Leukaemic involvement of the pleura. A case report.

In chronic myelocytic leukaemia (CML), the pleura is a most uncommon site of extramedullary involvement. A 34-year-old man with CML presented with a massive pleural effusion. His peripheral blood contained few blast cells and the leucocyte alkaline phosphatase level was low. Cytological examination of the pleural fluid revealed cells with the morphological features of myeloblasts and monoblasts. The patient was treated with systemic chemotherapy, with no effect on the leukaemic pleural effusion.

[Plasmapheresis in the combination therapy of patients with pyogenic destructive diseases of the lungs and pleura].

Plasmapheresis is an effective method for controlling the resorption factor in patients with pyogenic destructive diseases of the lungs and pleura. It facilitates rapid normalization of the parameters of the patients' clinical condition and the radiological picture, and reduces endogenous intoxication. Protein-volemic disorders are more manifest in patients with a fatal outcome, which increase with the gradual progression of the disease and must be corrected before performing plasmapheresis.

Fungal diseases of the pleura: clinical manifestations, diagnosis, and treatment.

Fungal respiratory infections were once considered rare and limited to certain endemic areas in the southeastern United States or the western desert areas. With the development and increasing use of agents that alter host immunity, as well as the spread of human immune deficiency virus infections in this country, the incidence and clinical features of fungal infections are changing. Opportunistic fungi, ubiquitous in our environment, have become common infectious agents in hospitalized patients. The pathogenic fungi are no longer considered to be limited to certain geographic areas because of the increased mobility of the population and reports of infections outside the usual endemic areas. Both opportunistic and pathogenic fungi may infect the pleural space, and symptoms of pleural disease may be the presenting complaint in patients with respiratory mycoses. This review summarizes the common clinical findings in fungal infections of the pleura, including those findings that suggest a fungal etiology. Diagnostic studies used to define the etiologic agent and recommended therapeutic approaches are also discussed.

[Immunological indicators as the criteria of prognosis and treatment of infectious complications in patients operated on for chronic nonspecific diseases of the lungs and pleura].

A correlation of clinical specific features of the postoperative period and dynamics of immunological indices in patients operated upon for chronic nonspecific diseases of lungs and pleura was established. Pyo-inflammatory complications were three times more often in decreased amount of T-lymphocytes after operation. HBO-therapy has an immunostimulating effect in the form of increased total and active T rosette-forming lymphocytes, causes more rapid elimination of purulent intoxication, purification of the pleural cavity from the purulent discharge and makes the period of treatment at the hospital shorter.

[Cytology of pleura exudate in lymphoretic sarcoma (author's transl)].

In 2.9% of 1762 patients with pleura exudate, the underlying disease was a lymphoreticular sarcoma. The experienced cytologist has no difficulties to differentiate typical patterns of plasmocytoma, reticulumcell sarcoma or lymphogranulomatosis. But there may be borderline forms which can only be evaluated by critical examination. This holds true especially for differentiated types of lymphosarcoma which resemble the pattern of lymphocytic leukemia or observed through low magnification look like lymphocytic exudate. Therefore the use of oil immersion is necessary for evaluation of the smear. Fluorescence microscopy offers some advantages in special problems.

Diffuse malignant mesothelioma of the pleura--intrapleural treatment with doxorubicin.

The diagnosis of diffuse mesothelioma of the pleura means a very bad prognosis. The average survival of our own cases, controlled by autopsy, was 16.4 months (n = 36) for symptomatically treated patients and 16.1 months (n = 26) for those treated with Cyclophosphamide and other cytotoxic drugs. The difference between the two groups is statistically without significance. Doxorubicin was applied in a third group. The drug was applied intrapleurally for cases with pleural fluid in doses of 20 to 30 mg, weekly during the first month of treatment and later on monthly. The effect will be controlled directly and during the continued treatment, using cytological examination of the sediment of the pleural fluid and by simultaneous measurement of the level of hyaluronic acid in the same fluid. The advantages of this method are the small dosage, the possibility of applying over a long time and few side effects. 21 patients were treated: 9 without response, 8 lived for a longer period, 4 patients are still alive. The average survival of all the 21 patients observed is now 25.3 (8-79) months. The difference to an untreated control group of 36 patients is proved as statistical significant in favour of the therapeutic group. Doxorubicin, linked with a really early diagnosis, seems to be a suitable drug for essentially improving the prognosis of disease.

[Medical-work capacity examination of patients with chronic empyema of the pleura after pulmonectomy].

Treatment and analysis of immediate results were described in 269 patients with chronic empyema of the pleura as well as their long-term results in 207 of them. Under consideration were main criteria of the examination of labor capacity in such patients. It was shown that the establishment of the phase of "compensation at the limit" (IIa degree of pulmonary insufficiency) and subcompensation (IIb degree of pulmonary insufficiency) allowed more exact evaluation of the degree of compensatory processes of respiratory organs in patients after operation and the degree of loss of their working capacity.

[Autoimmune diseases in the lungs and pleura (author's transl)].

