The histological and ultrastructural features of the epiphyseal plate in Morquio type A syndrome (mucopolysaccharidosis type IVA).

Tissue from the epiphyseal plate of a patient with Morquio type A syndrome (mucopolysaccharidosis type IVA) was studied by undecalcified histological and electron microscopical techniques. Cartilage cells in the plate were vacuolated to a variable degree. The orderly proliferating structure of the epiphyseal plate was absent and calcification was markedly reduced. The related bone tissue was morphologically normal although reduced in quantity.

Facs analysis of myeloid differentiation stages in epiphyseal bone marrow, adjacent to joints affected with rheumatoid arthritis.

To analyze the differentiation stages of myeloids statistically, we adopted a two-color FACS system and used appropriate monoclonal antibodies belonging to CD15, CD16 and CD11b. By using HL60 treated with DMSO or human bone marrow MNCs from patients with rheumatoid arthritis, it was proved that with this system, myeloids could be clearly separated according to differentiation stages. Furthermore, the number of myeloids at certain stages of differentiation in the epiphyseal bone marrow of patients with RA or OA was measured. Nine of 15 samples from RA patients showed immature and relatively mature myeloids, while none of the 8 OA samples did. When the proportions of myeloids in epiphyseal bone marrow MNCs were compared with the clinical features, disease subsets in RA and the degree of synovitis, seemed to be important factors for abnormal myelopoiesis.

Contribution of the cortex to epiphyseal strength. The upper tibia studied in cadavers.

Non-destructive measurements of compressive stiffness were carried out on 20 proximal tibial autopsy specimens. The tibial epiphyses were first loaded through a template covering all but the peripheral 2 mm of the subchondral resection surface, then through the whole resection surface, and finally, after removal of the peripheral shell. A slight increase of the stiffness coefficient resulted from peripheral contact. Stiffness increased significantly after removal of the shell, but several potential sources of systematic error in this part of the investigation raise questions as to the validity of this finding. The area-corrected stiffness showed a decrease as a result of peripheral contact; this result indicates that the peripheral rim of bone has a lower area-corrected stiffness than the central bone, a finding which is incompatible with the concept of a true cortical shell at the epiphyseal level.

Influence of manganese deficiency on the characteristics of proteoglycans of avian epiphyseal growth plate cartilage.

The need for manganese for normal skeletal development appears to be related to its role in proteoglycan biosynthesis. The purpose of this research was to characterize the proteoglycans synthesized under conditions of manganese deficiency. The proteoglycans were extracted from epiphyseal growth plate cartilage and the monomers separated by cesium chloride density gradient centrifugation followed by column chromatography. The proteoglycan monomers from normal cartilage contained primarily (92%) chondroitin sulfate side chains with keratan sulfate being a minor (8%) component. Manganese deficiency reduced the total amount of cartilage proteoglycans. Of the monomers present in deficient cartilage, the majority (75%) were similar to those found in normal cartilage. Cartilage from deficient chicks also contained a second monomer fraction (25%) characterized by a reduced carbohydrate content. Thus, in addition to a reduction in total proteoglycan content, manganese deficiency results in qualitative changes in the proteoglycans present in epiphyseal growth plate cartilage.

Temporal and spatial expressions of transforming growth factor-betas and their receptors in epiphyseal growth plate.

The activities of TGF-beta and its receptors in enchondral ossification have not yet been fully elucidated. The temporal and spatial expressions of TGF-beta and its receptors were examined immunohistochemically in epiphyseal growth plate of rats, in order to clarify the roles of TGF-beta and receptors in cartilage cells during enchondral ossification. TGF-beta1 and TGF-beta3 were expressed from 6 to 24 weeks in resting, proliferating, and maturing zone. TGF-beta2 was expressed at 6 week, but decreased during growth. The expression of TGF-betas in hypertrophic chondrocytes was weak. TbetaR-I was co-expressed with the ligand in resting, proliferating, and maturing zone throughout the development. The expressions of TbetaR-II was recognized at 6 weeks but decreased after that. This study suggests that TGF-betas play a major role in modulating endochondral ossification during growth in epiphyseal growth plate.

[Epiphyseal distal torsion of the femur in osteoarthritic knees. A computed tomography study of 75 knees with medial arthrosis].

