Cemented tungsten carbide pneumoconiosis.

An autopsy case of cemented tungsten carbide pneumoconiosis, the first lethal case in our country, is presented. A 28-year-old woman, who had been engaged in grinding presintered metallic matrix for four years, developed respiratory symptoms. X-ray examinations were indicative of interstitial pulmonary fibrosis. Corticosteroid therapy revealed only little effect. She expired five years after the onset of the symptoms. Postmortem examination showed nonspecific interstitial pneumonitis resulting in marked lung fibrosis. Ultrastructurally, crystals were observed in cytoplasm of presumable macrophages in the fibrotic lung tissue. Electron probe microanalysis of the lung tissue showed the presence of tungsten and other constituents of tungsten carbide except for cobalt. Metal analysis demonstrated a large amount of tungsten in the lung. Cobalt was detected tenfold of the normal value in the bone. In pathogenesis of the pneumoconiosis in the cemented tungsten carbide workers, toxicity of cobalt is most suspectable, and in addition, individual susceptibility may be also important.

A case of the calcium pyrophosphate dihydrate (CPPD) deposition disease without condylar destruction of the temporomandibular joint.

The case of calcium pyrophosphate dihydrate (CPPD) deposition disease, which occurred at a rare site in the temporomandibular joint (TMJ), is presented. A 48-year-old woman noted swelling in the preauricular area of her left cheek, which restricted her mouth opening. Radiological examination revealed a radiopaque lesion in the posterior and medial area of the left space of the TMJ. During the operation, the superior joint space was entirely filled with a solid and whitish-gray mass of chalky appearance. The lesion was partly attached to the retrodiscal tissue without any destructive change around the condyle and mandibular fossa. Histological examination revealed the foci of amorphous crystalline material to contain rhomboidal, rod and needle shaped crystals, which had cartilaginous tissue in the dense fibrous background admixed with chronic inflammatory cells and foreign body-type giant cells. The crystals were positive for von Kossa's stain and spectral peaks for phosphorus and calcium were evident by electron probe microanalysis, thus suggesting the presence of CPPD.

Cold-impregnated aluminium. A new source of nickel exposure.

A new technique for finishing anodized aluminium was introduced during the 1980s--cold impregnation with nickel. Nickel is available on the surface of cold-impregnated aluminium, as shown by the dimethylglyoxime test. Chemical analysis with EDXA showed that nickel was in the form of NiSO4. A case of work-related allergic contact dermatitis in an engraver with nickel allergy is reported. It transpired that the patient was exposed to nickel in connection with aluminium. It is concluded that cold-impregnated aluminium is a new source of nickel exposure, probably previously unknown to dermatologists.

Metallothionein expression and concentrations of copper and zinc are associated with tumor differentiation in hepatocellular carcinoma.

Metallothionein is the carrier protein of heavy metal ions, such as copper (Cu) and zinc (Zn). In this study, the relationships among immunohistochemical expression of metallothionein, concentrations of Cu and Zn, histological differentiation and proliferative activity of hepatocellular carcinoma were investigated in 51 cases. The concentrations of Cu and Zn in both tumor and non-tumor tissues were determined using electron probe microanalysis. Immunohistochemical expression of metallothionein in tumor tissues decreased with the degree of differentiation, whereas the number of hepatocytes positive for Ki-67 increased. Furthermore, the concentrations of Cu and Zn in tumor tissues decreased with the degree of histological differentiation in human hepatocellular carcinoma.

Total cytoplasmic calcium in relaxed and maximally contracted rabbit portal vein smooth muscle.

The concentration of total cytoplasmic Ca in vascular smooth muscle was measured by electron probe microanalysis of strips of rabbit portal anterior mesenteric vein that were rapidly frozen either when relaxed or during a maintained (30 min) maximal contraction stimulated with high K and noradrenaline. Strips were also frozen and analysed after incubation in Ca-free, high-Mg2+ solution. Probe diameters of 0.1-0.2 micron and 1.0-1.5 micron were used to measure, respectively, cytoplasmic and cellular (including stored) Ca. There was a highly significant increase (P less than 0.0005) in cytoplasmic Ca of 1.0 +/- 0.2 (S.D.) mmol Ca/kg dry wt. from 0.8 +/- 0.2 (S.E. of mean) mmol/kg dry wt. (n = 262 spectra, six animals) to 1.8 +/- 0.2 (S.E. of mean) mmol Ca/kg dry wt. (n = 296 spectra, six animals), during maximal contraction. This increase is greater than can be accounted for by Ca binding to calmodulin and to myosin, suggesting the presence of other Ca-binding proteins in smooth muscle. A small amount (0.4-0.6 mmol/kg dry wt.) of cytoplasmic Ca remained after incubation in Ca-free, high-Mg2+ EGTA solution. This tightly bound, cytoplasmic Ca is insufficient to account for the total amount of divalent cation known to be bound to F-actin. We conclude that Mg is the major inexchangeably bound cation in F-actin in smooth as in striated muscle. In the contracted muscles, the cellular Ca concentration, measured with the large probes that include Ca stored in the sarcoplasmic reticulum (s.r.), was 3.2 +/- 0.3 (S.E. of mean) mmol Ca/kg dry wt. (n = 93), significantly higher than the cytoplasmic Ca concentration measured with small probes. This value of cellular Ca is probably an underestimate, as the large-diameter probes did not cover all of the peripheral s.r. The cellular Ca (measured with large probes) was highest in the contracted and lower in the relaxed tissue, and was significantly reduced in the muscles incubated in Ca-free solution. In contracted muscle, cytoplasmic Mg significantly decreased and mitochondrial Mg increased. In 0 Ca, high-Mg2+ solution, the cytoplasmic Mg increased significantly. Mitochondrial Ca did not significantly change during a maintained contraction, but was significantly lower (0.0 +/- 0.2 (S.E. of mean) mmol Ca/kg dry wt.) after incubation in Ca-free, high-Mg2+ solution than in the relaxed tissue (1.6 +/- 0.2 mmol Ca/kg dry wt.) in normal Ca-containing solution.