Diseases of autoaggression can be produced if immunotolerance to endogenous tissue is lost. Such diseases become manifest in the pulmonary and pleura regions mainly within the framework of systemic diseases of the connective tissue (collagenoses) and of the vessels (angiitides). Lupus erythematodes disseminatus belongs to the first group which also includes rheumatoid arthritis, sclerodermia, and dermatomyositis, whereas panarteritis nodosa belongs to the second group, which comprises among others arteritis of the arteria pulmonalis, Wegener's granulomatosis, Goodpasture's syndrome and Ceelen's disease. Pleuropulmonary involvement is frequent in these systemic diseases; it is in fact the principal sign in Goodpasture's syndrome and in Ceelen's disease.

[Disorders of the functional properties of thrombocytes causing thromboembolism in acute suppurative destructive diseases of the lungs and pleura].

An examination of 96 patients has established that the development of a pyo-destructive process in the lungs and pleura is accompanied by an increase of concentration of circulating thrombocytes as well as their adhesive-aggregating activity and degenerative-dystrophic alterations. These parameters were most pronounced in patients 1-3 days before their sudden death from thromboembolism of the pulmonary artery without a clinically detected source. Persantine, aspirin, pyrazolones and heparin in generally accepted therapeutic doses were shown not to possess evident anti-aggregative properties.

[Recurrence of a benign pleural mesothelioma 18 years after excision. Review of a series of 33 patients with a primary tumor of the pleura from 1958 to 1986].

A 60 year old woman developed recurrence of a localized pleural mesothelioma 18 years after initial exeresis. Based on data from 33 cases of primary tumor of pleura treated in the UCH, Vaudois, Switzerland between 1958 and June 1986, symptomatology, treatment and prognosis of pleural mesothelioma a are discussed. Findings suggest the need for long-term follow up surveillance of patients with benign mesothelioma.

Trends in mortality from malignant mesothelioma of the pleura, and production and use of asbestos in Australia.

Annual mortality from malignant mesothelioma of the pleura (MMP) in Australia (as represented by ICD8 codes 163.0 and 212.4, and ICD9 codes 163 and 212.4) increased in men aged 30 years and older from about 0.5/100 000 in 1968-1970 to 2.1/100 000 in 1983. Corresponding rates in women varied from 0.1/100 000 to 0.2/100 000 between 1968 and 1980, then rose to 0.3/100 000 in 1983. The rise in MMP mortality in men probably corresponds to the increasing use of asbestos, particularly amosite, in Australia during and after World War II. That production and importation of amosite and crocidolite in Australia reached a peak in 1958 may mean that peak mortality from MMP will not be reached until the 1990s (allowing a 35-year lag period). Substantial increases in importation and later production, of chrysotile in the 1950s, 1960s and 1970s may lead to increases in the incidence of other asbestos-related cancers, not reflected in trends in the incidence of MMP.

Biochemical and cytological features of diffuse mesotheliomas of the pleura.

The malignant mesothelioma was proved by autopsy as cause for death in 55 cases--42 men and 13 women. 4 men had the tumor without pleural effusion. Transthoracic fine needle biopsy confirmed the suspicion 3 times. The fluids caused by the tumor were exudates 50 times. One fluid was a transudate. Cytological examinations led to the partition into four sections with the following main findings: histiocytes, mesothelial cells also in clusters, tumor cells--21 cases; tumor cells without perceptibility of the matrix--18 cases; cytological empty sediments--8 cases; finding of inflammation--4 cases. The determination of the level of hyaluronic acid in pleural fluids by a colorimetric test proved a malignant mesothelioma in one third of these cases. The synopsis of the named parameters together with clinical dates in general use gives the possibility to put up the diagnosis of the primarily malignant disease of the pleura during lifetime of a patient.

[Haemangioma capillare of the pleura with ipsilateral haemopneumothorax (author's transl)].

1. The article introduces a patient with left-side haemangioma capillare of the pleura parietalis with haemopneumothorax and ipsilateral cystic changes of the left upper lobe of the l,ng. 2. The aetiopathogenesis of the disease is discussed as an abortive type of Rendu-Osler-Weber disease (hereditary haemorrhagic telangiectasia), the diagnosis being based exclusively on histological findings as the only possibility of diagnosis. 3. The therapeutic approach is discussed.

[Aggregative properties of the erythrocytes in acute suppurative-destructive diseases of the lungs and pleura].

The investigation of aggregation properties of erythrocytes in 218 patients with acute pyo-destructive diseases of lungs and pleura at different stages of the conservative and operative treatment has revealed a pronounced hyperaggregation of erythrocytes. The aggregation mechanisms are to be studied and new methods of correction of the aggregation properties of blood red cells are to be searched for.

[Osmometry as a method of control of homeostasis in acute suppurative destructive diseases of the lung and pleura].

The osmolar status was studied in 63 patients with acute abscesses of lungs and empyema of pleura at different stages of the conservative and operative treatment. The hypoosmolarity syndrome was revealed at the acute stage of the disease against the background of a pronounced purulent intoxication. An increased excretion of nonidentified osmotically active metabolites with urine was registered at the height of pyodestructive process and postoperative disease.
