classification.
A canal was surgically created in the lower lip of male Sprague-Dawley rats and used as a reservoir for moist snuff. A total of 230 animals were randomized into six groups, five containing 40 rats and one containing 30 rats. After 2 wk of recuperation, the animals were treated as follows. Group I was initiated with 7,12-dimethylbenz(a)anthracene 3 times/wk for 4 wk followed by cotton pellet administration. Group II was initiated with 7,12-dimethylbenz(a)anthracene for 4 wk followed by snuff twice a day, 5 days/wk. Group III received snuff twice a day, 5 days/wk. Groups IV and V were initiated with 4-nitroquinoline N-oxide 3 days/wk for 4 wk. Thereafter Group IV received a cotton pellet, and Group V rats were treated with snuff twice a day, 5 days/wk. Group VI received a cotton pellet once a day, 5 days/wk. Treatment of all groups continued for a maximum of 104 wk. Group V rats had a significantly lower mean survival time than did the other groups because of the development of lip sarcomas in 66% of the rats as compared with 23% in Group II and 26% in Group III. One rat in each of Groups IV and VI developed lip sarcomas. The incidence of sarcomas in Group V as compared with the other groups is statistically significant (P less than 0.05 to 0.001). Spindle cell proliferation, a possible precursor lesion of lip sarcoma, was found in five rats of Group II, seven of Group III, and four of Group V. These results show that snuff has strong promoting capability with regard to the development of lip sarcomas after 4-nitroquinoline N-oxide initiation, but not after 7,12-dimethylbenz(a)anthracene initiation. Snuff by itself caused three squamous carcinomas of the palate, two squamous cell papillomas of the lip, and ten lip sarcomas (in 38 rats as compared with one lip sarcoma in 30 control rats), showing snuff to be carcinogenic for the lip and oral cavity.
A case report of epithelial carcinoma of the inferior lip which spread in cervical lymph nodes previously affected by a chronic lymphocitic leukaemia, is presented. The coexistence of both histological findings in some of the nodes (collision tumor) is remarked. We have made a review of the scanty literature about metastases on second tumors and compared and commented those features.
The authors describe rabdomyosarcoma of different location in two patients of different age. One patient, 51 years old, had rabdomyosarcoma located in the larynx. Its detection was late because its late manifestation. The patient refused the suggested treatment and the issue was lethal within the first year after the first symptoms had been discovered. The second patient was a child, aged 5, with rabdomyosarcoma on the upper lip. The tumour was detected relatively early and it was cured (operatively, by radiation and chemio-therapy). Unfortunately the result was lethal two years after the beginning of the illness.

We report 3 cases of head and neck malignant fibrous histiocytoma (MFH). The first case is a xanthomatous subtype of the inferior lip with lymph node metastasis. The two others cases are storiform-pleomorphic MFM of the hypopharynx and Killian area. A summary of actual knowledges about this entity is reviewed.
Children aged 15 years or less usually suffer from mesenchymal malignancies. Observations on four rare cases of juvenile epithelial malignancy in larynx, maxillary antrum, upper lip, and parotid are reported. No particular etiological factor could be found in these cases. Age, as such, had no effect on the clinical presentation of the malignancy, its management and prognosis.
A 58-year-old man is described with fibrillar neuroma of the lips, tongue, mucous membranes of the mouth, and larynx. Also tumors of the capillitium, the neck the trunk, and the arms were found. There were no clinical or blood chemical signs of pheochromocytoma, medullary carcinoma of the thyroid or medullated corneal nerve fibers. It is considered that there is no association with the multiple endocrine adenomatosis type IIb, or with the neurofibromatosis von Recklinghausen.
