Treatment for chronic depression using Cognitive Behavioral Analysis System of Psychotherapy (CBASP).

The article presents an overview of the chronic depression construct in the Diagnostic and Statistical Manual of Mental Disorders (DSMs) since 1980, including developmental etiology and psychopathological patterns of the chronically depressed adult. Then the Cognitive Behavioral Analysis System of Psychotherapy (CBASP), an empirically supported psychotherapy and the only therapy developed specifically to treat chronic depression, is described. CBASP addresses the idiosyncratic problems of the chronically depressed adult using Situational Analysis, the Interpersonal Discrimination Exercise, and Behavioral Skill Training/Rehearsal techniques. The first two techniques are illustrated using a verbatim case example.

[Analysis system of microsatellite polymorphism using automated laser fluorescent DNA sequencer].

Microsatellite polymorphisms are very important tool for linkage analysis. Now we can obtain some very fine linkage maps and many highly polymorphic microsatellite markers for linkage analysis. We developed a new efficient system for analysis of microsatellite polymorphism using automated laser fluorescent DNA sequencer. Ordinary PCR method with RI labeled primer has been used for analysis of microsatellite polymorphism, but our system mainly consists of FITC labeled primer and DNA autosequencer. There are some benefits in this system such as short analysis time, easy handling, high safety, and use of computer that enable to automatically detect allele size, save raw data, and connect to linkage analysis program on line.

[Anterior chamber dimensions in patients with angle-closure glaucoma measured by an anterior eye segment analysis system].

The corneal curvatures, iridocorneal angles, anterior chamber depths, and anterior chamber volumes in 26 (5 male and 21 female) patients with primary angle-closure glaucoma were measured using an anterior eye segment analysis system (EAS-1000, Nidek). The radius of corneal curvature was 7.65 +/- 0.25 mm (mean +/- standard deviation), significantly smaller than age-matched controls, and the iridocorneal angles ranged between 19 and 21 degrees. The anterior chamber depth and volume of angle-closure glaucoma eyes did not correlate with age; the depth averaged 1.90 +/- 0.41 mm, and the volume averaged 83.3 +/- 28.4 microliters.

[Physician-patient interactions: a comparison of analysis systems].

OBJECTIVE: The medical interview has important diagnostic and therapeutic functions and requires the integration of doctor-centred and patient-centred interviewing techniques to collect accurate and complete biopsychosocial data from the patient. Analysis of the interaction between patient and doctors which occur during the medical interview allow to evaluate physicians' interview techniques and to eventually improve them. OBJECTIVE: 1. To review different Interaction Analysis Systems (IAS) used to describe doctor-patient communication in terms of clinical relevance, observational strategy, reliability and behavioural and verbal contents. 2. To critically evaluate these IASs on the basis of their relevant research outcomes. METHOD: Previous reviews on interaction and keywords for Medline research (HealthGate) listed above were utilised to collect the relevant literature. RESULTS: Seventeen classification systems were identified and ten were discussed in a chronological order. Starting from a general sociological or psycholinguistic approach, the IASs over the years have became more specific and detailed, focusing more on the medical interview and on specific topics, such as cancer or hospital medical consultations. CONCLUSIONS: When studying interactions in general practice medicine, it is important to define the significant units of interaction which allow to identify a "patient-centred approach", since this is relevant not only for obtaining reliable and complete medical and social data, but also for the recognition of patients with emotional disorders and their correct diagnosis. Listening to the patient and facilitating the expression of emotions is an important aspect of patient education too, as patients learn that talking about psychological problems to their physician is appropriate and may be therapeutic.

Protein classification using comparative molecular interaction profile analysis system.