Solid-state energy-dispersion spectrometer for electron-microprobe x-ray analysis.

Improved lithium-drifted silicon solid-state detectors allow detection and energy dispersion of x-rays of about 3 to 30 kiloelectron volts in the electron-microprobe x-ray analyzer. Energy resolution is sufficient to separate peaks of characteristic x-rays of elements adjacent in the periodic system at atomic number 20 and higher. The detected x-ray spectrum emitted from an unknown sample can be recorded with a multichannel analyzer in approximately 60 seconds.

Spin-labeling studies on the membrane of a facultative thermophilic bacillus.

Spin-labeling studies were conducted to elucidate the rigidity and viscosity of the membrane of a facultative thermophilic bacillus. By employing nitroxide stearate probes, it was observed that this fatty acid was incorporated into the membrane fraction. Electron spin resonance spectroscopy indicated that cells grown at 55 C contained a membrane which was more rigid and viscous than the membrane from cells grown at 37 C.

Aluminium and injection site reactions.

AIMS: To alert pathologists to the spectrum of histological appearances that may be seen in injection site reactions related to aluminium. METHODS: Four cases of injection site reaction were examined microscopically using routine staining with haematoxylin and eosin, electron microscopy and by electron probe microanalysis. RESULTS: As in previous reports, all four cases included collections of histiocytes which contained faint granular brownish refractile material within their cytoplasm; ultrastructural examination showed this to be aluminium. Two cases showed a prominent inflammatory reaction with numerous lymphoid follicles and a notable eosinophilic infiltrate. Two cases showed unusual features not described previously. In one, there was a sclerosing lipogranuloma-like reaction with unlined cystic spaces containing crystalline material. The other case presented as a large symptomatic subcutaneous swelling which microscopically showed diffuse and wide-spread involvement of the subcutis by a lymphoid infiltrate with prominent lymphoid follicles. CONCLUSIONS: This report highlights the changes encountered in aluminium injection site reactions and emphasises that the lesions have a wider range of histological appearances than described previously.

Barium granuloma of the transverse colon.

A case of barium sulphate granuloma of the transverse colon following gunshot wounds to the abdomen has been described. Scanning electron microscopy with electron probe microanalysis was used to confirm the presence of barium sulphate and the absence of lead or other elements related to the gunshot wounds.

Stretch induced accumulation of total Ca and Na in cytosol and nucleus: a comparison between cardiac trabeculae and isolated myocytes.

By means of electron probe microanalysis (EPMA), we quantified changes in total sodium [Na] and calcium [Ca] concentration owing to the following: (i) local axial stretch (LAS) of isolated rat myocytes and (ii) end-to-end stretch (ETES) of rat ventricular trabeculae. For LAS, the distance between patch pipette and a cell-attached stylus was increased by maximally 20%; this activated a nonselective cationic current I(SAC) of approximately -0.5 nA, which was blocked by streptomycin. Trabeculae were stretched end-to-end from 85% L(max) to L(max). Stretch increased cytosolic [Na](total) by 34% in isolated myocytes (p < 0.001) and by 43% in trabeculae (p < 0.001). The increment in nuclear [Na](total) was 21% in myocytes (p < 0.01) and 20% in trabeculae (p < 0.001). Stretch increased [Ca](total) in isolated myocytes, in both cytosol (from 0.63 +/- 0.09 to 1.09 +/- 0.20 mmol/L, p < 0.05) and nucleus (from 0.33 +/- 0.05 to 0.64 +/- 0.13 mmol/L, p < 0.05). In trabeculae, the stretch-induced increment of 51% in cytosolic [Ca](total) remained nonsignificant (p < 0.15). In the nucleus, [Ca](total) did not change. We interpret the difference of stretch on nuclear calcium in myocytes vs. trabeculae with the assumption that LAS, but not ETES, produces shear-stress components that translate the mechanical stimulus deeply into the cell where it may modulate [Ca](total) by signals independent of I(SAC).

Preparation of individual human diploid fibroblasts and study of ion transport.

