Genealogy and geographical distribution of CFTR mutations in Saguenay Lac-Saint-Jean (Quebec, Canada).

Saguenay Lac-Saint-Jean (SLSJ), a region located in northeastern Quebec, has a high incidence of cystic fibrosis (CF). During the past few years the majority of the CF patients have been genotyped. The geographical distribution of the birth places of the patients and obligate carriers of the 621 + 1G-->T, the A455E and the delta F508 mutations (which accounted for 89% of the CF chromosomes) showed differences that can be explained by some degree of isolation but also by differential migration. The mean inbreeding and kinship coefficients were higher among the various CFTR mutation groups than in the general population. An ancestor couple common to most of the A455E carriers was identified. These data further substantiate the role of founder effect in the CF population of SLSJ.

Food habits of French Canadians in Montreal, Quebec.

OBJECTIVES: Since the 1960's, marked sociocultural and economic changes have affected lifestyle, religious practices, and family structure among French Canadians in Quebec. Recent nutritional assessment was unavailable, despite indications of change. METHODS: A survey was carried out in 1988 in a representative sample living in Greater Montreal, to obtain current data on food habits, nutrient intakes and sociodemographic factors, using interviewer-administered questionnaires and seven-day food records. RESULTS: Some 845 families (1450 individuals from different age groups) were studied in two phases: summer/fall and winter/spring. Mean household size in the study population was 2.7. Among adults, extremes of educational level were observed, with 33% having elementary school only and 28% having completed university. On average, each household spent $CAN 89.90 a week for food, $26.00 for tobacco, and $13.50 for alcohol. Breakfast was eaten regularly by 90% of subjects while 96% ate lunch and 99% ate dinner. Morning snacks were consumed by 36% and afternoon snacks were taken by 50%. Milk was consumed with breakfast by 24% of respondents, with lunch by 19%, and with dinner by 24%. On weekdays, 81% of subjects ate their main meals at home, while on weekends this figure was 95%. Time spent for meals varied by meal, and was slightly longer on weekends. Specialty diets, including vegetarianism, were followed by 7% of the study subjects, while 22% adhered to health-related diets. CONCLUSIONS: The food record analyses revealed adequate nutrient intakes overall in relation to the 1990 Canadian Recommended Nutrient Intakes, although further investigation is needed before addressing dietary quality in specific age-sex groups. Protein comprised 16% of energy, fat 38%, and carbohydrates composed 45% or 46% for males and females, respectively. Subsequent analyses will evaluate nutrient intakes in relation to health and sociodemographic indicators in this population.

Development of the Quebec User Evaluation of Satisfaction with assistive Technology (QUEST).

This study's purpose was to develop a clinical instrument designed to evaluate user satisfaction with assistive technology devices. This paper describes the methodology used to develop the instrument entitled the Quebec User Evaluation of Satisfaction with assistive Technology (QUEST). Based on the theoretical and practical foundations of assistive technology as well as on the concept of satisfaction, preliminary versions of the instrument were created and examined by a panel of team participants. After the panel's recommendations were incorporated, a pretest of the revised instrument was conducted and the final French version of QUEST emerged. The originality of QUEST lies in its inter-activeness and user-directed approach to assessing satisfaction with assistive technology. From a set of 27 variables, the user is asked to indicate the degree of importance he/she attributes to each of the satisfaction variables and then to rate his/her degree of satisfaction with each of the variables considered (quite or very) important. While QUEST remains a clinical instrument undergoing pilot testing, it holds much promise in our quest for a reliable and valid means of assessing assistive technology outcome from the user's perspective.

Item analysis of the Quebec User Evaluation of Satisfaction with Assistive Technology (QUEST).

