Malignant melanomas in the small intestine: a study of 103 patients.

OBJECTIVES: Malignant melanoma shows an unusual predilection to metastasize to the small intestine. A proportion of small bowel melanomas occur without history of an antecedent primary. We evaluated a group of patients with malignant melanoma in the small intestine to further our understanding of this disease. METHODS: We reviewed 103 cases of malignant melanoma in the small intestine (77 surgical resections and 26 autopsies) accessioned at the Armed Forces Institute of Pathology between 1945 and 1991 for demographic, chronological, and pathological features. RESULTS: Mean age at time of primary was 45.6 yr for surgical and 34.1 yr for autopsy cases (p = 0.01). Mean age at time of small intestinal involvement was 52.2 yr for surgical and 42.7 yr for autopsies (p = 0.03). Primary lesions preceded intestinal disease by an average of 5.6 yr for surgical and 2.1 yr for autopsies. The age distribution of surgical patients with and without known primary melanomas at the time of small intestinal melanoma was not significantly different. The same was true for autopsy patients. Using regression analysis, the linear relationship of age at primary melanoma (AAP) on age at small intestinal melanoma (AASI) was given by AAP = 2.30 + 0.85 (AASI), and that of AASI on AAP was given by AASI = 3.94 + 1.02 (AAP) (r = 0.93 and p < 0.0001 for both regressions). CONCLUSIONS: Our data and results support the concept that small bowel involvement by melanoma, even without a known primary, is most probably metastatic. The age at which an unknown primary occurred in cases of intestinal melanoma, or the age at which intestinal metastasis may appear in cases with known primary melanoma, can be estimated. There appear to be two subsets of primary melanoma: one that occurs among younger patients and is more aggressive with rapid metastasis and early death and one that occurs among older patients, is more indolent, and metastasizes less rapidly.

Renal tubular damage: an extraintestinal manifestation of chronic inflammatory bowel disease.

OBJECTIVE: To investigate whether treatment of inflammatory bowel disease (IBD) with 5-aminosalicylate or sulphasalazine in IBD may induce renal tubular damage. DESIGN AND METHODS: The urinary enzymes beta-N-acetyl-D-glucosaminidase ( beta-NAG), dipeptidylpeptidase 4 (DPP4) and alanine aminopeptidase (AAP) were measured as markers of renal tubular damage in 104 consecutive patients with Crohn's disease and in 43 consecutive patients with ulcerative colitis (all with normal serum creatinine values). Control values were gained from 65 healthy persons. RESULTS: The normal values (mean +/- SD) for the urinary enzymes investigated (U/g creatinine in the urine) were: DPP4 4.5 +/- 2.2, beta-NAG 1.6 +/- 1.4, AAP 11.4 +/- 6.5. In 28% of the patients with ulcerative colitis elevated beta-NAG levels of more than the mean + 2 x SD were measured. This pathological enzymuria was nearly exclusively found in patients with active disease (CAI > 6): DPP4 15.6 +/- 25.3, beta-NAG 8.3 +/- 10.1, AAP 24.7 +/- 50.1 (all three enzymes were significantly elevated). The highest values were measured in patients with active ulcerative colitis before start of therapy. Nineteen per cent of the patients with Crohn's disease had elevated beta-NAG levels of more than the mean + 2 x SD. There was no significant difference in enzymuria between patients with active (CDAI > 150) and patients with inactive Crohn's disease (CDAI < or = 150). DPP4 and AAP were normal in both groups. A correlation between the enzymuria and the cumulative doses of 5-aminosalicylic acid, sulphasalazine or prednisolone could not be found. The courses of enzymuria in three patients who presented with the first severe manifestation of IBD are described. They were treated with either corticosteroids and 5-aminosalicylic acid or corticosteroids and sulphasalazine. Before onset of therapy, very high urine enzyme values were measured. They almost normalized in the course of successful medical therapy despite increasing cumulative doses of 5-aminosalicylic acid or sulphasalazine. CONCLUSIONS: Renal tubular damage can frequently be observed in IBD. Our results suggest that this is an extraintestinal manifestation of the disease and not a toxic side-effect of anti-inflammatory therapy using 5-aminosalicylic acid or sulphasalazine.