We recently introduced a new molecular description factor, interaction profile Factor (IPF) that is useful for evaluating molecular interactions. IPF is a data set of interaction energies calculated by the Comparative Molecular Interaction Profile Analysis system (CoMIPA). CoMIPA utilizes AutoDock 3.0 docking program, and the system has shown to be a powerful tool in clustering the interacting properties between small molecules and proteins. In this report, we describe the application of CoMIPA for protein clustering. A sample set of 15 proteins that share less than 20% homology and have no common functional motifs in primary structure were chosen. Using CoMIPA, we were able to cluster proteins that bound to the same small molecule. Other structural homology-based clustering programs such as PSI-BLAST or PFAM were unable to achieve the same classification. The results are striking because it is difficult to find any common features in the active sites of these proteins that share the same ligand. CoMIPA adds new dimensions for protein classification and has the potential to be a helpful tool in predicting and analyzing molecular interactions.

Nonlinear system analysis of renal autoregulation in normotensive and hypertensive rats.

We compared the dynamic characteristics in renal autoregulation of blood flow of normotensive Sprague-Dawley rats (SDR) and spontaneously hypertensive rats (SHR), using both linear and nonlinear systems analysis. Linear analysis yielded only limited information about the differences in dynamics between SDR and SHR. The predictive ability, as determined by normalized mean-square errors (NMSE), of a third-order Volterra model is better than for a linear model. This decrease in NMSE with a third-order model from that of a linear model is especially evident at frequencies below 0.2 Hz. Furthermore, NMSE are significantly higher in SHR than SDR, suggesting a more complex nonlinear system in SHR. The contribution of the third-order kernel in describing the dynamics of renal autoregulation in arterial blood pressure and blood flow was found to be important. Moreover, we have identified the presence of nonlinear interactions between the oscillatory components of the myogenic mechanism and tubuloglomerular feedback (TGF) at the level of whole kidney blood flow in SDR. An interaction between these two mechanisms had previously been revealed for SDR only at the single nephron level. However, nonlinear interactions between the myogenic and TGF mechanisms are not detected for SHR.

The effect of inhaled leukotriene D4, histamine, or antigen on central and peripheral airways of guinea pigs: analysis of bronchograms with an interactive image analysis system.

The effect on guinea pig airways of the inhalation of leukotrienes, histamine or antigen was investigated by measuring changes in lung volume and analyzing the airway area on bronchograms with the Zeiss interactive image analysis system (IBAS). LTC4, LTD4, LTE4 and histamine inhaled through an ultrasonic nebulizer caused inflation of the lung: LTD4 was the most potent of the leukotrienes and was 100 times more potent than histamine on a molar basis. The results of analyses of areas of large and small bronchi and bronchioles on the bronchograms indicated that LTD4 selectively decreased the area of the peripheral airways. Inhalation of an antigen in actively sensitized animals resulted in inflation of the lung and in a selective decrease in the area of the peripheral airways. Anaphylactic bronchoconstriction provoked by antigen inhalation was clearly inhibited by AA-861, a 5-lipoxygenase inhibitor but not significantly by mepyramine, an antihistamine. These observations indicate that LTD4 is a potent constrictor of the peripheral airways in guinea pigs and that the anaphylactic bronchoconstriction provoked by antigen inhalation could be mediated by LTD4 in actively sensitized guinea pigs.

Staining mast cells for morphometric evaluation on an image analysis system.

Multiple skin sections from three nonhuman primates (Macaca mulatta) and three hairless guinea pigs (Cavia porcellus) were stained with 12 different histologic stains to determine whether mast cells could be selectively stained for morphometric analysis using an image analysis system (IAS). Sections were first evaluated with routine light microscopy for mast cell granule staining and the intensity of background staining. Methylene blue-basic fuchsin and Unna's method for mast cells (polychrome methylene blue with differentiation in glycerin-ether) stained mast cell granules more intensely than background in both species. Toluidine blue-stained sections in the guinea pig yielded similar results. Staining of the nuclei of dermal connective tissue was enhanced with the methylene blue-basic fuchsin and toluidine blue stains. These two stains, along with the Unna's stain, were further evaluated on an IAS with and without various interference filters (400.5-700.5 nm wavelengths). In both the methylene blue-basic fuchsin and toluidine blue stained sections, mast cell granules and other cell nuclei were detected together by the IAS. The use of interference filters with these two stains did not distinguish mast cell granules from stained nuclei. Unna's stain was the best of the 12 stains evaluated because mast cell granule staining was strong and background staining was faint. This contrast was further enhanced by interference filters (500.5-539.5 nm) and allowed morphometric measurements of mast cells to be taken on the IAS without background interference.