The Quebec User Evaluation of Satisfaction with Assistive Technology (QUEST) is an outcomes assessment tool designed to measure satisfaction with assistive technology in a structured and standardized way. The purpose of this article is to present the results of an analysis of the 24 items comprising QUEST and to explain how a subset of items demonstrating optimal measurement performance was selected. The criteria against which the items were measured were general acceptability, content validity, contribution to internal consistency, test-retest stability, and sensitivity. The items that ranked best in terms of these measurement properties were submitted to factorial analysis in order to complete the item selection. The first series of analyses reduced the item pool approximately by half, and the second series of analyses led to the final selection of 12 items. Factor analysis results suggested a bidimensional structure of satisfaction with assistive technology related to the assistive technology device (eight items) and services (four items). The 12-item revised version that will result from this study should prove to be a reliable and valid instrument for measuring outcomes in the field of assistive technology.

The validity and applicability of the Chinese version of the Quebec User Evaluation of Satisfaction With Assistive Technology for people with spinal cord injury.

The Quebec User Evaluation With Assistive Technology (Version 2.0; QUEST 2.0) has become an important outcome assessment to capture user satisfaction in the field of assistive technology. The purpose of this study is to investigate the psychometric properties of the Chinese version of the QUEST 2.0 (C-QUEST) in terms of content and substantive and factor validity and to explore its applicability on user satisfaction on mobility and equipment among Chinese people with spinal cord injury. A group of six expert members were invited to evaluate the content validity and translation quality of the 12-item C-QUEST. The revised version, along with the brief version of the World Health Organization Quality of Life Questionnaire (WHOQOL-BREF [HK]), was administered on user satisfaction of people with spinal cord injury in the community. The content validity and item performance were evaluated to be satisfactory. Exploratory factor analysis with varimax rotation agreed with the bidimensional structure of Device and Services in the construct of user satisfaction (61.06% variance explained). Items in WHOQOL-BREF (HK) were shown to have positive and moderate correlations with C-QUEST Device items (r = .412-.567, p < .05) but no significant associations with the Services items (p > .05). The 12-item C-QUEST was shown to be a valid and relevant instrument to capture the user satisfaction among Chinese people with spinal cord injury in the context of mobility and seating equipment.

Environmental contamination and human exposure assessment to manganese in the St-Lawrence River ecozone (Quebec, Canada) using an environmental fate/exposure model: GEOTOX.

Methylcyclopentadienyl manganese tricarbonyl (MMT) is an organic derivative of manganese (Mn) used as an additive in unleaded gasoline. The combustion of MMT leads to the formation of oxides of manganese. The objective of the present study is to predict the environmental levels of Mn and the human exposure in the St-Lawrence ecozone (fluvial section, Quebec, Canada) using an environmental fate/exposure model: GEOTOX. The results of our MMT research program on abiotic and biotic components of the ecosystem and on the human exposure were used to validate the model estimations. Air and surface soil were selected as source terms with an annual Mn input rate in each compartment of 0.083-0.113 mol km-2 d-1 and 0.44-0.87 mol km-2 d-1 respectively (Mn3O4 equivalent). The predicted air, soil, plant, surface water and sediment concentrations were similar (+/- 50%) to values measured in the Montreal region. As expected, the ingestion pathway was the main absorption route for adults (> 99%), with vegetables and fruits contributing almost 80% of the dietary intake of Mn. The multimedia exposure doses for adult men predicted by the model ranged between 0.04 and 0.08 mg kg-1 d-1 compared to 0.004 and 0.201 mg kg-1 d-1 (average = 0.05) for workers from the MMT study. Considering the landscape configuration and the source vectors (air and soil) included in the model, GEOTOX estimations were in good agreement with measured values.

Correlates of psychotropic drug use in the elderly compared to adults aged 18-64: results from the Quebec Health Survey.