[Aminopeptidases in the serum and urine of patients with hyperthyroidism].

In patients with hyperthyroidism the serum activities of the leucine aminopeptidase (LAP) and the alanine aminopeptidase (AAP) as well as the alanine aminopeptidase excretion in the urine were determined. A significantly increased activity of the leucine aminopeptidase in the serum and an increased excretion of alanine aminopeptidase in the urine were found. The AAP in the serum did not show a significant increase of activity. On account of the changes in the serum and in the urine before and during therapy a low-grade participation of the hepatobiliary and renal systems which are clinically not uppermost is to be assumed. Increased excretion of AAP and hyperthyroidism coincide nearly without exception in out patients. Correlation-statistical investigations make it probable to regard the increased excretion of AAP in the urine as an indirect parameter of the peripheral metabolism in hyperthyroidism.

[Ear acupuncture has a hypotonic effect on the gastrointestinal tract].

The effect of auricular acupuncture (AAP) on the motility and tone of gastrointestinal tract was investigated in 60 patients, by comparison of the width of corpus and antrum of the stomach, as well as duodenum before and after the application of AAP. The obtained results showed that the effects of AAP and usual drugs were equal. The fact that AAP was applied without any side effects and contraindications pointed to the possibility of its wider application in clinical practice.

International child health electives for pediatric residents.

BACKGROUND: International child health (ICH) electives can strengthen the skills and shape the values of pediatric residents. Much can be learned from the literature on ICH electives during medical school. Yet there is little published information regarding ICH electives during residency, nor do educational guidelines for such electives exist. OBJECTIVES: To describe existing ICH electives among pediatric residency programs and to develop guidelines for ICH electives during residency training. PARTICIPANTS AND METHODS: A survey of 248 pediatric residency programs in the United States, Canada, and Puerto Rico was conducted in November 1995. Consensus guidelines were developed by the executive committee of the American Academy of Pediatrics (AAP) Section on International Child Health. Consensus was achieved via full agreement among the 11 committee members. RESULTS: Survey response rate was 65%. International child health electives were offered by 25% of respondents. Most had no formal educational structure. An additional 42% of respondents indicated interest in ICH electives and requested more information. The AAP consensus guidelines for ICH electives focus on 4 principles: prerequisites, preceptorship, preparation, and evaluation. The guidelines are based on a conceptual framework that emphasizes reciprocity and continuity. CONCLUSIONS: While only 25% of pediatric residency programs currently offer ICH electives, many more express an interest in doing so. Educational structure for such electives is important and lacking. The AAP consensus guidelines provide a template for meaningful ICH experiences during pediatric residency. These guidelines may be applicable to other specialties as well.

Preferences of pregnant women and physicians for 2 strategies for prevention of early-onset group B streptococcal sepsis in neonates.

OBJECTIVES: To assess preferences of pregnant women, pediatricians, and obstetricians for the policies of the American College of Obstetrics and Gynecology (ACOG) and American Academy of Pediatrics (AAP) for reducing the incidence of neonatal group B streptococcal (GBS) sepsis. DESIGN: An interactive interview using a computer-based decision-making model (analytic hierarchy process) and a self-administered survey assessing the interview process. SETTING: An obstetric clinic at a university center and offices of practicing physicians from the Birmingham, Ala, area. PARTICIPANTS: Ninety-two pregnant women selected by a systematic sampling technique and 40 pediatricians and 40 obstetricians selected randomly. MAIN OUTCOME MEASURE: Ranking of the ACOG and AAP policies and the 5 criteria on which the decision was based: risk of infection to an infant, knowledge of maternal GBS status, risk of anaphylaxis to mother, diagnostic tests received by healthy infants, and cost. Satisfaction with the interview process also was measured. RESULTS: Eighty-three women (90%), 40 pediatricians, and 40 obstetricians (100%) provided responses suitable for analysis. Sixty-seven pregnant women (81%), 26 pediatricians (65%), and 6 obstetricians (15%) preferred the AAP strategy. The ACOG policy was the preferred strategy by 34 (85%) obstetricians. The 3 groups ranked risk of infection in an infant as the most important criterion in their decisions. Ranks for the other criteria differed among the 3 groups. Women ranked knowledge of maternal GBS status more important than did pediatricians and obstetricians. Thirty obstetricians (75%), 35 pediatricians (87.5%), and 72 pregnant women (86.7%) liked the interview. Seventy-three women (88%), 29 pediatricians (72.5%), and 17 obstetricians (42.5%) thought physicians should use this type of interview to assist in managing patients. CONCLUSIONS: Pregnant women, pediatricians, and obstetricians had different priorities when making a decision about GBS policies. These differences led obstetricians to prefer a different policy than that of pediatricians and pregnant women. Obstetricians were less likely to endorse the use of this decision-making technique in their practice than were patients and pediatricians.

