Hyperbaric oxygen therapy in hemorrhagic radiation cystitis: a report of 20 cases.

Radiation cystitis with macroscopic hematuria has been a frustrating clinical problem for urologists. Since 1985 hyperbaric oxygen (HBO) has been used to treat this disease, showing favorable results. Between November 1989 and October 1992, 20 female patients with hemorrhagic radiation cystitis were treated with HBO at a pressure of 2.5 atm abs, breathing 100% O2 for 100 min in our multiplace hyperbaric chamber. After an average of 44 HBO sessions, macroscopic hematuria was completely halted in 16 patients (80%) and markedly decreased in 2 patients (10%). Comparison of the cystoscopic findings before and after HBO showed a significant decrease in hemorrhagic sites and telangiectasis of the bladder mucosa. One patient had urinary frequency and urgency without hematuria during her hospital stay. After 30 sessions of HBO therapy, her symptoms subsided, and the cystoscopic findings were much improved. Only one patient failed to respond to HBO and underwent ileal conduit diversion. The mean follow-up period was 14 mo. (5-41 mo.). From our clinical results and cystoscopic findings, we suggest that HBO is an effective and safe treatment for hemorrhagic radiation cystitis.

[Squamous cell carcinoma of the face].

The main morphologic, diagnostic and therapeutic features of squamous cell carcinoma of the facial region are described. It occurs predominantly in exposed areas with preexisting actinic elastosis, keratosis, telangiectasies or fissurae. The primary lesion of squamous cell carcinoma more encountered is a firm erythematous nodule with poorly-defined margins and smooth, verrucous or papillomatous surface, frequently surrounded by satellite nodules and central ulceration. The indications of surgical excision, radiation therapy, and electrocoagulation or cryosurgery are discussed. Squamous cell carcinoma is more malignant than the basal cell variety and will metastatize to regional glands more rapidly.

[Pulmonary arteriovenous fistula: a case in childhood].

The authors describe a rare case of congenital, bilateral arteriovenous fistulas of the lung, without other symptoms of hemorrhagic telangiectasis. The disease was diagnosed in a ten-year-old child, because of a chronic hypossiemia state. The angiography with digitalized technique defined, at the best, the vascular anomalies; the examen cannot be substituted by other imaging methods. First a therapeutic embolisation in angiography was tried without success because of the dimension of the arteriovenous shunt. Then a local excisional therapy was carried out, on one lung. One year after the operation, the oxygenative and clinical conditions of the patient are excellent.

[Results of treatment in malignant epithelial conjunctival tumors].

Tumors of the conjunctiva are rare, especially malignant epithelial tumors. Although these tumors grow very slowly and only exceptionally with local invasion, intensive treatment is necessary. We studied the results of combined treatment with excision and brachytherapy with strontium-90 in 30 patients with carcinoma in situ and squamous cell carcinoma. After a follow-up between 3 months and 8 years (average 3 years) only one patient suffering from a squamous cell carcinoma developed a recurrence after 1 year. Two patients with squamous cell carcinoma developed telangiectasis, one patient developed glaucoma, and one patient developed a scleral ulcer.

[Clinico-genetic diagnosis of ataxia telangiectatica (Louis-Bar syndrome)].

We describe the possible difficulties in getting the diagnosis "ataxia telangiectasia" using the example of a 16 years old girl. If cases of cerebellar ataxia in childhood present without classical symptoms, the diagnosis of ataxia telangiectasia should not be excluded before chromosome analysis. In our case, first signs of cerebellar ataxia were observed from the age of 11 years and we found only mild, atypical located and late onset telangiectasis. Other signs of the syndrome, such as elevated alpha-fetoproteine and deficiency of IgA or IgE could not be detected. Chromosome analysis, however, demonstrated a breakage syndrome with chromosome 14 to 7 translocation and established a firm diagnosis of ataxia telangiectasia. Patients with chromosome breakage syndromes including the Louis-Bar-syndrome have an increased risk for malignomas. Therefore chromosome analysis should be undertaken in cases of children with cerebellar ataxia, and frequent radiological examination avoided.

Vaginal telangiectasias: unusual presentation of the Osler-Weber-Rendu syndrome.

BACKGROUND: The Osler-Weber-Rendu syndrome (hereditary hemorrhagic telangiectasia) is an autosomal dominant disorder of blood vessels associated with mucocutaneous telangiectasis and arteriovenous malformations of internal organs. CASE: A 23-year-old woman was found on routine gynecologic examination to have asymptomatic vaginal telangiectasias, a previously unreported initial site of involvement in this syndrome. CONCLUSION: The finding of asymptomatic vaginal telangiectasias on routine pelvic examination should alert the clinician to the possibility of the Osler-Weber-Rendu syndrome.