Ultrasonic motion analysis system--measurement of temporal and spatial gait parameters.

The duration of stance and swing phase and step and stride length are important parameters in human gait. In this technical note a low-cost ultrasonic motion analysis system is described that is capable of measuring these temporal and spatial parameters while subjects walk on the floor. By using the propagation delay of sound when transmitted in air, this system is able to record the position of the subjects' feet. A small ultrasonic receiver is attached to both shoes of the subject while a transmitter is placed stationary on the floor. Four healthy subjects were used to test the device. Subtracting positions of the foot with zero velocity yielded step and stride length. The duration of stance and swing phase was calculated from heel-strike and toe-off. Comparison with data obtained from foot contact switches showed that applying two relative thresholds to the speed graph of the foot could reliably generate heel-strike and toe-off. Although the device is tested on healthy subjects in this study, it promises to be extremely valuable in examining pathological gait. When gait is asymmetrical, walking speed is not constant or when patients do not completely lift their feet, most existing devices will fail to correctly assess the proper gait parameters. Our device does not have this shortcoming and it will accurately demonstrate asymmetries and variations in the patient's gait. As an example, the recording of a left hemiplegic patient is presented in the discussion.

Inclusion counter: a public domain image analysis system for measurement of Chlamydiaceae growth in vitro.

The traditional method of measuring chlamydial growth in vitro, counting Chlamydiaceae inclusions by eye, is time-consuming and error prone. This paper describes a novel automated image analysis system suitable for high-throughput screening of novel anti-Chlamydiaceae compounds. The software, Inclusion Counter v3.0, is freely available in the public domain (http://www.image-analysis.co.uk).

[Effect of photocoagulation on retinal mean circulation time in proliferative diabetic retinopathy by use of fluorescein video angiography and image analysis system].

The effect of panretinal photocoagulation (PRP) on mean circulation time (MCT) was investigated in a main nasal superior artery and vein of 13 eyes with proliferative diabetic retinopathy, using fluorescein video angiography and an image analysis system. MCT before photocoagulation averaged 5.67 +/- 1.40 seconds. Two weeks after PRP, MCT averaged 4.29 +/- 0.89 sec., one month after, 4.29 +/- 0.88 sec., three months after, 4.03 +/- 0.86 sec., and six months after, 3.61 +/- 0.31 sec. The MCTs after PRP were shorter than MCT before PRP (p less than 0.01). These results suggest that MCTs in proliferative diabetic retinopathy shortened soon after PRP.

Quantitative changes in macrophage distribution in normal mouse ovary over the course of the estrous cycle examined with an image analysis system.

PROBLEM: The current study considered the distribution of macrophages within the major ovarian structures throughout the estrous cycle. METHODS: Immunohistochemical analyses were carried out using an avidin-biotin-peroxidase staining method and the rat anti-mouse macrophage monoclonal antibody anti-Mac-l was applied to stain macrophages. A computer-assisted image analysis system was used to quantify and compare the distribution of macrophages within individual ovarian structures during the estrous cycle. The following morphological structures were analyzed: primordial, preantral, antral, pre-Graafian, and atretic follicles; first-, second-, and third-generation corpora lutea; and the interstitium. The analysis included follicular and corpus luteum substructures: theca, granulosa cells, and interstitium. The system allows the estimation of macrophage distribution as a macrophage density per microns2 of the defined area. RESULTS: Primordial and preantral follicles did not contain macrophages during all stages of the estrous cycle. In antral, pre-graafian, and graafian follicles, macrophages were located and quantified only in the theca and were not detected in the granulosa cell layer. In contrast, atretic follicles showed macrophage localization in both thecal and granulosa cell layers. Macrophages were present in small numbers in the granulosa luteal cell layer and in high numbers in the thecal layer of newly developing corpora lutea. In the second generation of corpus luteum, macrophages followed the same pattern of distribution, while old corpora lutea contained significantly higher numbers of macrophages in both thecal and luteal cell layers. Surprisingly, significant quantitative changes in the macrophages distribution were detected over the course of the estrous cycle. Macrophage density was significantly higher in proestrus and metestrus when compared with the density in diestrus and estrus in most of the studied substructures with the exception of atretic follicles. Atretic follicles showed high macrophage density throughout the cycle with a two-fold higher density at metestrus. CONCLUSION: Macrophages were present in the mouse ovary over the course of the estrous cycle. The greatest numbers of macrophages appearing in corpora lutea and in atretic follicles suggest a role for macrophages in corpus luteum differentiation and follicular atresia. Their patterns of distribution at proestrus and metestrus within microenvironmental compartments suggests a functional correlation with the events of ovarian development.