Impact of recommendations to suspend the birth dose of hepatitis B virus vaccine.

CONTEXT: In July 1999, due to concerns about thimerosal content, the American Academy of Pediatrics (AAP) and the Public Health Service (PHS) recommended suspending hepatitis B virus (HBV) vaccination at birth except for mothers who had positive or unknown hepatitis B surface antigen (HBsAg) status. In September 1999, the Centers for Disease Control and Prevention recommended that hospitals resume HBV vaccination at birth with a new thimerosal-free vaccine. Whether the 2 changes in recommendations within 3 months led to less-than-optimal compliance in hospital nurseries is unknown. OBJECTIVE: To determine hospital HBV vaccination policy before the recommendation for delay of HBV vaccination and 1 year later. DESIGN, SETTING, AND PARTICIPANTS: Survey of all 46 hospitals with obstetric services and neonatal nurseries in Cook County, Illinois. MAIN OUTCOME MEASURES: Hepatitis B virus immunization practices before July 1999 and in August 2000; hospital factors associated with routine HBV immunization and compliance with AAP and PHS recommendations. RESULTS: Before July 1999, 74% of surveyed hospital nurseries offered HBV vaccine to all neonates; only 39% did so in August 2000. Being located in the Chicago city limits (88% vs 57%; P =.02) and having an academic affiliation (93% vs 66%; P =.05) were positively associated with routine neonatal immunization before July 1999. Both academic affiliation and city location were associated with routine immunization in August 2000 (71% vs 25% [P =.003] and 60% vs 14% [P =.002], respectively) and with compliance with recommendations for suspension (57% vs 25% [P =.03] and 56% vs 10% [P =.001]). CONCLUSIONS: We documented a 35% decrease in hospital nurseries that routinely offered HBV immunization 1 year after the AAP and PHS recommendations were made. Special efforts may be required to make at-birth administration of HBV vaccination universal.

Surface modification based on Si-O and Si-C sublayers and a series of N-substituted acrylamide top-layers for capillary electrophoresis.

Two approaches were used to prepare a series of surface-modified capillaries. In the first, a sublayer was formed by coupling gamma-methacryloxypropyltrimethoxysilane to the surface silanol groups forming an SI-O bond; a top layer was then formed by polymerizing acrylamide in the capillary, which reacted with the sublayer. In the second approach, a sublayer was formed by silanol chlorination, followed by Grignard coupling of vinylmagnesium bromide to form an Si-C bond at the surface; a top layer was formed by polymerizing either acrylamide (AA), dimethylacrylamide (DMA), N-acryloylaminoethoxyethanol (AAEE), or N-acryloylaminopropanol (AAP) onto the sublayer. The Si-Cpoly(AA) capillaries were more stable and produced an approximately 10-fold lower electroosmotic flow compared to the Si-O-poly(AA) capillaries. The Si-C sublayer was used to compare the performance of all four top layers. Electroosmotic flow decreased in the order: Si-O-poly(AA), Si-C-poly(AA), Si-Cpoly(AAEE), Si-C-poly(DMA), and Si-C-poly(AAP). Si-C-poly(AA) showed evidence of irreversible degradation at pH 9 already after 40-50 runs. Si-C-polyAAP-coated capillaries demonstrated superior efficiency and migration time reproducibility for a number of alkaline proteins and for fluorescently labeled ovalbumin. Excellent performance was maintained, in the case of poly(AAP), for a least 300 runs (of 30 min duration) at pH 9.0.