A method for analyzing individual mammalian cells with electron probe microanalysis has been developed using human diploid fibroblasts. Cells were grown on the same support that is used for experimental manipulations and analysis. Steady-state cation and anion concentrations and kinetic processes during experimental perturbations could be measured on populations of less than 1,000 cells. Human diploid fibroblasts in normal tissue culture medium had the following intracellular concentrations (in mM): K, 168; Na, 25.0; Cl, 51.2; P, 84.1; S, 16.5; Ca, 6.04; and Mg, 10.0. The ratios of K to Na were equivalent when measured in the nuclear or cytoplasmic area of the cells. Serum in the incubation medium was found to increase the cellular effective permeability to Na by a factor of 2.5, while leaving the effective permeability to K unchanged. When returned to control medium after 7 h of incubation in K-free medium, the cells recovered normal K/Na in less than 1 h. In some experiments the coupling ratio of the ouabain-inhibitable cellular transport of Na to K was 3:2 and the ratio of Cl to K was 1:2. The sum of intracellular content (Na + K) (an estimate of cellular volume) did not change when the cells were placed in K-free medium and increased by less than 30% after ouabain treatment. After 5-7 h of ouabain treatment or of incubation in K-free medium, long after the intracellular K had been replaced by Na, the cellular chloride content had not changed significantly.

Distal site of action of parathyroid hormone on phosphate reabsorption.

The sites of inhibited phosphate transport following administration of bovine parathyroid hormone (bPTH) to thyroparathyroidectomized (TPTX) dogs were investigated. Phosphate reabsorption by the proximal and distal nephron was studied using recollection micropuncture, stop-flow methodology, and electron-probe microanalysis. Following bPTH, delivery of phosphate from the proximal tubule increased from 26 to 37% of the filtered load, P less than .01. Fractional phosphate excretion increased from 2.3 +/- 1.5 to 21.4 +/- 2.3%, P less than .001. The increased delivery of phosphate at the point of micropuncture in the proximal tubule accounted for approximately half of the phosphaturia. In six TPTX dogs, which were saline loaded, similiar increases in phosphate delivery from the proximal tubule from 27 +/- 1 to 36 +/- 2% of the filtered load resulted in a strikingly smaller phosphaturia, 5.1 +/- 1 to 9.8 +/- 2.4%, NS. In stop-flow experiments, phosphate concentratin ratios were slightly increased in the proximal nephron and markedly increased in the distal nephron following bPTH. It is concluded that parathyroid hormone markedly decreases phosphate transport in the distal nephron.

Structural and functional heterogeneity of mammalian nephrons.

Since the anatomical descriptions of Bowman showing differences between nephrons originating in the superficial and deep cortex, the concept of heterogeneity has been extended from identification of dissimilarities between nephrons to recognition of inhomogeneity within major portions of individual nephrons. We are now aware of functional correlates for the anatomical differences between nephrons, between analogous parts of different nephrons, and between the three portions of the proximal tubule and the three or more parts of the distal tubule. The implications of all of these differences for major renal processes, such as isosmotic fluid transport, salt balance, hypertension, urinary acidification, and the concentration or urine are now being defined. It seems likely that new conceptual and technical approaches, especially electron probe microanalysis, will add appreciably to defining the role of heterogeneity in these and other processes. Despite the increasing complexity of nephron heterogeneity, it is recommended that our basic nomenclature be retained and that new findings be incorporated into the schema set forth by Karl Peter. It would be very helpful if reports of investigations on single nephrons or segments of nephrons were to include diagrams delineating the structures on which the work was performed.

Regulation of ion content and cell volume in isolated rat renal IMCD cells under hypertonic conditions.

The adaptive responses of cell volume and intracellular inorganic electrolytes to hypertonicity were investigated in isolated inner medullary collecting duct (IMCD) cells in vitro. Intracellular water and element content were determined simultaneously by a rapid centrifugation technique and electron-probe microanalysis, respectively. In the absence of adenosine 3',5'-cyclic monophosphate (cAMP), cells isolated at 600 mosM (268 mM NaCl) shrank in 900 mosM buffer (418 mM NaCl) within 100 s to 79 +/- 2% of the control volume and showed no regulatory volume increase (RVI). K+ content decreased steadily during the first 5 min to 18% below control. K+ concentration initially increased 25 +/- 2% above control and was normalized after 5 min to 144 +/- 10 mM (n = 6). Na+ and Cl- content increased slowly during the 60-min incubation period; at this point intracellular Na+ and Cl- concentrations were 97 +/- 6 and 59 +/- 7% (n = 6) above control levels (75 +/- 12 mM Na+ and 143 +/- 29 mM Cl-), respectively. In the presence of 10(-4) M 8-bromo-cAMP (8-BrcAMP; used as a substitute for antidiuretic hormone), IMCD cells showed RVI within 5 min. K+ content initially dropped 18 +/- 2% below control and recovered during the next 4 min but was 12 +/- 4% below control after 60 min. K+ concentration showed similar changes. Na+ and Cl- content increased to a larger extent than in the absence of 8-BrcAMP to 95 +/- 10% (Na+) and 47 +/- 6% (Cl-) above control after 60 min, whereas Na+ and Cl- concentrations were the same as without 8-BrcAMP.(ABSTRACT TRUNCATED AT 250 WORDS)
