Midtarsal dislocations in children.

Four children with midtarsal dislocations are described. Two of the four patients had a delay in diagnosis because of associated midtarsal injuries detracting from the main pathology. The key pathology was a dislocation or subluxation of the calcaneocuboid joint. The lateral radiograph was the most consistent view in making the diagnosis.

[Fractures and luxations of the mid- and forefoot].

These injuries are rare. Additional soft tissue lesions and multiple traumatic disorders of the foot are common, due to crush and traffic injuries. Surgery is needed for the reconstruction of the arch of the foot, its joints and soft tissue. The traumatic lesion of the soft tissue requires meticulous debridement, an exact reduction and osteosynthesis and primary wound closely by means of a synthetic material like Epigard. Providing reduction is adequate, it should be stabilised with percutaneous Kirschner wires.

Foot Injuries↗

[X-ray examination of foot injuries].

Fractures and luxations of the phalanges, metatarsus, tarsus and their joints are discussed. Correct radiological projections, knowledge of their pitfalls and a systematical interpretation are necessary for correct diagnosis.

[Treatment of chronic bursitis by a local cryogenic procedure].

Treatment of 92 patients with bursitis of different localizations by routine methods was followed by recidivations in 51.1% of the cases. Seventy patients were treated by cryodestruction. The follow-up observation of the patients during 1-2 years did not reveal any recidivations.

Adolescent hallux valgus: its etiology and surgical management.

Although most patients who seek relief are adults, the problem of hallux valgus frequently begins at an earlier age. In the juvenile patient, except for congenital anomalies or inflammatory juvenile rheumatic disease, hallux abducto valgus appears to be related to the biochemical malfunction. It requires not only surgical management, but also the use of orthotics or surgical stabilization.

Arthrocentesis.

Aspiration of the synovial joints is an important part of the diagnostic and therapeutic armamentarium of the physician and may provide vital information that can be obtained in no other way. As with any other technique in medicine, skill and safety in the aspiration of joints can be acquired only through careful study and continued practice in arthrocentesis. When appropriate preparations and precautions are observed, obtaining fluid from synovial joints is safe, relatively pain free, inexpensive, and extremely beneficial to the patient.