Localization of alanyl aminopeptidase and leucyl aminopeptidase in cells of Pseudomonas aeruginosa by application of different methods for periplasm release.

Various methods for the isolation of periplasm were examined and compared with regard to the complete release of known periplasmic marker enzymes and the contamination of the periplasm by cytosol for Pseudomonas aeruginosa PAO1 as a significant Gram-negative test strain. The aim of the investigations was to clarify the exact localization of alanyl aminopeptidase (AAP) and leucyl aminopeptidase (LAP) of this microorganism and to evaluate these methods. The osmotic shock of NOSSAL and HEPPEL (1996) was the most effective method with the lowest contamination by the cytosolic marker enzyme malic enzyme, but some proteins, which are located near the inner side of the cytoplasmic membrane, can be released additionally into the periplasm. All other procedures like chloroform or polymyxin treatment, the magnesium chloride washing of intact bacteria and spheroblasting by lysozyme in the presence of EDTA or magnesium chloride resulted only in a partial, sometimes only very low release of periplasm. The periplasmic enzymes are bound either more by hydrophobic or more by ionic interactions to the cell envelope and show a different behaviour with the different releasing agents. These methods are useful for a further differentiation between really periplasmic protein, and those proteins, which were false positive found in periplasm as a result of the osmotic shock. Our results show that AAP from Pseudomonas aeruginosa is a periplasmic enzyme with hydrophobic interactions to the cytoplasmic membrane, corresponding to the early results of LAZDUNSKI and MURGIER for Escherichia coli (LAZDUNSKI et al. 1975a and b, MURGIER et al. 1977), and LAP is cytosolic, but located near the cytoplasmic membrane. The AAP is not a real amphipatic membrane protein, as could be demonstrated by phase separation experiments with Triton X-114.

Chronic renal effects in three studies of men and women occupationally exposed to cadmium.

We measured sensitive indicators of renal damage in three different populations occupationally exposed to cadmium, and examined the degree of variation in damage and the relative sensitivity of different types of indicators. The three studies included (1) men exposed in a cadmium recovery plant, (2) men exposed in a nickel/cadmium battery plant, and (3) women exposed in the latter plant. The indicators of renal damage were urinary proteins in three categories: (1) the high molecular weight enzymes alanine aminopeptidase (AAP) and N-acetyl-beta-D-glucosaminidase (NAG), (2) the intermediate molecular weight protein albumin (ALB), and (3) the low molecular weight proteins retinol-binding protein (RBP) and beta 2-microglobulin (B2M). These tests indicate that exposed groups with higher urine cadmium levels had varying degrees of renal damage. All exposed groups showed evidence of renal damage when compared with their respective control groups. A higher percentage of elevated protein levels was noted in the exposed group of Study 1 than in the exposed groups of Studies 2 and 3. In Study 1, the means of all five protein levels and ALB, RBP, and B2M fractional clearances were significantly elevated in the group with higher urine cadmium concentrations when compared with the groups with lower urine cadmium concentrations. Highly significant dose-response relationships for all of the urinary protein tests, including fractional clearances, were found. All of the tests were more sensitive in detecting evidence of subclinical renal damage than serum creatinine, a commonly used indicator of renal function. The order of test sensitivity in men was determined by considering three factors: (1) the magnitude of the correlation coefficient between the test and the urine cadmium concentration in the study with the most advanced damage, (2) the relative cadmium level predicted by the dose-response model at which there is a 10% chance of observing an elevated test value, and (3) the ability of the tests to detect renal effects in the population with less advanced damage. The tests in order of decreasing sensitivity in men are ALB, AAP, NAG, RBP approximately B2M. The women with higher urine cadmium levels in Study 3 had a higher percentage of elevated AAP and NAG values when compared with the control group.

Aminopeptidases in seeds of picea abies (L.) Karst.: characterization of leucine aminopeptidase by molecular properties and inhibitors.

