BACKGROUND: This retrospective study investigated three very similar cases of bilateral lower leg amputation. The aim was to determine which of two therapeutical procedures is associated with better long-term outcome: replantation or primary treatment of the stumps and subsequent prosthetic replacement. METHODS: Evaluation included clinical examination, gait analysis, and a workup of the psychosocial background. Health problems were documented using the Nottingham Health Profile. Follow-up assessments were performed 6, 7, and 18 years after the trauma. RESULTS: One patient underwent successful bilateral lower leg replantation and continued to work for the same employer. Two patients underwent prosthetic replacement. One became a social outcast confined to a wheelchair. The other patient had a good psychosocial background, similar to that of the patient who underwent replantation. He showed a better gait analysis on even ground than the replantation patient, but the findings were vice versa for uneven ground. CONCLUSIONS: The decision between replantation and prosthetic replacement after bilateral lower leg amputation is case related and cannot be generalized. Patients who have undergone these procedures require long-term psychological and physiotherapeutic care to achieve a good long-term surgical outcome.

OBJECTIVE: To show that a simple, easily learned and relatively inexpensive method of reversal of sterilisation provides good results. DESIGN: Retrospective observational study. SETTING: An NHS Trust Hospital. SAMPLE: Sixty-seven women aged 19 to 39 years who underwent reversal of sterilisation between January 1984 and December 1993. MAIN OUTCOME MEASURES: Pregnancy rate following reversal; rate of ectopic pregnancy. RESULTS: Thirty-four women (51%) had intrauterine pregnancies after reversal of sterilisation; intrauterine pregnancy rate in women who had been sterilised with rings or clips was 64%. Five women (7.5%) had ectopic pregnancies. CONCLUSION: This technique provides results that compare favourably with those of microsurgery. It offers hope to couples who do not meet the restrictive criteria for microsurgery and in vitro fertilisation and should appeal to purchasing authorities.

Levels of insect infestation, insect spatial distribution, and the relationship between the number of insect-damaged kernels (IDK) and the number of insects present in grain samples in three-hopper railcars transporting wheat from country elevators to a mill were studied. Six of eight sampled railcars were infested with more than two species of insects. The most abundant species collected were the lesser grain borer, Rhyzopertha dominica (F.), and rusty grain beetle, Cryptolestes ferrugineus (Stephens), with the larval stage of the two species being the most prevalent (>90%). The spatial distributions of these two species within the grain mass were typically clumped in railcar compartments containing >0.4 insect/2.75-kg sample of wheat, and these foci of high-infestation levels varied in compartments within the railcars and among the sampled railcars. There were no significant correlations between IDK and insect density for any of the different stage-specific insect populations that were collected in the grain samples. Mean numbers of immatures and IDK differed among railcars and compartments within railcars, but not among grain depths. Number of insects in the first discharge sample was not correlated with mean numbers of insects in the entire compartment. This indicates that each compartment of a railcar should be sampled to determine level of insect infestation but that sampling at different depths within a compartment is less important.
A group of 132 engineers from the north of Sweden was included in the study. A subsample of about 50 subjects was selected for further laboratory investigations during a light and warm summer period and a dark and cold winter period. The mean for the hours of sleep noted on the sleep records was significantly lower for night work than for day work. The amount of sleep during night work was significantly less during the light period than during the dark period as was the amount of sleep during the day off. Body temperature measured during work followed a daytime pattern and had a low amplitude. Potassium excretion and the blood levels of cortisol displayed a stable circadian rhythm with a daytime pattern. Many environmental factors made the results of catecholamine data difficult to interpret. The frequency of reported peptic ulcers was higher in the engineer group than in some other groups.