PURPOSE OF THE STUDY: We used computed tomography (CT) to measure external torsion of the distal femur preoperatively in patients with osteoarthritic knees presenting genu varum in order to assess the correlation between femoral distal external torsion (FDET) and radiological or epidemiological data obtained during the general preoperative work-up. MATERIAL AND METHODS: Seventy-five knees were studied in 38 patients. Mean age was 70.4 years; there were 13 men and 25 women. The degree of external torsion of the distal femur was defined as the angle measured on the CT-scan between a line drawn tangent to the most posterior part of the condyles and the epidondylar line drawn from the lateral epicondyle to the most prominent point of the medial epicondyle. This angle was compared with the angle of the epiphyseal tibial varum measured on the anteroposterior radiograph and the overall deviation of the lower limb (HKA), and with the angle between the mechanical axis and the femoral shaft axis (HKS). We assessed the effect of gender and sex and looked for correlations between the FDET angle and measurements made on standard radiographs. Student's t test was used compare the FDET angle by sex and side. The alpha risk was set at 5%. RESULTS: The FDET angle measured a mean 5.36 +/- 1.87 degrees (0-9 degrees ). We did not find any correlation between the FDET angle and tibial epiphyseal varum or HKS angles. A weak statistical correlation between the FDET and the HKA angles had no clinical value. Finally, the FDET angle was not correlated with sex or side. DISCUSSION: Computed tomography provides reliable and reproducible measurements. Our work clearly demonstrated the wide interindividual variability of the FDET angle. We were unable to identify any measurement on standard radiographs allowing an indirect prediction of the FDET angle. CONCLUSION: Our findings suggest that measuring the FDET angle on preoperative CT-scans in candidates for total hip arthroplasty can be useful for adapting the angle of rotation for the femoral component to each patient.

Effects of two epiphyseal-stimulating procedures on local growth of long bones in rabbits.

OBJECTIVE: To study the effects of two epiphyseal stimulating procedures on local growth of long bone in rabbits. METHODS: Osteotomy was performed in the metaphysis near the proximal tibial epiphyseal plate and hemicircumferential periosteal excision was made on the proximal tibial epiphysis. Tibia roentgenography, tetracycline labelling, histological method and electron microscopy were used. RESULTS: The local stimulating effect following the hemicircumferential periosteal excision was more remarkable than the osteotomy. CONCLUSIONS: Periosteal excision is a better treatment for children's knee deformity.

Pseudoachondroplasia and multiple epiphyseal dysplasia: New etiologic developments.

Pseudoachondroplasia (PSACH) (OMIM#177170) and multiple epiphyseal dysplasia (MED) are separate but overlapping osteochondrodysplasias. PSACH is a dominantly inherited disorder characterized by short-limb short stature, loose joints, and early-onset osteoarthropathy. The diagnosis is based on characteristic clinical and radiographic findings. Only mutations in the cartilage oligomeric matrix protein (COMP) gene have been reported in PSACH, and all family studies have been consistent with linkage to the COMP locus on chromosome 19. Multiple epiphyseal dysplasia (MED) is a relatively mild chondrodysplasia but like PSACH, MED causes early-onset joint degeneration, particularly of the large weight-bearing joints. Given the clinical similarity between PSACH and MED, it was not surprising that the first MED locus identified was the COMP gene (EDM1). Mutations causing MED have now been identified in five other genes (COL9A1, COL9A2, COL9A3, DTDST, and MATN3), making MED one of the most genetically heterogeneous disorders. This article reviews the clinical features of PSACH and MED, the known mutations, and the pathogenetic effect of COMP mutations on the cartilage extracellular matrix.

Pre-cultivation in suspension obtained chondrocyte-enriched cultures from articular-epiphyseal cartilage in fetal rats.

To establish a reliable chondrocyte culture system is valuable for investigating the phenotypic properties or the drug effects on chondrocytes. In this study, we established a chondrocyte-enriched culture system by pre-cultivating cells isolated from fetal rat articular and epiphyseal cartilage in suspension prior to cultivating in monolayer. Our results showed that the cultures with 5 days of preculture in liquid suspension produced the highest chondrocyte representative matrix of sulfated glycosaminoglycan compared to those with 1 or 3 days in suspension. We also found that PGE2 (10 and 100 nM) effects on DNA synthesis showed biphasically in 1 day-suspension chondrocyte cultures, similar to those of fibroblast cultures, while PGE2 (1-100 nM) revealed suppressive effects on DNA synthesis in 3 day-suspension chondrocyte cultures, similar to those of osteoblast cultures. However, PGE2 (0.1-100 nM) had no significant effects on the DNA synthesis of 5 days-suspension chondrocyte cultures. These results suggest that the monolayer chondrocyte culture following 5 days of suspension may be a reliable method to exclude the contaminated cells and obtain the chondrocyte-enriched cultures. In this study, we also found that 24-hour treatment of PGE1 (100-1000 nM), but not PGE2 or PGF2 alpha, revealed inhibitory effects on DNA synthesis in chondrocyte-enriched cultures. These phenomena may be as markers to check the characteristics of chondrocyte cultures derived from rat articular and epiphyseal cartilage, and also provide the information for further investigation about chondrocytic functions.