This study documents correlates of psychotropic drug use among older adults and compares the results observed in this group to those observed in individuals aged 18-64. A multivariate logistic regression analysis using data from the Quebec Health Survey (1992-1993) was employed to study predisposing and facilitating factors of this behavior. Results showed that 22% of the elderly reported having used anxiolytics, sedatives or hypnotics during the two days preceding the survey, compared to 4.9% of the respondents aged 18-64. Combining both samples, perceived health status, physical functional status, and health services utilization for psychological distress symptoms were the main health factors associated with psychotropic drug use. A significant difference was observed between the two age groups with regard to the association between the respondents' health status and the use of anxiolytics, sedatives or hypnotics. When the effect of other explanatory factors was controlled, older adults in poor health used anxiolytics, sedatives or hypnotics 2.21 times more than individuals aged 18-64 with a similar health condition, whereas older adults in good health used these drugs 7.49 times more than healthy individuals aged 18-64. Furthermore, after controlling for the effect of the respondents' physical and psychological health status, our results showed that more women than men used psychotropic drugs (OR = 1.57; 99% CI = 1.26-1.94). Low-income respondents were also more likely to report using these medications (OR = 1.53; 99% CI = 1.22-1.90). These results were interpreted as supporting the socio-cultural hypothesis of psychotropic drug use, which suggests that the prescribing and utilization of psychotropic drugs is influenced not only by symptoms but also by the social characteristics of individuals. It is suggested that future research may contribute to a better understanding of psychotropic drug utilization in the older adult community-dwelling population by examining consumers' attitudes and health care providers' social values concerning the appropriateness of this behavior.

Early lexical and syntactic development in Quebec French and English: implications for cross-linguistic and bilingual assessment.

BACKGROUND: Although a number of studies have been conducted on normal acquisition in French, systematic methods for analysis of French and normative group data have been lacking. AIMS: To develop a systematic method for the analysis of language samples in Quebec French, and to provide preliminary normative data on early lexical and syntactic development in French with a comparison with English. METHODS & PROCEDURES: Language samples were collected for groups of monolingual French- and English-speaking children (n=39, age range 21-47 months) with normal language development. Coding conventions for French were developed based on similar principles as English SALT conventions. However, due to structural differences between the languages, coding of inflectional morphology was considerably more complex in French than in English. OUTCOMES & RESULTS: The French procedure provided developmentally sensitive measures of lexical and syntactic development, including mean length of utterance in morphemes and in words, and number of different words, and should be an important addition to the assessment procedures available for French. Cross-linguistic similarities and differences were noted in the language sample measures. Although the same elicitation context was used in the English and the French language samples, and the analysis methods were designed to rest on similar principles across languages, systematic differences emerged such that the French-speaking children exhibited a higher mean length of utterance, but smaller vocabulary sizes. Differences were also noted in error patterns, with much lower error rates occurring in samples of the French-speaking children. CONCLUSIONS: The findings have important implications for language assessment involving cross-linguistic comparisons, such as occurs in the assessment of bilingual children, and in the matching of participants in cross-linguistic studies. Given differences in the mean length of utterance and vocabulary scores across the languages, the finding of the same mean length of utterance or vocabulary obtained in the two languages for a given bilingual child or for monolingual speakers of the two languages does not imply equivalent levels of language development in the two languages.

CARD15 genetic variation in a Quebec population: prevalence, genotype-phenotype relationship, and haplotype structure.

The caspase recruitment domain gene (CARD15) was recently identified as the underlying gene associated with the IBD1 locus that confers susceptibility to Crohn disease (CD). CARD15 is related to the NOD1/Apaf-1 family of apoptosis regulators, and three sequence variants (Arg702Trp, Gly908Arg, and Leu1007fsinsC) in the gene were demonstrated to be associated with CD. We collected a cohort of 231 patients with CD and 71 healthy control individuals from the Canadian province of Quebec, to determine the prevalence of these sequence variants in an independent population. Clinical records of all patients were systematically reviewed, and detailed phenotypic information was obtained. All patient DNA samples were genotyped for the three variants, thus enabling an analysis of genotype-phenotype correlations. In this cohort, 45.0% of patients with CD carried at least one variant in the CARD15 gene, compared with 9.0% of control individuals (P<10-7). Allele frequencies of Arg702Trp, Gly908Arg, and Leu1007fsinsC were 12.9%, 5.2%, and 10.3% in patients with CD, compared with 4.2%, 0.7%, and 0.7% in control individuals, respectively. Importantly, CARD15 mutants were seen with equal frequency in patients with familial and sporadic CD. Analysis of the relationship between genotype and phenotype convincingly demonstrates that CARD15 variants are significantly associated with ileal disease involvement, as opposed to strictly colonic disease (P<.001). Moreover, we were able to determine the haplotype structure surrounding this disease gene by genotyping 45 single-nucleotide polymorphisms (SNPs) in a 177-kb region that contained the CARD15 gene. This structure helps clarify the history of these causal mutations. Finally, this analysis shows that CARD15 involvement with CD is detectable by use of publicly available SNPs alone.