[Application and evaluation of multiple exposure photography and motion analysis system in gait measurement].

Accurate techniques to evaluate and quantitate gait patterns have been available in clinical applications for the last 20 years. In this study, by employing multiple exposure photography, the walking velocity, stride length, step cadence and joint angle of the lower limbs were recorded for normal young men during the gait cycle. Data were compared with results from the motion analysis system and related researches. The results showed that multiple exposure photography can also provide quantitative analysis of several important time-distance parameters in the gait cycle.

Validity and reliability of measurements taken by the Peak 5 motion analysis system.

Many new measurement tools are becoming available for physiotherapists; however, there is often a lack of data to support their validity and reliability. This study attempts to address this issue with respect to the Peak 5 video analysis system. Five experiments were conducted. Experiment A investigated the validity of static angular measurements by filming a universal goniometer. A high level of agreement was recorded with a mean difference between the two instruments of 0.2 degree. Experiment B investigated the validity of angular joint velocity by filming a Biodex isokinetic dynamometer. A high level of agreement was recorded with a mean difference between the two instruments of 0.96 degree s-1. Experiment C investigated the reliability of reflective marker placements on the hip, knee and ankle on 17 healthy volunteers, in weight bearing on three separate days. Reliability was high with the 95% confidence interval (CI) for marker placement being 1.5 cm. Experiment D investigated the normal intrasubject variation of knee joint 'critical angle' and angular velocity measures over time, by filming 17 healthy volunteers performing an eccentric step test on three separate days. For 'critical angle' measures the standard deviation was 5.72 degrees and the 95% CI was +/- 11.44 degrees. For angular velocity measures the standard deviation was 8.51 degrees s-1 and the 95% CI was +/- 17.02 degrees s-1. Experiment E repeated analysis of the reflective marker placement (mean standard deviation 0.6 cm), the 'critical angle' (mean standard deviation 1.32 degrees) and the angular velocity (mean standard deviation 1.29 degrees s-1 showed high consistency, when analysing three pieces of film three times. Data generated in this study suggest that the Peak 5 provides valid angular and angular velocity data when compared to other measuring devices. Reliable data are produced when filming healthy volunteers performing an eccentric step test. These data will be used in the next stage of the project investigating Anterior Knee Pain Patients.

[Studies with the fully automated EEG sleep analysis system QUISI].

To judge sleep reliably would be interesting not only for outpatient sleep diagnostics, but also in the inpatient sphere. A step in this direction is the easy-to-apply fully automated sleep-EEG-analysis system QUISI. Nine males patients with obstructive sleep apnoea syndrome were examined simultaneously by polysomnography and the QUISI system. A comparison of the sleep profiles evaluated by polysomnographic recordings using Rechtschaffen and Kales' rules with the automated classification using QUISI recording shows that the difference of the sleep parameters sleep period time (SPT), total sleep time (TST), REM, stage 1, stage 2, slow-wave sleep and the awake stage of maximum five patients ranged between +/- 10 minutes, respectively +/- 5 per cent. Only two of nine sleep profiles could be compared as far as the sequence of the sleep stages was concerned. According to our results it is important to edit the original data and to treat the fully automated sleep profile. We cannot recommend QUISI to establish sleep profiles for the obstructive sleep apnoea syndrome.
