Acute appendicitis in the elderly.

A prospective study was done on 104 patients more than 60 years old with appendicitis. The clinical features are by and large, similar to that of the younger patient. However, four patients presented atypically with abdominal distension with little or no pain. A statistically significant increase in appendiceal perforation is found in patients more than 70 years old and in patients with prolonged delay in operation. However, delay on the part of the surgeon contributed only slightly to the high appendiceal perforation as 85.6 per cent of patients who were operated upon within 24 hours of admission. Appendicitis in the elderly patient appeared to progress to perforation faster and perforation occurred in about 40 per cent of patients within 24 hours of onset of symptoms. Five patients died and 29 postoperative complications occurred in 24 more patients. An associated medical disease is linked to a statistically significant increase in mortality. Appendiceal perforation and a total delay of more than 37 hours are associated with a statistically significant increase in morbidity.

[Surgical treatment in acute occlusion of the large intestine caused by carcinoma].

Fifty four cases of acute large bowel obstruction were observed in an overall groups of 266 patients with colonic carcinoma. Pre-operative elements which seems to contra-indicate a primary radical surgery and advantages of the same procedure are considered. In the experience of the Authors, primary and delayed staged resection of the cancer seems to give the same survival rate at short and long term. Choice of surgery of obstructive large bowel carcinoma should be based pratically on general patient's conditions.

Obstruction of the appendix lumen in relation to pathogenesis of acute appendicitis.

The role of obstruction of the pathogenesis of acute appendicitis was studied by measuring the pressure in the appendix peroperatively in 33 patients undergoing appendectomy because of suspected acute appendicitis. The technique involved insertion of a fine needle via the apex into the lumen of the appendix and measurement of the hydrostatic pressure required to inject a saline solution. In all six patients with gangrenous appendix and in two patients with phlegmonous appendix there were signs of obstruction of the appendix lumen expressed as raised intraluminal pressure. In 19 patients found at operation to have phlegmonous appendix there were no signs of obstruction. The experimental data suggest that obstruction is not an important factor in the causation of acute appendicitis, but may develop as a result of the inflammatory process.

[Diagnosis and treatment of ceco-ileal reflux in children].

A cecoileal reflux detected in children under the age of 5 years may be related to the manifestations of relative immaturity. In older children experiencing continuous pain, irrigography must be performed to verify the diagnosis and the degree of the reflux. In a group of 111 patients, 51 with Degree 2 and 3 passive reflux underwent operation. Appendectomy was performed in which the purse-string suture created conditions for tightening the frenuli, which explains the effectiveness of interventions verified by late results.

[Appendico-vesical fistula secondary to acute appendicitis. Review of the literature].

Acute appendicitis, particularly pelvic, may cause a suppurative process that spontaneously drains to the bladder and an appendicovesical fistula may result. The pelvic appendix and the bladder may occasionally become fused in an inflammatory and necrotic focus, directly creating a fistula. A case of vesicointestinal fistula secondary to appendiceal pelvic peritonitis is described and the cases reported in the literature are reviewed.

[Endometriosis of the appendix. A case report].

The authors report a case of endometriosis of the appendix found by chance during the course of left adnexectomy owing to endometriotic cysts. Endometriosis of the appendix is usually associated with right ovarian endometriosis, which is sometimes only present at a microscopic level. A situation of acute endometriotic appendix is the result not only of the disintegration of the appendix muscles, as was thought in the past, but also recent catamenial hemorrhage within the endometriotic tissue.