By either acrylamide or starch gel electrophoresis of Norway spruce (Picea abies) seed extracts, two prominent isoenzyme bands were obtained after staining for leucine aminopeptidase (LAP). These bands were proved to correspond to each other by reelectrophoresis in both gel media. Single endosperm studies with acrylamide gels showed clearly that, in addition to LAP, two bands are expressed after staining for alanine aminopeptidase (AAP) as well. Both the LAP and the AAP activities appeared together as a single peak between catalase and ferritin after gel chromatograhy on Sepharose. Isoelectric focusing in sucrose gradients proved the two LAP activities to have identical isoelectric points revealed that LAP, but not AAP, is detectable by standard starch gel electrophoretic procedures. The two LAP bands refer to approximate molecular weights of 71,000 and 131,000, respectively. Disaggregation studies did not conclusively determine whether these two bands represent different enzymes or not. only inhibitors succeeded in producing a definite differentiation by selective inhibition of one of the two bands. It is concluded that in both gel media the isoenzyme bands reflect the activity of two distinct leucine aminopeptidases.

Effect of aminoglycosides on proximal tubular membranes of the human kidney.

The effect of the aminoglycosides amikacin, gentamicin, netilmicin sisomicin and tobramycin on the proximal tubule of the human kidney was investigated in 78 healthy subjects. Fifteen adults were each given gentamicin, sisomicin or tobramycin 3 mg/kg bodyweight, 10 subjects received netilmicin 3 mg/kg or amikacin 15 mg/kg additionally seven subjects amikacin 10 mg and six subjects netilmicin 6 mg on three consecutive days. The principal enzyme of the brush border membrane of the proximal tubule, alanine aminopeptidase (AAP), was determined enzymatically and immunologically in 24-hour urines. The effects of the various aminoglycosides on the membranes were different. Less of membrane AAP was greatest after amikacin and was least after tobramycin. There was no difference between gentamicin, netilmicin, and sisomicin, which had an effect intermediate between the other two compounds. The elimination of AAP occurred at intervals which might possibly have been caused by impairment of cell cycles.

[On the differentiation of aminopeptidases in urine by a synthetic tripeptide (author's transl)].

In contrast to findings in healthy people two aminopeptidases with alanine-specifity were found in urine samples of patients after allogenic kidney transplantation. The two aminopeptidases revealed different enzymatic and immunologic properties: one alanine-aminopeptidase (AAP) could be specifically inhibited by a synthetic Thr-Phe-Pro-(tertiary butyl) tripeptide, but was not immunoreactive against an antibody, raised against plasma membranes of human kidney tubule. The tripeptide however was not capable to inhibit the second AAP which exhibited immunoreactive precipitins against the antibody. The appearance of the latter AAP had a close relation to tubular damage during graft rejection episode.

Urinary alanine aminopeptidase and beta 2-microglobulin as measurements of aminoglycoside-associated renal impairment.

The value of urinary alanine aminopeptidase (AAP) and urinary beta 2-microglobulin as predictors of aminoglycoside-associated nephrotoxicity was studied in 46 patients treated with gentamicin or tobramycin. In three patients serum creatinine increased by more than 50 mumol/l. Urinary AAP increased in virtually all patients. The degree of these increases could not be correlated to subsequent increases in serum creatinine. Increases in urinary beta 2-microglobulin were also seen in many patients who did not show subsequent increases in serum creatinine. Moreover, urinary beta 2-microglobulin was elevated before the onset of aminoglycoside treatment in many patients with septicaemia and malignant diseases, thus making an evaluation of antibiotic-induced changes impossible. These results indicate that neither urinary AAP nor urinary beta 2-microglobulin can be used to predict aminoglycoside-associated nephrotoxicity of clinical importance in individual patients.

Tobramycin nephrotoxicity: failure of cefotaxime to potentiate renal toxicity.