Management and outcomes of a first recurrence of Clostridium difficile-associated disease in Quebec, Canada.

BACKGROUND: During an epidemic of Clostridium difficile-associated disease (CDAD) caused by a strain that is a hyper-producer of toxins A and B, the frequency of a first recurrence after metronidazole treatment of the initial episode doubled in 2003-2004, compared with 1991-2002. METHODS: To examine whether administration of metronidazole as treatment for a first recurrence of CDAD remained appropriate, we reviewed data for patients with CDAD diagnosed in a hospital in Quebec, Canada, during 1991-2005, who experienced a first recurrence. The frequency of a second recurrence within 60 days after the first one was measured using Kaplan-Meier analysis. Cox regression was used for multivariate analysis. RESULTS: A total of 463 patients had a first recurrence of CDAD, of whom 154 (33.3%) experienced a second recurrence. Independent predictors of a second recurrence were age and duration of hospitalization after the first recurrence; this latter finding suggested that many such episodes were reinfections rather than relapses. Neither choice of treatment drug (metronidazole or vancomycin) nor use of the same drug for treatment of first recurrence, as had been used during the initial episode, was associated with increased risk of a second recurrence. However, 51 patients (11.0%) developed at least 1 complication (i.e., shock, need for colectomy, megacolon, perforation, or death within 30 days) during the first recurrence. Older age, a high leukocyte count, and renal failure at first recurrence were strongly associated with a complicated CDAD. CONCLUSIONS: Metronidazole is not inferior to vancomycin for treatment of patients with a first recurrence of CDAD, but the risk of complications with any treatment of CDAD may be higher than has previously been documented.

Complications and sequelae of meningococcal disease in Quebec, Canada, 1990-1994.

To study complications and sequelae of serogroup B and C meningococcal disease, a retrospective survey examined the outcome of all culture-proven cases reported in the province of Quebec, Canada, from January 1990 through December 1994 (serogroup B, 167 cases; serogroup C, 304 cases). Data were collected from medical files, postal questionnaires, and telephone interviews. Age groups having the most cases were the 10-19-year age group for serogroup C and the < 1-year age group for serogroup B. Fatality rates were 7% for serogroup B and 14% for serogroup C disease. Only 3% of survivors of serogroup B disease had physical sequelae, compared with 15% of survivors of serogroup C disease (skin scars, 12%; amputations, 5%; hearing loss, 2%; renal problems, 1%; and other sequelae, 4%). These results confirm the gravity of disease caused by serogroup C, serotype 2a Neisseria meningitidis and justify liberal use of vaccination for outbreak control.

Nutritional status of Quebec Indians.

The nutritional status of more than 80% of adult Indians in three bands in a remote area of Quebec was studied during the summer of 1978 and the findings were compared with the results of the 1970 and 1972 Nutrition Canada national and Indian surveys in which the Indian participation rate was 30%. In the present study, hematocrit, serum iron, transferrin saturation, serum vitamin E, serum folic acid, and urine thiamin excretion patterns showed only minor differences from Nutrition Canada's national and Indian survey. Serum total protein values were slightly higher in Indians than in the national sample, consistent with previous findings. A higher percentage of subjects had low serum concentrations of vitamin A and ascorbic acid than in the previous Indian survey, but not to such a marked degree.

Waist and hip circumferences have independent and opposite effects on cardiovascular disease risk factors: the Quebec Family Study.