[Transforming growth factor-beta can induce differentiation of hypertrophic chondrocytes into osteoblast-like cells in epiphyseal plate].

OBJECTIVE: To investigate the effect of transforming growth factor-beta (TGF-beta) on the differentiation of hypertrophic chondrocytes in epiphyseal plate in order to understand its role in the mechanism of bone and cartilage formation. METHODS: We observed the effect of TGF-beta in varied time and dosage on the differentiation of hypertrophic chondrocytes in embryonic chick femora which were cultured in serum-free medium, using histochemical staining to detect the expression of alkaline phosphatase (ALP) and immunohistochemical staining to detect the expression of type I collagen and fibronectin. RESULTS: TGF-beta enabled the chondrocytes to express type I collagen and fibronectin, and it enhanced the expression of ALP in hypertrophic zone. Time-effect and dosage-effect relationships were observed. CONCLUSION: TGF-beta can induce the differentiation of hypertrophic chondrocytes into osteoblast-like cells in epiphyseal plate of embryonic chick femora. The inducing effect might be closely related to time and dosage of TGF-beta.

Immunohistochemical localization of basal lamina components in the developing rat epiphyseal cartilage canals.

The purpose of this study was to investigate the relationship between the appearance of basal lamina components (Type IV collagen and laminin) and the development of cartilage canals. In the distal femoral epiphyses from developing rats, the distribution of basal lamina components in the cartilage canal was examined immunohistochemically. The formation of cartilage canals from the perichondrium was first observed on the fifth day after birth. By Day 8, a few cartilage canals penetrated the epiphyseal cartilage and considerably increased in size and length. By light microscopic immunohistochemistry, reticular structures stained with anti-Type IV collagen and antilaminin antibodies were observed in the cartilage canals. In the early development of cartilage canals, however, immunostaining by anti-Type IV collagen antibodies was weaker than that by antilaminin antibodies. In eight-day-old rats, the laminin-positive reticular structures were more densely colored and more widely distributed in the canal than the Type IV collagen-positive ones. Type IV collagen was found around the endothelial cells of the developing capillaries by electron microscopic immunohistochemistry. Laminin was observed in the cytoplasm of the mesenchymal fibroblastic cells and their pericellular matrix as well as in the capillary basal lamina. These immunohistochemical electron microscopic observations can explain the differences that are observed in Type IV collagen and laminin immunostaining patterns as cartilage canals develop. Laminin synthesized by the mesenchymal fibroblastic cells may promote the migration and the outgrowth of endothelial cells in the formation of cartilage canals.

Delayed treatment of a malreduced distal femoral epiphyseal plate fracture.

Fractures of the epiphyseal plate are considered rare when compared with the more prevalent injuries found in competitive sports, but the complications associated with this type of trauma are a major concern. The factors affecting the success or failure of healing include the severity of injury, patient age, and the type and expedience of treatment. This case study examines the clinical presentation and treatment of a 15-yr-old high school football player who sustained a displaced, distal femoral epiphyseal Salter II fracture. Primary treatment consisted of nonmanipulative, nonweight bearing knee immobilization. The treatment resulted in malunion, pain, decreased range of motion and physical deformity; therefore, the patient sought a second opinion. On physical exam, the displacement and rotational deformity of the fracture site were unacceptable. The fracture was treated 20 days post-injury via open reduction with internal fixation. On follow-up, the athlete demonstrated radiographic healing, normal physical exam, and no significant leg length discrepancy or deformity. The athlete successfully returned to full competitive sport activity.

[Effects of XW630 on the expression of type I collagen protein in the epiphyseal plates of mice].

OBJECTIVE: To identify the effect of XW630 on the expression of type I collagen in the epiphyseal plates of mice. METHODS: The immunohistochemical method was used to measure the change of type I collagen protein after being treated with XW630. RESULTS: With 10(-7) mol/L or 10(-7) mol/L of XW630, the area of positive stain cells in every zone increased significantly compared to the control group. A more significant increase of the area of positive cells in the proliferative zone and hypertrophic zone was observed in the XW630 group than in the estrone group at a concentration of 10(-7) mol/L. When the XW630 was decreased to 10(-9) mol/L, the increase of positive cells was only found in the hypertrophic zone. The area of positive cells increased with the concentration of XW630. CONCLUSION: The XW630 upregulates the dose-dependent expression of type I collagen protein in the resting zone, proliferative zone, and hypertrophic zone in the epiphyseal plates of mice in vitro.