The aim of our prospective clinical study was to determine whether the combination of tobramycin plus cefotaxime is more nephrotoxic than tobramycin alone. The studies were carried out in 30 patients with serious infections and normal renal function. Groups of ten patients each received either 2 g cefotaxime or 1 mg/kg body weight tobramycin or cefotaxime and tobramycin in the same dosage every eight hours intravenously for at least seven days. Serum creatinine, creatinine clearance and alanine aminopeptidase (AAP) excretion in 24-hour urine were determined before, during and five days after the antibiotics had been discontinued. These were used as parameters for glomerulotubular injury. The plasma levels of tobramycin and cefotaxime (assayed by agar diffusion) did not differ when the drugs were given alone or in combination. None of the patients treated with cefotaxime alone showed any signs of renal damage. In contrast, tobramycin alone or in combination with cefotaxime caused an increase in urinary enzymes in all patients. This activity was a mean five to six times greater than the initial values prior to antibiotic therapy. There were no significant differences between the AAP increase during treatment with tobramycin and tobramycin plus cefotaxime. Four to five days after discontinuing antibiotic therapy, AAP activity decreased to values similar to those measured prior to therapy. In some of the patients receiving tobramycin alone or in combination with cefotaxime, increased creatinine levels, a reduction in creatinine clearance to 60 ml/min and an increase in renal enzyme excretion could be observed. Thus, treatment with high doses of cefotaxime does not seem to increase tobramycin nephrotoxicity in patients with normal renal function. The nephrotoxicity of this drug combination is obviously due to the aminoglycoside.

The Adelaide Activities Profile: a measure of the life-style activities of elderly people.

A new instrument for the measurement of life-style activities of the elderly is described. The Adelaide Activities Profile (AAP) was developed in response to the shortcomings of the Frenchay Activities Index. The AAP was validated on a random sample of 1799 people aged 70 years and over, living outside of institutional care. Principal components factor analysis revealed four consistent factors from which four scales were derived; these scales were named domestic chores, household maintenance, service to others, and social activities. Analyses demonstrated the construct validity of the four life-style scales by showing them to be differentially sensitive to a range of domestic, health and social circumstances. It was concluded that the distinct and meaningful clusters of activities represented by the AAP scales should have broad application as health and social indicators.

The accuracy of proxy ratings of life-style activities using the Adelaide Activities Profile.

The accuracy of proxy ratings with the Adelaide Activities Profile (AAP) has not been previously described. The current study sought to redress this situation by sampling spouses and adult children (the two most plausible classes of proxy), and by selecting healthy community dwelling people aged 65 years and over as index cases. The aim was to examine the level of agreement, and the nature of any disagreement, between index case and proxy AAP ratings. Data were obtained from 87 family units, of which 61 comprised husband, wife and adult child, while 26 consisted of husbands and wives only. Index-proxy agreement for the AAP scales was in general very satisfactory. Wives were more accurate proxies than their husbands, and both were more accurate than their children, although the latter were still adequate. Proxy accuracy was not further enhanced by age, sex, or frequency of contact with index cases.

Chronic administration of olanzapine induces metabolic and food intake alterations: a mouse model of the atypical antipsychotic-associated adverse effects.

RATIONALE: Most of atypical antipsychotics (AAPs) are highly related to a major risk of metabolic drawbacks leading to dyslipidemia and obesity. OBJECTIVE: To set up a mouse model of the AAP-associated weight gain in mice under the influence of chronic olanzapine regimen. MATERIALS AND METHODS: Female mice were housed in pairs and habituated to spontaneous feeding with a high-palatable diet (10% sucrose wet mash). Firstly, we orally administered olanzapine (0.75, 1.5 and 3 mg/kg), evaluating body weight and periuterine fat mass, as well as insulin, non-esterified fatty acids, triglycerides, and glucose levels. In a second experiment, we assessed the effect of olanzapine on energy expenditure through indirect calorimetry (IC). A third experiment was conducted to investigate the effects of olanzapine on a high fat-high sweet palatable diet (10% sucrose + 30% fat, HF-HS) in mice implanted with subcutaneous osmotic mini-pumps. Locomotor activity was also assessed. RESULTS: In experiment 1, the highest dose of chronically administered olanzapine (3 mg/kg) induced significant weight gain accompanied by augmentation of periuterine fat depots, with no changes in locomotor activity. In experiment 2, chronic administration did not alter energy expenditure, whereas, decreased respiratory quotient (RQ). In experiment 3, subcutaneously infused olanzapine evidenced a dose and time-dependent increase of body weight and HF-HS diet consumed. Notably, serum analyses revealed a hyperinsulinemia together with increased levels of triglycerides and glucose. CONCLUSIONS: In this study, we describe in female mice metabolic alterations matching the metabolic syndrome, thus resembling the clinical situation of schizophrenic patients taking AAPs.