BACKGROUND: A high waist-to-hip ratio is associated with unfavorable cardiovascular disease risk factors. This could be due to either a relatively large waist or a small hip girth. OBJECTIVE: We sought to define the separate contributions of waist girth, hip girth, and body mass index (BMI) to measures of body composition, fat distribution, and cardiovascular disease risk factors. DESIGN: Three-hundred thirteen men and 382 women living in the greater Quebec City area were involved in this cross-sectional study. Percentage body fat, anthropometric measurements, and abdominal fat distribution were obtained and BMI (in kg/m2) and waist-to-hip ratio were calculated. Serum blood lipids were determined from blood samples collected after subjects had fasted overnight RESULTS: A large waist circumference in men and women (adjusted for age, BMI, and hip circumference) was associated significantly with low HDL-cholesterol concentrations (P < 0.05) and high fasting triacylglycerol, insulin, and glucose concentrations (P < 0.01). In women alone, a large waist circumference was also associated with high LDL-cholesterol concentrations and blood pressure. A narrow hip circumference (adjusted for age, BMI, and waist circumference) was associated with low HDL-cholesterol and high glucose concentrations in men (P < 0.05) and high triacylglycerol and insulin concentrations in men and women (P < 0.05). Waist and hip girths showed different relations to body fat, fat-free mass, and visceral fat accumulation. CONCLUSIONS: Waist and hip circumferences measure different aspects of body composition and fat distribution and have independent and often opposite effects on cardiovascular disease risk factors. A narrow waist and large hips may both protect against cardiovascular disease. These specific effects of each girth measure are poorly captured in the waist-to-hip ratio.

Associations between daily cause-specific mortality and concentrations of ground-level ozone in Montreal, Quebec.

The authors investigated the association between daily variations in ozone and cause-specific mortality. Fixed-site air pollution monitors in Montreal, Quebec, provided daily mean levels of ozone, particles, and other gaseous pollutants. Information on the date and underlying cause of death was obtained for residents of Montreal who died in the city between 1984 and 1993. The authors regressed the logarithm of daily counts of cause-specific mortality on mean levels of ozone, after accounting for seasonal and subseasonal fluctuations in the mortality time series, non-Poisson dispersion, and weather variables. The effect of ozone on mortality was generally higher in the warm season and among persons aged 65 years or over. For an increase in the 3-day running mean concentration of ozone of 21.3 microg/m(3), the percentage of increase in daily deaths in the warm season was the following: nonaccidental deaths, 3.3% (95% confidence interval (CI): 1.7, 5.0); cancer, 3.9% (95% CI: 1.0, 6.91); cardiovascular diseases, 2.5% (95% CI: 0.2, 5.0); and respiratory diseases, 6.6% (95% CI: 1.8, 11.8). These results were independent of the effects of other pollutants and were consistent with a log-linear response function.

The combination of effects on lung cancer of cigarette smoking and exposure in quebec chrysotile miners and millers.

Although it is well known that both cigarette smoke and microscopic airborne asbestos fibres can cause lung cancer, evidence as to how these two agents combine is nebulous. Many workers have believed in the multiplicative theory, whereby asbestos increases the risk in proportion to the risk from other causes. However, evidence against this theory is mounting: a recent review concluded that the multiplicative hypothesis was untenable, and that the relative risk of lung cancer from asbestos exposure was about twice as high in non-smokers as in smokers, a finding largely independent of type of asbestos fibre. The criteria for entry to the current study were met by 7279 men in the 1891-1920 birth cohort of Quebec chrysotile miners and millers. The data consisted of date of birth, place of employment, smoking habit, asbestos exposure accumulated to age 55 and, for those 5527 who died between 1950 and June 1992, date and cause of death; 533 of the deaths were from lung cancer. For the principal analyses, ex-smokers were excluded from the study cohort, which comprised 5888 men, of whom 473 died of lung cancer. The conventional form of analysis is simply of the double dichotomy: non-smokers of cigarettes, 'unexposed' and exposed; all others, 'unexposed' and exposed. The respective standardized lung cancer mortality ratios (SMRs) were 0.29 and 0.62; and 1.37 and 1.72. Thus, the differences in relative risk, due to exposure, were closely similar, 0.33 and 0.35. On the other hand, the effects of asbestos measured by the corresponding ratios, 2.12 and 1.25, did differ, being 1.7 times as high in non-smokers as in others. The principal analysis was much more penetrating: the method was to fit models to a 'disaggregated' 6 x 10 array, by smoking habit (excluding ex-smokers) and asbestos exposure, of lung cancer SMRs. Both linear and log-linear models were fitted: the former included the additive and linear-multiplicative; the latter embraced the more conventional multiplicative form. The additive model fitted much the best. The fit of each multiplicative model was improved by the introduction of an interaction term that implied a less than multiplicative relationship. Thus smoking and exposure to chrysotile appear to have acted independently in causing lung cancer, with 10 cigarettes a day having an effect roughly equivalent to exposure amounting to 700 million particles per cubic foot x years. The refutation of the multiplicative hypothesis in these data reinforces its inapplicability in general; but the additive hypothesis is not generally applicable either. Indeed, there seems to be no good reason to believe that interactions conform to any simple theory. The implications are important.