Long-term progression of a case of trans-epiphyseal fracture-dislocation of the proximal epiphysis of the femur.

Epiphyseal detachment-dislocation of the proximal femur constitutes a traumatic lesion, the immediate resolution of which always requires surgical treatment. How early treatment is carried out is important in improving the prognosis of the lesions which, however, in most cases is poor. In our case, clinical examination made 6 years post-surgery shows satisfactory preservation of range of movement with no important subjective disorders and with a minimal amount of hypometria (1 cm) even if structural epiphyseal changes visualized on radiographic examination and CT scan predict progression towards early arthritic degeneration.

[Production of monoclonal antibody to chicken epiphyseal cartilage matrix vesicle metalloprotease to determine its localization].

Previous reports demonstrated that metalloprotease was isolated from matrix vesicles (MVs) of chicken epiphyseal cartilage. In this study, we produced the monoclonal antibody against MV metalloprotease (MVPase) and its location was studied by immunohistochemical technique. Isolated MVs were injected into Balb/c mice and spleen cells were collected for fusion with myeloma cells. A monoclonal hybrid cell producing antibody against MVPase was isolated. The immunoglobulin isotype was IgG1. On western blots, this antibody reacted with a single band protein of 33KDa which corresponded with purified MVPase. In the light immunohistochemical study, immuno reactions were observed in the mesenchymal cells of bone marrow at the lowest area in hypertrophic zone of the epiphyseal cartilage. Ultrastructural immunolocalization of protein A-gold particles revealing MVPase showed that MVs contained this enzyme, and in the calcifying area, gold particles were observed at the apatite needles of ruptured MVs. According to these observations, it is suggested that MVPase was already synthesized in undifferentiated marrow bone cells and this enzyme promotes the initial MV calcification and relates closely to the growth crystalline needles of apatite.

Growth retardation after experimental limb lengthening by epiphyseal distraction.

Growth retardation occurring as a result of limb lengthening by epiphyseal distraction in two animal species was quantified by using femur or tibia as the experimental bone. The growth plate subjected to distraction appeared radiolucent after the lengthening procedure but was less distinct and had reduced height as compared with the control. In seven of 22 animals, the growth plate of the lengthened bone appeared to close earlier than in the control. After lengthening, bilateral longitudinal growth was evaluated semiquantitatively from radiographs until growth had ceased or residual growth was negligible. Growth retardation was consistently experienced in all animals, with an average reduction in growth potential of each study ranging from 40 to 70%. The results show that epiphyseal distraction is a valid method of limb lengthening, but it appears to have a consistently harmful effect on the growth plate and should be used clinically only in patients close to maturity.

[X-ray diagnosis of multiple epiphyseal dysplasia (a report of 3 cases from the same family)].

This is a case report of 3 cases from the same family with multiple epiphyseal dysplasia. The disease is an uncommon congenital dominant hereditary disease of the bone characterized by multiple and symmetrical skeletal involvement with retardation of epiphyseal development, accompanied by irregularity and shortening of toes and fingers and yet still maintaining the normal proportion between the trunk and extremities. The intelligence is not affected. In the adult patients, sloping of distal tibial articular surface associated with degenerative changes of both hip joints is a reliable diagnostic sign. The aforementioned X-ray signs are helpful to distinguish this condition from other chondrodysplasia diseases.

[Morphologic and metabolic studies on the effect of ethane-1-hydroxy-1, 1-diphosphonate (EHDP) on the epiphyseal plate and metaphysis of the rat].

Ethane-1-hydroxy-1, 1-diphosphonate (EHDP) has been widely used in recent clinical and laboratory investigations. The author examined epiphyseal and metaphyseal morphology as well as cartilage matrix and calcium metabolism during EHDP administration and after drug discontinuation by 35S, 45Ca and 3H-thymidine autoradiographic study. The effect of low-dose EHDP was limited to osteoclast dysfunction of the metaphysis with resultant widening of trabeculae. High dose administration of EHDP induces a variety of disorders with a manifestation of marked widening of the epiphyseal growth plate. This process is probably derived from a reduction in matrix calcification that relates an inhibition of chondrocyte metabolism with a resultant decreased rate of chondroitin sulfate turnover. Furthermore, high dose EHDP causes a metaphysis which exhibits rickets+-like changes with little calcification and a decrease in the normal rate of osteoprogenitor cell differentiation to osteoblasts and osteocytes.