[Breast-preserving treatment of breast carcinoma: analysis of surgical, pathologic-anatomic and radiotherapeutic data of 291 cases].

291 breast cancer patients treated with conserving surgery and subsequent locoregional irradiation were studied. The method of surgery consisted of 200 lumpectomies, 76 quadrantectomies and 15 with either a wide excision or with an atypical resection. In at least 42.6% (124 out of 291), the tumor had been within 10 mm distance to the margin of resected specimen, in 13.4% (39 out of 291) the tumor even reached the border. In the other reports, a numerical description of the distance to the margins was not given. Postoperative radiotherapy was applied with 50 Gy cobalt-60/6-MeV photons to include the whole breast. A boost of tangentially opposed photon beams was added, replacing a technique with direct electron fields used in an earlier period during three years. The supraclavicular fossa received 46 Gy, the retrosternal pathway was covered with 45 to 50 Gy photons/electrons. Locoregional failure after an average observation time of 39.5 months was 4.5% (13 out of 291), within the target volume 3.8% (11 out of 291. The 39 cases on which the tumor reached the margin of resection, 92.3% (36 out of 39) remained free of recurrences. With a tumor-free resection line but a maximal distance of 10 mm, one recurrence (1.2% or 1 out of 85) was found. With the application of tangential photon beams to the tumor bed, which technically allowed sparing skin parts but also slightly reduced daily doses, the incidence of telangiectasis could be lowered to 2.5%. An anatomical was model was constructed to resemble a patient in the treatment position. Dosimeters were implanted and verified by computed tomography. Measurements of the radiation fields were performed within the breast, the lung and along the mammaria-interna chain. Minimal angle changes of 5 degrees decreased the retrosternal dose down with the amount of 56.5%, 10 degrees of 75% and 15 degrees of 85.7%. In contrast, moving the central axis medially by 1 cm and 2 cm increased the lung dose 2.2 and 4.4 times, respectively, whereas the dose within the breast remained almost equal. This emphasizes the importance of identical positioning, which must be realized 25 to 35 times throughout the course of therapy, often with different staff! Precision and consistent positioning are the main priorities. If these goals are achieved, lumpectomy with subsequent high-dose radiotherapy respecting cosmetics provides patients with excellent quality of life.

[Ambulatory therapy in varicose veins].

The treatment of varicose veins comprises conservative and active options. Every patient with varices has to be informed on the conservative modalities and should apply them in daily life. Compression therapy, as the most important part of the conservative treatment, should be considered individually for any patient according to the varicose-type, the grade of chronic venous insufficiency and the compliance of the patient. Active treatment modalities are clearly indicated in varices with complications such as trophic skin changes, varicophlebitis or when varices cause pain. But the cosmetic problem should not be underestimated. Morphologic and hemodynamic information obtained by noninvasive duplex technique allows the individualization of the surgical strategy for each patient. Besides surgical techniques used being less and less traumatic (invagination stripping, stab evulsion phlebectomy), more and more interventions are realized under ambulatory conditions in local anesthesia, even crossectomy with partial stripping of truncal varices. More important and complex operations, interventions involving more than one saphenous vein or reinterventions in recurrent varices are still performed under hospital conditions. They require only a short hospitalization time (2 to 4 days). Considering this very favourable evolution in surgery with a net trend to ambulatory, thus more economic treatment, the indications for sclerotherapy--a traditionally ambulatory modality with high recurrence-rate--are limited to reticular varices and telangiectasies.

Clinical and endoscopic features of hereditary hemorrhagic telangiectasia (Osler-Weber-Rendu disease) in India.

Hereditary hemorrhagic telangiectasis (HHT) is an autosomal dominant disease characterized by recurrent epistaxis and telangiectasia of the skin and mucous membranes. Most reports of HHT are from Europe and N. America. In this report of 7 patients from India we postulate that increased skin pigmentation in Asians and Negroids masks the cutaneous manifestations of the disease but without any discernible effect on mucosal lesions. The median hemoglobin value in these patients was 4 g/dl. Endoscopic lesions in the stomach or duodenum were detected in six patients. Most patients in our report (5/7) presented with a chronic iron deficiency anaemia. The treatment of HHT is mainly supportive although our preliminary data shows that low dose ethinyl estradiol therapy decreases transfusion requirement in these patients.