Phenotypic and genotypic characterization of macrolide-resistant group A Streptococcus strains in the province of Quebec, Canada.

Resistance to macrolides among group A streptococci is an increasing problem worldwide. We examined 496 strains phenotypically and genotypically for resistance to erythromycin and clindamycin. Strains were isolated in five different geographical areas representing about 45% of the total Quebec population. The overall resistance rate was 4.6% but varied from 0% in rural areas to 9.4% in Montreal. Of the 23 strains showing resistance to erythromycin, 15 (65%) had an identical pulsed-field gel electrophoresis pattern, were of serotype M28T28 and harboured the erm(TR) gene, suggesting the spread of a single clone. Of the remaining eight strains, two strains had the erm(B) gene, five had the mef gene and one with a different serotype also had the erm(TR) gene.

Genotypic characterization of macrolide-resistant strains of Streptococcus pneumoniae isolated in Quebec, Canada, and in vitro activity of ABT-773 and telithromycin.

Increasing resistance of Streptococcus pneumoniae to macrolides represents a challenge for clinicians. New ketolides have an enhanced activity against macrolide-resistant strains. Four hundred and seventy-four strains of S. pneumoniae were collected during the 2000-2001 season in Quebec through a surveillance network. Macrolide resistance was 20.2%, and significantly higher in non-invasive strains versus invasive ones (22.4% versus 14.8%), and in children (30%) versus adults (14.8%). For susceptible strains, MIC(90)s of ABT-773 and telithromycin were 0.008 and 0.015 mg/L. Among the 96 macrolide-resistant strains, 56 (58%) were erm(B), 35 (37%) carried the mef(A) gene, four were carrying both genes and one none. ABT-773 and telithromycin were very active against all these resistant strains irrespective of the resistance mechanism, with MIC(90)s of 0.25 and 0.5 mg/L, respectively.

The arthropod fauna of Quebec vineyards with particular reference to phytophagous arthropods.

A 3-yr study using different sampling and trapping techniques showed that the arthropod pest fauna in two commercial vineyards in southwestern Quebec was qualitatively and quantitatively different than that of Ontario, Canada, and New York state. We hypothesize that a colder winter climate in addition to the agronomic activity of earthing up around the vines in autumn to protect the roots from freezing in winter contributed to low numbers of pests, such as the grape berry moth, Endopiza viteana Clemens (Lepidoptera, Tortricidae). Once in 3 yr, the density of this pest approached, in one of the vineyards, the action threshold recommended for New York. Therefore, it should be monitored on an annual basis. Another phytophagous arthropod that has the potential to cause sporadic economic damage is the potato leafhopper, Empoasca fabae (Harris). The Asiatic garden beetle, Maladera (= Autoserica) castanea (Arrow), was reported for the first time in Canada. The tarnished plant bug, Lygus lineolaris (Palisot de Beauvois), was also captured by sampling. However, its status as a pest has yet to be clarified.