A canal in the lower lip to function as a reservoir for snuff was surgically created in 150 male Sprague-Dawley rats. The animals were randomized into five groups of 30 each: Group I received snuff twice a day, 5 days a wk; Group II was painted with propylene glycol (solvent control) on the hard palate 3 times a wk during 4 wk; Group III underwent painting on the hard palate with 4-nitroquinoline N-oxide (4-NQO) dissolved in propylene glycol, 3 times a wk for 4 wk; Group IV received 4-NQO as in Group III followed by snuff application as in Group I; and Group V received a cotton pellet dipped in saline twice a day, 5 days a wk. Treatment continued for up to 108 wk. There was no significant difference in mean survival time between the groups. Squamous cell tumors of the lip, oral and nasal cavities, esophagus, and forestomach were seen only in Groups I, III, and IV. Nine tumors of these organs were found in Group I (six carcinomas and three papillomas), nine in Group III (seven carcinomas and two papillomas), and ten in Group IV (eight carcinomas and two papillomas). The difference between each of these groups and the control groups (II and V) with regard to tumor incidence is statistically significant (P less than 0.05). In Group I, four oral cavity or lip carcinomas were found in 29 rats, a significant difference in relation to control rats (P less than 0.05). In addition, hyperplastic lesions of the lip, palate, and forestomach were significantly more common in Groups I and IV compared with Groups II, III, and V. The study has shown that snuff and 4-NQO by themselves have the potential to induce malignant tumors. Initiation with 4-NQO followed by snuff did not significantly enhance tumor formation.
BACKGROUND: Cancer rates of the head and neck are traditionally linked to public health issues. OBJECTIVE: To describe the epidemiology of head and neck cancer in the United States. DESIGN AND SETTING: National Cancer Institute's Surveillance Epidemiology and End Results (SEER) program. RESULTS: A total of 75,000 cases of head and neck cancer were diagnosed in 2001. Incidence is rising in thyroid (up 52%), bone (43%) soft tissues (20%), salivary (20%), tongue (16%), tonsil (12%), and nose (12%). Incidence is falling in lip (down 58%), hypopharynx (35%), cervical esophagus (32%), oropharyngeal mucosa (26%), and larynx (26%). There were 30,000 deaths from head and neck cancer in 2001. Mortality has decreased to some degree at all sites except thyroid where it was stable. CONCLUSION: Many head and neck cancers have changing incidence and mortality rates contrary to expected changes given trends in public health issues. Further investigation of risk factors, diagnostic practices, and management strategies is warranted. EBM RATING: 2C.
The relative frequency of histologically diagnosed cancer at the Chiang Mai Medical School in North Thailand in 1964-67 inclusive is examined. Most of the 1877 cancers seen are in Thais (males, 927; females, 908), the remainder arising in Chinese (males, 21; females, 12), Hill People (males, 6; females, 1) and others (females, 2). The cancers in Thais are presented by site, sex and 10-year age-group together with the relative frequency (crude and corrected for age).In Thais, the outstanding finding is the extraordinarily high frequency of cancer of the hypopharyngeal-laryngeal region in both sexes (males, 18.4 per cent; females, 3.4 per cent). This may be associated with the smoking of a local variety of the cigar called "keeyo" This cigar, smoked in the usual manner, contains approximately equal quantities of home-grown, sun-dried, Thai tobacco and the chopped bark of the "koi" tree (Streblus asper). In women, who also smoke "keeyo", the frequency of hypopharyngeal-laryngeal cancer is unusually high by occidental standards. There is no sex difference in the frequency of bronchial cancer (4 per cent). In males cancer of the penis, in second rank (6.6 per cent) is much more frequent than cancer of the prostate and testis combined. Stomach and skin cancers (ICD 191) are in third place (each 5.7 per cent). In females the most frequent cancers are cervix uteri (19.8 per cent), breast (8.8 per cent) and skin (ICD 191) (6.1 per cent). Cancers of the lip and skin of the head and neck are more frequent in females than in males. Choriocarcinoma is common (1.9 per cent) and there is a large number of vulvar cancers in young women (2.6 per cent). The geography, economy and medical facilities of Chiang Mai Province are described. It is considered that, although there is likely to be considerable under-reporting of internal cancers, the high frequency of hypopharyngeallaryngeal cancer is not due to selective bias.