[Water-filtered infrared-A-hyperthermia combined with radiotherapy in advanced and recurrent tumors. Initial results of a multicenter phase I-II study].

BACKGROUND: Water-filtered infrared-A-radiation (IR/A-HT) can be used to heat superficial malignant tumors. A prospective multicenter phase I-II study was conducted to evaluate toxicity and efficacy of IR/A-HT combined with external beam radiotherapy (RT). PATIENTS, MATERIAL AND METHODS: From December 1991 to June 1994, a total of 53 patients with 58 malignant lesions were entered in the study. There were 14 primary, 36 recurrent and 8 metastatic tumors which were located in the head and neck region (14), chest wall (31), abdominal wall (2) and the extremities (11). The mean tumor volume was 100 cm3. IR/A-HT was applied 1 to 2 times per week with up to 3 IR/A-HT-radiators directly before or after external RT for 1 hour at 40.5 to 44 degrees C. Temperatures were controlled at various locations at the skin surface and invasively at depth. RESULTS: IR/A-HT was well tolerated: in 31 (53%) lesions acute (pain, pulse or blood pressure changes, increased skin reaction etc.) and in 25 (43%) chronic side-effects (atrophy, telangiectasis, fibrosis etc.) were noted; usually the toxicity was minor and temporary. At 3 months FU, 32 (55%) lesions achieved a local CR and 19 (35%) a PR; at 12 months FU, 25 (43%) had persistent CR; 16 patients (18 lesions) were deceased and 3 (4 lesions) not yet in FU. In univariate analysis the following prognostic factors for CR at 3 or 12 months FU were found: Karnofsky, metastatic status, tumor size, total RT-dose, thermal parameters T min(av) and T mean. For acute toxicity maximum temperature Tmax(av) was prognostically decisive. Significant differences were also found when considering the "quality of the HT-application". The microwave technique was superior to the infrared-A-HT-technique with regard to the penetration depth of energy deposition. CONCLUSIONS: Water-filtered infrared-A-radiation can be safely and effectively applied to heat localized superficial tumors (up to 1 cm depth). To increase the area of HT application multiple infrared-A-radiators have to be combined. A multi-element-system is in progress.

[Rendu-Osler-Weber disease. Report of a clinical case].

Rendu-Osler-Weber disease is an hereditary disorder characterized by cutaneo-mucous telangiectasis and vascular abnormalities in several organs. Bleeding, especially epistaxis, represents the most important clinical feature. Pulmonary arteriovenous fistulae can cause hypoxaemia, haemoptysis, polycythaemia and clubbing. Diagnosis is based on family and personal history, teleangiectasis, laboratory (haemochrome, fibrinogen, PT, PTT) and instrumental findings (endoscopy and/or roentgen). Therapy depends on symptoms. Embolization of pulmonary arteriovenous fistulae and laser treatment of intestinal vascular abnormalities have been successful. Danazol treatment yielded controversial results. We report the case of a patient admitted for arterial hypertension and recurrent epistaxis. Rendu-Osler-Weber disease diagnosis was made based on positivity at family and personal history, clinical examination, laboratory and instrumental findings. In conclusion we underline the pivotal role of anamnesis and clinical examination in the differential diagnosis of hereditary bleeding disorders and emphasize the importance of early diagnosis for the correct therapeutic approach.

[A pathological analysis of 180 cases of vascular malformation of brain].

180 cases of vascular malformation in brain (BVM) including 156 cases of brain biopsy and 24 autopsy cases were examined with routine pathological methods. The attempt of this study is to realize the pathological change and incidnce of each type and to find out the main cause of hemorrhage in BVM. According the morphological characteristics, 180 cases of BVM were classified into five types. They are: arteriovenous type 159 cases, venous type 3 cases, cavernous type 6 cases, capillary telangiectasis 1 case, and mixed type 11 cases. Hemorrhage was the most common clinical manifestation and main cause of death in BVM. The mechanism of hemorrhage may be due to either hemodynamic disturbance in local area or changes of the blood vessel wall. Besides these, there are five cases of BVM accompanied with brain tumor. They may be two independent diseases occurred in the same person. No relationship was found between the occurrence of these 2 entities.

Osler-Rendu-Weber with liver involvement.

Osler-Rendu-Weber disease, or hereditary hemorrhagic telangiectasia, is a vascular disease with autosomal dominant transmission. The liver is involved in 31% of patients with hereditary hemorrhagic telangiectasia (1). It appears that the lesions uncommonly causes serious disease. We report the imaging findings and the therapy in a case with extensive telangiectasis in the liver.
